Rabbit Model for in vivo Study of Intervertebral Disc Degeneration and Regeneration

  • Objective: The purpose of this study is to verify the usefulness of the rabbit model for disc degeneration study. Materials: The L1-L2, L2-L3, L3-L4. or L4-L5 lumbar intervertebral disc (IVD) of 9 mature male New Zealand White rabbits were injured by inserting a 16-gauge needle to a depth of 5 mm in the left anterolateral annulus fibrosus while leaving L5-L6 IVD uninjured. Three other rabbits also received intradiscal injections of rabbit disc cells transfected with adenovirus and bone morphogenetic protein-2 (ad-BMP-2) at L4-L5 in addition to injury by 16-gauge needle at the L1-L2 level. Using digitized radiographs, measurements of IVD height were made and analyzed by using the disc height index (DHI). Magnetic resonance imaging (MRI) scans of the injured discs, injected discs, and uninjured L5-L6 discs were performed at 15 weeks post surgery and compared with preoperative MRI scans. Results: All twelve rabbits showed consistent results of disc degeneration within 15 weeks following annular puncture. DHIs of injured discs were significantly lower than that of the uninjured L5-L6 discs (p<0.05). The mean value of disc degeneration grade of injured discs was significantly higher than that of uninjured discs (p<0.05). The injection of disc cell transfected with ad-BMP-2 did not induce disc regeneration at 15 weeks after injection. Conclusion: This study showed that the injured disc had a significant change in DHI on simple lateral radiograph and disc degeneration grade on MRI scans within 15 weeks in all rabbits. Rabbit annular puncture model can be useful as a disc degeneration model in vivo.

악관절장애환자의 하악과두 크기에 관한 연구 (A Study on the Size of TMD Patient's Condyle Head)

  • Objective: In many TMD cases, deformed and reduced condyle heads were frequently observed. This study was prepared to compare the dimensions between normal and symptomatic condyles, using MR images. Materials: One hundred and twenty one patients with clinical signs and MRI-confirmed diagnosis of disc displacement were selected for this study. Thirty eight TMJs from nineteen asymptomatic volunteers who had no clinical symptoms and no disc displacement on sagittal and coronal view of MRI, were served as normal. Methods: Symptomatic condyles were classified according to the severity of the anterior disc displacement. The amount of anterior disc displacement was evaluated at sagittal section, and they were classified into 4 groups as normal(N), little(G0), mild(G1), moderate(G2) and severe displacement(G3). The dimentions of condyle were measured at the 200% magnified view, by digitizing program. All dimensions were compared among each groups on the central section of sagittal and coronal views, and the statistical analysis was performed. Results: The mean value of anteroposterior length of normal condyle was $0.79{\pm}0.13cm$ at sagittal section and mediolateral length was $2.12{\pm}0.22cm$ on coronal section. The mean value of anteroposterior length of symptomatic condyle was $0.67{\pm}0.16cm$ at sagittal section and mediolateral length was $1.97{\pm}0.28cm$ on coronal section. Conclusions: The size of symptomatic condyle was smaller than normal TMJ. The size of condyle was decreased as the amount of the disc displacement was increased. The dimensional change was found on the anterior articular surface of condyle at the mild or moderate disc displacement. And at the case of severe disc displacement, dimensional change was found on the superior articular surface.

  • The present research aims to develop the aluminum disc brake replacing the existing cast iron disc brake. Material such as aluminum using FEM numerical analysis in order to improve the characteristics of each element, we analyze the performance characteristics and braking time you try to change. We try to lay the foundation for the development of an aluminum disc by investigating performance characteristics of the existing cast iron disc brake and comparing them with those of the aluminum disc. This involves FEM dynamics analysis for disc materials and experimental tests using the brake dynamometer. From this study, the results of 7075 aluminum braking performance can be seen that the best.

악관절원판의 전방변위에 따른 관절간극의 변화 (CHANCE OF JOINT SPACE ACCORDING TO ANTERIOR DISC DISPLACEMENT OF TMJ)

  • Objective: Disc displacement may affect the joint space narrowing between condylar head and glenoid fossa. This study was designed to evaluate the correlation between the joint space change and the severity of anterior disc displacement. Materials and Methods: Two hundreds temporomandibular joints MR images of TMD patients(170 joints) and asymptomatic volunteers(30 joints) were evaluated for this purpose. Anterior disc displacement was divided into 3 stages(normal, little to mild, and moderate to severe displacement) based on sagittal images, then joint spaces were measured at medial, central and lateral parts of condyle head on coronal MR images, respectively The joint spaces of 3 groups divided according to the severity of anterior disc displacement were compared. Results: The reduction of joint space was related to the severity of the anterior disc displacement at lateral, medial side and especially at center Conclusion: The temporomandibular joint space was affected by the severity of the anterior disc displacement.

