말속도와 강도 변조에 따른 경도 마비말장애 환자의 말 용인도 변화 (The Change of Acceptability for the Mild Dysarthric Speakers' Speech due to Speech Rate and Loudness Manipulation)

  • This study examined whether speech acceptability was changed under various conditions of prosodic manipulations. Both speech rate and voice loudness reportedly are associated with acceptability and intelligibility. Speech samples by twelve speakers with mild dysarthria were recorded. Speech rate and loudness changes were made by digitally manipulating habitual sentences. 3 different loudness levels (70, 75, & 80dB) and 4 different speech rates (normal, 20% rapidly, 20% slowly, & 40% slowly) were presented to 12 SLPs (speech language pathologists). SLPs evaluated sentence acceptability by 7-point Likert scale. Repeated ANOVA were conducted to determine if the prosodic type of resynthesized cue resulted in a significant change in speech acceptability. A faster speech rate (20% rapidly) rather than habitual and slower rates (20%, 40% slowly) resulted in significant improvement in acceptability ratings (p <.001). An increased vocal loudness (up to 80dB) resulted in significant improvement in acceptability ratings (p <.05). Speech rate and loudness changes in the prosodic properties of speech may contribute to improved acceptability.

파킨슨병 환자 대상 조음교대운동의 음향적 분석 (An Acoustic Analysis of Diadochokinesis in Patients with Parkinson's Disease)

  • The acoustic analysis of diadochokinesis(DDK) has been used to evaluate dysarthria. However, there has not been an automatic method to evaluate dysarthria. The aim of this study was to introduce a new automated program to measure DDK tasks and to apply this to clinical patients with idiopathic Parkinson's disease(IPD). Fourty-seven patients with IPD and a healthy control group of twenty participants were selected with every DDK task recorded three times. Twenty-five acoustic parameters in the program were developed. The relevant parameters were times of DDK, pitch related parameters, intensity parameters which were analyzed by 2-way ANOVA. Significant differences between the groups were found in the times of DDK, pitch related parameters, and intensity parameters. The findings indicated that the pitch of control group was more stable than that of the IPD. Even though the patients with IPD had a higher intensity value, this phenomenon was caused by the weakness of the IPD group who could not control their speech with a breath.

  • Schizencephaly is a form of the porencephaly in which there is a cystic cavity in the cerebral hemispheres. Schizencephaly is a congenital disease which consist of clefts that extend through the entire hemisphere from the ependymal lining of the lateral ventricles to the pial covering of the cortex. The etiology and the pathogenesis of this malady are not clearly estalilished. Typical symtoms is seizures, mental retardation, spastic tetraplegia and blindness. A case of schizencephaly which was treated through methods of the Oriental Medicine, and that met with good results, is reported.

퇴행성질환과 말언어장애 재활 (Neurodegenerative Disease and Speech Rehabilitation)

  • Neurodegenerative diseases such as Parkinson's disease and amyotrophic lateral sclerosis may induce impairment of speech motor system. This review discusses the characteristics of dysarthria and symptom management for these conditions. Given the progressive nature of the neurodegenerative diseases, speech-language pathologists must be aware of appropriate augmentative and alternative communication equipment at the early stage of the disease course. Patients with neurodegenerative diseases can maintain functional communication with augmentative and alternative communication supports.

간이식 환자의 뇌경색에 대한 한의학적 치료 증례보고 (Case report of The liver transplant patient's cerebral infarction treated with Korean Medicine)

  • Objectives : Liver cancer usually appears in the setting of underlying liver disease. Liver transplantation has potentials to improve survival for patients with liver cancer. This case report was designed to assess the clinical effect of Korean medicine on liver transplant patient's cerebral infarction.Methods : We provided Chiganpung Granule(Chodeungsan) to a patient who was 58 year old man complained of cognitive impairment and dysarthria. We checked the change of symptoms(K-MMSE, Aphasia Rapid Test) and liver function test and kidney function test.Results : We observed that Chiganpung Granule(Chodeungsan) decreased cognitive impairment and dysarthria. Liver function test and Kidney function test was improved.Conclusions : From the above results, it is considered that Korean medicine treatments is effective on liver transplant patient's cerebral infarction.

  • Diadochokinetic (DDK) speech task has been utilized as an evaluating tool for speakers with dysarthria for many years. This study attempted to differently diagnose multiple system atrophy (MSA) from idiopathic Parkinson's disease (PD) using patients' performance of DDK (i.e., alternate motion rate (AMR)). The subjects included 11 cases of pathologically confirmed MSA and 16 IPD patients who commonly presented with parkinsonian syndrome. The speech sample of each patient was analyzed acoustically using the MSPTM(Motor Speech Profile, a module of CSL). The results showed that the average DDK rate was significantly faster in the IPD than the MSA groups in all three syllables (i.e., /puh/, /tuh/. and /kuh/). We propose the average DDK rate variable as a core clinical trait in differentiating the two pathological conditions.

