Clinical Comparison between Paclitaxel Liposome (Lipusu®) and Paclitaxel for Treatment of Patients with Metastatic Gastric Cancer

  • Aim: To compare the efficacy and safety of paclitaxel liposome (Lipusu$^{(R)}$) with paclitaxel in combination with tegafur and oxaliplatin in treating patients with advanced gastric cancer. Materials and Methods: Patients with advanced gastric cancer receiving chemotherapy were retrospectively collected, and divided into two groups. Patients in group A received paclitaxel liposomes at a dose of 135 $mg/m^2$ on day 1 of each cycle, and patients in group B were given paclitaxel at the same dose with the same timing. All patients received tegafur at a dose of 500 $mg/m^2$ on days 1-5, and oxaliplatin at a dose of 80-100 $mg/m^2$ on day 1 for 2 cycles (each cycle was 21 d in total). Results: Fifty-eight patients could be evaluated for efficacy. The overall response rate was 47% in group A (14/30), and 46% in group B (13/28). Disease control rate was 73% in group A (22/30), and 71% in group B (20/28) (P>0.05). No significant differences were detected in hematologic and neurologic toxicities between the two groups (P>0.05). However, nausea, vomiting and hypersensitive reactions were significantly lower in group A than in group B (P<0.05). Conclusion: Paclitaxel liposomes are as effective as paclitaxel when combined with tegafur and oxaliplation in treating patients with advanced gastric cancer, but adverse reactions with paclitaxel liposomes are less common.

Prognostic factors in breast cancer with extracranial oligometastases and the appropriate role of radiation therapy

  • Purpose: To identify prognostic factors for disease progression and survival of patients with extracranial oligometastatic breast cancer (EOMBC), and to investigate the role of radiation therapy (RT) for metastatic lesions. Materials and Methods: We retrospectively reviewed the medical records of 50 patients who had been diagnosed with EOMBC following standard treatment for primary breast cancer initially, and received RT for metastatic lesions, with or without other systemic therapy between January 2004 and December 2008. EOMBC was defined as breast cancer with five or less metastases involving any organs except the brain. All patients had bone metastasis (BM) and seven patients had pulmonary, hepatic, or lymph node metastasis. Median RT dose applied to metastatic lesions was 30 Gy (range, 20 to 60 Gy). Results: The 5-year tumor local control (LC) and 3-year distant progression-free survival (DPFS) rate were 66.1% and 36.8%, respectively. High RT dose (${\geq}50Gy_{10}$) was significantly associated with improved LC. The 5-year overall survival (OS) rate was 49%. Positive hormone receptor status, pathologic nodal stage of primary cancer, solitary BM, and whole-lesion RT (WLRT), defined as RT whose field encompassed entire extent of disease, were associated with better survival. On analysis for subgroup of solitary BM, high RT dose was significantly associated with improved LC and DPFS, shorter metastasis-to-RT interval (${\leq}1month$) with improved DPFS, and WLRT with improved DPFS and OS, respectively. Conclusion: High-dose RT in solitary BM status and WLRT have the potential to improve the progression-free survival and OS of patients with EOMBC.

Experimental Study of CheonghunHwadam-tang on the Cerebral Blood Flow in Rats (청훈화담탕이 국소뇌혈류량에 미치는 실험적 연구)

  • CheonghunHwadam-tang(CHT) have been used in oriental medicine for many centuries as a therapeutic agent of vertigo by wind, fire and phlegm. CheonghunHwadam-tangGamypang(CHTG) was CHT adding Aurantii Fructus(AF), Gastrodae Rhizoma(GR). The effects of CHT on the cerebral blood flow and cardiacvascular system is not known. The purpose of this Study was to investigate effects of CHT and CHTG on the regional cerebral blood flow(rCBF), mean arterial blood pressure(BP), cardiac muscle contractile force(CMF), heart rate(HR). The changes of cerebral blood flow and movement of cardiacvascular system(BP, CMF, HR) was determinated by Laser-Doppler Rowmetry(LDF). The results were as follows; 1. CHT extract increased rCBF, but decreased BP, HR in a dose-dependent manner. 2. AF extract accelerated rCBF and movement of cardiacvascular system in a dose-dependent manner. 3. GR extract increased significantly rCBF(10.0mg/kg, p<0.05) as well as accelerated BP and rCBF in a dose-dependent manner. 4. CHTG extract increased significantly rCBF(10.0mg/kg, p<0.01) in a dose-dependent, but was not changed movement of cardiacvascular system. This results suggest that CHTG is significantly treated more than CHT in vertigo by wind, fire and phlegm.

