흉막에서 발생한 침습성 흉선종,정상 흉선을 가진 예 (Invasive Thymoma Originating from Right Pleura with Normal Thymus A case Report)

  • 42세 남자는 약 10년간 석면에 노출되는 직업에 종사하다가 그만둔지 5년 후, 진행하는 호흡곤란을 주소로 검사를 받던중 우측 늑막에 다양한 크기의 종괴를 보이는 암종으로 입원하였다. 환자는 부분 마취하에 늑막 조직 생검을 받고 침윤성 흉선종의 의심하에 우 전늑막 및 우전폐 절제술을 받았다. 수술중 흉선의 상태는 정상이었으며 폐 첨부로부터 횡격막부위까지의 늑막과 폐엽간, 부분적인 폐 침습을 보이고 심낭의 침윤을 보이는 Masaoka 분류 Stage 로 보이는 침윤성 흉선종으로 진단되 었다. 환자는 술후 600의 방사선 조사를받고 술후 8개월인 현재까지 재발의 징후 없이 경과관찰중이다.

보강토옹벽의 배부름현상 분석 및 보강 방법에 대한 실험적 연구 (A Study on Reinforcement Method of Reinforced Soil Retaining Wall Through Field Experiment)

  • 본 연구는 공용 중 보강토옹벽에 발생하는 배부름현상을 보강하기 위한 방법을 제안하고 현장실험으로 보강방법의 신뢰성을 파악하고자 하였다. 보강토옹벽의 배수시설을 미설치하여 강우에 의해 배부름현상이 유도되도록 하였으며, 시공 후 15일간 변위속도가 크게 발생한 후 점차적으로 감소하였다. 유도된 배부름현상의 변위는 미연방도로국(FHWA)의 시공 중 수평변위 기준을 적용하여 판단하였다. 배부름 현상을 보강하기 위해 보강토옹벽에 소일네일을 설치하고, 네일의 두부를 수평으로 서로 연결하여 보강토 옹벽의 블록들을 서로 지지할 수 있도록 하였다. 보강재들의 보호를 위해 숏크리트로 보강면을 마감하고 계측장치를 설치하였다. 배부름현상 보강방법의 신뢰성을 확인하기 위하여 변위의 진행을 검측한 결과 보강 후 수평변위는 거의 발생하지 않았으며, 보강방법의 신뢰성을 확인할 수 있었다.

농업기반정비사업의 재정정책변화에 따른 지방자치단체의 사업추진 전망 및 실태 분석 (An Analysis on the Prospects and Realities of Local Governments' Project Promotions according to the Changes in Financial Policy of Agricultural Infrastructure Improvement Projects)

  • This study attempted to investigate the changes in the perception of local government officials about agricultural infrastructure improvement projects due to the financial policy changes. A total of 141 local government officials were surveyed on the priority, the current status of the agricultural infrastructure improvement projects. Irrigation facilities renovation project and Small drainage improvement project had generally high priority, while Support project of cultivation basis for upland crops in paddy, Large scale land readjustment project, and Upland infrastructure improvement project had low priorities. After the introduction of the block grant system, agricultural infrastructure improvement projects had often been conducted by collecting opinions from local residents. The officials in charge of local governments recognized that the agricultural infrastructure improvement projects were reduced due to lack of financial resources and project potential sites. In response to the changes in the agricultural infrastructure improvement projects, most of the surveys showed that the influence of cities and provinces was strengthened. In addition, respondents most needed to improve the subsidy rate and there were a large number of cases requiring differential support to meet the characteristics of the local government. Therefore, it will be necessary to present a new perspective on the agricultural infrastructure improvement projects and to suggest differentiated projects for each regions.

하수관거 통수능 해석을 위한 Huff 모형과 ABM 법의 적용성 분석 (Applicability of Huff Model & ABM Method for Discharge Capacity of Sewer Pipe)

  • The sewer capacity design have been based on the Huff model or the rational equation in South Korea and often failed to determine optimal capacity, resulting in frequent urban flooding or over-sizing. A time distribution of rainfall (i.e., Huff or ABM method) could be used instead of a rainfall hyetograph obtained from statistical analysis of previous rainfalls. In this study, the Huff method and the ABM method, which predict the time distribution of rain intensity, which are widely used to calculate sewage pipe drainage capacity using the SWMM, were compared with the standard rainfall intensity hyetograph of Seoul. If the rainfall duration was 30 minutes to 180 minutes, the rainfall intensity value calculated by the Huff model tended to be less than the rainfall intensity value of the standard rainfall intensity in the initial 5-10 minutes. As a result, more than 10% to 30% of under-design would be made. In addition, the rainfall intensity value calculated by the Huff model from the section excluding the initial 5-10 minutes of rainfall to the rainfall duration was calculated larger than the value using the standard rainfall intensity equation, which would result in an over-design of 10% to 30%. In the case of a relatively long rainfall duration of 360 minutes (6 hours) to 1,440 minutes (24 hours), it showed an lower rainfall intensity of 60 to 90% in the early stages of rainfall, but the problem of under-design had been solved as the rainfall duration time had elapsed. On the other hand, in the alternating block method (ABM) method, it was found that the rainfall intensity at the entire period at each assumed rainfall duration accurately matched the standard rainfall intensity hyetograph of Seoul.