  • An auditory-perceptual evaluation has long been utilized in assessing dysarthric speech. The process involves subjective judgement and the results might vary depending on clinical experiences or training of listeners. This study aimed to investigate reliability of the auditory-perceptual evaluation of 22 multi -dimensional variables on 6 patients with Parkinsonian speech disorders. Listeners were divided into two groups: one consisted of 6 speech therapists with clinical experiences for three years or more, and the other 6 graduate students without any previous clinical background. The results showed that the former evaluated dysarthric speech with higher inter-rater and intra-rater reliabilities than the latter. Furthermore, such speech variables as 'precise consonant: 'speech intelligibility: and 'SMR regularity' were more influenced than others by clinical experiences. We, therefore, postulated that a reliable auditory-perceptual evaluation of dysarthric speech may require adequate amount of clinical training of listeners.

비유창성 화자의 후두 교호운동 특성 (Characteristics of Laryngeal-Diadochokinesis (L-DDK) in Nonfluent Speakers)

  • Laryngeal DDK involve with the rate, pattern, and regularity (periodicity) in opening and closing of vocal fold. This study was aimed at investigating the characteristics of laryngeal DDK between nonfluent and fluent speakers. One with an ataxic dysarthria (with cerebellar lesion) and the other with stuttering, and 13 normal speakers were evaluated. L-DDK were analyzed with MSP (motor speech profile, CSL 4400). Measures of DDK included: DDKavr, DDKcvp, DDKjit, DDKavp. An ataxic dysarthric speaker and a stutterer showed more reduced rate and aperiodic L-DDK (both adductory and abductory movement) than normal speakers. But the average L-DDK period (ms) in adductory movement in a speaker with stuttering showed more decreased than the other. Results from this study are preliminary. Nonetheless, results of L-DDK produced by nonfluent speakers suggested the possibility to have relation with slow rate of phonatory initiation and connected speech. In the future, perceptual studies are needed in conjuction with acoustic and speech production.

순수 소뇌실조증의 임상 양상으로 SCA 1의 과도한 CAG 반복서열을 보인 유전성 소뇌실조증 가족 1례 (Pure Cerebellar Ataxia Presenting in the SCA 1)

  • SCA 1 is an autosomal dominant disorder. The phenotypic manifestations of SCA 1 are not specific, and thus, the diagnosis of SCA 1 rests on molecular genetic testing. The number of CAG repeats ranges from 6-44 in normal alleles and from 39-81 repeats in disease-causing alleles(chromosomal locus 6p22-23). The main clinical features of SCA 1 are ataxia, dysarthria, ophthalmoparesis, extrapyramidal signs without retinal degeneration. A 24-year-old woman with suspected family history presented with progressive cerebellar ataxia, dysarthria, ptosis, titubation and general weakness. Brain MRI revealed a moderate cerebellar atrophy. A genomic polymerase chain reaction(PCR) analysis showed 66 repeats at the SCA 1 locus.

중풍으로 인한 마비성 조음장애 환자의 침술 후 말소리의 음향학적 평가 연구 (Acoustic Evaluation of acupuncture therapy effects on post-stroke dysarthria)

  • Stroke makes several physical deficits. Dysarthria is one of the most difficult problems in conventional medicine because of the weakness of neuromotor control. The purpose of this study is to find the acoustic characteristics of acupuncture therapy effects on post-stroke dysarthria. Seven patients with stroke(infarction or hemorrhage) were selected by CT or MR imaging. The authors applied acupuncture therapy by inserting needles into 8 acupuncture points, ipsilateral ST4, ST6 and contralateral LI4, ST36 on facial palsy side, and CV23, CV24, bilateral "Sheyu" for 4 weeks. Speech sample were composed of five simple vowels /a,e,i,o,u/ and meaningless polysyllabic words CVCVC(C: stops, affricated, fricative sounds, v: /e/). .VOT, total duration of each speech samples and vowel formant (F1&F2) were analyzed on Spectrogram. The results are as follows: 1. VOT of bilabial and velar stops was decreased post treatment. The VOT of bilabial glottalized pre and post treatment were statistically significant (p < 0.05). 2. Total duration of polysyllabic words was decreased post treatment. Decrement of total duration containing the bilabial was statistically significant (p<0.05). 3. First formant of round vowel /o/ pre and post treatment was statistically significant (p<0.05).
