Customized healthcare service platform connecting EMR system (병원 내 EMR 시스템 연동을 통한 맞춤형 헬스케어 서비스 시스템 개발)

  • Recently, university hospitals have problems as patient datas grows. And it is difficult that medical staffs check patients living on their prescription. Patients don't often remember their prescription, and don't know how to apply the prescription to their life. To solve these problems, To solve these problems, it is customized healthcare service platform using MQTT push notification. It is provided in form of a web application having good accessibility that. Based on doctor's prescription, the patients take notifications that is the timing of medication, medical appointments, and other prescription informations. The results can be entered into the web application. The healthcare professionals can check that the patients are following their prescription well. In this paper, we provide the above functions via the web application and a RESTful API server platform, and notification function can be efficiently performed by MQTT Broker. So we checked the customized healthcare service can be effectively provide.

  • Nurses have performed their nursing practice according to the standard guidelines such as NANDA, NIC, and NOC, and recorded the information on nursing process into EMR system. In particular, NANDA, nursing diagnosis taxonomy, has difficulty expressing nursing diagnosis in detail because it represents abstract concepts of nursing diagnosis. So, the hospitals in KOREA have developed and used the list of nursing diagnosis on their own without referring the international standard terminologies, and it caused the delay of computerization of nursing records. Therefore, we proposed a ontology development methodology on nursing diagnosis based on NANDA and SNOMED-CT. The developed ontology, systematically developed with the frequently used nursing diagnosis terminologies in each hospital, based on the proposed methodology enables knowledge expansion and interoperable exchange of nursing records between EMR systems. We developed an ontology using the 112 nursing diagnosis terms defined by extracting and refining information on nursing diagnosis recorded in Kyungpook National University Hospital. We also confirmed the content validity and the usefulness of the developed ontology through expert assessment and experiment.

Nephropathy related to computed tomography in emergency department patients with serum creatinine <1.5 mg/dL (정상 혈청 크레아티닌을 가진 응급실 환자에서의 조영제 연관 신증)

  • Background: Contrast-induced nephropathy (CIN) can cause serious adverse effects. To reduce the occurrence of CIN related computed tomography (CT) in emergency patients, we assessed the respective roles of serum creatinine (SCr) alone and estimated glomerular filtration rate (eGFR) as an early predictor for CIN related CT. Methods: For patients with SCr <1.5 mg/dL who underwent CT in emergency department (ED) between September 2012 and October 2013, we assessed the prevalence of CIN and its adverse effects. The Modification of Diet in Renal Disease Study (MDRD) and Cockcroft-Gault (CG) formula was used for the calculation of eGFR. Practical calculation was performed by electronic medical record (EMR) system for MDRD and internet calculating service for CG. And we investigated the prevalence of CIN in eGFR $<60mL/min/1.73m^2$ before CT. Results: A total of 1,555 patients were enrolled. The prevalence of CIN after CT was 4.6% and it showed correlation with renal deterioration, increased in-hospital mortality, and prolonged hospitalization. Despite baseline SCr <1.5 mg/dL, among enrolled patients, 11.3% as MDRD equation and 29.5% as CG formula were $<60mL/min/1.73m^2$ and in this condition, the prevalence of CIN was significantly high (odds ratio was 2.87 [1.64-5.02] as MDRD equation and 2.03 [1.26-3.29] as CG formula). Conclusion: Just SCr <1.5mg/dL was not appropriate to recognize preexisting renal insufficiency, but eGFR using MDRD equation was useful in predicting the risk of CIN related CT in ED. Using EMR, calculation of eGFR can be easier and more convenient.

A Convergence Investigation on Nursing Task Performance, Appropriate Performer, and Job Satisfaction of Nurses with Shift Work in General Ward (교대근무를 하는 일반병동 간호사의 간호업무 수행실태, 수행주체와 직무만족에 대한 융복합적 조사연구)

  • This study is a secondary data analysis to investigate the nursing tasks performance, appropriate performer, and job satisfaction in the general wards with sift work. We found that nurses performed direct (45%) and indirect nursing (55%). The workload per shift was 37.2% in the day, 35.6% in the evening, and 27.2% at night. The tasks performed after handover were 'direct nursing (34.5%)' and 'documentation and notification (25.8%)'. Nurses responded that there were some tasks that could be delegated to nursing assistants or had ambiguous boundaries with other medical personnel. There was a significant correlation between compliance to the job description of the night shift and job satisfaction (rs=.43, p=.01). These results imply that it is necessary to establish strategies that will enhance work efficiency based on job analysis by shift work, reduce handover time using EMR system, stmart devices, and clarify appropriate performers.