Evaluation of antitumor. hepatoprotective and antimutagenic potentials of Phellinus gilvus (Phellinus gilvus의 항암활성, 간보호 및 항돌연변이성에 대한 평가)

  • This study was carried out to evaluate the antitumor, hepatoprotective and antimutagenic activities on hot water extract of Phellinus gilvus (PGE). Growth of tumor in mice that were orally given $0.25,0.5,1.0,2.0g\;kg^{-1}$ dose of PGE was inhibited in a dose-dependent manner (p < 0.05). The hepatoprotective effect of PGE in the carbon tetrachloride ($CCl_4$)-intoxicated rats was studied. In $CCl_4$ + PGE group, PGE was orally administered with 100 mg/kg/day dose 7 days before the treatment of $CCl_4$. The serum activity of aspartate aminotransferase and alanine aminotransferase in $CCl_4$ + PGE group were decreased at a rate of 59.6% and 54.1% compared with those in $CCl_4$ group, respectively (p < 0.05). Also, total cholesterol and triglyceride in $CCl_4$ + PGE group were significantly decreased at a rate of 90% and 73.6% compared with those in $CCl_4$ group (p < 0.05). In the Ames test, we confirmed PGE doesn't have any activity as a mutant, and PGE showed inhibitory effect against mutagenesis induced by 2-amino fluride and sodium azide in Salmonella typhimurium TA98, TA100 and TA1535 in a dose-dependent manner. From the above results, we may suggest that PGE might have useful as a material for functional food and/or animal pharmaceutics.

Effects of Prostacyclin [PGI2] on Myocardial Protection in the Isolating Working Heart Model (적출활동심장에서 Prostacyclin [PGI2]의 심근보호효과)

  • The effect of prostacyclin[PGI, ] on myocardial preservation during global ischemia was studied in the isolating working rabbit heart model. Forty hearts underwent a 15 minute period of retrograde nonworking perfusion with Krebs-Henseleit buffer solution [37*C] and were switched over to the working mode for 15 minutes. After baseline measurement of heart rate, peak aortic pressure, aortic flow, and coronary flow, all hearts were subjected to 60 minutes of ischemic arrest at 10*C induced with St. Thomas Hospital cardioplegic solution: Group I had single dose cardioplegia, Croup II double dose, Croup III oxygenated double dose, and Group IV single dose with PCI, infusion [10ng/min./gm heart weight]. Hearts were then revived with 15 minute period of nonworking reperfusion at normothermia, followed by 30 minutes of working perfusion. Repeat measurements of cardiac function were obtained and expressed as a percent of the preischemic baseline values. Oxygen content of arterial perfusate and coronary effluent was measured by designed time interval. Leakage of creatine kinase was determined during post-ischemic reperfusion period. Finally wet hearts were weighed and placed in 120*C oven for 36 hours for measurement of dry weight. In the PGI, treated group [IV], heart rate increased consistently throughout the period of reperfusion from 100*5.0% [p<0.001] to 107*6.2% [p<0.001]. The percent recovery of aortic flow showed 95*5.7% [p<0.001] at the first 3 minute and full recovery through the subsequent time. Coronary flow was augmented significantly in the 3 minute [96*6.2%, p<0.001] and then sustained above baseline values. Among the Croup I, II, and III, all hemodynamic values were significantly below preischemic levels. PGI2 relatively increased oxygen delivery [1.22*0.19ml/min, p<0.001] and myocardial oxygen consumption [0.90*0.13ml/min, p<0.001] during reperfusion period. Leakage of creatine kinase in the PGI2 group was 9.3*1.58IU/15min [p<0.001]. This was significantly lower than Group I [33.0*2.68 IU/15min]. The water content of PCI2 treated hearts [81*0.9%, p<0.001] was also lower than the other groups.

Effects of the Mixture of Morphine, Bupivacaine and Antidepressants by Continuous Epidural Infusion in Cancer and Non-cancer Pain (모르핀, 국마제 및 항우울제의 경막외강내 지속혼주의 효과)

  • This study was objected to evaluate clinical progressions about both the degree of pain relief and the occurrence of morphine tolerance while the epidural analgesia with low dose of morphine, bupivacaine and antidepressant continued repeatedly at every 5 day intervals of the constant-rate infusion(0.5 ml/hr, 60 ml capacity). The subjects were divided to 56 cancer and 36 non-cancer patients who failed to respond to palliative treatments. Before the relief of pain, the pain severity was moderate(10%) and severe(90%). The dose escalation of morphine noted to 11(20%)patients in cancer pain and to one(5%) case only in non-cancer. During the epidural analgesia, the effect of pain relief was moderate(11%) and good(89%). It suggest that the morphine tolerance may be reduced to some degree such as an initial minimum dose of epidural morphine with local anesthetic and antidepressant should be adjusted on an individual basis using the constant-rate infusor, even though rapid dose escalation occurrs in some patients who the diseases progress over a short period of time.

