Epidural hematoma (EDH) can sometimes be life-threatening, although small-volume EDHs can resolve spontaneously like other intracranial hematomas. However, in rare cases, EDH can transform into a chronic form instead of disappearing. In contrast to subdural hematoma, there is no agreed-upon definition or treatment of chronic EDH. A 41-year-old male patient with acute EDH in the bilateral paravertical area due to partial rupture of the sagittal sinus was operated first, and then remnant contralateral hematoma was treated conservatively. One month after surgery, he showed hemiparesis, and brain imaging revealed chronic EDH at the location of the remnant acute hematoma. We performed surgery again to treat chronic EDH through a large craniotomy. Although many cases of EDH are self-limited, clinicians must keep in mind that some cases of EDH, especially those of venous origin and arising in young people, can become chronic and require surgical treatment.

According to the very rapid technological change and market mega-trends, the ITU(International Telecommunication Union) is now under Reform. In line with whole ITU reform, ITU-T (ITU- Telecommunications Standardization Sector) as global standards development organization has setup the two major environment such as ITU-T resolutions and new standards approval process for new study period 2001-2904 in the WTSA-2900. In this paper, we analyze on detailed EDH(Electronic Document Handling) activities, EDH related resolution and Methods as strategic tools within the ITU-T and its application to the New approval process which is described in ITU-T Recommendation A.8(AAP). Finally, we discuss future ITU-T EDH policy and suggest direction of Korean EDH policy.

Spontaneous intracranial epidural hematoma (EDH) due to dural metastasis of hepatocellular carcinoma is very rare. A 53-year-old male patient with hepatocellular carcinoma, who was admitted to the department of oncology, was referred to department of neurosurgery because of sudden mental deterioration to semicoma with papillary anisocoria and decerebrate rigidity after transarterial chemoembolization for hepatoma. Brain computed tomography (CT) revealed large amount of acute EDH with severe midline shifting. An emergent craniotomy and evacuation of EDH was performed. Active bleeding from middle cranial fossa floor was identified. There showed osteolytic change on the middle fossa floor with friable mass-like lesion spreading on the overlying dura suggesting metastasis. Pathological examination revealed anaplastic cells with sinusoidal arrangement which probably led to spontaneous hemorrhage and formation of EDH. As a rare cause of spontaneous EDH, dural metastasis from malignancy should be considered.

Objective : Management guidelines for single intracranial hematomas have been established, but the optimal management of multiple hematomas has little known. We present bilateral traumatic supratentorial hematomas that each has enough volume to be evacuated and discuss how to operate effectively it in a single anesthesia. Methods : In total, 203 patients underwent evacuation and/or decompressive craniectomies for acute intracranial hematomas over 5 years. Among them, only eight cases (3.9%) underwent operations for bilateral intracranial hematomas in a single session. Injury mechanism, initial Glasgow Coma Scale score, types of intracranial lesions, surgical methods, and Glasgow outcome scale were evaluated. Results : The most common injury mechanism was a fall (four cases). The types of intracranial lesions were epidural hematoma (EDH)/intracerebral hematoma (ICH) in five, EDH/EDH in one, EDH/subdural hematoma (SDH) in one, and ICH/SDH in one. All cases except one had an EDH. The EDH was addressed first in all cases. Then, the evacuation of the ICH was performed through a small craniotomy or burr hole. All patients except one survived. Conclusion : Bilateral intracranial hematomas that should be removed in a single-session operation are rare. Epidural hematomas almost always occur in these cases and should be removed first to prevent the hematoma from growing during the surgery. Then, the other hematoma, contralateral to the EDH, can be evacuated with a small craniotomy.

In an era of very rapid technological change, standardization organizations have been confronted with the problem that they should develop more new and more complex standards as strategic tools for enforcement of competitive power in shorter time according to the reduction of Life Cycle of products and the increase in the number of standards and their complexity. To cope with this challenge, worldwide Standardization Bodies now implement and operate their own EDH (Electronic Document Handling) system which is electronic means applied to development and delivery of standards based on their standardization workflow. In this paper, we analyze the EDH guidelines and methods of major Standardization Bodies, International bodies such as ITU-T, ISO/IEC JTC1, Regional bodies such as ETSI, Committee T1 of USA and TTA (Telecommunications Technology Association) of Korea. Finally, we discuss future EDH policy and suggest architecture of Advanced Standards Information System.

