지체장애인의 건강관련 삶의 질에 영향을 미치는 요인 (Influencing Factors on HRQOL of Physically Disabled Persons)

  • Purpose: The purpose of this study was to investigate the influencing factors on health-related quality of life of physically disabled persons. Method: Data was collected from 96 persons with a physical disability in three cities in Korea from June to September, 2003. Social support and depression were measured by a Social Support Questionnaire 6 ($SSQ_6$) and Center for Epidemiologic Studies Depression questionnaire (CES-D). The Rosenberg Self-esteem Scale was used to measure self-esteem. Health-related quality of life (HRQOL) was measured using Smith Kline Beecham Quality of Life Scale (SBQOL). The SPSS WIN 11.0 version program was used for data analysis. Result: There were significant differences of HRQOL according to monthly income and economic status. All subjects had a high level of depression and low self-esteem. Conclusion: The findings of this study show that self-esteem and the size of the social support network are significant influencing variables on HRQOL in physically disabled persons. Nursing intervention and counseling programs which improve self-esteem and increase the size of a social support network are needed to promote HRQOL in physically disabled persons.

슬관절주위 외상에 의한 슬와동맥 손상의 재혈관화 (Revascularization of Popliteal Artery Injury in Trauma Around Knee Joint)

  • Popliteal artery injury in blunt trauma of knee joint is not common but poses high rate of amputation due to anatomical characteristics or delayed diagnosis and treatment. The aim of the present study is to review the authors' experiences with this condition and identify factors contributing to disability. We reviewed 7 cases of popliteal artery injury in trauma around knee. Injury mechanism, type of vessel damage, associated injuries, mangled extremity severity scores (MESS), ischemic time and additional treatments were analyzed. Tibial fracture, distal femoral fracture and serious soft tissue defect were combined. Mean MESS was 9.9 point and mean time of revascularization was 7.1 hours. Transfemoral amputation was performed in 2 cases due to vascular insufficiency and devastating infection, and 4 patients were able to walk without any support at the last follow up. Age, the severity of soft tissue injury, ischemic time and MESS are thought to be related to prognosis, and young patients with short ischemic time show best results, but authors experienced one exceptional case. We have to consider multiple factors related to the prognosis in popliteal artery injury with fractures around knee, and careful decision is needed regarding to early amputation.

지적장애아동의 식습관 유형과 식품군별 기호도가 적응행동문제에 미치는 영향 (Impact of Eating Habits and Food Preferences on Adoptive Behavior of Children with Intellectual Disabilities)

  • The purpose of this study was to investigate the effects of eating habits and food preferences on the adoptive behavior of children with intellectual disabilities. Survey questionnaires were distributed to six special education schools located in the Daegu-Kyungbuk area, and data were collected from 552 families and teachers with intellectually disabled students. Identification of eating habits began with a factor analysis, and the results were a five-factor solution. Among the five patterns, factors 1 and 2 were significantly related with behavior problems of intellectually disabled children. Further, food preferences of the children were significantly related with adoptive behavior problems. The findings of this study clearly indicate that eating habits as well as food preferences are important factors in identifying adoptive behavior problems in intellectually disabled children. Based on the findings of this study, similarities and differences in eating habits are discussed, and implications for children are provided.

고령 경비원의 직무스트레스와 관련요인 (Occupational Stress and Related Factors among Aged Security Guards)

  • Purpose: Occupational stress is a determinant risk factor for cardiovascular disease and work disability. This study aims to describe occupational stress and identify its related factors among aged security guards. Methods: A random sample of 76 aged security guards were interviewed by three research assistants. The criteria of the subjects were as follows : 50 or older male with 40 or more working hours a week working with colleagues ranged from 3 to 50. The data were collected in September, 2007 and analyzed by multiple regression using SAS Version 9.1. Results: The mean score of occupational stress among aged security guards was 47.2. The mean scores of its sub-scales were 31.3(physical environment), 33.8(occupational demand), 80.8occupational control), 44.6(interpersonal conflict), 51.3 (occupational insecurity), 53.6(occupational system), 57.5(lack of reward), and 30.3(work environment). The scores of occupational control, interpersonal conflict, and occupational insecurity were higher than the scores of Korean average occupational stress sub-scales. Significant variables affecting occupational stress and its sub-scales were work place, work type, and employment type. Conclusion: In order to reduce occupational stress among security guards, sufficient occupational control and interpersonal conflict control, appropriate reward, and employment stability should be considered. Also long working hours and work shift should be improved.

