골반강 내 방사선 치료 환자에서 Electronic Portal Imaging Device(EPID)를 이용한 Portal Image의 유용성에 관한 연구 (Evaluation of Usefulness of Portal Image Using Electronic Portal Imaging Device (EPID) in the Patients Who Received Pelvic Radiation Therapy)

  • 목적 : matrix ion chamber type의 EPID와 video camera based EPID를 이용한 portal image와 기존의 film을 이용한 port film의 영상의 질을 객관적으로 비교 평가하여 EPID의 유용성을 알아보고자 본 연구를 계획하였다. 대상 및 방법 : 1997년 4월부터 10월까지의 인하대 병원과 세브란스 병원에서 방사선 치료를 받은 골반강내 치료 환자 각 10명씩을 대상으로 환자 1명 당 5-10회의 port film과 EPID를 이용한 portal image를 동시에 얻어 비교하였다. 환자의 나이는 32세에서 79세이었고 2명의 AP영상을 제외하고는 모두 PA영상을 얻었다. 환자의 두께는 17cm에서 20cm으로 비교적 균일하였다. beam energy는 10MV X-ray를 사용하였고 dose rate은 100-300MU/min으로 2-10MU을 주어 영상을 얻었다. port film은 Kodak diagnostic film을 사용하였고 film을 넣는 cassette는 납을 전후에 부착한 것을 이용하였다. source to detector(film) distance는 140cm으로 하였다. 영상의 판독은 4명의 치료방사선과 의사에 의해서 시행되었으며 pelvic brim, sacrum, acetabulum, iliopectineal line, symphysis, ischium, obturator foramen, sacroiliac joint를 각각 very clear(1), clear(2), visible(3), not clear(4), not visible(5) 다섯 단계로 나누어 점수를 주었다. 결과 : video camera based EPID를 이용하여 얻은 영상을 비교하여 보았을 때 film을 이용한 port film과 enhancement를 시행하지 않은 portal image는 각 해부학적 구조에서는 차이를 보이지 않았다. 그러나 portal image를 window level로 영상의 변화를 주었을 때는 sacrum과 obturator는 영상의 판독에 도움이 되었다. 또한 portal image를 CLAHE로 enhance를 하였을 때는 모든 해부학적 구조물의 판독이 film보다 용이한 것으로 나타났다. matrix ion chamber type의 EPID를 이용하여 얻은 영상에서도 역시 port film과 영상의 변화를 주지 않은 portal image간에는 커다란 차이를 보이지 않았으나, portal image를 window level로 변화를 주었을 경우는 portal film에 비하여 영상의 질이 더욱 좋아지을 알 수 있었다. 결론 : 방사선 치료를 받는 환자 중에서 골반강의 영상에서는 EPID의 영상의 질은 기존의 port film과 비교하여 차이가 없었으며, window level로 영상에 변화를 주거나 enhance를 하였을 경우는 port film보다 더 나은 영상을 얻을 수 있어 기존의 port film을 대체 할 수 있을 것으로 생각된다.

노인의 의미에 관한 청소년의 태도 유형 연구 - Q 방법론 적용 - (Study on the Adolescent′s Attitude Patterns toward the Meaning of Aging and the Elderly - Q-Methodology -)

