THE DIMENSIONAL CHANGE OF CAST IMPLANT BARS AFTER LABORATORY PROCEDURE

  • Statement of Problems. The precision of fit between the bearing surfaces of implant abutments and the prosthesis framework has been considered fundamental to implant prosthodontic protocol. Purpose. The study aimed to investigate the effect of laboratory procedure on the dimensional accuracy of cast implant bars. Material and methods Thirty implant bars were fabricated on a metal master model. The gap distances were measured at the right implant abutment replica-gold cylinder interface after casting procedure. The bar length data of precasting and postcasting state were collected and analyzed. Results. The mean gap distance found after casting was $106.3{\mu}m$ for buccal side, $122.1{\mu}m$ for distal side and $117.1{\mu}m$ for the lingual side. The mean bar length was $17964.7{\mu}m$ at precasting measurement, $17891.6{\mu}m$ at postcasting measurement. The mean change of bar length was $-73.1{\mu}m$. Conclusion. Even though the techniques used in this study strictly followed the guidelines established in the literature, the 30 cast implant bars evaluated all yielded gap distances that were beyond acceptable accuracy. There was a statistically significant difference between precasting and postcasting bar length (P<0.01). There was a decreasing tendency in bar length after casting procedure. It was necessary to correct this dimensional change from laboratory procedure by some corrective methods.

  • The ductus arteriosus (Botallo's duct) is a vascular communication between the pulmonary artery near its bifurcation and the aorta distal to the origin of the left subclavian artery. A surgical operation was tried in the shepherd dog which was affected by patent ductus arteriosus. The patient was six months old and weighed about 11kg. Pentobarbital sodium, 26 mg/kg was used intravascularly for the anesthesia. And then a respirator was used for the patient. The thoracotomy incision was made over the fourth intercostal space on the left and the ductus arteriosus was ligated twice. But the patient revealed ventricular fibrillation at three minutes after ligations of ductus arteriosus. So the cardiac massage was done after incision of pericardium and then the defibrillator was used for the defibrillation. But the patient was not recovered without effect. It was considered that if there is complication or cyanosis, the adaptability of surgical operation should be specially considered. Histologically, chronic passive congestion of liver was observed.

  • The aim of this investigation was to study the effect of orthodontic force on the flow of gingival crevicular fluid and activities of arylsulfatase and brta-glucuronidase in crevicular fluid. The material consisted of 12 persons between the ages of 13 years and 22 years and all were categorized Class I, 4-4 extraction cases Crevicular fluids were sampled from distal crevis of each canine before treatment (phase 1), after bracketing (phase 2), after application of force (phase 3) and after run out of orthodontic force (phase 4). Crevicular fluid flow did not show any significant changes during the period of treatment. The activities of arylsulfatase increased significantly after setting of orthodontic appliance without application of force, but did not show any significant difference after application of force. The activities of beta-glucuronidase increased significantly after application of orthodontic force and decreased with force deminished. These indicated that beta-glucuronidase was good indicator of bone remodelling resulted from initial orthodontic force.

Fractal analysis of the surgical treatment of ligature-induced peri-implantitis in dogs (임플란트 주위염 치료 효과의 프랙탈 분석)

  • Purpose : To evaluate the effect of surgical treatment of ligature-induced peri-implantitis in dogs using fractal analysis. Also, the capabilities of fractal analysis as bone analysis techniques were compared with those of histomorphometric analysis. Materials and Methods : A total of 24 implants were inserted in 6 dogs. After a 3-months, experimental periimplantitis characterized by a bone loss of about 3 mm was established by inducing with wires. Surgical treatment involving flap procedure, debridement of implants surface with chlorhexidine and saline (group 1), guided bone regeneration (GBR) with absorbable collagen membrane and mineralized bone graft (group 2), and $CO_2$ laser application with GBR (group 3) were performed. After animals were sacrificed in 8 and 16 weeks respectively, bone sections including implants were made. Fractal dimensions were calculated by box-counting method on the skeletonized images, made from each region of interest, including five screws at medial and distal aspects of implant, were selected. Results : Statistically significant differences in the fractal dimensions between the group 1($0.9340{\pm}0.0126$) and group 3($0.9783{\pm}0.0118$) at 16 weeks were found (P<0.05). The fractal dimension was statistically significant different between 8($0.9395{\pm}0.0283$) and 16 weeks in group 3 (P<0.05). These results were similar with the result of the evaluation of new bone formation in histomorphometric analysis. Conclusions : Treatment of experimental peri-implantitis by using $CO_2$ laser with GBR is more useful than other treatments in the formation of new bone and also the tendency of fractal dimension to increase relative to healing time may be a useful means of evaluating.

