목적: 본 연구는 의료인과 간호학생의 웰다잉 의미와 주관성 유형을 확인하고, 유형별 특성을 분석하기 위해 Q 방법론을 적용한 조사연구이다. 방법: 간호학과 학생 8명, 호스피스전문병원 임상간호사 3명을 대상으로 심층면담과 선행연구고찰을 통해 102개의 Q-모집단을 구성하였으며, 이후 Q-방법론의 전문가인 간호학과 교수 1명과 간호학과 박사과정 중인 2명의 의견 교환 및 재조정을 통하여 총 33개의 Q-표본을 선정하였다. 4년제 간호학과 학생 11명과 경기도 일개 3차 종합병원의 간호사 9명과 의사 2명을 대상으로 총 22명의 P-표본을 대상으로 하였다. P-표본은 선정된 33개의 진술문을 강제 정상분포가 되도록 7점 척도 상에 분류하도록 하였고, 양극단에 분류한 진술문과 관련하여 추가 진술문을 작성하도록 하였다. 수집된 자료는 PC-QUANL Program으로 요인 분석하였다. 결과: 분류된 의료인의 웰다잉 의미 유형은 모두 3가지로 나타났으며 이들 유형에 의해 설명된 전체 변량은 57.97%였다. 제1형은 '현실중시형'으로 부담 없이 아름다운 모습으로 임종하는 것에 의미를 두었다. 제2형은 '관계중시형'으로 죽기 전 자선을 베풀고 주변인들과 화해와 용서의 시간을 갖는 것에 의미를 두고 있었다. 제3형은 '자연순응형'으로 살만큼 살다 가족의 간호를 받으며 임종하는 것에 의미를 두었다. 결론: 웰다잉에 관한 의료인과 간호학생의 주관성은 임종을 맞이하는 환자간호에 영향을 미친다. 따라서 의료인과 간호학생은 삶과 죽음에 대한 깊은 통찰을 가져야 하며, 이를 위해 웰다잉에 대한 주관성 유형별 특징에 맞는 죽음교육이 필요하다.
Purpose: This study reviewed what the location of death (LOD) means as an outcome and how to use LOD to assess end-of-life (EOL) care. This study also examined the reason why LOD is significant for the quality of EOL care. Methods: A literature review was performed, using LODs and home deaths as outcomes in the field of EOL care, and analyzed the findings associated with key fields in regards to LOD. Results: Palliative care research used LOD, in particular, hospital death (versus home death) as a significant outcome when examining cost savings, quality of life care, and patient and family preferences. Based on substantial evidence from previous research, home hospice or continuous palliative care in non-hospital settings (i.e. homes, nursing homes) have been designed and available for dying patients in developed countries. Conclusion: The LOD delivers practical significance as an outcome for diverse reasons. In-depth examination on LOD in South Korea is needed despite limitations to interpretation of its meaning in the country.
2016년 2월 3일에 제정된 "호스피스 완화의료 및 임종과정에 있는 환자의 연명의료결정에 관한 법률(이하, 연명의료결정법)"이 2017년 8월 4일에 시행되면서, 2011년부터 "암관리법"에 의해 추진되던 호스피스 완화의료(이하, 호스피스)가 새로운 법에 근거하여 추진되게 되었다. 이에 연명의료결정법의 호스피스에 대한 내용을 암관리법에서의 내용과 비교하여 주요 변경사항에 대해 살펴보아 새로운 정책의 내용과 방향에 대하여 숙지하여, 제도 시행에 참여하도록 해야 한다.
Oriental medicine thinks life and death as the following. 1. The universe seems to be a kind of organism which is divided into 3 branches, as Heaven, Earth and Man. Man is not created from nihil by the Creator. Heaven and Earth by their interaction operate to produce man. This is similiar that zygote is not created from nihil, and that sperm and ovum are transformed into zygote by their interaction. The symbolic meaning of sperm is Heaven, and that of ovum is Earth. Mind and body, as well as spirit and body, are not the real, but artificial words for the purpose of observing and expressing one man. So there is not spiritual substance as distinct from body. The expected life span of man is subjected to change, and is always becoming through life. Fate, the Creator and the world to come cannot be said to be. 2. After one's death, man is transformend into Heaven and Earth. Dying is this process of transformation. Although man comes into existence and closes one's life, the total life of the universe does not change. The criteria of determination of death is not in cell death, but in somatic death. Somatic death divided into 2 branches, one is heart-lung death, the other is brain death. For the standard of health changes ceaselessly as time goes by, aging and dying is not the process of losing health. Because of mind cannot be seperated from body, we'll feel at ease bodily and mentally in healthy dying. The completion of lifetimes is the value of healthy dying. 3. From the viewpoint of these, we must think to let a person die healthily is the right medical ethics. The way to let a person die healthily is divided into 3 branches, one is treatment, another is prevention and the other is promotion of health. We should treat and prevent death of sickness, but take care of healthy dying.
