Association of periodontitis-related bacteria complex with socio-demographic and oral health condition among the elderly in a rural area (일부 농촌지역 노인의 일반 특성 및 구강상태와 치주염유발세균의 관련성)

  • Objectives: The objectives of this study were to investigate the distribution and level of periodontopathic bacteria with the general characteristics and oral health condition of the elderly. Methods: A total of 335 elderly individuals aged 65 years or older who lived in Ganghwa-gun, Incheon, were included in the study. Oral examination, investigation through a questionnaire, and collection of saliva were carried out. The collected saliva was analyzed for the distribution and levels of bacteria (red and orange complex bacteria) by real-time polymerase chain reaction. Statistical analyses were performed using chi-square test, t-test, one-way analysis of variance, and Pearson's correlation coefficient with SAS statistical software version 9.4. Results: Among the general characteristics, there were significant differences in the distribution of Porphyromonas gingivalis, Treponema denticola, and Parvimonas micra depending on sex, age, and dental visits (p<0.05). The number of remaining teeth and denture use were related to the distribution of periodontopathic bacteria, except T. denticola (p<0.05). Additionally, periodontitis was related to the distribution of P. gingivalis (p<0.05). As the number of remaining teeth increased, the copy number of red and orange complex bacteria also increased (p<0.05). Those individuals who did not use dentures and had periodontal disease had more periodontopathic bacteria (p<0.05). Conclusions: The distribution and copy number of periodontopathic bacteria in the elderly were more related to oral health condition than to general characteristics. In particular, the distribution and copy number of periodontopathic bacteria were higher in subjects with multiple remaining teeth, no dentures, and periodontal disease.

A Case of Gorham-Stout Disease with Life-threatening Chylothorax Successfully Treated with the Combined Therapy of mTOR Inhibitor and Beta-blocker (mTOR inhibitor와 beta-blocker 병합요법으로 성공적으로 치료된 Gorham-Stout 질환)

  • Gorham-Stout disease is a rare disorder characterized by lymphovascular proliferation and destruction of osseous matrix. The etiology of this condition remains poorly understood. Chylothorax as a consequence of lymphatic leakage in thoracic cage may cause a severe life-threatening complication, accompanying respiratory difficulty. Currently, there is no standard management for this extremely rare condition. Here we describe a patient affected by Gorham-Stout disease successfully managed by the combined treatment of mTOR inhibitor and beta-blocker. A previously healthy 11-year-old female developed dyspnea and chest pain with a massive pleural effusion. The ligation of right thoracic duct and bilateral pleurodesis temporarily decreased her pleural effusion, which was aggravated repetitively and required frequent admission and tube thoracotomies. Along with bilateral pleural adhesiolysis with thoracotomy, the combined treatment of oral beta-blocker and mTOR inhibitor was commenced. After 1 month of oral medication, her pleural effusion was not increased and she was free of respiratory difficulty on room air without chest tubes. Over eleven months of treatment, no serious adverse reaction was noted and her condition has been stable with no further admission required.

  • Familial idiopathic basal ganglia calcification[FIBGC] is an inheritable neurological condition characterized by calcium deposits in the basal ganglia and extra-basal ganglia areas. The condition manifests as parkinsonism and other variable neuropsychiatric symptoms. FIBGC is a rare condition, and its pathophysiology has not yet been fully elucidated. Here we report the results of a clinical study of two related patients diagnosed with FIBGC.

Ecology of Disease Outbreak of Circular Leaf Spot of Persimmon and Inoculum Dynamics of Mycosphaerella nawae (감나무 둥근무늬낙엽병의 발생과 병원균(Mycosphaerella nawae)의 전염원 동태)

  • The circular leaf spot of persimmon is occurred almost every place where persimmon is cultivated, especially the disease outbreak severely in southern part of Korea. The disease reveals unusually long incubation period after pathogen invade into leaf tissue and no practical control measure is available once the symptom has appeared. Most of the farmers just follow the suggested spray schedules calculated on the basis of weather condition of ordinary years. Therefore the damages due to circular leaf spot greatly differ year after year. In this article, we tried to describe and summarized the investigation on the circular leaf spot pathogen, Mycosphaerella nawae, related to disease outbreak such as overwintering of pathogen, inoculum formation and spread, incubation period after infection, and secondary inoculum. With the summary of these results, we suggest the disease cycle of circular leaf spot of persimmon. The pathogen overwinters in diseased leaves as mycelial form or pseudoperithecial premodium. The pseudoperitheria become matured in spring as the temperature raise and forms asci and ascospores. The maturation of pseudoperithecia are closely related to the temperatures during March and early April. The ascospores completely mature in early May and the ascospores released when the pseudoperithecia absorbed enough moisture after rainfall. The release of ascospores are diverse greatly with the variation of maturity of pseudoperithecia. Generally the spore start to release from middle of May to early of July. Duration of ascospore release is depend on the weather condition of particular year, especially amount and number of precipitation. The ascospores produced from pseudoperithecia is known to the only inoculum for circular leaf spot disease. But according to the results obtained from our investigations, the conidia formed on the lesions which incited by natural infection. This conidia are infectious to persimmon leaves and formed identical symptom as natural infection. The time of producing secondary inoculum of circular leaf spot of persimmon is considered too late to develop new disease. Generally the importance of secondary inoculum is low but the conidia produced in early September are competent to develop new disease and new infection also significantly affect to harvest of persimmon. The importance of circular leaf spot disease is recognized well to farmers. The approaches to control of the disease should be initiated on the basis of the knowledges of inoculum dynamics and ecology of disease development. The forecasting system for circular leaf spot is need to be developed.

