  • 1. 순측접근법은 치수치료를 순측으로 시행한 후 변색된 순측의 치질을 확대하여 삭제하고 레진으로 순면전체를 수복함으로써 치수치료와 동시에 심미성을 회복할 수 있다. 2. 순측으로 치수치료를 시행하므로 시야가 확보에 유리하고 기구조작이 용이해 행동조절이 잘 되지 않는 어린이에게 추천된다.

지르코니아 코어의 두께에 따른 변색치와 메탈 포스트의 색조차단 효과 (Effect of Zirconia Core Thickness on the Tone Blocking of Discolored Tooth and Metal Post)

  • Purpose: The purpose of this study was to investigate the correlation between zirconia core thickness and color tone blocking of discolored tooth and metal post. Methods: For this study, we made 20-porcelain sample and 4-metal sample( liechtenstein IPS e.max) bonded to zirconia core of different thickness with cement(Relyx ARC-3M USA)for produce discolored. We measured the color-spectral characteristics, using Shadepilot equipment(Degudent USA).We measured it with Shadepilot equipment set by automated average mode in 3 times And applied the average value obtained from 2 times of measurement in the middle of each sample. Results: As a result of analysing color-spectral characteristics on zirconia core sample, Depending on the thickness of zirconia core, the value of brightness(${\Delta}L*$:color-spectral characteristic) was increased within limited range, value of ${\Delta}a*$, ${\Delta}b*$ was decrease. Conclusion: Consequently, we obtained the following results: Changes of sample color were observed depending on the thickness of zirconia, but the range of change did not exceed the scope range of shade guide. The case of metal posts, shade guide color D2 were observed in 0.5mm of zirconia core thickness. As a result, in case of porcelain, increasing the zirconia thickness of 0.3mm or more is unnecessary for color blocking effect, in case of metal post, considering the discolored tooth, thickness of zirconia with at least 0.5mm or more is recommended.

Use of ultrasound Doppler to determine tooth vitality in a discolored tooth after traumatic injury: its prospects and limitations

  • When a tooth shows discoloration and does not respond to the cold test or electric pulp test (EPT) after a traumatic injury, its diagnosis can be even more difficult due to the lack of proper diagnostic methods to evaluate its vitality. In these case reports, we hope to demonstrate that ultrasound Doppler might be successfully used to evaluate the vitality of the tooth after trauma, and help reduce unnecessary endodontic treatments. In all three of the present cases, the teeth were discolored after traumatic injuries and showed negative responses to the cold test and EPT. However, they showed distinctive vital reactions in the ultrasound Doppler test during the whole observation period. In the first case, the tooth color returned to normal, and the tooth showed a positive response to the cold test and EPT at 10 wk after the injury. In the second case, the tooth color had returned to its normal shade at 10 wk after the traumatic injury but remained insensitive to the cold test and EPT. In the third case, the discoloration was successfully treated with vital tooth bleaching.

Anatomical analysis of the resected roots of mandibular first molars after failed non-surgical retreatment

  • Objectives: Understanding the reason for an unsuccessful non-surgical endodontic treatment outcome, as well as the complex anatomy of the root canal system, is very important. This study examined the cross-sectional root canal structure of mandibular first molars confirmed to have failed non-surgical root canal treatment using digital images obtained during intentional replantation surgery, as well as the causative factors of the failed conventional endodontic treatments. Materials and Methods: This study evaluated 115 mandibular first molars. Digital photographic images of the resected surface were taken at the apical 3 mm level and examined. The discolored dentin area around the root canal was investigated by measuring the total surface area, the treated areas as determined by the endodontic filling material, and the discolored dentin area. Results: Forty 2-rooted teeth showed discolored root dentin in both the mesial and distal roots. Compared to the original filled area, significant expansion of root dentin discoloration was observed. Moreover, the mesial roots were significantly more discolored than the distal roots. Of the 115 molars, 92 had 2 roots. Among the mesial roots of the 2-rooted teeth, 95.7% of the roots had 2 canals and 79.4% had partial/complete isthmuses and/or accessory canals. Conclusions: Dentin discoloration that was not visible on periapical radiographs and cone-beam computed tomography was frequently found in mandibular first molars that failed endodontic treatment. The complex anatomy of the mesial roots of the mandibular first molars is another reason for the failure of conventional endodontic treatment.

Choice of resin cement shades for a high-translucency zirconia product to mask dark, discolored or metal substrates

  • PURPOSE. The aim was to study the masking ability of high-translucency monolithic zirconia and provide guidance in selecting resin luting cements in order to mask discolored substrates. MATERIALS AND METHODS. 160 high-translucency zirconia specimens were divided into 32 groups depending on their thickness and shades. Using five shades of try-in paste, the specimens were luted onto the substrates (Co-Cr, precious-metal, opaque porcelain-sintered Co-Cr, opaque porcelain-sintered precious-metal, and 5M3-shade zirconia). All CIELAB color parameters were measured and statistically analyzed. RESULTS. Zirconia shade and thickness and try-in paste shade affected CIELAB color parameters (P=.000) in different substrates groups, and there were interactions among these factors (P=.000). All five try-in paste shades can be chosen to achieve ${\Delta}E$ values of zirconia with 1.2 - 1.5 mm for masking dark-tooth-like 5M3-shade and zirconia with 1.5 mm for masking precious-metal groups < 2.6. Only suitable try-in paste shades were used, can ${\Delta}E$ values that less than 2.6 be achieved when applied translucent monolithic zirconia with 0.7-1.0 mm for masking dark-tooth-like 5M3-shade and zirconia with 0.7 - 1.2 mm for masking precious-metal groups. CONCLUSION. Choosing suitable resin cement shades is necessary for high-translucency monolithic zirconia to achieve ideal masking ability (${\Delta}E$ < 2.6) on the dark-tooth.