본 연구는 웰다잉을 어떻게 준비하고 있는가에 대한 질문을 통해 노인들이 생각하는 웰다잉을 위한 준비의 의미를 알아보기 위해 시도된 질적 연구이다. 경기도 B시의 노인복지관을 방문하는 65세 이상의 노인 10명을 대상으로 포커스그룹 인터뷰를 진행하였으며, 5명의 대상자를 한 그룹으로 하여 총 2그룹을 인터뷰 하였다. 연구결과 노인들은 신체적, 심리적, 사회적, 영적 준비 영역 4개 차원에서 웰다잉을 준비하고 있었고, 각 차원에서 총 8개의 주제가 도출되었다. 신체적 준비에서는 '건강 관리하기', '하고 싶은 것 하기'가 도출되었고, 심리적 준비에서는 '후회하지 않는 마음 갖기', '주변 사람에게 베풀기'가 도출되었다. 사회적 준비에서는 '재산 정리하기', '희망하는 죽음의 장소 정하기', '사전 연명의료 의향서 작성하기'가 도출되었으며, 영적인 준비에서는 '종교에 의지하기'가 도출되었다. 노인들은 웰다잉을 위해 하나의 측면이 아닌 신체적, 심리적, 사회적, 영적의 다양한 측면에서 웰다잉의 준비를 하고 있었다. 따라서 웰다잉을 위한 프로그램 개발 시 다양한 측면에서 실제적인 죽음 준비가 이루어지도록 프로그램이 기획되어야 할 것이다.
Hospice is defined by the National Hospice and Palliative Care Organization (NHPCO, USA) and WHO, as a program of care that provides comprehensive medical, nursing and support services to dying patients and their family. Despite its broad definition, however, hospice care in Korea has been focused mostly on terminal cancer patients. Thus hospice eligibility for patients with advanced cancer is relatively easier to predict than those with other fatal chronic illnesses such as heart, lung, renal or liver diseases, and dementia. This makes it more difficult for patients and families to prepare for death and gain full benefits of hospice care. This article introduces the medical guidelines for selected non-cancer patients who are expected to live for only six months, this making it possible for patients, who are nearing the end of life, to avoid unwarranted suffering.
As cost pressures have escalated, policy makers, politicians, health care providers and families have tried to devise ways to reduce health care costs. While originally developed to enhance patient control and to provide better care at the end of life, hospice care has recently received significant attention as a mean of reducing health care costs. As a program providing care for patients who are dying at their homes, hospice has expanded slowly since the opening of the first hospice in Korea in 1963. Therefore, a variety of services that responds to the needs and concerns of many dying people and their families is limited The purpose of this study was to determine the potential cost savings at the end of life among patients who used home hospice compared with the patients who received institutional care in Korea. This study used a retrospective, descriptive design. The sample for this study included 46 patients who died of lung cancer: 25 patients who received home hospice care and 21 patients who received institutional care. Data on patient characteristics, kinds and frequencies of provided treatment and nursing services, and hospice and hospital charges during the last month before death were collected. Cost of care was measured by the average cost per patient per day in the last month of life. The results of the study indicated that there were significant differences in average cost of care between home hospice sample and institutional care sample (t=9.956, p<.001; home hospice sample: M=18,102 won, institutional care sample: M=317,578 won). The cost of the home hospice sample was approximately 6% of the cost of institutional care. The majority of the home hospice nursing services were education (35.7%) and supportive counseling (25.2%), followed by medication management (13.6%), assessment (12.1%), basic nursing (7.2%), treatment (5.5%) and others. In institutional care sample, basic nursing and treatment were more emphasized than education or supportive counseling among the nursing services provided. The results of this study showed the potential for hospice to reduce costs and implications for policymakers and clinicians to incorporate hospice program into the formal health care delivery system in Korea.
