Extramedullary plasmacytoma is rare tumor of plasma cell tumor, which involve soft tissue without any signs of systemic spread and occurs predominantly in the head and neck, especially the nasal cavity and the paranasal sinuses. Ten to twenty percent of extramedullary plasmacytoma have regional lymph metastasis at the time of diagnosis and approximately one third of patients with extramedullary plasmacytoma will subsequently develop multiple myeloma during long-term follow-up. Primary treatment modality is moderate-dose radiation and surgery is rarely used. Extramedullary plasmacytoma has good prognosis, but requires long-term systemic evaluation and follow-up. We report a case of extramedullary plasmacytoma of the sinonasal cavity with a review of literature.

This experiment was performed to study the effect of TSH (thyroid-stimulating hormone) on the expression of endoplasmic reticulum (ER) chaperones in the rat thyrocytes FRTL-5 cells. Although the expressions of ER membrane kinases (ATF6, IRE1 and PERK) were specially enhanced under absence of TSH, no remarkable up- or down regulations of ER chaperones (BiP, CHOP and Calnexin) were detected by TSH. We firstly report here that TSH by dose up-regulated expression of ER membrane kinases in FRTL-5 culture thyrocytes.

Endosulfan, one of organochlorine insecticides, is $\gamma$-aminobutyric acid (GABA) antagonist. In sufficient dose, this pesticide lower the seizure threshold and produce CNS stimulation, with resultant seizures, respiratory failure, and death. In patients with endosulfan intoxication, the first manifestation of toxicity is largely a generalized seizure without prodromal signs or symptoms. So the management of airway and seizure control are essential for survival and prognosis of intoxicated patients. We report two cases of acute endosulfan poisoning who manifest 'status epilepticus' similarly, but have different prognosis.

The anaphylactic allergic reaction is involved in many allergic diseases such as asthma and allergic rhinitis. In this report, I investigated the effect of ripe fruits of Rubus coreanus Miq. (Rosaceae) (RFRC) on the anaphylactic allergic reaction and studied its possible mechanisms of action. RFRC inhibited compound 48/80-induced systemic anaphylactic reaction in mice. RFRC dose-dependently decreased the IgE-mediated passive cutaneous anaphylaxis. In addition, RFRC decreased the gene expression and production of tumor necrosis factor-$\alpha$ (TNF-$\alpha$) and interleukin (IL)-6 in phorbol 12-myristate 13-acetate (PMA) plus calcium ionophore A23187 (A23187)-stimulated human mast cells. These findings provide evidence that RFRC could be a candidate as an anti-allergic agent.

A case of mediastinal seminoma is presented. A male driver, 27 years old Korean, has been suffered from substernal pain, cough and moderate swallowing difficulty since 5 months prior to this admission. At the time of onset, he visited at a local clinic to find some mass in his anterior mediastinum on chest P-A and lateral X-ray check. Recently, intermittent hiccups with much aggravated dysphagia forced him to visit our hospital, and admitted for radical resection under the impression of anterior superior mediastinal tumor of thymus origin. Median sternotomy was done and total resection of the tumor of 8.0X11.0X3.5cm was done without any specific complication and biopsy of the tumor revealed as primary mediastinal seminoma of the mediastinum. Supplementary prophylactic irradiation therapy was done with a tumor dose of 4,000 rad in 4 weeks after operation. Postoperative hospital course was uneventful and patient was joyful with his occupation for 3 months after discharge.

The CYP1A gene is one of Cytochrome P450 drug-metabolizing enzymes with dose-dependant manner of gene expression and is useful to get the information of alterations on gene expression upon environmental contaminants as well as the biomarker of environmental contamination at the specific places. In this report, we further discuss the usefulness of CYP1A gene in relation to aquatic environmental contamination at several aspects.

The piriformis syndrome is a condition allegedly attributable to compression of the sciatic nerve by the piriformis muscle. Recently, magnetic resonance neurography and electrophysiologic study have helped to diagnose piriformis syndrome. High dose radiotherapy could induce acute and delayed muscle damage. We had experienced piriformis syndrome with fatty atrophy of piriformis muscle after radiotherapy for recurrent cervical cancer.

Chlorination has been the most common antifouling method, but alternatives are under searching. In this article, we report how the hydrogen peroxide could enhance the effect of chlorination to prevent fouling by inhibiting larvae settlement and abatement of mussel colonization or by extinct of them; through marine mussel Mytilus edulis. The addition of hydrogen peroxide shows synergic effect on the veliger larvae (up to 19 folds) and effectively reduces required time of mussel mortality by 8-22%. For resolution of micro- and macro-fouling caused by the marine mussel, as well as diminishing of time and conventional chlorine dose could be important factor in favour of environment and economics.

Single phase, narrow beam X-ray attenuation data were obtained using various construction materials concrete, white block, red block, 3 hole block, gypsum board, artificial marble, cement, plate glass, wood, and lead. Tube voltages of 60, 80, 100, 120 kVp were employed and the resulting curves were compared to transmission data found in this report. The shielding methodology and the derivation of equations used for determination of barrier requirements were presented in NCRP 49. We could calculate the X-ray exposed dose after attenuation and thickness of protection barrier in the clinic facilities accordingly. For the purpose of maximizing the benefit/cost ratio to diagnostic shielding, various construction materials must be installed carefully and attnuation rate considered thoroughly.

We report that the crack-free organic-inorganic hybrid silica thin films were fabricated by sol-gel process using organometallic compounds as a precursor and that we have established very reproducible fabrication condition with systematic investigation of thickness and refractive index variations for various control parameters, such as, coating type, coating speed, chemical composition, prebake and postbake temperature. Additionally, we measured and compared the change of optical property with the UV exposure dose for three different kinds of photoinitiators. Furthermore, the fabrication of Ix4 MMI optical power splitter using the sol-gel thin film provides the possibility of various applications to the optical waveguide devices. vices.
