Wilson's Disease 환자 1례의 증례 보고 (A Case of Wilson's Disease)

  • Wilson's disease is a rare inborn error of metabolism inherited as a autosomal recessive trait. The disease has varied mode of manifestations. It is characterized by different neurologic disorder and hepatic disease. I experienced a case of Wilson's disease in 40 year old woman who was suffered from liver cirrhosis, severe anorexia, and classical neurologic symptoms such as tremour, dysarthria and ataxia. The symptoms was not relieved by D-penicillamine, Youngyanggaksan and Samchulgunbi-tang but anorexia was improved significantly by same medication.

중풍언어장애에 대한 동서의학적 고찰 (The Oriental and Western Medical Study of Communication Disorder with Post-Stroke)

  • Objectives : The purpose of this study is to search for more effective methods of diagnosis and treatment of Communication Disorder with Post-Stroke. Methods : Literature review on Communication Disorder with Post-Stroke in view of oriental and western medicine. Conclusions : Communication Disorder with Post-Stroke is relative with aphasia in western medicine, Sul-um(舌瘖) oriental medicine Aphasia, apraxia of speech, and dysarthria come after strock Each of them has its own mechanism. Yomchon(CV23), Amun(GV15), Chuldol(CV22), Tongni(HT5), Pungnyung(ST40), Pungbu(GV16), Chigu(TE6), Yongchon(KI1), Hapkok(L14), Peakoe(GV20), Kokchi(LI11), Puryu(KI7), Shinsu(BL23), Kumjin-okaek, and Chohea(KI6) was used on Communication Disorder with Post-Stroke treatment.

Preoperative Extrapontine Myelinolysis with Good Outcome in a Patient with Pituitary Adenoma

  • Few preoperative extrapontine myelinolysis (EPM) cases with pituitary adenoma have been reported. No such case had long follow-up to see the outcome of EPM. We reported a 38-year-old man complaining of nausea, malaise and transient loss of consciousness who was found to have severe hyponatremia. Neurologic deficits including altered mental status, behavioral disturbances, dysarthria and dysphagia developed despite slow correction of hyponatremia. Endocrine and imaging studies revealed hypopituitarism, nonfunctional pituitary macroadenoma and extrapontine myelinolysis. Transsphenoidal surgery was performed after three weeks of supportive therapy, when neurological symptoms improved significantly. The patient recovered function completely 3 months after surgery. Our case indicates that outcome of EPM can be good even with prolonged periods of severe neurologic impairment.

  • The relations between words intelligibility and sentences intelligibility were tested on adults with cerebral palsy(athetoid type). Intelligibility is used as an important evaluation value in the field of diagnosis and therapy of dysarthric patients. In order to develop one syllable phonetic contrast intelligibility test using specific phonetic contrasts, the correlation with sentences intelligibility was tested to find out the validity. Pearson's simple correlation coefficient was .83 that shows a high correlation. Also, comparing the range and standard deviation given by seven evaluators on each subject, it was shown that when evaluating patients of moderate intelligibility, words intelligibility was more reliable than sentences intelligibility.

  • Bickerstaff's brainstem encephalitis (BBE) is an autoimmune central nervous system disorder. It can occur in more limited forms and may overlap with Guillain-Barr$\acute{e}$ syndrome (GBS). A 49-year-old female presented with rapidly progressive paralytic ileus, urinary retention, deep drowsiness, ophthalmoplegia, dysarthria, ataxia, quadriparesis and hyporeflexia after viral meningitis. She was diagnosed as BBE with GBS and treated with immunoglobulin. She was completely recovered after 1 month. It is a rare case of BBE overlapping with GBS presenting with severe paralytic ileus.

만성 알콜중독증 환자에서 발생한 중심성 뇌교 수초용해증 치험 1례 (A Case of Central Pontine Myelinolysis in a Chronic Alcoholic)

  • Objective : This study was case report to show the good effects of herbal medicine (Daekumeum-ja) and acupuncture complex therapy for central pontine myelinolysis (CPM). Method : A 44-year-old man suffering from CPM was chronic alcoholic without electrolyte disturbance. He showed symptoms of dysarthria, dysphagia, dizziness, weakness of lower extremities and gait disturbance. CPM was diagnosed at his former hospital by MR imaging. He had no positive response to western treatments. We treated the patient with herbal extract (Daekumeum-ja) and acupuncture. Results : After our treatments, there was no change of Brain CT. But overall symptoms of CPM were improved. Conclusion : This study suggests that herbal medicine (Daekumeum-ja) and acupuncture complex therapy is probably effective in the treatment of CPM.