본 연구는 좋은 죽음을 맞이하고 싶어 하는 사람에게 웰다잉을 하지 못하도록 저해하는 요인이 무엇인지 파악하고자 한다. 연구대상은 전국 17개 시와 도의 만 19세 이상 만 75세 미만의 성인 1,000명을 대상으로 선정하였다. 설문구성은 일반적 특성 4문항과 리커트의 7점 척도를 활용한 웰다잉의 저해요인과 관련된 20문항으로 구성하였다. 분석은 기술통계, 상관분석, 의사결정나무 분석을 수행하였다. 연구결과는 웰다잉을 저해하는 요인들에 관한 문항 중 '퇴행성 질병(치매 등)'은 5.502점, '통제력 상실(정신/신체)'는 5.268점 등으로 나타났다. 웰다잉 저해요인들 간의 상관성 분석에 있어서는 '안 좋은 부부관계와 안 좋은 자녀관계', '죽음교육을 받지 못함과 의료정책 홍보부족(웰다잉)', '안 좋은 자녀관계와 사람들의 무관심'의 순서로 유의한 양의 상관관계가 높게 나타났다. 이러한 연구결과를 고려하여 좋은 죽음을 맞이하기 위한 죽음교육을 구성 및 실시한다면 사회전체가 좋은 죽음을 위한 인식과 실천적 삶을 위해 노력하고 삶과 죽음 교육의 확대가 이루어 질 것으로 사료된다.
목적: 호스피스완화의료의 목적은 환자와 가족의 고통을 줄이고 삶과 죽음의 질을 향상시키는데 있다. 편안한 임종을 맞이하는 것은 죽음의 질을 향상시키는데 역할을 한다. 이를 위해 환자와 보호자는 남은 삶 동안 임종을 준비할 수 있도록 임종과정 중 발생하는 다양한 문제와 요구에 대한 이해가 필요하다. 따라서 호스피스 환자와 가족을 위한 매뉴얼화된 임종교육 시청각 자료를 개발하고자 한다. 방법: 시청각 자료 개발을 위해서는 문헌고찰을 토대로 접근하기 쉬운 파워포인트 유인물로 초기 교육자료 개발하였고 다섯 차례에 걸친 전문의의 자문과 수정 및 보완, 평가 과정을 거쳐 완성하였다. 결과: 호스피스 환자와 가족들을 대상으로 한 시청각 교육자료는 3차시 5단계로 분류하였으며 교육 소요시간은 총 26분 34초이다. 최종 교육프로그램은 시청각 교육실 협력 하에 촬영하였으며 시청각 자료를 완성하였다. 결론: 본 연구는 완화의료병동의 구축과 전문성 증대를 위해 개발된 매뉴얼화 된 임종교육 시청각 자료로써 큰 의의가 있으며, 호스피스 환자와 가족뿐 아니라 호스피스 영역 전문 종사자들이 호스피스 환자와 가족을 대상으로 효율적인 임종교육 자료로 적극 활용되길 기대한다.
목적: 호스피스 완화간호 실무를 향상시키기 위하여 간호사를 위한 호스피스 완화의료 기본 교육과정을 개발하기 위함이다. 방법 간호사를 대상으로 하는 국내외 호스피스 완화의료 7개 교육과정의 내용을 비교하였으며, 전국의 호스피스 완화의료 기관에서 근무하는 간호사 162명에게 우편으로 교육 요구도를 조사하였다. 결과: 1. 국내외 교육과정에서 공통적으로 다루고 있는 내용은 호스피스 완화요법의 이해, 삶과 죽음의 이해, 말기 환자의 통증 및 증상관리, 기관견학 및 실습, 호스피스 병동 운영의 실제, 가정 호스피스, 건강사정(신체사정), 치료적 의사소통, 아동 호스피스, 호스피스 운영관리, 호스피스 완화의료 팀, 호스피스 완화의료의 윤리와 법, 심리적 사회적 영적 돌봄, 임종 관리, 사별가족관리 등이었다. 2. 간호사의 호스피스 완화의료에 대한 교육 요구도가 3.5점 이상인 문항은 34개였다. 교육 요구도가 높았던 문항은 '죽음의 이해 ', '죽음에 대한 태도와 반응', '통증의 이해와 평가' 등이었고, 이전에 교육받은 경험이 많았던 문항은 '통증 및 증상완화', '호스피스의 윤리와 법', '호스피스 협력 및 홍보체계 구축'이었다. 3. 17개 내용을 강의, 토론 및 증례 등의 교육방법을 통하여 이론교육 48시간과 실습교육 30시간, 총 78시간의 기본 교육과정 을 구성하였다. 결론: 앞으로 개발된 교육과정으로 교육을 시행하여 효과를 평가하고, 호스피스 완화의료기관 실무자들의 교육 요구도를 정기적으로 파악하여 실무의 발전에 기여할 수 있는 표준 교육과정을 개발하는 것이 필요하다.
