Hydraulic Impact Analysis of Geum River with the Operation of Diversion Tunnel for Low Flow Augmentation of Boryong Dam (금강-보령댐 도수터널 운영에 따른 수리영향 분석)

  • 본 연구에서는 최근 기후변화에 따른 가뭄으로 어려움을 겪고 있는 충청남도 서부권의 물 부족을 해결하기 위하여 금강 본류로부터 보령댐으로 일정 유량을 도수(diversion)하는 계획이 금강 본류에 미치는 수리적 영향을 검토하였으며 특히 금강 본류의 농공용수 취수에 제한을 초래할 수 도 있는 수위에 미치는 영향을 중심으로 분석하였다. 본 연구의 공간적 범위는 금강 하굿둑으로부터 하류 외해 방향으로 약 9km 지점에 위치한 금강 하구로부터 상류 방향으로는 금강 하굿둑으로부터 약 76km 지점에 위치한 공주시 반포면 검상동의 검상천 합류지점까지 총 85km에 이르는 구간에 대한 1차원 수리 모형을 구축하였으며, 시간적 범위는 보령댐으로의 도수가 갈수기에 이루어지는 것으로 가정하여 2015년 2월 1일부터 5월 31일까지 진두수위표의 시간별 유량과 8개 지천의 유입 유량, 장항검조소의 시간별 조위, 농공용수 취수시설 취수량 및 회귀수, 금강 배수갑문 운영 등을 고려하여 모형의 보정을 실시하였고, 유황분석에 따른 2016년 갈수기 예측 유량을 추정하여 보령댐으로의 도수에 따라 금강 본류 내 주요 구간의 수위에 미치는 영향을 분석하고자 하였으며, 서해안 조위에 따른 금강하굿둑 배수갑문의 운영에 직접적인 영향을 받는 배수위 구간임을 감안하여 비정상류 해석(unsteady flow analysis)을 수행하였다. 갈수기 예측 유량 추정 결과 2015년과 같은 가뭄이 지속될 경우, 2016년의 유황은 2015년 대비 21%로 분석되었으며 이는 보령댐 도수 여부에 관계없이 물 부족이 발생할 수 있는 상태인 것으로 분석되었고, 하천유지유량 정도의 유황이 유지되는 상태에서 보령댐으로 도수를 실시할 경우, 도수시 금강호 최저 수위는 각각 EL(+)0.94m와 EL(+)0.72m로 모의되어 농업용수를 비롯한 기존 취수시설 뿐만 아니라 보령댐 도수시설의 운영에도 어려움이 생길 것으로 예상되었다. 2016년 갈수기 동안 과업 대상 구간인 금강 하류부의 유황은 농업용수 취수가 본격화되기 이전인 2월에는 하천유지유량 정도의 유황을 유지하는데 별 어려움이 없지만 3월 이후농업용수 취수가 본격화되면 농업용수 취수시설이 집중된 입포수위표 ~ 금강 하굿둑 사이 구간은 하천유지유량 이하로 유량이 크게 감소할 것으로 예상되었다.

Comparison of the Degree of Pain According to Nursing Intervention Method during Arteriovenous Fistula Needle Insertion for Patients on Hemodialysis (혈액투석 환자의 동정맥루 천자 시 간호중재요법에 따른 통증정도 비교)

  • Purpose: This research was done to compare the pain relief effects of various nursing interventions, such as cold therapy, attention diversion and 10% Lidocaine spray during arteriovenous fistula needling for patients on hemodialysis, and also to identify and develop more effective nursing interventions for pain relief in these patients. Methods: This research was conducted from October 1, 2010 to January 31, 2011 with 8 hemodialysis patients, who were on regular dialysis (3 times a week) at K University Hospital in Seoul and had an arteriovenous fistula within the past 3 months. Each patient received the three nursing interventions (cold therapy, attention diversion and 10% Lidocaine spray therapy) prior to the arteriovenous fistula needling and applied in turn with the series being repeated 4 times. After each intervention, physiologic indexes, subjective and objective pain were measured at the time of needling. ANOVA was used with SPSS/WIN 12.0 to analyze pain scores and comparison of physiologic indexes (BP, pulse). Results: No significant differences were found for subjective pain (p=.574), objective pain (p=.562) and total pain (p=.506) among the 3 interventions. Systolic blood pressure (p=.689), diastolic blood pressure (p=.969) and pulse (p=.980) also showed no significant difference among the 3 interventions. Conclusion: These 3 interventions are all possible for pain relief during arteriovenous fistula needling for these patients. However, the only interventions that nurses can do independently are cold therapy and attention diversion so we recommend that these nursing interventions be used.