A Clinical Study on Liver Disease Sequelae in Soyangin Treated by Sasang Constitutional Treatment (사상체질방으로 간질환 후유증을 호소하는 소양인(少陽人) 치험 1례)

  • Objectives : The purpose of this case study is to report the improvement after Sasang Constitutional treatment to patient who has liver disease sequelae. Methods : This patient was treated by Soyangin's Constitutional Medications according to the result of Sasang Constitutional diagnosis. We checked visual analog scale(VAS) to evaluate the symptoms of liver disease sequelae. Results : The general condition of the patient improved by the end of hospitalization. The visual analog scale(VAS) of general weakness dropped from 10 to 3. Other symptoms also showed improvement. Conclusions : This case study shows Sasang Constitutional treatment has effectiveness on liver disease sequelae.

Diagnosis of incomplete Kawasaki disease

  • Several authors suggested that the clinical characteristics of incomplete presentation of Kawasaki disease are similar to those of complete presentation and that the 2 forms of presentation are not separate entities. Based on this suggestion, a diagnosis of incomplete Kawasaki disease in analogy to the findings of complete presentation is reasonable. Currently, the diagnosis of incomplete Kawasaki disease might be made in cases with fewer classical diagnostic criteria and with several compatible clinical, laboratory or echocardiographic findings on the exclusion of other febrile illness. Definition of incomplete presentation in which coronary artery abnormalities are included as a necessary condition, is restrictive and specific. The validity of the diagnostic criteria of incomplete presentation by the American Heart Association should be thoroughly tested in the immediate future.

  • Purpose: The study was conducted to find out clinical characteristics for coronary artery disease patients with diabetes mellitus. Methods: We retrospectively reviewed the electronic medical records which included the data of 6,792 patients, who had been diagnosed coronary artery disease (CAD) such as angina or acute myocardial infarction and admitted to a university hospital in Seoul from January, 2005 to November, 2010. Results: Of the 6,792 patients, 43% had been diagnosed diabetes as comorbidity. The CAD patients with diabetes had lower left ventricular ejection fraction, stayed longer at hospital, and spent on more time from the first symptom to hospital visit than those without diabetes. In addition, they were more likely to have multi vessel coronary artery disease. Conclusion: The CAD patients with diabetes lay on the various factors which can make more worsen condition. Hence, we need to pay attention to specialized nursing care and patient education for the CAD patients with diabetes.

Mondor's Disease after Immediate Breast Reconstruction with Silicone Implant (실리콘 보형물을 이용한 즉시 유방재건 후 발생한 Mondor's Disease 치험례)

  • Purpose: Mondor's disease is commonly known as a benign breast condition after augmentation mammaplasty, and some authors have also reported its association with other breast surgeries such as reduction mammaplasty or axillary lymph node biopsy. Here we report two cases of Mondor's disease after immediate breast reconstruction with silicone implant. Methods: Two women, 51-year-old and 36-year-old, underwent immediate breast reconstruction with silicone implants after nipple-areolar skin-sparing mastectomy. Results: Subcutaneous cord-like firm lesion appeared on upper abdomen, axillary area following surgery. The lesion was painless and spontaneously subsided with no medications. Conclusion: To our knowledge, this is the first report of Mondor's disease developed after immediate breast reconstruction using silicone implant.

  • I studied on the correlativity with an individual constitution and etiological factor, the results are as follows. As the external wickedness invade into human body, the condition of an individual constitution determine whether being attacked with a disease or not. Moreover the contracting and the tendency of a disease are determined from the peculiarity of an individual constitution. And whether being attacked with a disease or not, the contracting and the tendency are possible explained of DONG-GI-SANG-GU(the indentical vigour looking for mutually). The division on the malice for cause of a disease and the symptom of a disease are more and more necessary.

Survey on Actually Infected Condition of Aujeszky′s Disease to the Consigned Pigs in Seoul from 1990 to 1993 (90~93년도 서울 지역에 출하된 돼지의 Aujeszky′s병 감염 실태 조사)

  • The porcine Aujeszky's disease was surveyed by Enzyme Immunodiffusion method and serologic neutralization test to the slaughtered pigs at slaughtehouse only in Seoul from March, 1990 to October, 1993. After detecting the positive by enzyme immunodiffusion method primary, we decided finally the positive by serologic neutralization test secondary. Results obtained through the experiments were summarized as followed; 1. The positive of Aujeszky's disease in March, 1990 was 2 of 1,000 sera. 2 positive were decided as the consigned pigs in Kalsan, Hongsung, Chungnaa 2. The positive of Aujezsky's disease in May, 1991 was decided 1 serum at Pogok, Yongin, Kyeonggi. 3. All of the positive detected by Enzyme immunodiffusion Method in 1993 were decided finally in the negative sera. 4. The positive sera detected in 1992 were decided 32 sera at Gyeonggi, 6 at Chungnam, and 1 at Gangwon. Especially, the positive sera percentage detected by Kit Latex Aujeszky Test appeared 78.04% at Gyonggi and by enzyme immunodiffusion Method appeared 11.11% at Chungnam and Gangwon.