Research on the Quality of Life of the Parents by Functions, Activities, Participation, and Environmental Factors of Children with Cerebral Palsy Using ICF-CY Checklist

  • Purpose: This study examined whether the quality of life of parents of children with cerebral palsy is affected by the functions, activities, participation, and environmental factors using an ICF-CY check list. Methods: This study recruited in 26 parents of children with cerebral palsy. The functions, activities, participation, and environmental factors of the children were evaluated using ICF-CY checklist. To measure the quality of life of parents, this study used world health organization quality of life (WHOQOL-BREF), which was composed of a total of 26 questions. The GMFCS (gross motor function classification system) was used to assess the degree of disability in the children. Multiple regression analysis was performed to examine the effects of the ICF-CY checklist on the quality of life. Correlation analysis was performed to examine the correlation between GMFCS and WHOQOL. Results: The functions, activities, participation, and environmental factors were significantly different from WHOQOL-BREF. On the other hand, the contextual factor showed a significant difference in the neuromusculoskeletal and movement-related functions (b7), and service, systems, and policies (e5)(p<0.05). Conclusion: This study suggests that the functions and environmental factors affect the quality of life of parents of children with cerebral palsy. Therefore, these findings suggest that contextual factors, such as neuromusculoskeletal and movement-related functions (b7), and service, systems and policies (e5), which can be facilitators, should be considered for improving the quality of life of parents of children with cerebral palsy.

Factors Related to Preoperative Shoulder Pain in Patients with Atraumatic Painful Rotator Cuff Tears

  • Background: Patients with rotator cuff tears are usually afflicted with shoulder pain and disability. However, it is unclear which factors are related to shoulder pain in patients with rotator cuff tears. This study was therefore undertaken to determine the factors correlated with shoulder pain in patients with painful rotator cuff tears, but without any history of trauma. Methods: We evaluated a cohort of 745 patients with painful rotator cuff tears having no trauma history, and analyzed the relationship between pain and multiple factors including demographic data, tear characteristics, and passive range of motion. Pain was analyzed with a questionnaire concerning the visual analogue scale (VAS) for pain. Tear characteristics were determined by evaluating tear size, muscle atrophy, number of torn tendons, and presence of arthritis. Multivariate linear regression analysis and chi-squared test were applied to evaluate the relationship between the VAS for pain and variable factors. Results: Shoulder pain was associated with young age (p=0.01), male sex (p=0.01) and the presence of diabetes mellitus (p<0.001). Measurements of rotator cuff tear characteristics including tear size (p=0.53), muscle atrophy (p=0.16) and the number of torn tendons (p=0.34) did not correlate with shoulder pain. Symptom duration (p=0.60) and range of motion (p>0.05) also showed no correlation with VAS for pain. Conclusions: Young age, male sex and the presence of diabetes mellitus correlated positively with preoperative shoulder pain in patients with painful rotator cuff tears without a trauma history. Combined treatment of pain management and risk factor correction could be helpful to control preoperative shoulder pain.

일개지역 저소득층 노인의 신체적 기능상태에 관한 연구 (A Survey on Functional Status among Low-Income Older Adults Living at Home)

  • The purpose of the study was to analyze the functional status of low income elderly living at home according to their socio-economic factors, sensory function, health status, medical service utilization, commodity and types of chronic disease. Method: Functional status was defined by the level of mobility, ADL and IADL categorized as independently functional, mildly impaired, moderately disabled, and severely disabled. The data was collected by home-visit interviews with 567 community dwelling adults who were 65 years of age or more with low a income status subsidized by government in ChonAn. Results: 9.9% of community dwelling older adults were severely disabled, and 44.4% were moderately disabled in their functional status. There were significant differences in the functional status by age, education, religion, and types of family structure. The older adults with hearing impairment or dental problems had a significantly higher rate of severe disability. Self-rated health status and medical service utilization were also significant factors to the differences in functional status. The functional status of older adults was also significantly related to the presence of chronic health problems such as chronic back pain, stroke, and Alzheimer-dementia. Conclusion: The results confirmed that community dwelling older adults with low income status were more functionally disabled in comparison to general older adults at national level, while the relating factors to their functional status seemed similar to other studies on older adults. Further studies were suggested to look into functional status longitudinally and focus on the changes of functional status by managing modifiable influencing factors.

