PHOTOELASTIC STRESS ANALYSIS ON THE SUPPORTING TISSUE OF MANDIBULAR DISTAL EXTENSION REMOVABLE PARTIAL DENTURE WITH VARIOUS DESIGN OF DIRECT RETAINERS (직접유지장치 설계 변화에 따른 하악 후방연장 국소의치 지지조직의 광탄성 응력분석)

  • The purpose of this study was to evaluate the stress distribution developed in supporting structures by distal extension removable partial denture with 4-types of direct retainer. The direct retainers examined were Akers clasp, RPI clasp, RPA clasp and RPL clasp in bilateral & unilateral free end case. 3-dimensional photoelastic stress analysis was used to record the isochromatic fringe patterns and to calculate the compressive stress at measuring points. The results were as follows. 1. In bilateral free end case, RPI clasp exhibited the similar stress distribution on distal and mesial alveolar crest but Akers clasp exhibited higher stress concentration on distal alveolar crest than mesial alveolar crest. 2. In bilateral free end case, RPA clasp and RPL clasp exhibited the similar stress distribution on distal and mesial alveolar crest and RPL clasp exhibited higher stress concentration on buccal alveolar crest than lingual alveolar crest. 3. Akers clasp produced high stress concentration on residual alveolar ridge distally, but RPI clasp, RPA clasp and RPL clasp produced even stress distribution on residual alveolar ridge. 4. Removable partial denture in unilateral free end case exerted higher stress on abutment tooth root apex than bilateral distal extension removable partial denture.

  • Osteochomdromas, which are uncommon in the hand, are encountered most frequently with hereditary multiple exostosis. They can occur away from the epiphyseal plate region at the distal end of the proximal and middle phalanges. But little has been written about exostosis that occur at the distal end of the distal phalnges. We report one case of hereditary multiple exostosis that arose at the distal end of the distal phalnges. Complete excision was done and the patient was disease-free of 4 years follow-up.

Surgical Treatment of Carpal Tunnel Syndrome through a Minimal Incision on the Distal Wrist Crease: An Anatomical and Clinical Study

  • Background An anatomical analysis of the transverse carpal ligament (TCL) and the surrounding structures might help in identifying effective measures to minimize complications. Here, we present a surgical technique based on an anatomical study that was successfully applied in clinical settings. Methods Using 13 hands from 8 formalin-fixed cadavers, we measured the TCL length and thickness, correlation between the distal wrist crease and the proximal end of the TCL, and distance between the distal end of the TCL and the palmar arch; the TCL cross sections and the thickest parts were also examined. Clinically, fasciotomy was performed on the relevant parts of 15 hands from 13 patients by making a minimally invasive incision on the distal wrist crease. Postoperatively, a two-point discrimination check was conducted in which the sensations of the first, second, and third fingertips and the palmar cutaneous branch injuries were monitored (average duration, 7 months). Results In the 13 cadaveric hands, the distal wrist crease and the proximal end of the TCL were placed in the same location. The average length of the TCL and the distance from the distal TCL to the superficial palmar arch were $35.30{\pm}2.59mm$ and $9.50{\pm}2.13mm$, respectively. The thickest part of the TCL was a region 25 mm distal to the distal wrist crease (average thickness, $4.00{\pm}0.57mm$). The 13 surgeries performed in the clinical settings yielded satisfactory results. Conclusions This peri-TCL anatomical study confirmed the safety of fasciotomy with a minimally invasive incision of the distal wrist crease. The clinical application of the technique indicated that the minimally invasive incision of the distal wrist crease was efficacious in the treatment of the carpal tunnel syndrome.

Surgical Treatment of Giant Serpentine Aneurysm of A2-A3 Segment Distal Anterior Cerebral Artery : Technical Case Report

  • Objective : To report our surgical experience using in situ end-to-side bypass for giant serpentine distal anterior cerebral artery aneurysm, unsuitable for microsurgical clipping. Methods : A 49-year-old woman presented with headache and intermittent loss of consciousness. The brain computed tomography scan revealed a partially calcified mass in the interhemispheric fissure. On cerebral angiography, that was giant ($30{\times}18mm$ sized), serpentine aneurysm originating from the A2 to A3 segment of the distal anterior cerebral artery (DACA). The aneurysm was trapped with clips, and the right A3 segment to left A3 segment of DACA, end-to-side in situ bypass was performed. Surgical result was favorable, with no newly developed ischemic event in the acute recovery period. Postoperative angiography showed total occlusion of the aneurysm and good patency, with preserved distal flow. Conclusion : Giant fusiform aneurysms of the DACA are extremely rare and can be particularly challenging to treat. End-to-side A3 : A3 bypass with aneurysm trapping could be a treatment modality for these locations.

In vitro apical pressure created by 2 irrigation needles and a multisonic system in mandibular molars

  • Objectives: The aim of this study was to evaluate the apical pressure generated by 2 endodontic irrigation needles and the GentleWave system in mandibular molars. Materials and Methods: The mesial and distal root canals of 12 mandibular molars were irrigated with a 30-gauge close-end needle or with a 30-gauge open-end needle. Procedures were performed in the mesial and distal canals. The GentleWave procedure and irrigation at 1 mm from the apex in the distal roots using an open-end needle were used, respectively, as negative and positive controls. The apical pressure was measured using a data acquisition pressure setup. Apical pressure exerted by the different needles in the 2 different canal types was statistically compared using 2-way analysis of variance. Results: Significant differences were found in the apical pressure for both needles and the canal type. The lowest values were obtained with close-end needles and in mesial canals. Negative apical pressure values were obtained using GentleWave. Conclusions: The needle and the canal type influenced the apical pressure. The GentleWave procedure produced negative apical pressure.

