  • 인터넷과 같은 정보기술의 눈부신 발전으로 인해 환자들의 보건의료정보에 대한 접근성이 매우 향상되었다. 이는 곧 환자들이 당면하고 있는 보건의료정보의 비대칭 문제, 즉 의료서비스 공급자에 비해 적은 양의 정보를 보유함으로 인해 의료서비스 소비에 있어 그 효율성이 침해될 수 있다는 기존의 주장이 갖고 있는 타당성에 한계가 있음을 지적할 수 있는 변화라고 할 수 있다. 따라서 이러한 변화를 감안해 볼 때, 환자들의 정보비대칭 문제에 있어 핵심적인 사항은 환자들이 획득한 많은 의료정보를 그들의 합리적인 의료서비스 소비를 위해 어떻게 활용하느냐 하는 것으로 정리할 수 있다. 이러한 맥락에서 의료서비스 공급자인 의사가 환자들과의 효과적인 의사교환을 통해 보건의료정보에 대한 환자들의 이해와 효율적인 활용을 도와주고자 노력하는 것은 환자의 의료서비스 소비의 효율성을 증진함에 있어 매우 중요한 역할을 담당할 것으로 생각할 수 있다. 이에 본 논문은 의사-환자 간 의료서비스 소비에 대한 이론모형 구축을 통해 우선 환자의 비효율적인 의료서비스 이용이 그들의 정보 문제, 즉 의료서비스의 치료효과에 대한 오해에서 비롯됨을 밝혔으며, 두 번째로 의사가 이러한 환자의 정보문제를 해결하기 위해 충분한 노력을 투입했을 때 환자의 의료서비스 소비에 있어 파레토 효율성이 증대됨을 보였다. 아울러 정책적 관점에서 의사들로 하여금 환자를 상대로 한 충분한 의사교환을 유도하기 위한 정책방안으로 의사의 지불보상체계가 잘 작동함을 보였다. 즉, 환자의 진료비에 대한 의사의 책임 부분을 증가시킴으로 인해 의사의 진료상의 노력이 충분히 증가함을 이론적으로 입증할 수 있었다.

  • Objectives : This study aimed for attain to ideal aspects in medical ethics-clinical medicine, through researching doctor's ethics based on Confucian ideas, Methods : Research materials are datum of Confucian ideas included eastern medical books and the Confucian old books, Stand on above-mentioned, I investigate indispensable ethical condition clinical doctor must have in diagnose and treat. Results and Conclusions : 1. Combination of people-oriented ideas-Confucian and applicative learning-Eastern Medicine, doctor's principle mind is initiated and Confucian physician is the center of that. 2. For practice morality and ethics using the Confucianist's good character, Eastern medical doctor read and acquire the Confucian books. 3. Eastern medical doctor make an effort for application a kind love ideas(perfect religious scholarship ideas of Confucian), serve parents and country as for practice and sublimate into a kind love ideas through practice of integrity and the self-sacrifice. 4. Occasion to examine the female patient, emphasize good manners by Confucian ideas's sexual distinction 5. According to the doctor-doctor ethics, partnership is important due to Confucian faith and good manners 6. Confucian physician often effected by the people around patient and the protector of patient and criticize positive influence by these kind of peoples(傍人) when diagnose and treat. 7. Owing to Confucian's a notion of preferring a son to a daughter(男兒選好思想), come out the methods of convert a son into a daughter(轉女爲男法)-manipulation of sex distinction. this method is criticized aspect of medical ethics.

The Impact of Healthcare Provider Characteristics in Telemedicine App Services

  • Purpose: his study aims to explore how healthcare provider service characteristics in telemedicine services, which have become more common since the pandemic, affect rapport formation and service satisfaction with healthcare providers. Research design, data and methodology: A group of actual telemedicine users underwent data collection and empirical analysis. After analyzing reliability and validity, hypotheses were tested using a structural equation model. Results: Key perceived attributes of healthcare providers in telemedicine services were identified as doctor effort, doctor listening, and doctor expertise. Each of these variables had a significant positive impact on trust in telemedicine. Moreover, these attributes significantly positively impacted rapport formation and user service satisfaction, which was mediated by trust. However, the direct impact of rapport formation on service satisfaction was not supported. Conclusions: The study's findings have academic and practical implications for expanding telemedicine services. As an initial empirical study on telemedicine services, it confirms the importance of trust and rapport formation even in non-face-to-face medical situations. In order to overcome the limitations of non-physical contact, telemedicine services should strive to develop UI/UX designs that are more interoperable and boost trust in service apps.