Data Processing and Visualization Method for Retrospective Data Analysis and Research Using Patient Vital Signs (환자의 활력 징후를 이용한 후향적 데이터의 분석과 연구를 위한 데이터 가공 및 시각화 방법)

  • Purpose: Vital sign are used to help assess the general physical health of a person, give clues to possible diseases, and show progress toward recovery. Researchers are using vital sign data and AI(artificial intelligence) to manage a variety of diseases and predict mortality. In order to analyze vital sign data using AI, it is important to select and extract vital sign data suitable for research purposes. Methods: We developed a method to visualize vital sign and early warning scores by processing retrospective vital sign data collected from EMR(electronic medical records) and patient monitoring devices. The vital sign data used for development were obtained using the open EMR big data MIMIC-III and the wearable patient monitoring device(CareTaker). Data processing and visualization were developed using Python. We used the development results with machine learning to process the prediction of mortality in ICU patients. Results: We calculated NEWS(National Early Warning Score) to understand the patient's condition. Vital sign data with different measurement times and frequencies were sampled at equal time intervals, and missing data were interpolated to reconstruct data. The normal and abnormal states of vital sign were visualized as color-coded graphs. Mortality prediction result with processed data and machine learning was AUC of 0.892. Conclusion: This visualization method will help researchers to easily understand a patient's vital sign status over time and extract the necessary data.

  • 본 논문에서는 의료기관내 이기종 시스템(OCS, PACS, EMR 등) 및 외부 기관(타 의료기관, 정부기관 등)과의 원활한 의료정보 공유를 위한 Framework을 제시하고자 한다. 의료기관 내/외부의 이질적인 Legacy System 간의 자료 교환을 위하여 현재 범용적으로 사용되는 HL7 Messaging standards를 기반으로 정보를 생성 공유하게 한다. 또한, HL7 Message 생성시 표현기법이 다른 데이터들을 HL7 표준에 해당하는 데이터로 자동 변환시킬 수 있는 방안을 제시하고, 이렇게 모인 의료정보를 다기종 Platform(PC, Mobile phone 등)에서 효율적인 접근을 위한 모델도 함께 제시한다.

  • Recently, the environment of a hospital information system is a trend to combine various SMART technologies. Accordingly, various smart devices, such as a smart phone, Tablet PC is utilized in the medical information system. Also, these environments consist of various applications executing on heterogeneous sensors, devices, systems and networks. In these hospital information system environment, applying a security service by traditional access control method cause a problems. Most of the existing security system uses the access control list structure. It is only permitted access defined by an access control matrix such as client name, service object method name. The major problem with the static approach cannot quickly adapt to changed situations. Hence, we needs to new security mechanisms which provides more flexible and can be easily adapted to various environments with very different security requirements. In addition, for addressing the changing of service medical treatment of the patient, the researching is needed. In this paper, we suggest a dynamic approach to medical information systems in smart mobile environments. We focus on how to access medical information systems according to dynamic access control methods based on the existence of the hospital's information system environments. The physical environments consist of a mobile x-ray imaging devices, dedicated mobile/general smart devices, PACS, EMR server and authorization server. The software environment was developed based on the .Net Framework for synchronization and monitoring services based on mobile X-ray imaging equipment Windows7 OS. And dedicated a smart device application, we implemented a dynamic access services through JSP and Java SDK is based on the Android OS. PACS and mobile X-ray image devices in hospital, medical information between the dedicated smart devices are based on the DICOM medical image standard information. In addition, EMR information is based on H7. In order to providing dynamic access control service, we classify the context of the patients according to conditions of bio-information such as oxygen saturation, heart rate, BP and body temperature etc. It shows event trace diagrams which divided into two parts like general situation, emergency situation. And, we designed the dynamic approach of the medical care information by authentication method. The authentication Information are contained ID/PWD, the roles, position and working hours, emergency certification codes for emergency patients. General situations of dynamic access control method may have access to medical information by the value of the authentication information. In the case of an emergency, was to have access to medical information by an emergency code, without the authentication information. And, we constructed the medical information integration database scheme that is consist medical information, patient, medical staff and medical image information according to medical information standards.y Finally, we show the usefulness of the dynamic access application service based on the smart devices for execution results of the proposed system according to patient contexts such as general and emergency situation. Especially, the proposed systems are providing effective medical information services with smart devices in emergency situation by dynamic access control methods. As results, we expect the proposed systems to be useful for u-hospital information systems and services.