Clinical and Electrophysiological Changes after Open Carpal Tunnel Release: Preliminary Study of 25 Hands (수근관증후군 수술 전후 임상증상과 전기생리학적 검사소견의 변화: 25손을 대상으로 한 예비연구)

  • Background: Electrophysiological study has been known as a useful method to evaluate the therapeutic effect of operation in idiopathic carpal tunnel syndrome (CTS). The purpose of this study was to evaluate the clinical and electrophysiological changes after carpal tunnel release (CTR) compared to the preoperative results. Methods: We analyzed the changes of nerve conduction study (NCS) before and after minimal open carpal tunnel release in 18 patients (25 hands) with CTS. Follow-up study was performed over 6 months after operation. Results: Clinical improvement was seen in all cases after CTR. In contrast, electrophysiological improvement was various depending on the parameters; the mean median sensory latency and nerve conduction velocity (NCV) improved significantly (p = 0.001). The mean median motor latency also improved, but NCV and compound muscle action potential (CMAP) amplitude did not change. The extent of improvement was evident in moderate CTS, but not in severe CTS. Conclusions: In this preliminary study, all subjects who underwent CTR achieved a clinical relief along with a significant improvement of electrophysiological parameters such as median sensory latency, sensory NCV and median distal motor latency. After CTR, a number of cases with mild to moderate CTS showed a prominent improvement of clinical and electrophysiological parameters, while fewer improvements were seen in severe CTS, although it did not reach the statistical significance.

Periodontal regeneration capacity of equine particulate bone in canine alveolar bone defects

  • Purpose: This study was performed to evaluate the periodontal wound healing effect of particulate equine bone mineral on canine alveolar bone defects. Methods: Twelve adult male beagle dogs were used as study subjects. The mandibular second and fourth premolars were extracted prior to the experimental surgery, and the extraction sites were allowed to heal for 8 weeks. After periodontal probing, two-walled defects were created at the mesial and distal sides of the mandibular third premolars bilaterally, and the defects were filled with equine particulate bone with collagen membrane or bovine particulate bone with collagen membrane, or collagen membrane alone. The defects without any treatment served as negative controls. After probing depth measurement, animals were sacrificed at 10, 16, and 24 post-surgery weeks for micro-computed tomographic and histomorphometric analysis. Results: The equine particulate bone-inserted group showed significantly decreased values of probing depth and first bone contact compared to the negative control and collagen membrane alone groups at weeks 10, 16, and 24 (P<0.05). There were no significant differences in the new cementum length, newly-formed bone area, or newly-formed bone volume between equine particulate bone- and bovine particulate bone-inserted groups, both of which showed significantly increased values compared to the negative control and collagen membrane alone groups (P<0.05). Conclusions: Equine particulate bone showed significant differences in probing depth, first bone contact, new cementum length, newly formed bone area, and bone volume fraction values when compared to the negative control and collagen membrane alone groups. There were no significant differences between equine and bovine particulate bone substitutes in these parameters; therefore, we can conclude that equine particulate bone is equivalent to bovine bone for periodontal regeneration.

Guided bone regeneration using demineralized allogenic bone matrix with calcium sulfate: case series

  • PURPOSE. The purpose of this case series was to evaluate the effect of guided bone regeneration using demineralized allogenic bone matrix with calcium sulfate. MATERIALS AND METHODS. Guided bone regeneration using Demineralized Allogenic Bone Matrix with Calcium Sulfate ($AlloMatrix^{TM}$, Wright. USA) was performed at the time of implant placement from February 2010 to April 2010. At the time of the second surgery, clinical evaluation of bone healing and histologic evaluation were performed. The study included 10 patients, and 23 implants were placed. The extent of bony defects around implants was determined by measuring the horizontal and vertical bone defects using a periodontal probe from the mesial, distal, buccal, and lingual sides and calculating the mean and standard deviation of these measurements. Wedge-shaped tissue samples were obtained from 3 patients and histologic examination was performed. RESULTS. In clinical evaluation, it was observed that horizontal bone defects were completely healed with new bones, and in the vertical bone defect area, 15.1% of the original defect area remained. In 3 patients, histological tests were performed, and 16.7-41.7% new bone formation was confirmed. Bone graft materials slowly underwent resorption over time. CONCLUSION. $AlloMatrix^{TM}$ is an allograft material that can be readily manipulated. It does not require the use of barrier membranes, and good bone regeneration can be achieved with time.

