본 연구의 목적은 토모다이렉트를 이용한 토모테라피 방사선치료에서 두상 각도(head tilting)에 따라 3차원 입체조형 전뇌 방사선치료 계획을 평가하고 자 하였다. 총 다섯 가지 두상 각도(-20°, -10°, 0°, +10°, +20°)를 비교 평가하였다. 두상 각도에 따라 표적에 대한 선량균질지수(homogeneity index, HI)와 처방선량지수(conformity index, CI)를 계산하였고, 정상 장기인 수정체, 안구, 귀밑샘에 대한 최대선량과 평균 선량을 구하였다. 선량균질지수와 처방선량지수는 두상 각도가 +10°와 +20°에서 수치 1에 가까웠다. 두상 각도 +10°에서 수정체와 안구의 선량은 기준 두상각도(0°)에 비해 각각 약 74%와 30% 감소하였다. 턱을 올린 상태의 +10°의 두상 각도는 표적에 대한 선량균일지수와 처방선량지수가 적합하며 렌즈와 안구의 선량을 줄일 수 있는 각도로써 권장한다.
The purpose of this study was to investigate the dose-volume indices and radiobiological indices according to the change in dose calculation grid size during the planning of nasopharyngeal cancer VMAT treatment. After performing the VMAT treatment plan using the 3.0 mm dose calculation grid size, dose calculation from 1.0 mm to 5.0 mm was performed repeatedly to obtain a dose volume histogram. The dose volume index and radiobiological index were evaluated using the obtained dose volume histogram. The smaller the dose calculation grid size, the smaller the mean dose for CTV and the larger the mean dose for PTV. For OAR of spinal cord, brain stem, lens and parotid gland, the mean dose did not show a significant difference according to the change in dose calculation grid size. The smaller the grid size, the higher the conformity of the dose distribution as the CI of the PTV increases. The CI and HI showed the best results at 3.0 mm. The smaller the dose calculation grid size, the higher the TCP of the PTV. The smaller the dose calculation grid size, the lower the NTCP of lens and parotid. As a result, when performing the nasopharynx cancer VMAT plan, it was found that the dose calculation grid size should be determined in consideration of dose volume index, radiobiological index, and dose calculation time. According to the results of various experiments, it was determined that it is desirable to apply a grid size of 2.0 - 3.0 mm.
The utilization of x-ray for diagnosis and examination is increasing by about 5-15% every year, therefore, it would be mandatory to protect the patients from exposures and so, studies in this field are performed even now. In dental field, the area of irradiation is limited any to the head and neck area, but the irradiated angle is varied following the objected tooth, so the adjacent structures lens and thyroid gland would be fragile to radiation. And the scattered radiation is one of the complicated problems in the protection because of specificity of dental x-ray and its object structures. The author, by using TLD (Thermo luminescent Dosimeter; Teledyne Isotopes-Model 7300, Element; TLD 200(CaF₂:Dy) and Capintec(Capintec Model 192, PM-30 Diagnostic chamber 28㎖ active volume), tried a measurement of air dose distribution of the scattered radiation and the irradiated dose of lens and thyroid gland under the condition of taking the film on the left maxillary molar. The results were as follows: 1. The half value layer of adapted dental x-ray machine was measured, and is 1.44㎜ Al. 2. The time of irradiation on the left maxillary molar in the Alderson Rando Phantom, the measured doses of left and right lens, and thyroid gland were 8,9mR, 1,2mR and 2,8mR. Under the same conditions, the scattered radiation at the distance of 1 meter from the phantom were 84 μR at the front side, 11μR at the back side, 18μR at the right side and 72μR at the left side. 3. Under the same conditions, the dose showed higher value by about 5% in the presence of object(phantom) than in the case of absence.
