Self-Care, Symptom Experience, and Health-Related Quality of Life by COPD Severity (만성 폐쇄성 폐질환의 중증도에 따른 자가관리, 증상경험 및 건강관련 삶의 질)

  • Purpose: This study aimed to define the relationship among self-care, symptom experiences, and health-related quality of life(HRQoL) according to the severity of disease in chronic obstructive pulmonary disease(COPD) patients. Methods: The participants, 195 COPD patients, were recruited at a pulmonology clinic of a universityaffiliated medical center in B metropolitan city. Inclusion criteria were patients who were diagnosed as COPD and had less than 0.7 of $FEV_1$/FVC(Forced Expiratory Volume for 1 second/Forced Vital Capacity) ratio. The severity was classified with GOLD(Global initiative for Obstructive Lung Disease) stages, $FEV_1%$ predicted value from mild to very severe. The data were gathered by the medical records and self-reported questionnaires. Results: The self-care, symptom experience, and HRQoL were all significantly different by COPD severity based on pulmonary function(p<.05). There were significant moderate negative relationships between symptom experiences and self-care(r=-.54, p<.001) and between symptom experiences and HRQoL(r=-.64, p<.001). There was a moderate positive correlation between self-care and HRQoL(r=.63, p<.001). Conclusion: It suggests that the assessment of disease severity based on pulmonary function and subjective symptom experience in patients with COPD may be a key component to develop a tailored self-management program and to improve their quality of life.

Relationship between Psychosocial Factors and Symptom Experience in Patients with Cardiovascular Disease: Over a Six-month follow-up Period (심장질환자의 심리사회적 요인 및 증상 경험간의 관계 : 6개월 추적조사)

  • Purpose: The purpose of this study was to identify the relationship between psychosocial factors and symptom experience of patients with cardiovascular disease over a 6-month follow-up period. Method: Baseline data for each of the 138 patients were collected. Eighteen patients were dropped for a final total of 120 patients in the present study. To monitor patients' symptom experience after discharge, each patient was interviewed by telephone, and interviewed directly with a questionnaire at each clinic visit. Results: Mean scores for hostility, anger, anxiety, and depression were 51.63, 22.87, 48.95, and 41.21, respectively. Mean scores for the level of symptom experience at discharge, 3 months and 6 months after discharge were 32.83, 24.79, and 26.70, respectively. There were significant differences in the level of hostility by gender, BMI, job, and monthly income. Also the differences in the level of anxiety and depression were statistically significant according to gender and existence of spouse. In the regression analysis, depression was identified as the significant factor associated with symptom experience at discharge, 3 months, and 6 months after discharge. Conclusion: This study presented baseline data to elucidate whether psychosocial factors were associated with the progression of cardiovascular diseases. Accordingly, it suggests that nurses need to take into consideration psychosocial factor as well as physiological factor in symptom management for patients with cardiovascular disease.

AI-powered Paralichthys olivaceus Disease Symptom Detection and Classification Method (인공지능 기술 기반 넙치 질병 증상 감지 및 분류 기법 )

  • In this study, we propose a deep learning-based disease symptom detection and machine learning-based disease classification technique to quickly diagnose Paralichthys olivaceus diseases. The disease symptom detection technique, using YOLOv9 model, showed excellent accuracy of about 85.4% of mAP and 75.2% of F1-score. The disease classification, an ensemble model using SGD classifier and random forest classifier was used, and the overall good correct answer rate was about 72% for Top-1, 83% for Top-2, and 94% for Top-3. As a result of testing by combining the two algorithms, when the threshold was not set in the disease classification result, it showed good performance of 96%. The Paralichthys olivaceus disease symptom detection and classification technique proposed in this study can take quick and effective disease treatment and prevention measures by the two algorithms working together to accurately detect disease symptoms and classify disease information.

  • In order to nursing practice of the patient in Oriental medicine, it is suggested that the fundamental recognition of Oriental medicine should be preceded. It is, however, difficult that we comprehend Oriental medicine generally since it is different from Western medicine in the point of th symptom. We have thought that is to be carried out to select and analyze a disease by the sense of Oriental medicine for the establishment of Oriental medical nursing. So we have tried out the analysis of Tae-Yang In view of the results so far achieved, it has been suggested that Tae-Yang Symptom applies to the first step of Sanghan Yug Kyung Symptom and consists of Palsy, Sanghan, Onbyung, Dropsy, and Congestion. In the Oriental medicine, Tae-Yang Symptom is recognized to be concerned with a common cold and the respiratory, renal and hepatic diseases. In the points of Orintal medicine, it is noticed that Tae-Yang Symptom is caused by the wind and cold evil, is related to human resisting force, and is fused with each other. And the treatment of Tae-Yang Symptom is various by the cause and the pathological mechanism. In the points of Western medicine. it is difficult to comprehend that various disease germs revolving each disease are implied by identical symptom. The summary of this study are as follows ; 1. In the outer-caused diseases. so called Tae-Yang Symptom, it is an important index to the patient's resisting force and the type of a disease whether he sweats, chills and the pulse is tense or not. 2. The treatments are various according the body's resisting force and the type of symptoms; harmonizing Yung & Wee(調榮衛) to Weaknees of surface(表虛證), sudorifics flourishing of evils(表實證), and antifebriles to On-byung(溫病). 3. If Tae-Yang Symptom is not cured, it progresses to develop complications ; Dropsy(蓄水) & congestion(蓄血), the former brings about renal diseases and the latter hepatic diseses. According to the resuslts mentioned above, we have come to the conclusion that the Oriental - medical nursing must emphasis the body's sesiting force and the type of symptoms rather than the name of a disease.