Functions (Mobility, Self-care, Social Ability) and Health-related Quality of Life in Children with Cerebral Palsy

  • Purpose: The purpose of this study was to describe the functional (mobility, self-care, social ability) and health-related quality of life in children with cerebral palsy (CP). Methods: A cross- sectional survey of 202 children with CP, mean age $5.91{\pm}1.57$ years, was carried out using the Gross Motor Function Classification System (GMFCS), Gross Motor Function Measure (GMFM), Pediatric Evaluation of Disability Inventory (PEDI), and Child Health Questionnaire (CHQ). Results: The functional assessment of children with CP showed that a more severe GMFCS level was associated with lower functional abilities (p<0.05). The health-related quality of life assessment showed that psychosocial well-being was less impaired than physical well-being. The internal consistency of the three instruments was satisfactory (cronbach's ${\alpha}$>0.80). The three different scales were correlated from moderate to strong (r=0.44 to 0.92). It was also found that mobility, tone distribution, and the parents' education level exerted a significant effect on the quality of life of children with CP (p<0.05). Conclusion: These findings suggest that children with CP have reduced function and quality of life and these are influenced by various factors. However, planning and application of various task-oriented functional interventions to childhood CP may be useful.

슬관절(膝關節)의 퇴행성 관절염에 대한 고찰 (A Study on Osteoarthritis of the Knee)

  • Osteoarthritis(degenerative joint disease), the most common arthropathy affecting the aged people, is characterized by degeneration of articular cartilage with proliferation and remodeling of subchondral bone. Osteoarthritis of the knee, which probably has greater social cost and more associated disability than osteoarthritis of any other joint, prevalence is known to increase with age, and females have higher rates than males; radiographic abnormalities are present in more than 30% of persons more than 65 years old, with approximately 40% of these persons symptomatic. Though the etiology of osteoarthritis is not entirely understood, much information is available regarding risk factors for the development of knee osteoarthritis that permit some reasonable guideline for preventive strategies. Traumatic damage or occupational or recreational overuse of knee joint may result in osteoarthritis, and obesity also is related to osteoarthritis of the knee. A variety of other possible risk factors for the development of knee osteoarthritis have been proposed, including increased bone mass, smoking, diseases such as diabetes or elevated serum uric acid, and some metabolic factors, but the contribustions of these and other factors such as smoking or race and diseases such as diabetes are as yet undetermined. The usual clinical manifestations include pain, stiffnesss, crepitus and loss of function. In oriental medicine, osteoarthritis of the knee is very similar to diseases such as Bijung(痺症), Haksulpung(鶴膝風), Youkjeolpung(歷節風) in symptoms. The diseases such as Bijung(痺症), Haksuipung(鶴膝風), Youkjeolpung(歷節風) is related to the of function liver(肝) and kidney(腎) and risk factors are regarded as Pung(風), Han(寒), sub(濕). The diagnosis of osteoarthritis of the knee has often been based on radiographic appearance and clinical manifestations. The acupuncture therapy of osteoarthritis of the knee has often been based on Stomach meridian(ST), Spleen meridian(SP), Kidney meridian(KI), Liver meridian(LR).

ICF 관련 범주에 따른 근골격계질환 장애인의 사회참여 관련요인 (Factors Influencing Social Participation in People with Musculoskeletal Conditions - Applying ICF relevant categories -)

  • 본 연구는 WHO의 국제건강관련분류체계인 ICF를 적용하여, 한국의 근골격계질환 장애인(척수손상, 근육디스트로피, 왜소증, 류마티즘) 352명의 사회참여 관련요인을 분석하였다. 연구방법은 기술분석, 집단별 평균분석 및 분산분석과 사후검정, 단순상관분석, 다중회귀분석을 사용하였다. 연구결과 근골격계질환 장애인의 정신기능, 신경근육골격 및 운동과 연관된 기능, 비뇨생식기와 생식기능, 피부 및 관련구조, 학습과 지식 적용, 일반적 과제와 요구, 이동의 수준이 높을수록 사회참여의 수준도 높았다. 개인요인에서 소득이 근육디스트로피질환 장애인의 사회참여 관련요인으로 나타났고, 환경요인에서 서비스, 시스템 및 정책이 척수손상 장애인의 사회참여에 영향을 미치는 것으로 나타났다. 본 연구는 장애에 관한 보편주의적 접근을 의미하는 ICF 구성요소로 연구모형을 설정하고, ICF 코어셋을 검토하여 질환관련범주를 측정하였다는 점에 의의가 있다.