Safety and Efficacy of Flow Diverter Therapy for Unruptured Intracranial Aneurysm Compared to Traditional Endovascular Strategy : A Multi-Center, Randomized, Open-Label Trial

  • Objective : Endovascular treatment of large, wide-necked intracranial aneurysms by coil embolization is often complicated by low rates of complete occlusion and high rates of recurrence. A flow diverter device has been shown to be safe and effective for the treatment of not only large and giant unruptured aneurysms, but small and medium aneurysms. However, in Korea, its use has only recently been approved for aneurysms <10 mm. This study aims to compare the safety and efficacy of flow diversion and coil embolization for the treatment of unruptured aneurysms ≥7 mm. Methods : The participants will include patients aged between 19 and 75 years to be treated for unruptured cerebral aneurysms ≥7 mm for the first time or for recurrent aneurysms after initial endovascular coil embolization. Participants assigned to a flow diversion cohort will be treated using any of the following devices : Pipeline Flex Embolization Device with Shield Technology (Medtronic, Minneapolis, MN, USA), Surpass Evolve (Stryker Neurovascular, Fremont, CA, USA), and FRED or FRED Jr. (MicroVention, Tustin, CA, USA). Participants assigned to a coil embolization cohort will undergo traditional endovascular coiling. The primary endpoint will be complete occlusion confirmed by cerebral angiography at 12 months after treatment. Secondary safety outcomes will evaluate periprocedural and post-procedural complications for up to 12 months. Results : The trial will begin enrollment in 2022, and clinical data will be available after enrollment and follow-up. Conclusion : This article describes the aim and design of a multi-center, randomized, open-label trial to compare the safety and efficacy of flow diversion versus traditional endovascular treatment for unruptured cerebral aneurysms ≥7 mm.

Methodologies for Survey and Retrofit of Small Dams Pierced by Diversion Tunnel (복통을 갖는 저수지의 결함 조사 및 보수보강 방안)

  • There are almost 18,000 dams including about 1,200 large dams in Korea. The large dams are well operated and maintained by KWATER(Korea Water Resource Corporation), KRC(Korea Rural Community & Agriculture Corporation) and KHNP(Korea Hydro & Nuclear Power Co., Ltd.). Several research reports concern with the safety of these large dams are presented but there is no paper concerned with small dams which has diversion tunnel through the dam body. The purpose of this study is to show the common defects of small dams according to various cases of degradation of dams and the repair and retrofit methods which applied to the damaged dams. And this study performed resistivity survey to evaluate the effect of retrofitting dam. Also, this study tries to present the solution which concerned with these common defects in maintenance and design steps.

Clinical and Angiographic Outcomes of Endovascular Treatment for Acute Intracranial Vertebral Artery Dissecting Aneurysms Using Double-Overlapping Stents : Low-Profile Visualized Intraluminal Support within Enterprise Stents

  • Objective : The use of reconstructive treatment with a double-overlapping stents has proven to be effective and safe in the current treatment of intracranial vertebral artery dissecting aneurysms (VADAs). We employed a combination of overlapping stents, using low-profile visualized intraluminal support (LVIS) within the Enterprise stent. This combination was chosen to minimize the outward bulging of the inner LVIS by overlapping it with the Enterprise stent while maintaining flow diversion and stability. This study aimed to evaluate the clinical and angiographic outcomes following the use of double-overlapping stents with LVIS within the Enterprise stent for the treatment of VADAs. Methods : From March 2016 to January 2022, total 28 patients with unruptured VADAs were treated with the double-overlapping stent technique using LVIS within an Enterprise stent in our institute. The Enterprise stent was deployed first, followed by the LVIS stent. Patient clinical and angiographic characteristics, procedural complications, and follow-up outcomes were retrospectively reviewed. Results : All 28 patients (18 males and 10 females) were successfully treated with double-overlapping stent deployment. There were no procedural complications or new neurological deficits in any patient. Of the 28 patients, four VADAs had posterior inferior cerebellar artery involvement. Procedure-related parent artery occlusion did not occur during the angiographic follow-up conducted 6 to 12 months after the procedure. Out of 28 patients, 24 showed complete healing, three had focal residual stenosis or dilatation with residual sac and only one had a residual dissecting flap with aneurysm. All patients, including the four patients, did not require any additional procedures. The postoperative modified Rankin scale scores were 0-1 for all patients. Conclusion : A double-overlapping stent, with a flow-diversion effect, is a safe and effective treatment for patients with VADAs. In particular, when using the LVIS stent within an Enterprise stent, it minimizes the bulging of the inner LVIS stent while maintaining flow diversion and stability. Therefore, both can be effectively utilized as overlapping stents.

