Surgical Repair of Interruption of the Aortic Arch[Type A] -A Report of 5 Cases- (대동맥 결손증 (Type A) 의 외과적 치험)

  • Between 1981 and 1987, five patients with an interruption of the aortic arch were operated upon. All had a ventricular septal defect and a patent ductus arteriosus as associated anomalies. A two-stage procedure was employed in these cases, the initial procedure being repair of the interrupted arch, ligation of the patent ductus arteriosus, banding of the main pulmonary artery and a lung biopsy which was followed, 5 to 49 months later, by the repair of the ventricular septal defect. Four patients completed the two-stage procedure with one postoperative mortality. The remaining patient is yet to complete the second stage procedure.

Acoustic variations associated with congenital heart disease (선천성 심장병 환아에서 음향학적 요소의 변동)

  • Purpose : To investigate the nature of deviant voice physiology in preoperative children with congenital heart disease. Methods : Ninety-four children with congenital heart disease were enrolled. Their cries and related acoustic variables (fundamental frequency, duration, noise to harmonic ratio, jitter and shimmer) were analyzed using a multi-dimensional voice program. Results : The average fundamental frequency showed a significant decrease in patent ductus arteriosus, ventricular septal defect, and tetralogy of Fallot, except in atrial septal defect and pulmonary stenosis. The length of the analyzed sample (duration) did not show a significant difference when compared with the control group. There was a significant increase in jitter percent in ventricular septal defect, patent ductus arteriosus, and atrial septal defect. There was an increase in shimmer in ventricular septal defect, patent ductus arteriosus, and atrial septal defect. The noise-to-harmonic ratio increased in ventricular septal defect, patent ductus arteriosus, and atrial septal defect but there was no significant difference in pulmonary stenosis and tetralogy of Fallot. While analyzing acoustic variables, the voice change was significantly higher, especially in patent ductus arteriosus followed by ventricular septal defect and atrial septal defect. Most of these acoustic variables were deviant in left-to-right shunt lesions in congenital heart disease, especially in patent ductus artriosus. Conclusion : The results of the voice change analysis of preoperative children with congenital heart disease revealed that the acoustic variables differed by each congenital heart disease. Moreover, the acoustic variables were prominently deviant in congenital heart disease with left-to-right shunts.

  • Of several techniques for closure of the patent ductus arteriosus, the less invasive surgical ranscatheter closure without thoracotomy(the Porstmann's procedure) may have a place between the Rashkind procedure and thoracotomy. We describe here a 68 year old woman with calcified patent ductus arteriosus which was successfully closed by the Porstmann's procedure.

Closure of Patent Ductus Arteriosus in a Maltese Dog Using Amplatz Canine Duct Occluder through Femoral Vein

  • An 8 month-old male Maltese (weighing 2.0 kg) was referred with loud heart murmur at routine physical exam in local animal clinic. Electrocardiogram found left ventricular hypertrophy pattern (4.5 mV R-wave). Diagnostic imaging studies revealed the elongation of left ventricle (LV) with classic triple bumps on the main pulmonary artery, aorta and left atrium on the ventrodorsal view of radiograph. Echocardiography revealed patent ductus arteriosus (PDA) duct and continuous turbulent shunt flow (maximal velocity 4.83 m/s) between the aorta and pulmonary artery with left to right direction. The PDA in this dog was successfully closed through femoral vein (transvenous approach) using a 5 mm Amplatz$^{(R)}$ Canine Duct Occluder. To the best of author's knowledge, this is the first case of PDA occlusion treated with Amplatz Canine Duct Occluder through femoral vein.

Reverse Patent Ductus Arteriosus with an Aberrant Left Common Carotid Artery in a Shih Tzu Dog (시츄개에서 발생한 비정상적 좌경동맥 분지장애를 동반한 우-좌형 동맥관 개존증)

  • A 4-year-old intact male Shih Tzu dog (weighing 5 kg), was presented with signs of severe dyspnea, exercise intolerance and cyanosis. Diagnostic studies suggested a reverse patent ductus arteriosus (rPDA) with pneumonia. Despite emergency treatments with oxygen, diuretics, antibiotics and medical therapy for hyperkalemia, the dog died of ventricular fibrillation from hyperkalemia and dehydration. Necropsy revealed the rPDA with a marked descending aortic dilation and abnormally arisen left common carotid artery.