경동맥 협착을 동반한 관상동맥 질환의 수술적 치료 -2례 보고- (Surgical Management of Coronary Artery Disease Combined with Carotid Artery Stenosis -A Report of Two Cases-)

  • The optimal surgical approach to the patients with coronary artery disease combined with carotid artery stenosis is controversial. We report two cases of successful surgical management of the patients with combined obstructive coronary and carotid artery disease. The first case was a 69-year-old female who had unstable angina pectoris and a past medical history of left carotid endarterectomy. She was revealed to have triple vessel coronary disease and nearly total occlusion of right internal carotid artery. She was undergone staged right carotid endarterectomy 10 days before coronary bypass surgery. The second case, a 54-year-old male with a past medical history of left hemiparesis and dysarthria, was admitted due to unstable angina pectoris. He was revealed to have triple-vessel coronary disease and more than 90% stenosis of left internal carotid artery and 50% stenosis of right internal carotid artery. In the latter case, a combined coronary bypass surgery and left carotid endarterectomy was done. In both cases, postoperative neurologic complications were not observed.

인숙산(仁熟散)으로 치료한 불안을 주소로 하는 환자 1례(例)에 대한 임상보고 (A clinical report on anxiety improved by Insooksan(仁熟散))

  • This study is designed to evaluate the effects of an oriental medicine therapy, namely insooksan(仁熟散), on anxiety disorders. The clinical data was analyzed on a patient with anxiety disorders due to Simdamheagub(心膽虛技) whose main symptoms were anxiety, dysarthria, tremor of hands. The patient was admitted at the Oriental Neuropsychiatry Department of DaeGu Hanny University DaeGu Oriental Medicine Hospital on May 25, 2005, and was treated with herbal medicine(insooksan), acupuncture. After treatment, the patient's main symptoms were disappeared and secondary symptoms were improved. This study suggests that Insooksan(仁熟散) is significantly effective in treatment of Anxiety disorders due to Simdamheagub(心膽虛怯).

  • A 77-year-old male patient was admitted to the hospital with acute appendicitis and underwent emergency appendectomy under inhalation anesthesia without any complications. After the operation, the patient was placed on epidural analgesia with the mixture of fentanyl-bupivacaine and scopolamine patch for nausea and vomiting. The patient experienced central anticholinergic syndrome (CAS) with hallucination, confusion, somnolence, dysarthria and delirium at 20 hours after patch placement and recovered from all symptoms of CAS in 24 hours after the removal of the patch. The CAS occurs when central cholinergic sites are occupied by specific drugs and also as a result of an insufficient release of acetylcholine. The diagnosis of CAS is often determined by a process of exclusion and not actually made until a positive therapeutic response to physostigmine, a centrally active anticholiesterase agent, has taken place. Treatment of CAS includes prompt removal of the patch, cleansing of the area, and physostigmine administration. The scopolamine patch should be used with extreme caution in elderly patient.

Atypical Metronidazole-Induced Encephalopathy in Anaerobic Brain Abscess

  • Metronidazole-induced encephalopathy is a very rare complication of the long standing use of metronidazole. The encephalopathy is bilateral and symmetric in nature. We report on the magnetic resonance imaging (MRI) and clinical course of metronidazole-induced encephalopathy in a 60-year-old female with a persistent anaerobic brain abscess after draining of the abscess. After 3 months of metronidazole administration, the patient complained of dysarthria, tingling sense of all extremities, and left hemiparesis. MRI revealed symmetric hyperintensity lesions in medulla, pons, dentate nuclei of cerebellum, and splenium of corpus callosum, all of which represent typical findings of metronidazole-induced encephalopathy. In addition, asymmetric lesions in midbrain, thalamus, putamen and cerebral subcortical white matter were noted. The patient recovered after discontinuation of metronidazole and the remaining abscess was successfully treated with meropenem and levofloxacine.