The Effects of Panax ginseng and P. quinquefolium on Hemodynamics and Body Temperature in Healthy Young Men (III)

  • The current study was performed to observe the effects of Panax ginseng (PG) and P. quinquefolia (PQ) on hemodynamics such as blood flow rate (BF), blood flow velocity (BV), heart rate (HR), systolic blood pressure (SBP) and diastolic blood pressure (DBP, and body temperature (BT) in healthy young men. After testing equality of variance, Student's t-test using PROC TTEST was examined to. prove statistical differences between control and ginseng conditions at each time point. It was found that the BF data were fluctuated by personal deviation. In order to minimize the deviation, the results obtained for 6 hrs were reconstituted after dividing them into two periods of the first half from 1 to 3 hrs and of the latter half from 3.5 to 6 hrs. And then the reconstitution data and dose-response curves were obtained. Blood flow such as BF and BV shows significant increases both two periods in the dose of PG 2.25 and PG 9.0, whereas significant decrease in the dose of PG 4.5. However, in the PQ groups, the middle dose PQ 4.5 shows the highest significant increase among the three doses. Except for PG 2.25 in HR, other doses show significant decreases both in the first half and latter half. SBP of PQ 9.0 shows only a significant decrease in the first half; on the other hand, in the latter half, PG 4.5, PG 9.0 and PQ 9.0 significantly increase SBP. In addition, DBP of PG 2.25 and PG 4.5 show significant increase in the both periods. In the BT, PQ groups show gradual decrease from PQ 2.25 to PQ 9.0; however, PG groups show differently. PG 4.5 shows significant decrease, but PG 9.0 shows a increase without statistical meanings. In summary, PG is more effective in respect to keeping homeostasis of hemodynamics.

Transmission Dose Measurement of Gamma-ray Using Tungsten Shield (텅스텐 차폐체의 감마선 투과선량 측정)

  • This study was conducted to investigate the penetration dose and shielding rates of tungsten shields used in apron material by changing the type of source used in the nuclear medicine department, thickness of shielding material and distance between the source and detector. For the experiment, the source, shield, and detector were arranged in a straight line and measured with an inspector at a height of 100 cm. The highest shielding effect of tungsten was measured for $^{201}Tl$, while $^{123}I$ showed a higher shielding effect than $^{99m}Tc$. For the sources used in the experiment, the penetration dose decreased with distance and the shielding rate was measured with thicker thickness. However, the shielding rate of $^{13}1I$ and $^{18}F$ sources was found to be lower than when there was no shielding at 0.25 mmPb shield. Therefore, even if the radiation shielding effect of tungsten is high, considering the characteristics according to the type of source and the thickness of the shielding material, it may be helpful to reduce the exposure.

Influence of the Low Dose ${\gamma}$-ray Radiation on the Old Seed Germination and Growth of Chinese Cabbage (묵은 배추종자의 발아와 생육에 미치는 저선량 ${\gamma}$선 효과)

  • To investigate the stimulating effects of the low- dose radiation, the seeds of chinese cabbages (Brassica campestris L. cv. Hanyoreum and cv. Tropic emperor) were irradiated at the dose of 0.5 Gy~20 Gy with the ${\gamma}$-ray radition. In case of ‘Hanyoreum’cultivar, there were no significant promoting effects on the germination rate except the 0.5 Gy irradiated group. The increase in seedling height and fresh weight in 8 Gy and 12 Gy irradiated groups was 20% and 40%, respectively, which suggested the stimulating effect of the low-dose radiation on the early growth. In case of‘Tropic emperor’cultivar, the germination rate of 1 Gy irradiated group increased approximately 10% and the seedling height and fresh weight of 1 Gy and 12 Gy irradiated group in the greenhouse 20% on the average, and plant height and fresh weight of 0.5 Gy and 4 Gy irradiated group in the field increased 10% and 20% on the average, respectively.

  • Digital Mammography is very important for the early diagnosis of breast cancer. but excessive radiation dose to the patient may increase the probability of occurrence of breast cancer. absorbed dose rate and signal, noise, and contrast in accordance with the change of the filter, and analyzed the signal-to-noise ratio. absorbed dose rate GE Equipment is higher than that of Siemens and Hologic. using the material of Mo / Mo was lower than the other target-filter combination. SNR of Siemens equipment is high and measured high when Mo / Mo selected, the other two devices was measured high when Mo / Rh selected. when you choose a target-filter combination our results will be a good indicator.