Endovascular treatment for anterior inferior cerebellar artery-posterior inferior cerebellar artery (AICA-PICA) common trunk variant aneurysms: Technical note and literature review

  • The Anterior Inferior Cerebellar Artery-Posterior Inferior Cerebellar Artery (AICA-PICA) common trunk is a rare variant of cerebral posterior circulation in which a single vessel originating from either the basilar or vertebral arteries supplies both cerebellum and brainstem territories. We present the first case of an unruptured right AICA-PICA aneurysm treated with flow diversion using a Shield-enhanced pipeline endovascular device (PED, VANTAGE Embolization Device with Shield Technology, Medtronic, Canada). We expand on this anatomic variant and review the relevant literature. A 39-year-old man presented to our treatment center with vertigo and right hypoacusis. The initial head CT/CTA was negative, but a 4-month follow-up MRI revealed a 9 mm fusiform dissecting aneurysm of the right AICA. The patient underwent a repeat head CTA and cerebral angiogram, which demonstrated the presence of an aneurysm on the proximal portion of an AICA-PICA anatomical variant. This was treated with an endovascular approach that included flow diversion via a PED equipped with Shield Technology. The patient's post-procedure period was uneventful, and he was discharged home after two days with an intact neurological status. The patient is still asymptomatic after a 7-month follow-up, with MR angiogram evidence of stable aneurysm obliteration and no ischemic lesions. Aneurysms of the AICA-PICA common trunk variants have a high morbidity risk due to the importance and extent of the territory vascularized by a single vessel. Endovascular treatment with flow diversion proved to be both safe and effective in obliterating unruptured cases.

Structural Analysis of Time and Money Attitudes, Leisure Consumption Behavior and Leisure Satisfaction (시간과 화폐에 대한 태도가 여가소비행동과 여가만족도에 미치는 영향에 대한 구조분석)

  • This study compared types of time and money attitudes by respondents' characteristics, and compared leisure consumption behavior and leisure satisfaction among different groups, which are classified by attitudes toward time and money. It also verified a structural model for the relationship of leisure satisfaction and the attitudes toward time and money, and investigated the moderating effects of leisure consumption behavior. Data were obtained from a questionnaire completed by 512 adult consumers. Using factor analyses and cluster analyses, this study classified the types of time and money attitudes. The time attitude was categorized according to four clusters: time-insensibility, future- oriented, present-oriented, and past-oriented. The money attitude was divided into four clusters: future safety, stinginess/anxiety, diversion, and power/prestige. The major findings of this study were as follows: First, the propensity of leisure consumption behavior to seek fashion and conspicuousness was higher for time-insensibility group than for the others. However, the propensity of rational consumption behavior for leisure was higher for the future-oriented group. The level of leisure satisfaction was the highest for the time-insensibility group. Second, the propensity of leisure consumption behavior to seek fashion and conspicuousness was higher for those who considered money as tools for diversion and power/prestige. The propensity of rational consumption behavior for leisure was higher for the future safety and stinginess/anxiety groups. The level of leisure satisfaction was significantly higher for the diversion group. Third, from the results of structural analyses, the time-insensibility attitude showed a positive effect on the leisure consumption behavior to seek fashion and conspicuousness. The future-oriented attitude had a positive effect on the propensity of rational consumption behavior for leisure. The money attitudes of diversion and power/prestige had positive impacts on the leisure consumption behavior to seek fashion and conspicuousness, although they had negative effects on the propensity of rational consumption behavior for leisure. The leisure consumption behavior to seek fashion and conspicuousness had a positive effect on leisure satisfaction, although the rational consumption behavior for leisure had a negative effect.