Research Trend on the Clinical Application of Dong-gi Acupuncture Treatment - Focusing on Korean and Chinese Journals - (동기침법의 임상 활용에 관한 연구 동향 - 한국과 중국을 중심으로 -)

  • The purpose of this study is to review the trend of Dong-gi acupuncture treatment and to suggest the future direction of its study. We performed literature search using KISS, RISS, NDSL, DBpia, OASIS, National Assembly Library and CNKI. We categorized literatures by year, diseases or symptoms, type of articles, motion types, acupoints, and clinical methods. There were few Korean researches found, but recently research in China has been growing rapidly. According to the researches, Dong-gi acupuncture treatment has been mainly used for painful symptoms of musculoskeletal diseases using distal acupoints. However, its field of application has been widening. It has been shown to be effective for a variety of diseases like cerebrovascular disease or neurologic disorder. We expect Dong-gi acupuncture treatment to be widely applied for various diseases and more clinical studies performed to prove the curative effect on various diseases.

A STUDY OF EFFECTS OF BONE MORPHOGENETIC PROTEIN BONE REGENERATION OF IMPLANTS IN DOGS (성견에서 임프란트 매식시 골형성단백 사용에 따른 골재생에 관한 연구)

  • The purpose of this study is to evaluate the effect of the bone morphogenetic protein, bone matrix gelatin and collagen matrix on the amount and shape of generating new bone adjacent to the implant. Implants were inserted in the mandible of adult dogs at 2 months after teeth extraction. Artificial bony defects, 3mm in width and 4mm in depth were made at the mesial and distal side of implant. Experimental groups were divided into three groups ; Group 1 : Defects filled with collagen matrix and bone morphogenetic protein, Group 2 : Defects filled with bone matrix gelatin. Control group : Defects filled with only collagen matrix. After implantation, the animals were sacrificed at 1,3,5 and 10 weeks for light microscopic examination. For the fluorescent microscopic examination. each tertracycline Hcl and calcein were injected at 1, 3, 5, 8 and 10 weeks after implantation. The results obtained were as follows : 1. The molecular weight of bovine BMP was about 18,100 by hydroxyapatite chromatography. 2. Osseointegration was observed in experimental groups 1 & 2, and BMG and BMP had an excellent bone forming capability as a filling materials to the repair of the bone defects. 3. The degree of healing of bone defect area, the experimental group 1 showed more prominent bone formation than control group, and the control group showed fibrous connective tissue between the implant and the bone. 4. In the fluorescent microscopic findings, bone remodelling was observed regenerative lamellar bone at defect area in experimental group 1, and partial remodelling in experimental group 2, In the control group, fibrous connective tissue was observed between the implant and bone surface and sign of remodelling was not apperaed. Above results suggest that BMP has rapid osteoinductive property and can be used clinically as a bone substitute on bone defects around implants.

Adhesion Prevention with Guardix® (Sodium Hyaluronate) After Flexor Tendon Repair in Rabbits (토끼 모델에서 굴근 인대 봉합 후 가딕스®의 유착 방지 효과)

  • Purpose: Adhesion is the most common and troublesome complication after repair of flexor tendon injury. Recently, use of sodium hyaluronate derivatives for adhesion prevention is increasing. A commercial product, Guardix$^{(R)}$, sodium hyaluronate(NaHe) combined with carboxymethylcellulose(CMC) has been newly developed as a preventive material for adhesion. We have investigated its effect in rabbits. Methods: Twenty seven male New Zealand white rabbits were operated under ketamine anesthesia. After tendon repair in zone II of the hind paw, Guardix$^{(R)}$(experimental group) or normal saline(control group) was administered. Biomechanical tests were performed to estimate adhesion formation at 2, 4, 8, and 12 weeks after the operation. Maximum tensile load to flex the distal interphalangeal joint 50 degree from its resting state(MTL50) was measured, depicting the amount of adhesion formed. Subsequently, breaking strength was assessed. Results: There were no postoperative complications such as infection, wound dehiscence, or hematoma. MTL50 was significantly lower in the experimental group than in the control group at 4, 8, 12 weeks (p<0.05). Mean value of MTL50 was 6.64N in the experimental group and 28.53N in the control group at 12 weeks after surgery. There were no significant differences in breaking strength. Conclusion: Our results indicate that Guardix$^{(R)}$ is helpful in reducing adhesion formation and does not interfere with normal healing processes of the tendon.