미백제가 법랑질 명도 변화에 미세경도에 미치는 영향 (The Effect of Tooth Whitener on the Color and Microhardness of Human Enamel in Vitro)

  • Human teeth vary widely in color. Practitioner and patients are concerned with preventing and correcting discolored or dark teeth to achieve and maintain stain-free, white teeth. Tooth brushing cannot alter tooth color but it can remove adhering films and stains. The esthetics of natural dentition can be improved by bleaching and this process can be applied to intrinsically and extrinsically stained teeth. The need for a brighter, more attractive smile has made rapid growth in the market for tooth whiteners. There is no doubt these products work as whiteners, at least on mild to moderate stains, but the safety of these products are unclear. In this experiment, the effect of tooth whitener application on the color and microhardness of extracted human enamel was measured. RMS, RMT and NWT were used as tooth whiteners, and tooth paste(ETQ) and hydrogen peroxide solution(HPO) were used as controls. 35 caries-free extracted human molars were embedded and polished with the exposed enamel diameter of 4 mm. The tooth whiteners and control agents were applied according to the manufacturers' instructions or clinically simulated procedures for eight weeks, and measurements were repeated every two weeks. Value(L*) difference was measured using Differential Colorimeter(Model TC-6FX, Denshoku Co., Japan), and microhardness was measured using microhardness tester(Mitsuzawa Seiki Co., Japan). The results were as follows; 1. After application of agents for eight weeks, the Vickers hardness increased significantly in the ETQ, RMS and RMT application group(p〈0.01), and that decreased significantly in NWT application group(p〈0.01), but in HPO application group there was no significant change. The change in microhardness was greatest in NWT application group(p〈0.01). 2. After application of tooth whiteners and controls for eight weeks, the value change of toothpaste application group was significantly lower than those of other agents groups(p〈0.01), and there was no significant difference in value(L*) change among tooth whitener groups(p〉0.01). 3. The application of tooth paste and paste type tooth whitener made gradual value change, but hydrogen peroxide gel type tooth whitener and hydrogen peroxide solution made rapid value change during initial application period.

전치부에서의 연조직 관리를 통한 심미적 접근 (Esthetic Approach Focusing on Soft Tissue Management in Anterior Dentition: Case Reports)

  • A tooth solely can not be an exclusive factor to attain our goal, esthetics. Other consideration, such a proportion of individual tooth, harmonized arrangement of the dentition and surrounding tissues including gingiva, lip and face should be taken into account. Esthetic gingival pattern and incisal level in the anterior area are the very important factors for esthetic restoration of the smile. We often propose adequate periodontal and prosthodontic approaches in order to improve the esthetics of the anterior area. Here, I would like to share some cases which include soft tissue management for ovate pontic placement, osteoplasty, resin filling for severe cervical caries, and bleaching approach to discolored root caused by endodontic treatment, and so on.

  • Author examined clinical procedure, clinical application and clinical failure of castable glass ceramic (Dicor) crowns which were made at the Dept. of Prosthodontics, Seoul National University Hospital during 19 months from Jan. 1988 to July, 1989(Observation period:19 months). The author obtained the following results. 1. A total of 86 crowns were constructed ; Most of them has been set on maxillary central incisors(53 cases, 62%):maxillary lateral incisors(20 cases, 23%):maxillary canines(6 cases, 7%):others(7 cases, 8%) 2. Dicer could be used as a lamiate veneer for discolored tooth. 3. During the examination period(1-19 months), there was no fracture case. 4. Dicer could be applied on the maxillary anterior tooth with minimal occlusal force.

배경 색조 차단 목적의 불투명 복합 레진의 두께와 반투명도 (Thickness and translucency of opaque shade composite resin for masking effect)

  • The aims of this study were to evaluate the adequate thickness of opaque resins for situations such as an oral black cavity and discolored tooth structure, as well as the translucency of each opaque material at various thicknesses. Six opaque-shade composite resins (Z-350 OA3, Amelogen Universal A2O, Esthet-X A2O, Esthet-X A4O, Charmfil UO and Aelite Universal OA3) were prepared in metal molds with a hole of 8 mm in diameter and various thicknesses (0.5mm, 1.0mm, 1.5mm, 2.0mm, 2.5mm, 3.0 mm and 4.0mm). Four backgrounds (white tile, black tile, C4 shade porcelain and opaque resin itself) were used to determine the translucency parameter (between black and white backgrounds). and to mimic a black oral cavity (between black and opaque resin backgrounds) and a discolored tooth structure (between C4 and opaque resin backgrounds). Color measurements were made by a colorimeter to determine the CIELAB values of each specimen with each background and to calculate the translucency parameter and ${\Delta}E^*$ value difference among the specimens on the backgrounds. The translucency parameter and ${\Delta}E^*$ obtained between black and opaque resin backgrounds decreased in similar pattern as thickness increased. A C4 background was masked by resin thicknesses of 0.5-1.0mm, while a black background required thicknesses of 1.0-2.0mm. Adequate knowledge about differences in the optical character like translucency of the materials used is essential, together with the accumulated experience of the individual clinician.