  • The lengthened average span of human life by virtue of recent developments in medicine has caused the Population of elders to increase. The development of modern industrial society has transformed family structure from the large family system to that of a nuclear family. Due to the shift in family structure, the problem of support for the aged has surfaced as a nursing problem as well as a social problem. With regard to this problem, this study aims to investigate the adolescent's understanding of elders and aging. By identifying their understanding and classifying their attitude patterns, this study will help the nursing assessment of the support of elders in the family. This study employed Q-methodology and the research was conducted from December 1998 to May 1999. One method of the research included deep interviews with elders, those who are in their 50's. 40's or 30's. and the adolescent. 183 Q-Populations taken from literary works such as poems or novels were also formed as another method. Finally. 36 Q-cards were made after consultation with Professors of the nursing department. The subjects of the P- sample were 30 high sohoolboys/girls - who were in first, second, and third years. The result showed that 3 factors provided an explanation for 59.14% of the whole variables: the first factor, 41.37%; the second factor. 11.49%, and the third factor. 6.28%. These three factors were analyzed and categorized as three types. Twenty subjects out of the 30 were included in Type 1: Respecting Elders. The statements which showed the most positive consent were as follows: 'The declining age is a perfect time to prosper completing a worthy life' ; 'Getting old. one needs financial stability' and 'Elders wish the best for their children' The statements showing the most negative response were as follows: 'It is better to die than to live as an older person' ; 'Elders are insignificant' ; and 'Getting old is the worst unhappiness that tortures human.' Four subjects were included in Type 2. Resenting Elders. The statements which showed the most positive consent were as follows: 'Aging is a process of dying that nobody can escape from'. 'Elders should be concerned about his health and try to maintain their health' ; and 'When you set older. you regret about the life in the Past.' The statements showing the most negative response were as follows: 'When You get older. You should stand aloof greed and worldly things' 'When You got older, You become generous and gentle' ; and 'When You set he gets old. You change to become a comfortable and warm person.' Six out of 30 subject were included in Type 3 Caring Elders. The statements which showed the most positive consent were as follows: 'Elders should be concerned about his health and try to maintain their health' ; 'Elders wish the best for their children' ; and 'Elders deserve to be treated with filial respects.' The statements showing the most negative response were as follows 'Elders are insignificant' ; 'Elders have freedom and plenty of free time.' and 'Elders are alienated form and drove out of the society.' The above-mentioned results show that most adolescents in Korea recognize aging as the time of fruition and development: it is a time of benefiting and giving back to society. Aging can also be seen as a time of generosity and magnanimity and the time of respect and favorable treatment from society. despite the change of modern society and the ostensible transformation of a family system. Their recognition seems deeply rooted in the traditional confucian values and the dual family system which is Peculiar especially to the Korea - one which maintains both the superficial form of nuclear family and the substantial mode of the enlarged family system. In sum, many Korean adolescents attribute the meaning of the elderly and aging to the type of the respect with the elderly and the type of the elderly's caretaking.

동북(東北)아시아 유의 기원(起源)과 그 교류(交流)에 관(關)한 연구(硏究) -$4{\sim}8$세기(世紀)를 중심(中心)으로- (A study on the origination and Transmission of Yu in Northeast Asia. -from the 4th Century to the 8th Century-)

  • Yu was a type of dress worn on the upper part of the body which was commonly used in Northeast Asia. It was originally used by the Northern race for the need of courtesy as well as protecting cold. It was believed that Yu in Northeast Asia, which was called Kaftan, was came from Scythai lived in North Eurasian land around the Black sea. Scythians were the first-formed horse-riding race in the world and their civilization influenced those of far Asiatic sector along the steppe route. As their power expanded, their costume culture transmitted to the East(China, Korea, Japan). The upper garment, Yu, was characterized by the left-sided collars, narrow sleeves belted at the waist to the length of the hip line and the tight trouser on the lower part, which we commonly called HoBok(胡服) style. 1. Yu in Northeast Asia was originated from the Eurasians, Scythian Culture. Being exchanged, active style costumes were widely used among Chinese, Koreans and Japanese throughout centuries' including $4{\sim}8$ century. 2. Chinese Yu had a style of wide-sleeves and right-sided collars. The traditional costumes of Han race are consisted of wide-sleeved Yu on the upper and long-skirt on the lower part of the body. Before the adoptation of HoBok during reign of King Jo Mooryung in 307. B.C., HoBok style had already found in the remains since the Sang period. There were various names among Yu during the Han period. Seup, Sean Eui, Kye, Kyu were one of the styles and several names were meant for collar and sleeves. During $4{\sim}8$ centuries, clothes of right-sided collar were found, superior to that of left-sided and narrow sleeves were widely used both the royal and the humble. Various styles of decoration were seen in Yu around neck, back and sleeves comparing other nations. 3. Yu, in Korea, was typical style of Northern-bound HoBok. Both men and women had similarity in Yu style, narrow sleeves, left-sided collar, belted at the waist and to the length of hip line. Influenced by Han race, in the $4th{\sim}8th$ centuries, dual system of collar was found. But we cannot see major change in Yu and finally was connected to the present. 4. The original design of the Japanese costumes was not similar to that of Northern nomadic hunting race, which was suitable for horse-riding activities. Owing to the climates along the island, we could see various conditions ranging from the cold and to the warm. Influenced by the climates, pulling over the neck(Pancho style) were major design in Japan. As Korea was advanced earlier than Japan, Korean landed Japanese territory showing clothes. So primitive costumes had changes in style. During the $4th{\sim}8th$ period. The Korean mode was found in Haniwa (which was built to make sacrifices to the dead King) and costumes in Jeong Chang Won. Among the costumes in Jeong Chang Won, we put 3 or more costumes to the category of Yu characterizing elements of Korea and Tang period. From the $4th{\sim}8th$ century, China, Korea, Japan fell into the same cultural category, Scythai. Styles in Yu among three nations, we saw little differences, basically along times. Originated from the West Asia, Yu was transmitted to the far East changing Chinese costumes, Koreans melted it into the traditional elements and then influenced Japan.