Diabetic cataract is a major complication of diabetes mellitus. Excess accumulation of sorbitol plays an important role in the pathogenesis of diabetic complications such as cataract formation. In this study, we investigated the inhibitory effect of the extract of the aerial parts of Aster koraiensis (AK) on diabetic cataractogenesis. To examine this further, we evaluated sorbitol accumulation during cataract development using streptozotocin-induced diabetic rat, an animal model of type 1 diabetes. Diabetic rats were treated orally with AK (100 mg/kg and 200 mg/kg body weight) once a day orally for 9 weeks. In vehicle-treated diabetic rats, lens opacity was increased, and lens fiber swelling and membrane rupture were observed. In addition, sorbitol accumulation in diabetic lens was markedly enhanced. However, AK treatment delayed the progression of diabetic cataract through the inhibition of sorbitol accumulation, and prevented lens fiber degeneration in a dose-dependent manner. These observations suggest that AK treatment can delay the progression of lens opacification in the diabetic rats during the early diabetic cataractogenesis.
목적: 전자빔(electron beam)을 이용한 HEMA(2-hydroxyethyl methacrylate)의 중합과정에 대한 최적의 전자빔조사(irradiation) 조건을 살펴보고, 전자빔과 일반적인 열중합 방법에 의해 제조된 콘택트렌즈의 물리적 특성을 각각 비교함으로써 콘택트렌즈 제조에 전자빔조사 방법의 활용가능성을 알아보고자 한다. 방법: 중합에 사용된 모노머(monomer)나 첨가제의 구성비 그리고 전자빔흡수선량(0~120 kGy)에 따라 HEMA의 중합정도를 관찰하여 전자빔조사(irradiation)에 의한 중합여부와 최적의 중합조건을 제시하였다. 동일한 반응물 구성비에 대해 전자빔과 열중합의 두 가지 다른 중합방법을 이용하여 고분자를 합성하였다. 각각의 고분자로부터 제조된 소프트콘택트렌즈에 대해 함수율, 산소전달률(Dk/t), 광투과율 등의 물리화학적 특성을 FT-IR 결과를 이용하여 비교 분석하였다. 결과: 전자빔조사선량(0~120 kGy)에 따라 살펴본 HEMA의 중합률은 100 kGy 이상에서 99% 이상으로 나타났으며, 사용된 모노머의 구성비나 광개시제와 가교제 등의 첨가에 무관하게 높은 중합률을 보였다. 전자빔 조사에 의해 제조된 렌즈의 함수율은 열중합 방법에 의해 제작된 렌즈에 비해 10% 이상 높게 나타났다. 산소전달률(Dk/t)도 함수율과 마찬가지로 전자빔조사방법에 의해 제조된 렌즈에서 더 높게 나타났으며, 순수한 HEMA의 경우는 약 2배 정도 높은 값을 보였다. FT-IR 분석결과, 전자빔 조사방법에 의해 제조한 렌즈에서 친수성 증가와 관계되는 OH group의 농도가 증가하였고 이에 따른 분자간 수소결합의 농도가 증가함을 확인하였다. 두 가지 다른 중합방법에 의해 제조된 렌즈의 가시광선(380~800 nm) 영역에서 광 투과율은 제조방법과는 상관없이 유사하였으며, 90% 이상의 높은 값을 나타내었다. 결론: HEMA를 기본으로 구성된 다양한 반응혼합물과 개시제나 가교제의 첨가가 없는 순수한 HEMA에 100kGy 이상의 전자빔을 조사할 경우 중합이 성공적으로 이루어졌다. 또한 순수한 HEMA에 100kGy의 전자빔을 조사하여 중합된 고분자로부터 제조된 콘택트렌즈에서 가장 높은 함수율과 산소전달률을 나타내어 전자빔조사조건에 따라 물리적특성이 다른 콘택트렌즈의 제조가 가능함을 확인하였다.