악관절 내장증 환자에서 자기공명영상 소견과 관절원판 후조직의 조직학적 소견과의 연관성 (RELATIONSHIP BETWEEN HISTOLOGIC FEATURES OF THE RETRODISKAL TISSUES AND MAGNETIC RESONANCE IMAGING FINDINGS IN TEMPOROMANDIBULAR JOINT INTERNAL DERANGEMENT)

  • Temporomandibular disorders arised from joint itself was caused by inflammation or mechanical irritation of the retrodiskal tissues. Histologic changes of the retrodiskal tissues of temporomandibular joint(TMJ) internal derangement, such as inflammatory cell infiltration, hyalinization, myxoid change, fatty change, arterial obliteration, and so on, were reported, but relationships between magnetic resonance imaging(MRI) findings and histologic findings of the retrodiskal tissue were few reported. The purpose of this study was to find histologic changes of the retrodiskal tissues for status of joint and joint effusion in MRI of the temporomandibular joint which had surgical treatment. Materials were surgically treated 52 temporomandibular joints with internal derangement or osteoarthritis in TMJ clinic, Yongdong Severance Hospital. All joints were confirmed by magnetic resonance T1- and T2-weighted imagings bofere surgery. Status of joint was categorized by normal position, disc displacement with reduction, disc displacement without reduction, disc displacement without reduction associated with osteoarthrosis using preoperative MR T1-weighted images. Magnetic resonance evidence of joint effusion was observed in T2-weighted images and classified by its amount; degree 0(not observed), degree 1(small amount), degree 2(moderate amount), degree 3(large amount). Histologic features were observed whether the retrodiskal tissue has inflammatory cell infiltration, myxoid change, hyalinization, chondroid metaplasia and arterial obliteration. The distribution of elastic fibers were also observed. The results were as follows; 1. Inflammatory cell infiltration was not observed in any retrodiskal tissues. 2. MRI findings such as status of joint and evidence of joint effusion did not have significant relationship with myxoid change, hyalinization, chondroid metaplasia, arterial obliteration of the retrodiskal tissues. 3. Hyalinization and chondroid metaplasia were found in 8 joints(15.4%) and 5 joints(9.6%). All of them were found in disc displacement without reduction and disc displacement without reduction associated with osteoarthrosis. 4. Arterial obliteration was observed more frequently in disc displacement without reduction(55.6%) than disc displacement without reduction associated with osteoarthrosis(28.6%). Further studies with proper controls and more materials will be necessary.

  • Objectives : To study the cervical lordosis and roentgenographic analysis of cervical disc space in neck pain with or without temporomandibular disorder(TMD). Methods : Radiographic measures of cervical lordosis, cervical disc space narrowing were collected, statiscally analyzed. Results : TMD group showed a significant increase in cervical lordotic angle as compared with non-TMD group, whereas no significant change in cervical disc degeneration. Conclusions : The findings from this study suggest that the curvature of the cervical spine is related to the subject's TMD.

Automated Pressure-Controlled Discography with Constant Injection Speed and Real-Time Pressure Measurement

  • Objective : This study was designed to investigate automated pressure-controlled discography (APCD) findings, to calculate the elastance of intervertebral discs, and to assess the relationship between the calculated elastance and disc degeneration. Methods : APCD was performed in 19 patients. There were a total of 49 intervertebral discs treated. Following intradiscal puncture, a dye was constantly injected and the intradiscal pressure was continuously measured. The elastance of the intervertebral disc was defined as unit change in intradiscal pressure per fractional change in injected dye volume. Disc degeneration was graded using a modified Dallas discogram scale. Results : The mean elastance was 43.0${\pm}$9.6 psi/mL in Grade 0, 39.5${\pm}$8.3 psi/mL in Grade 1, 30.5${\pm}$22.3 psi/mL in Grade 2, 30.5${\pm}$22.3 psi/mL in Grade 3, 13.2${\pm}$8.3 psi/mL in Grade 4 and 6.9${\pm}$3.8 psi/mL in Grade 5. The elastance showed significant negative correlation with the degree of degeneration ($R^2$=0.529, P=0.000). Conclusion: APCD liberates the examiner from the data acquisition process during discography. This will likely improve the quality of data and the reliability of discography. Elastance could be used as an indicator of disc degeneration.

자동차 브레이크용 디스크의 미세조직에 따른 편마모 특성에 관한 연구 (Wear Characteristics of Automotive Disc Brakes: Effect of Gray Cast Iron Microstructures)

  • The objective of the experiment is to investigate the effect of microstructures of gray cast iron oil wear characteristics of automotive disc brakes. Six different gray cast iron rotors were manufactured by changing carbon equivalent and cooling rate. The change of DTV (disc thickness variation) before and after wear tests was measured to examine the wear properties according to the microstructures of gray iron discs since the DTV generation is caused by the circumferential uneven wear. Experimental results showed that the morphology of graphite flake and hardness in gray cast iron were crucially associated with the change of DTV. In particular, the DTV changes of rotor decrease when the length and area fraction of graphite flake in brake rotors increase and hardness of brake discs reduces.