  • 선진국 진입의 길목에 있는 우리나라는 급경사면 축조 및 유지관리에 심혈을 기울이고 있으나, 급경사면 축조를 위해 철근콘크리트 옹벽이나 보강토 옹벽(블럭식, 판넬식) 등이 사용되어지고 있다. 이러한 구조체는 전면형태가 콘크리트 벽체로서 미관의 한계성과 친환경성이 부족한 상태이다. 본 연구는 녹화를 병행할 수 있는 식생토체를 형성하고 현장에서 발생된 불량토를 보강재(부직포) 감싸기(wrapping)방법을 적용한 $80^{\circ}$의 급경사 성토체를 현장에서 직접 축조하였으며, 보강성토체에서 발생될 수 있는 수평변위, 침하량 및 법면에서 발생된 토압 등에 대한 계측을 실시하여 분석하였다. 이러한 보강성토체의 계측 및 시간경과에 따른 법면의 식생활착 등에 대한 조사결과, 향후 녹화가 가능한 급경사 보강토 구조물에 대한 적용이 가능할 것으로 판단된다.

반응표면분석법을 이용한 제진기의 목메임 방지 개선 및 레이크 최적화 (A Study on the Optimization of Anti-Jamming Trash Screen with Rake using by Response Surface Method)

  • 제진기는 배수펌프장, 하수처리장, 발전소 등 유입 수로 전단에 설치되어 부유 협잡물의 유입을 차단하는 장치이다. 제진기의 스크린 사이의 협잡물이 끼는 목메임 현상과 노끈류가 제거되지 못하여 물의 유입로의 감소 또는 제진기 파손을 유발하는 문제점이 발생하고 있다. 본 논문에서는 목메임과 노끈류 끼임으로 인한 파손이 주로 발생하는 에이프런을 제거하고 스크린을 확장하여 대체하였다. 그리고 스크린 사이 공간에 내측 레이크를 추가하여 목메임과 노끈류를 제거하도록 개선하는 확장형 레이크를 사용하는 방식으로 설계하였다. 기존의 제진기 레이크의 허용응력을 만족하는 내측 레이크를 설계하기 위하여 레이크 수직 길이와 보강단면의 두께 값을 변수로 하여 실험계획법에 따라 실험점을 결정하고, 구조해석 툴인 ANSYS static structural module과 통계분석 툴인 R software를 이용하여 반응표면분석법에 따른 형상 최적화를 진행하였다. 결정된 최적 설계점 레이크 길이 210.2mm와 보강단면 두께 2mm에서 반응표면분석법 결과와 구조해석의 결과의 상대 오차는 1.63% 이다. 파일럿 제진기를 실제 크기로 제작하여 실증 실험을 수행하였으며 97%이상 목메임과 노끈류를 제거가 가능함을 확인하였다.

단심실 -III C Solitus 형의 수술치험- (Surgical Repair of Single Ventricle (Type III C solitus))