Purpose: We retrospectively reviewed the results of radiotherapy for localized ocular adnexal MALT lymphoma (OAML) to investigate the risk factors of cataract. Methods: Sixty-seven patients with stage IE OAML treated with radiotherapy at Seoul St. Mary's Hospital from 2001 to 2016 were included. Median treatment dose was 30 Gy. Lens protection was done in 52 (76%) patients. Radiation therapy (RT) extent was as follows: superficial (82.1%), tumor mass (4.5%), and entire orbital socket (13.4%). The risk factors for symptomatic cataract were analyzed using the Cox proportional hazard model. Results: Median follow-up time was 50.9 months (range, 1.9 to 149.4 months). All patients were alive at the time of analysis. There were 7 recurrences and there was no local recurrence. Median time to recurrence was 40.4 months. There were 14 cases of symptomatic cataract. Dose >30 Gy had hazard ratio of 3.47 for cataract (p = 0.026). Omitting lens protection showed hazard ratio of 4.10 (p = 0.008). Conclusions: RT achieves excellent local control of ocular MALT lymphoma. Consideration of RT-related factors such as lens protection and radiation dose at the stage of RT planning may reduce the risk of RT-induced cataract after radiotherapy.
Although the screening with a mammography has been shown to be economical, simple and effective in detecting breast cancer, it is accompanied by the risk from radiation. Therefore, this study analyzed the glandular dose and organ dose according to the target-filter combination and the presence and absence of implants using Monte Carlo simulation. The results indicate that at a tube voltage of 30 kV and a tube current of 50 mAs, the dose increased in the order of Mo/Mo. Mo/Rh, Rh/Rh and W/Rh in proportion to the atomic number of the target-filter. In addition, in phantom without implant a reduction in dose was seen when compared to the phantom with implant. The organ dose was highest in the lens except for the breast on the examination side regardless of the presence or absence of the implant. These results may contribute to use basic data for the diagnostic reference level of breast plastic surgery patients.
This study investigates the role of microbial extracellular polymeric substances (EPSs) as bioflocculants to harvest microalgae (water-microalgae separation). The EPS extracted from waste activated sludge (WAS) by heat extraction were fractionated into soluble EPS (S-EPS), loosely-bound EPS (LB-EPS) and tightly-bound EPS (TB-EPS) forms. All the EPSs facilitated the flocculation of microalgal cells from stable growth medium. Of those EPSs, the TB-EPS showed the highest flocculating activity (FA) resulting in the substantial decrease in the amount of EPS added in terms of total organic carbon (TOC) during flocculation. The FA of microalgae was improved with the increase in TB-EPS dose, however, excessive dose of TB-EPS adversely affected it due to destabilization. Both LB- and TB-EPS could be utilized for flocculating microalgae as a sustainable option to the existing chemical-based flocculants. In addition to the conventional assessments, the effectiveness of the two bioflocculants for floc forming was also confirmed using a novel assessment of lens-free shadow imaging technique (LSIT), which was firstly applied for the rapid and quantitative assessment of microalgal flocculation.
플라스틱 섬광체와 상용 50 mm, f1.8 렌즈 및 고감도 CMOS 카메라를 사용하여 방사선치료 시 흡수선량을 측정할 수 있는 광 도시메트리 시스템을 구축하였다. 아울러 촬영된 방사선 분포 영상에 대한 비네팅 보정, 기하학적 왜곡 보정, 스케일 보정을 통하여 화소값으로 선량을 교정하는 절차를 확립하였다. 개발된 광 도시메트리 시스템을 6 MV 의료용 선형가속기에 대하여 선량 특성 평가를 수행한 결과, 심부선량백분율은 이온챔버로 측정한 결과에 비하여 빌드 업 깊이 이상에서는 오차 범위 2% 이내로 일치하였으며, 90% 조사야에 대하여 2.8%의 평탄도가 측정됨에 따라 방사선치료선량 측정 시스템으로서의 충분한 활용가능성을 확인하였다.