The Mediating Effects of Resilience between Symptom Experience and Depression in Patients with Chronic Obstructive Pulmonary Disease (만성 폐쇄성 폐질환 환자의 증상경험과 우울의 관계에 미치는 극복력의 매개효과)

  • Purpose: The purpose of this study was to identify the relationship of resiliency and symptom experience and depression. Subjects (N=152) were patients with chronic obstructive pulmonary disease (COPD).admitted to D University Medical Center in B City. Methods: Data collection was conducted from July 20 to August 25, 2013. The levels of symptom, resilience, and depression were measured using a seven question symptom experience measuring instrument, a 25-question resilience instrument, and a 15-question Short Geriatric Depression Scale (SGDS). The collected data were analyzed by descriptive analysis, Pearson's correlation coefficients, and path analysis using SPSS/Win 19.0 and AMOS 5.0. Results: There was a significant negative correlation between symptom and resilience, a significant positive correlation between symptom and depression, and a significant negative correlation between resilience and depression. Symptom showed a direct effect on depression and an indirect effect on depression scores through resilience as a mediating variable. Conclusion: It is necessary to develop nursing intervention programs to relieve symptom in COPD patients, and to develop appropriate resilience enhancement programs to reduce their depression.

  • After We treated Beriberi disease belonging to the argument of interior-overheating-symptom of the Tae-Eum-In caused by liver's receiving heat due to Seven emotional excess with Constitutional prescription. we had a Significant effect on patient's Character and Beriberi disease. Therefore, if we add some Herbs to Constitutional prescription according to patient's Symptoms. we think we can maximize The Effect and Application of Constitutional prescription.Through this Clinical analysis. We think we need a Systematic study on applying This Principle to more Various diseases.

  • Bian-zheng(辨證) of Korean Medicine(KM) is based on four examinations(四診) of Korean medical doctor. The interrogation or questioning(問診) provides the most information of four examinations. The symptom obtained from the interrogation or questioning is the main basis of the Bian-zheng. KM is understood in the whole state of the body of a specified time without seeing the disease exist. So the observable symptom is disease itself. Symptom in KM is used as an important basis for the diagnosis. But if the interview when memories are not sure of the correct answer does not get much easier to find exactly the symptoms. So when recording original symptom(素證) and daily subjective symptom can be helpful for care. In this paper, we propose daily living symptom record system as a method that can be applied to the health care according to the importance of collecting the symptom in the KM. Daily living symptom record system can record the symptom in the individual to awaken daily. The system stores the symptom in structure and provides an open shared services. So it can be used as a symptom of other systems, such as PHR, EMR, CDSS. In addition, Doctor may be able to help in the treatment determined by reference to shared symptom.

  • Objectives This research was proposed to present Clinical Practice Guideline(CPG) for Taeeumin and Taeyangin Disease of Sasang Constitutional Medicine(SCM): Diagnosis and Algorithm. This CPG was developed by the national-wide experts committee consisting of SCM professors. Methods We searched the literature and articles related to Taeeumin and Taeyangin Symptomatology diagnosis and algorithm. For developing diagnosis and algorithm, we searched the classification, ordinary symptom, present symptom of the Taeeumin and Taeyangin Symptomatology Results and Conclusions We classified the Taeeumin Symptomatology by 4 steps: Exterior-Interior disease, favorable-unfavorable pattern, mild-moderate-severe-critical pattern, initial-intermediate-advanced pattern. And we classified the Taeyangin Symptomatology by 3 steps: Exterior-Interior disease, favorable-unfavorable pattern, mild-moderate-severe-critical pattern. At the unfavorable pattern, ordinary symptom is very important. So doctors are considered to need to focus on the symptom of unfavorable's ordinary symptoms.

Effect of Environmental-Friendly Control for White Stain Symptom on Grape (포도흰얼룩증상의 친환경방제 효과)

  • BACKGROUND: White stain symptom caused by Acremonium acutatum and Trichothecium roseum, is one of the most important diseases on the grape. This disease occurs national-wide in Korea and causes irreversible damage on grape, at harvest season. This study was conducted to develop environment-friendly control method against white stain symptom. METHODS AND RESULTS: Environment-friendly materials were tested for control activity against A. acutatum and T. roseum in vitro and in vivo. The effect of environment-friendly materials against white stain symptom on grape in farmer's greenhouse was examined. The materials, NaDCC and sulphur, were sprayed three times on the leaves and fruit of grape at the two sites of farmer's field, Anseong and Hwaseong of Gyeounggi. To evaluate control effect of NaDCC and sulphur against white stain symptom, the disease severity was investigated after the two materials were sprayed from the disease onset three times at the 7-day intervals. NaDCC showed efficacy of 59.71% for Kyoho and 72.26% for Campbell-Early, and sulphur showed 78.31% for Koyho and 66.19% for Campbell-Early. CONCLUSION: We were selected sulphur and Sodium-Dichloroisocyannurate(NaDCC) based on the results of experiments in vitro. In field test, sulphur and NaDCC showed suppressive effect in the white stain symptom of grape. These results suggested that NaDCC and sulphur selected can be used as control agents for controlling white stain symptom on grape.