  • For years, physicians and anatomists have been interested in the heart that has one functioning ventricle. Various terms have been suggested for this entity including single ventricle, common ventricle, double-inlet left ventricle, cor biatriatum triloculare, and primitive ventricle. In this report, the term "single ventricle" is utilized as suggested by Van Praagh, and is defined as that congenital cardiac anomaly in which a common or separate atrioventricular valves open into a ventricular chamber from which both great arterial trunks emerge. An outlet chamber, or infundibulum, may or may not be present and give rise to the origin of either of the great arteries. This definition excludes the entity of mitral and tricuspid atresia. An 11 year old cyanotic boy was admitted chief complaints of exertional dyspnea and frequent upper respiratory infection since 2 weeks after birth. He was diagnosed as inoperable cyanotic congenital heart disease, and remained without any corrective treatment up to his age of 11 year when he suffered from aggravation of symptoms and signs of congestive heart failure for 2 months before this admission. On 22nd of May 1979, he was admitted for total corrective operation under the impression of tricuspid atresia suggested by a pediatrician. Physical check revealed deep cyanosis with finger and toe clubbing, and grade V systolic ejection murmur with single second heart sound was audible at the left 3rd intercostal space. Development was moderate in height [135 cm] and weight[28Kg]. Routine lab findings were normal except increased hemoglobin [21.1gm%], hematocrit [64 %], and left axis deviation with left ventricular hypertrophy on EKG. Cardiac catheterization and angiography revealed 1-transposition of aorta, pulmonic valvular stenosis, double inlet of a single ventricle with d-loop, and normal atriovisceral relationship [Type III C solitus according to the classification of Van Praagh]. At operation, longitudinal incision at the outflow tract of right ventricle in between the right coronary artery and its branch [LAD from RCA] revealed high far anterior aortic valve which had fibrous continuity with mitral annulus, and pulmonic valve was stenotic up to 4 mm in diameter positioned posterolaterally to the aorta. Ventricular septum was totally defective, and one markedly hypertrophied moderator band originated from crista supraventricularis was connected down to the imaginary septum of the ventricular cavity as a pseudoseptum of the ventricle. Size of the defect was 3X3 cm2 in total. Patch closure of the defect with a Teflon felt of 3.5 x 4 cm2 was done with interrupted multiple sutures after cut off of the moderator band, which was resutured to the artificial septum after reconstruction of the ventricular septum. Pulmonic valvotomy was done from 4 mm to 11 mm in diameter thru another pulmonary arteriotomy incision, and right ventriculotomy wound was closed reconstructing the right ventricular outflow tract with pericardial autograft of 3 x 4 cm2. Atrial septal defect of 2 cm in diameter was closed with 3-0 Erdeck suture, and atrial wall was sutured also when rectal temperature reached from 24`C to 35.5`C. Complete A-V block was managed with temporary external pacemaker with a pacing rate of 110/min. thru myocardial wire, and arterial blood pressure of 80/50 mmHg was maintained with Isuprel or Dopamine dripping under the CVP of 25-cm saline. Consciousness was recovered one hour after the operation when his blood pressure reached 100 /70 mmHg, but vital signs were not stable, and bleeding from the pericardial drainage and complete anuria were persisted until his heart could not capture the pacemaker impulse, and patient died of low output syndrome 320 min after the operation.

GIS를 이용한 도심지 Nonpoint Source 오염 물질의 평가연구 (Urban Nonpoint Source Pollution Assessment Using A Geographical Information System)

  • 지리 정보시스템(Geographical Information System)을 이용하여 도심지 Non Pount Source오염 물질의 양이 오염원 종류별로 확인되고 적절하한 오염감소를 위한 대책이 마련되었다. 경험에 의한 공해물질 예측모델을 운용하기 위한 모든 입력 자료들이 도심지의 거리 구획별(Street block)별로 제공되어 각 거리 구획별 오염량이 계산되었다. 계산된 오염량은 각 우수 배출구별로 합산되어 오염량이 많은 지역이 판명되었다. 또한 오염량을 줄이기 위하여 인공호수를 만들기 위한 적지분석이 수행되었으며, 그에 따른 비용분석이 이루어졌다. 본 연구는 지형정보시스템의 도심지 공해연구에의 기여도를 입증시켜 주었다.

강우 초기특성에 따른 비점오염부하량 산정을 위한 FFC2Q 모형 (FFC2Q Model for NPS Load Analysis according to Characteristics of Early Stage of Runoff)

  • 이 연구에서는 FFC2Q모형의 WQUAL블럭의 기본이론과 적용성을 검토하고, 도시지역의 강우초기 시 집중적으로 발생하는 CSOs의 비점오염부하 특성을 분석하였다. 또한 비점오염물 축적량 산정법과 쓸림방정식에서 주요 매개변수들의 선택과 그 영향을 분석하였다. 군자 배수구역의 3개 강우사상에 대한 실측치와의 비교를 통해 첨두유출량, 총유출량, 총부하량, 첨두농도 발생시간 등을 적정하게 모의할 수 있었으며, 입력자료의 단순화에도 불구하고 SWMM, MOUSE 등과 대등한 계산 결과를 보였다. 강우분포와 관련하여서는, 초기에 집중도가 높은 Huff1 분포에서 오염부하가 다른 분포에 비해 빠르게 발생하였으며, 강우초기 $BOD_5$, COD의 부하량 증가율은 SS에 비해 크게 나타났다. 비점오염부하량은 일정 강우 초기에 집중되어 결국 총부하량에 이르며, 이후 강우지속에 따른 유출은 비점오염부하량에 큰 영향을 미치지 못하는 것으로 분석되었다.