The Function and Symptoms of Ankle Joint in the Distal Tibial Fractures Treated by Nailing (족관절 기능과 증상을 중심으로 평가한 경골 원위부 골절에서의 금속정 치료 결과)

  • Purpose: To evaluate the safety and prognostic factors of intramedullary nailing of distal tibia fractures in terms of function and symptoms of the ankle joint. Materials and Methods: We retrospectively analyzed 22 distal tibial fractures with intramedullary nailing. The mean duration of follow-up was 43 months. We reviewed medical records to describe each case. We measured radiographic parameters such as fracture configuration, arthritic change of the ankle joint and status of reduction. We also assessed clinical results by AOFAS ankle hind foot scoring system, degree of pain by VAS and range of motion to find out prognostic factors for functional result of the ankle joint. Results: Bone healing was obtained in all cases without any wound complications. Mean AOFAS ankle score was 94. There were 4 cases with mild (VAS<3/10) ankle pain and 2 cases with mild limitation of ankle motion. The comminution of fracture had a significant relationship with delayed angular deformity of ankle joint (p=0.032). There was no other significant parameter affecting ankle joint function except the location of nail-end. Conclusion: Intramedullary nailing in distal tibia fracture is a safe and effective procedure. But further study may need to evaluate the relationship between the position of nail-end and the function of ankle joint.

  • The apical foramen is not always found on the very tip of the root. The apical foramen may make its exit on the mesial, distal, labial or lingual side of a root slightly short of the root apex rather than at the root apex itself. The author collected 43 upper first molars and 84 lower first molars as a samples. Apical foramens were carefully checked and examined these location on each tip. (table 1) 1. About 33% to 49% of upper cases were found on extreme tip of roots and the remaining cases were on the distal side or mesial side. 2. Except distal simple canal of lower molars, approximately 40% to 50% were located on the very tip of the root. The remaining cases were on distal or mesial surface. 3. On lower distal simple canal, about 51% of cases made its exit on distal side. The remaining cases located on the tip end or mesial side.

In Situ Intersegmental Anastomosis within a Single Artery for Treatment of an Aneurysm at the Posterior Inferior Cerebellar Artery : Closing Omega Bypass

  • A 74-year-old patient was diagnosed with a subarachnoid hemorrhage suspected from a dissecting aneurysm located at the lateral medullary segment of the posterior inferior cerebellar artery (PICA). Because perforators to the medulla arose both proximal and distal to the dissecting segment, revascularization for distal flow was essential. However, several previously reported methods for anastomosis, such as an occipital artery-PICA bypass or resection with PICA end-to-end anastomosis could not be used. Ultimately, we performed an in situ side-to-side anastomosis of the proximal loop of the PICA with distal caudal loops within a single artery, as a "closing omega," followed by trapping of the dissected segment. The aneurysm was obliterated successfully, with intact patency of the revascularized PICA.

The Use of Hook Plate on the Management of Unstable Neer II Lateral End Fracture of The Clavicle (불안정한 Neer II형 원위쇄골 골절의 치료에서 Hook 금속판의 이용)

  • Purpose: Unstable Neer type II lateral end fracture of clavicle may be required operation. The purpose of this study is the effectiveness of the use of Hook plate in the management of unstable Neer type II fractures. We preliminary reported the results of Unstable Neer type II lateral end fracture of clavicle using by Hook plate. Material and Methods: From May 1998 to May 2002, we operated 6 cases unstable Neer type II lateral end fracture of clavicle with Hook plate by one surgeon. We checked radiologic evaluation and disappearance of pain per 1 week, and evaluated improvement of range of motion per 2 week. And we followed up at each interval 3 months,6 months, 12 months and after that per 6months interval, at that each time we checked stress radiogram of shoulder and functional evaluation. The shoulder function was evaluated using Modified Shoulder Rating Scale (MSRC) for Distal Clavicle Fracture and UCLA score. Average follow up was on 37.2 (12∼57) months. Results: All 6 patients were regained satisfactory function. Average MSRC for distal clavicle fracture was 17.3 (15∼20) and average UCLA score was 33.2 (31∼35) at last follow up. Conclusion: There are many advantage of the use of Hook plate on management of unstable Neer II distal clavicle fracture in spite of several disadvantage. There have not yet been reported in our country. So we obtained good to excellent clinical result in surgical treatment of 6 cases on type II displaced lateral end fracture of the clavicle. But we think that more cases will be review and longer follow up will be needed in the future.

A Numerical Study on the Steady and Pulsatile Flow with Various Diameter Ratios of Abdominal Aortic Aneurysm (복부대동맥류의 직경비에 따른 정상유동 및 맥동유동에 관한 수치적 연구)

  • The objective of the present study was to investigate the characteristics of flow and wall shear stress under steady and pulsatile flow in the aneurysm. The numerical simulation using the software were carried out for the diameter ratios ranging from 1.5 to 3.0, Reynolds number ranging from 900 to 1800 and Womersley number, 15.47. For steady flow, it was shown that a recirculating vortex occupied the entire bulge with its core located closer to the distal end of the bulge and the strength of vortex increased with increase of the Reynolds number and diameter ratio. The position of a maximum wall shear stress was the distal end of the aneurysm regardless of the Reynolds number and diameter ratios. For the pulsatile flow, a recirculating flow at the bulge was developed and disappeared for one period and the strength of vortex increased with the diameter ratio. The maximum values of the wall shear stress increased in proportion to the diameter ratio. However, the position of a maximum wall shear stress was the distal end of the aneurysm regardless of the diameter ratios.