Tunnelling in Bangkok - Two Case Studies (방콕의 터널공사 - 두 개의 사례연구)

  • This paper presents two case studies for tunnelling in Bangkok: a subway tunnel site and a flood diversion tunnel site. The first case study is related to ground displacement response for dual tunnel Bangkok MRT subway. The MRT subway project of Bangkok city consists of dual tunnels about 20 km long with 18 subway stations. The tunnels are seated in the firm first stiff silty clay layer between 15-22 m in depth below ground surface. The behavior of ground deformation response based on instrumentation is presented. The back analysis based on plain strain FEM analysis is also presented and agrees with field performance. The shear strain of FEM analysis is in the range of 0.1-1% and in accordance with the results of self boring pressuremeter tests. Meanwhile, the second case study is related to the EPB tunnelling bored underneath through underground obstruction. The Premprachakorn flood diversion tunnel is the shortcut tunnel to divert the flood water in rainy season into the Choapraya river. The tunnel was bored by means of EPB shield tunnelling in very stiff silty clay layer at about 20-24 m in depth. During flood diversion tunnel bored underneath the existing Bangkok main water supply tunnel and pile foundation of the bridge, instrumentation was monitored and compared with predicted FEM analysis. The prevention risk potential by means of predicting damage assessment is also presented and discussed.

Design of Sedimentary Rock Slopes in River Diversion Works (가배수로 터널공사의 퇴적암 사면 안정화 설계)

  • The Bakun hydroelectric project includes the construction of a hydroelectric power plant with an installed capacity of 2,520MW and a power transmission system connecting to the existing transmission networks in Sarawak and Western Malaysia, The power station will consist of a 210m height concrete faced rockfill dam. During the construction of the dam and the power facilities the Balui river has to be diverted by three diversion tunnels with a length of some 1,400m each. The inner diameter of the tunnels is 12m and the tunnel width is 16m at the portal area. This paper describes the stability analysis and design methods for the open cut rock slopes in the inlet and outlet area of the diversion tunnels. The geotechnical parameters employed in stability calculations were given as a function of four. defined Rock Mass Types (RMT) which were based on RMR system from Bieniawski. The stability calculations procedure of the rock slopes are divided into two stages. In the first stage, it is calculated for the stability of each 'global' slope without any rock support and shotcrete system. In the second stage, it is calculated for each 'local'slope stability with berms and supported with rock bolts and shotcrete. The monitoring instrumentation was performed continuously and some of the design modification was carried out in order to increase the safety of failed area based on the unforeseen geological risks during the open cut excavation.

Relationships Between Using Rate of Information Media on Diversion by Traffic Condition (소통상황에 따른 정보매체별 우회이용률 분석)

  • Although many studies have been carried out on the pattern of behavior of drivers which result from the provision of traffic information, there have been few detailed studies on the composition of message, method for message expression, timing of provision, point of provision, media for provision, changes by traffic condition, etc. This study was intended to provide an insight into the changes in the characteristics related to the provision of information by analyzing how the patterns of information utilization change depending on the traffic condition and reclassifying such patterns according to the characteristics of media. Unlike the existing studies, this study adopted the traffic condition, using rate of information media, and the correlation coefficient label as the basis for information media classification, and categorized them into passive utilization media, active utilization media, and past experience in order to ensure the statistical reasonability. The categorized using rate of information media and traffic condition was found to have a positive(+) correlation with the travel speed in the case of passive utilization media during both consecutive holidays(Korea's traditional Thanksgiving day) and weekends, but had a negative(-) correlation with the positive utilization media and past experience. The rate of decision to take a detour based on the past experience was high at the condition of congestion or slow during both consecutive holidays and weekends, but the rate of decision to take a detour through passive utilization media was high in a smooth traffic. In other words, if the traffic condition worsens, using rate of passive utilization media would be low while the diversion rate would be high which uses the active utilization media and past experience. Therefore, it should be established to suit the traffic condition and media characteristics for strategies of traffic distribution through drivers' diversion behavior on weekends and consecutive holidays.