급성일산화탄소 중독 후 발생한 중독정신병환자 1례에 대한 임상적 고찰 (A Psychotherapy of Oriental Medicine -Focus on Psychoanalysis-)

  • Objectives : In order to make a new psychotherapeutic model in the theory of oriental medicine by means of psychoanalysis method. Methods : The research was done by comparing the contents of the Tong-Ui Po-Kam, techniques of Yang Saeng(養生術), with the psychoanalysis Results : 1. There's no difference in the mind state which a doctor has to have between the oriental doctor and the occidental doctor. You should make effort not to project yourself on a patient. A conception of 'doctor' in oriental medicine is one who trains one's mind rather than just healer. 2. Heart(心) in oriental medical conception is revealed by circulation structure of Jeong(精), Gi(氣), and Sin(神). A healthy mind comes from harmony and smooth transport of Jeong(精), Gi(氣), and Sin(神). 3. The conception similar to unconsciousness doesn't exist in oriental medicine, but the contents about the attitudes or the methods of training one’s mind correspond to unconsciousness. 4. The psychotherapy of oriental medicine emphasized mutual dynamics between emotions at present, and presented a way of treatment by describing emotions from standpoint of 'Gi(氣)'. 5. The special feature of psychological treatment in oriental medicine is that treatment is done by understanding emotions of present time as Gi (氣), and using dynamics of Gi (氣)(; 相生相剋). Also the balanced state can be kept even after treatment and prevention can be done by observing one's own unconsciousness(; training one's mind helps circulation of Jeong(精), Gi(氣), and Sin(神)) I think that we need a new realistic therapy model to develop these psychotherapy methods in oriental neuropsychiatry, through studying emotions in the theory of oriental medicine, especially Korean's emotions and spiritual training.

기술과 자유로운 상상의 연결 패러다임 (The Connecting Paradigm between Skills and Free Imagination)

  • The status of medical doctors is relatively high in society. However, in spite of this acknowledged status, physicians are not aware of the extent to which they have the ability to care for patients or how much effort they should make to meet people's expectations. Therefore, we should examine what society asks of doctors and how doctors need to be educated to meet the expectations of society. In this article, the author asserts that physicians need four skills. First, doctors should know how to speak and communicate. In the work of a doctor, language is the most important for tasks such as understanding texts, communication with patients, analyzing data, and starting new projects. Second, doctors should have intuition. In a doctor's medical judgment, intuition is very important and it can initiate from an educated guess. In other words, good intuition can be developed based on a good educated guess, which in turn can derive from one's explored knowledge, communication with one's inner dialogues, and good interpretation skill. Third, doctors should have creativity. Doctors should produce an image about patients from intuition, and those intuitions are based on creativity. Usually, students in medical school have creative ability; therefore, the instructor should facilitate their learning to connect this creativity to free imagination ability and medical skills. Fourth, doctors should be humane. Patients want to communicate with doctors about their disease and further about their lives. The reason why a humane doctor is important is that this humane approach itself could cure patients and reduce their pain. When a doctor's humane attitude is realized in the hospital, the patients and doctors could be pleased sincerely.

선택 진료 제도 개선을 위한 소비자 인식도와 만족도 (Analysis of Consumer's Recognition and Satisfaction for the Improvement of the Doctor-Designation System)

  • 본 연구는 부산시민을 대상으로 선택진료제도에 대한 인식도와 선택진료의 경험이 있는 의료소비자에게 제도 인식율과 만족도 결과에 따른 개선방안을 도출하여 효과적인 선택진료 제도 운영에 필요한 기초자료 제공을 위하여 만 20세 이상 부산시민들을 연구대상자로 설문조사를 실시하여 분석한 결과, 여성, 20대, 대졸이상, 학생과 서비스직종, 월 평균소득은 200~299만원에서 가장 많은 분포를 보였다. 선택진료를 인지하고 있는 사람은 27.7%이고, 선택진료 경험자에 의한 인지율은 23.7%였다. 선택진료제도의 규칙에 대한 실제 인식율(정답율)은 전체 평균 66.3%이며 제도를 알게 된 경로는 대중매체를 통해서(31.9%)이며, 전문성이 높은 진료(57.5%)이기 때문에 선택진료를 이용하였다. 이들 대상자들은 재이용할 의사가 있다(76.3%)는 의견을 보였고 사유는 고급의료를 제공(35.2%)받기 때문이라는 결과를 보였다. 선택진료경험자의 만족도 전체 평균은 2.96점으로 만족도가 대체로 높지 않은 결과를 보였으며, 선택진료제도 개선을 위해서는 홍보강화(91.2%), 환자가 보기 쉬운 곳에 안내문 비치(96.7%), 선택진료비용 줄임(85.7%), 진료약관설명을 상세히할 것(65.4%)을 제시하여 효과적인 제도운영을 위해서는 지속적인 홍보를 통한 환자의 인식율 제고와 아울러 환자의 의료비 부담을 경감시킬 수 있는 제도적 전환이 필요할 것이다.