최근 국제표준화기구인 ITU를 비롯해 ETSI, T1 Committee, TTA, TTC, ATSC, TSACC 등 각 지역 표준화 기구 (Participating Standardization Organization: PSO)들은 빠른 기술개발에 따른 적기의 표준공급 및 전자적인 표준화문서유통을 통한 신속한 표준제정을 위해 EDH(Electronic Document Handling)라는 전자정보유통시스템을 구축하고 기고서 및 표준문서 등 표준화 진행에 관련된 각종 정보를 전자적으로 검색하고 처리할 수 있는 환경을 구축하는데 많은 노력을 기울이고 있다. 이와 관련하여, 본 고에서는 제2차 세계전기통신표준총회(WTSC-96)에서 가장 활발하게 논의된 사항 중의 하나인 ITU-T/TSAG/EDH 관련 표준화 활동 현황을 고찰하고, 향후 EDH의 방향을 소개하고자 한다.

정보통신분야의 대표적인 국제 표준화기구인 ITU는 급변하는 기술 및 시장 흐름에 대응하여, 최근 많은 개혁을 추진하고 있다. ITU 표준화 분야에서의 개혁 조치는 크게 보아 신속한 표준 제정을 위한 표준화 절차의 개선과 이를 위한 관련 규정의 변경으로 구분되며, 이를 위해 이번 회기(2001~2004) 동안 ITU 표준화 작업의 근거가 될 ITU 표준화 규정을 ITU-T 총회(WTSA-2000)에서 완료하였다. 본 고에서는 개혁내용 중 ITU-T가 표준화 절차의 효율화를 위해 전략적 도구로 내세우고 있는 EDH 관련 현황과 EDH 방법을 도입하여 새롭게 추진되고 있는 표준화 절차에서의 적용방법 등을 살펴보고, 향후 국내 표준화 활동의 대응방향을 살펴보고자 한다.

We present a case of acute spinal epidural hematoma[EDH] following unilateral laminectomy for bilateral decompression[ULBD]. A 45-year-old male presented with severe multi-level spinal stenosis underwent ULBD on the left side at the L2-3 and L3-4 level. Five hours after operation, paraparesis developed along with severe bilateral buttock pain. The CT scan showed an acute spinal EDH at the L2-3 level. The acute spinal EDH was successfully decompressed after emergency decompressive surgery with performing an additional laminectomy on the contralateral side at the L2-3 level. Although ULBD is an effective minimally invasive surgical technique for treating spinal stenosis, the possibility of acute spinal EDH should be kept in mind, as happened in our case.

국제 표준화 기구인 ITU, ISO, ISO/IEC/JTC1을 비롯해 지역표준화 기구인 ETSI, 국가기관인 Committee T1, TTA, TTC 등은 적기의 표준공급 및 전자적인 표준화 문서유통을 통한 신속한 표준보급 및 제정을 위해 EDH(Electronic Document Handling)라는 전자정보유통시스템을 구축해놓고 있다. 즉, 각자의 표준화 Workflow에 맞는 전자적 문서처리시스템을 구축하고, 이를 이용하여 표준화 진행에 관련된 각종 정보를 효율적으로 검색하고 처리할 수 있는 환경을 구축하는 데 많은 노력을 기울이고 있다. 본 고에서는 ITU-T의 최근 새롭게 구현된 ITU Web에서의 정보제공형태와 웹기반 EDH 활용지침 그리고, 이를 주도하고 있는 ITU-T/TSAG/WP3 관련 활동현황을 분석하고 향후 방향을 소개한다.

국제 표준화기구인 ITU는 급변하는 기술 및 시장 흐름에 대응하여 최근 많은 개혁을 추진하고 있으며, 표준화 분야인 ITU-T에서도 이를 반영하여 ITU-T 총회(WTSA-2000)를 통해 이번 회기(2001~2004) 동안 표준화 작업의 근거가 될 ITU 표준화 규정(ITU-T Resolutions, A-Series Recommendations)을 제.개정하였다. 본 고에서는 ITU-T가 신속한 표준화를 위해 전략적 도구로 내세우고 있는 EDH와 관련하여, ITU-T 규정을 고찰하고 지난 2001년 3월 첫번째 회의를 가진 ITU-T 전략 그룹이라 할 수 있는 TSAG 내의 WP3(EDH, Promotion and Publication Policy)에서 논의된 사항을 중심으로 주요 핵심동향을 살펴보고자 한다.