목 적: 상이한 치료장비를 이용하여 동일 부위에 중복하여 방사선치료 시 전산화치료계획장치간에 호환이 되지 않아 정상조직(Normal Tissue)의 총 흡수선량에 대한 평가의 한계점이 있다. 본 연구에서는 토모테라피(Tomotherapy)와 선형가속기(Linear accelerator)를 이용하여 동일 부위를 중복 치료할 때 환자가 받는 선량을 평가하고자 한다. 대상 및 방법: 인체모형팬텀(Anthropomorphic Phantom)을 대상으로 종양 및 정상조직을 묘사하여 선형가속기 치료계획장치 (Pinnacle 8.0: RTP)로 종양에 45 Gy 선량을 처방하고, 토모테라피 치료계획장치에서 종양에 15 Gy 선량을 처방하여 치료계획을 수립하였다. 토모테라피에서 수립된 치료계획을 통합가능 치료계획장치(Oncentra: RTP)에 전송한 후 동일한 조건으로 토모테라피 치료계획을 재현한 후 선형가속기에서 치료한 45 Gy를 합하여 총 선량 60 Gy의 치료계획을 구현하였다. 흡수선량 평가를 위해 두 개의 방사선 치료계획장치에서 얻어진 흡수선량(최소, 최대, 평균 선량) 및 관심 선량체적에 대해 서로 합(Total)한 값과 하나로 통합(Integration)하여 얻어진 값을 비교 분석하였다. 이를 바탕으로 본원에서 치료받은 환자 중 선형가속기와 토모테라피로 동일 부위에 치료 받은 5명(두경부 2명, 복부 1명, 골반부 2명)에 대하여 동일한 방법으로 종양 및 정상 조직의 흡수선량을 비교 분석하였다. 결 과: 인체모형팬텀에서 하나로 통합하여 얻어진 값과 서로 합한 값의 비교에서 최소선량은 비장(Spleen, 12.4%), 최대선량은 소장(Small bowl, 10.2%)과 척수(Spinal cord, 5.8%)에서 큰 차이를 나타냈다. 두경부 환자의 경우 최소선량은 구강(Oral cavity, 20.3%), 오른쪽 수정체(Rt lens, 7.7%)에서 큰 차이를 나타냈으며, 최대선량은 척수(22.5%), 뇌간(Brain stem; 12.0%), 시 신경교차(Optic chiasm; 8.9%), 오른쪽 수정체(11.5%), 하악골(Mandible bone, 8.1%), 뇌하수체(Pituitary gland, 6.2%)에서 뚜렷한 차이가 나타났다. 복부 환자의 경우 최대선량은 왼쪽 신장(Lt kidney, 20.3%), 위(Stomach, 8.1%)에서 큰 차이가 나타났고, 골반 부위 환자의 경우 최소 선량은 방광(Bladder, 15.2%), 최대선량은 소장(5.6%), 방광(5.5%)에서 큰 차이를 나타냈다. 또한 동일 부위 방사선치료계획 시 정량화되지 못했던 신장의 20 Gy를 받은 체적($V_{20}$)에서 37%, 간(Liver)의 25 Gy를 받은 체적($V_{25}$)에서 23%가 선량체적히스토그램(DVH)에서 나타났다. 결 론: 하나로 통합하여 얻어진 값과 서로 합한 값의 비교에서 최소선량은 높게, 최대선량은 낮게, 평균선량은 동일한 값으로 나타났다. 또한 관심장기의 선량 체적($V_{20}$)보다 낮은 선량을 처방했을 때 구현하지 못했던 종양 및 정상조직의 흡수선량에 대하여 평가를 할 수 있었다. 향후 상이한 치료 장치들을 이용한 동일 부위 치료 시 흡수선량의 통합 평가뿐만 아닌 정확한 선량분포를 구현할 수 있는 치료계획장치의 개발이 요구되어지며, 이에 관련된 연구가 진행되어져야 한다.