A Clinical Study on Postoperative Changes of Electrocardiographic Findings in Patients with PDA (동맥관개존증의 술후 심전도변화에 관한 임상적 연구)

  • At the Department of Thoracic and Cardiovascular Surgery, Hanyang university, from 1976 to 1984, 102 patients operated for isolated patent ductus arteriosus were studied. The correlation between the degree of left ventricular hypertrophy [LVH] and hemodynamic data, and postoperative changes of electrocardiographic findings were evaluated. The following results were obtained; [1] Of 102 patients with isolated patent ductus arteriosus, 78 patients [76.5%] were found to have LVH by EKG, preoperatively. [2] The patients with LVH had higher incidence of respiratory infection [79.4%] compared to patients without LVH. [3] Frequent physical findings in the patients with LVH were precordial bulging and P2 accentuation on auscultation. The typical continuous machinery murmur was not different in two group with or without LVH. [4] 66.1% of patients with LVH showed more than 56% of cardiothoracic ratio [CTR] on simple chest X-ray. 56.5% of patients without LVH showed less than 50% of CTR. [5] In the patients found to have LVH by EKG, the correlation between values of Svl+Rv6 and Qp/Qs in hemodynamic data was correlation coefficient 0.57 and between CTR and Qp/Qs was correlation coefficient 0.51. [6] In patent ductus arteriosus with LVH following surgery, the values of Svl+Rv6 was changed from preoperative 153.6$\pm$42.78% to 107.7$\pm$19.58% within 3 months and to 80.4$\pm$12.22%, which is within normal range of Svl+Rv6 on EKC, after 6 months.

Clip ligation for treatment of patent ductus arteriosus occlusion in three cats

  • Patent ductus arteriosus (PDA) is a rare congenital cardiovascular anomaly in cats. Due to their small body, intercostal thoracotomy is the most common option to close the PDA. However, few reports detail the surgical technique for ligating PDA in kittens. In this case report, three cats weighing 1.4 kg, 1.2 kg, and 2.9 kg were diagnosed PDA. Clip ligation via left fourth intercostal thoracotomy was performed and the cats were successfully treated. Postoperative echocardiography showed no residual flow in any of the cases. This case report highlights clip occlusion for small cats with PDA could be safe and effective.

Persistent right aortic arch with aberrant left subclavian artery originating from the patent ductus arteriosus in a dog: a case report

  • A 4-month-old intact male Sapsaree dog was referred due to a history of postprandial regurgitation following consumption of solid food. Thoracic radiography revealed focal leftward displacement of the thoracic trachea at T1 to T4 vertebrae levels. Barium contrast radiography revealed focal dilation of the cranial thoracic esophagus at the heart base level. Persistent right aortic arch (PRAA) with an aberrant left subclavian artery branching from the patent ductus arteriosus was diagnosed by computed tomography angiography (CTA). Although barium contrast radiography can presumptive diagnose PRAA, CTA should be considered for identifying additional vascular anomalies, specific types, and surgical planning.

  • The ductus arteriosus (Botallo's duct) is a vascular communication between the pulmonary artery near its bifurcation and the aorta distal to the origin of the left subclavian artery. A surgical operation was tried in the shepherd dog which was affected by patent ductus arteriosus. The patient was six months old and weighed about 11kg. Pentobarbital sodium, 26 mg/kg was used intravascularly for the anesthesia. And then a respirator was used for the patient. The thoracotomy incision was made over the fourth intercostal space on the left and the ductus arteriosus was ligated twice. But the patient revealed ventricular fibrillation at three minutes after ligations of ductus arteriosus. So the cardiac massage was done after incision of pericardium and then the defibrillator was used for the defibrillation. But the patient was not recovered without effect. It was considered that if there is complication or cyanosis, the adaptability of surgical operation should be specially considered. Histologically, chronic passive congestion of liver was observed.