의안(醫案) 『역시만필(歷試漫筆)』의 맥진(脈診) 실행(實行)에 대한 연구 (A Study on the Pulse Diagnosis of the Medical Charts of Youksimanpil)

  • Objectives : This paper studies the pulse diagnosis as found in Youksimanpil, which is a series of medical charts containing 150 diagnosis records of Yi Suki, a doctor who was active in Joseon during the 17-18th centuries. Through this effort, the paper aims to shed light on how pulse was utilized in the Korean medicine, and in process tries to reveal the essence of Korean medicine's treatment method. Methods : 60 charts where pulse method was used are selected in Youksimanpil and a table is created with them. Figures are drawn to explain four steps of pulse-sensing from the simple method to highly advanced method. Charts are presented with the corresponding original texts and their translations. With these efforts, the paper attempts to reveal the broad understanding of the doctor of Joseon period who consistently kept to the most basic principle of pulse diagnosis. Results : The efficiency of pulse diagnosis depends on the unity and simplicity in diagnosis and prescription. There were continued efforts between the doctors in Joseon to collect and compare the experiences they gained from clinical practices in order to organize their findings and form a system. These are: (1) individual pulse, (2) patternized pulse, (3) balance between left and right pulses, (4) balance between pulse and body, and (5) the doctor's extemporaneous diagnosis. In that efforts, they protect the principle of holistic diagnosis, which is one of Korean medicine's core principles. Conclusions : Thanks to the existence of medical charts that presents in detail how the texts of Donguibogam were applied in real clinical practices, today we can see Korean medicine's highly advanced synergy between textual knowledge and clinical experiences as recorded in the form of charts.

의료 지속성에 대한 일반인들의 태도 및 관련요인 - 사무직 직원들을 대상으로 - (A Study on the general population's attitude and related factor on the continuity of medical care)

  • This study is aimed to assess the general population's attitude toward the continuity of medical care and its related factors. Self administered questionnaire was performed on the 1,120 office workers in the C city, Gang-won province. The questionnaire included the attitude of the continuity of medical institute, the intention of medical service use on a given case, and the variables of the related factors. 58.8% of the total respondents agreed to sustaining treatment without changing medical institutes; on the other hand, 41.2% showed negative attitude. In case that a patient would gain a recommendation of a surgery, hospitalization, or a specific examination, the total respondents' 84.9%, 61.8%, and 50.8% of each recommended situation said that they would visit another doctor and gain a diagnosis. As a result of multiple logistic analysis of determinant factor on continuity, reliability of doctors was statistically significant factor. In order to reduce wastefully used medical resources and offer well-qualified medical service, a system of second opinion among peer group or beforehand agreement could be possibly adopted. In addition, improving the image and reliability of a doctor could be an important factor to make better the behavior of medical service shopping; therefore, an effort to improve the relationship between a doctor and a patient, and restore the reliability of doctors should be paralleled.

An Art of Arbitration:Dispute Resolutions in Shakespeare's The Merchant of Venice

  • The main narrative of Shakespeare's The Merchant of Venice deals with a dispute over the matter of bond in regard to moneylending, and its consequences upon the eventual default. Only the clever interference of a lawyer or judge brings the crisis to an end. In solving his dispute over the bond between Antonio, the merchant of Venice, and Shylock, the money lender and a Jew, Shakespeare offers one of the most famous trial scenes in literature. This trial scene presents the art of arbitration by Portia who was disguised as a Doctor of Law and sheds light on the nature of law, justice, equity, and divine law. What one cannot overlook in this trial scene is the importance of reading ability. After all, interpretation is the next stage of reading. Drawing just verdicts and wise arbitration while at the same time deconstructing the implicit violence and incongruity in law is based on ceaseless effort of analytic and creative act of reading.