A trial of Drug use Evaluation of oral levofloxacin in the Hospitalized Patients (입원환자의 경구용 levofloxacin 약물 사용 평가)

  • In July 2008, The National Police Hospital has developed the CPOE system(Computerized Physician Order Entry system) which links the medicine master and introduced the new program for the injectable antimicrobial agents. After introducing new system, we wanted to check the management of oral antimicrobial agents. Because new system has limitation that new system control only the use of the injectable antimicrobial agents. So we tried to evaluate the use of oral levofloxacin, which was chosen by a medical specialist of the infection. We retrospectively analyzed the appropriateness of oral levofloxacin through EMR(Electric Medical Record) of 72 inpatients who received oral levofloxacin in National Police Hospital in December 2008. We applied the modified ASHP(American Society of Health- System Pharmacists) DUE(Drug Use Evaluation) criteria, which is composed of justification of drug use, critical indicators, complications and outcome measures. Acceptable cases of use of levofloxacin were 67(93.01%) and 57cases(79.2%) showed good outcome. However critical indicators were not performed very well. We want to suggest the new system for the management of the medical therapeutics by the pharmacist in charge.

Development of Efficient Order Communication and Pharmacy Supporting System for Traditional Korean Medicine (효율적인 한의 처방조제지원시스템 개발)

  • The purpose of this study is to develop the order communication system for Traditional Korean Medicine(TKM) which can support prescribing decisions and provide the toxicological information. The relative vulnerability of the infrastructure of TKM has made us start the study. We carried out the benchmarking for TKM charting solution firstly, and then designed the intelligent search and supporting method for prescription decisions. We developed of the medical herbs database and the web-based order communication program which can be used in medical field actually. This system supplies a various functions to oriental medical doctors such as management for prescription history, search for herb's effects, generating prescriptions, inventory management, alerting of toxicity and taboo, guideline for taking medicine, and so on. The design and implementation process has been described in this research. We expect that this system will play an important role in electronic medical record(EMR) or electronic health record(EHR) binding diagnosis and management functions.

Reducing the waiting time of parkinson's patients in outpatient pharmacy by improving EMR and workflow (외래약국에서 파킨슨병 환자 투약대기시간 단축을 위한 전산 및 업무 흐름 개선)

  • Objectives: Prescriptions for Parkinson's can be dispensed at the outpatient pharmacy. In general, the treatment of Parkinson's disease requires a multitude of drugs, sometimes taken 4 to 6 times a day at specific times as prescribed by the medical practitioner. Said "time-specific therapy" is one of the major reasons of dispensing delay observed at the outpatient pharmacy. Because our establishment lacked a computerized system to support time-specific prescriptions, they were not recognized electronically. They had to be issued and dispensed manually, which required a greater amount of time than the automated process. To solve the problem, a new sig code was developed to handle time-specific prescriptions with a comprehensive automated dispensing system to support it. This study aims to create electronic programs and streamline the process to increase dispensing performance. And thus, ensure greater patient safety and dispensing accuracy within a shorter dispensing time and also increase employee satisfaction through a decreased workload. Methods: After identifying the problems caused by non-electronic prescriptions an automated system that allowed the issuance of time-specific prescriptions was developed. A new sig code was created that could be recognized by the Pharmacy electronic medical program, the label printer to group medications by administration times and the Automatic Tablet Counter(ATC) to count the grouped drugs accordingly. Result: With the new sig code, the practitioner became able to electronically select the times of drug administration while issuing the prescription. This 'time-specific prescription' can now be recognized by the pharmacy electronic medical program, the label printer and the ATC like any other prescription. Conclusion: The developed program started operating on September 2013. Although not all Parkinson's patients have been issued with the new electronic 'time-specific prescription', the overall dispensing process has become more streamlined and accurate. As the medical team continues to integrate the new system in their practice an additional decrease of the dispensing time is predicted. Future program upgrades and other new time-saving approaches are scheduled, which are expected to further increase the service quality of our outpatient pharmacy.
