A Qualitative Study on Attitude, Acceptability, and Adaptation for Home-delivered Meal Services in the Korean Elderly from the Perspective of Life Context (질적 연구를 통한 삶의 맥락적 관점에서의 노인의 도시락 및 밑반찬 배달 서비스에 대한 수용 태도 및 적응에 대한 이해)

  • Objectives: The purpose of this study was to suggest the strategies for improvement of home-delivered meal services for the elderly, to identify reasons for recipients to get started with the services and to evaluate the attitude, acceptability and adaptation of recipients to the services from the perspective of life context. Methods: The data was collected through face-to-face in-depth interviews with eighteen low-income elderly recipients of home-delivered meals and analyzed using a qualitative research method. Results: The results were deduced as four themes which comprised of long-term vulnerable socioeconomic contexts resulted in entry to the services, conflicting acceptability to the services, passive adaptation to taking the services, and positive practices to cope with supplement free meals or other services. The service participation was initiated because of a combination of prolonged, vulnerable socioeconomic contexts, including poverty and unexpected life events such as diseases, disability, living alone, aging and unemployment. With regard to taking the services, conflicting acceptability was observed: positive aspects including saving living cost and good quality of meals, and negative aspects including lack of a tailored service and feeling of stigma. Although the recipients needed an individualized service, they did not express their needs and demands for the services and they accepted the unavailability as an accustomed, prolonged vulnerable socioeconomic context. With regard to lack of tailored services, either self-solution such as modification of eating patterns or community-based network and services were used. Conclusions: We suggest that a system to concretely identify recipients' attitude, acceptability and adaptation for home-delivered meal services should be developed in the establishment of a tailored nutrition support system for the low-income elderly.

A Current Status of Low Back Pain in Rural Peoples (일부 농촌지역 주민들의 요통발병 양상에 관한 연구)

  • Low back pain is generally defined as a complex symptom rather than a specific diseases entity, especially when musculoskeletal lesions of the low back, neurogenic, vasculogenic and discogenic lesions are present. Sixty to eighty percent of all human being have experienced low back pain once or more times during in their lifetime. Low back pain is a one of the commonest cause of physical impairment and induces disability the activities of everyday living or the activities of work. Today, low back pain plays important part for the loss of working capability from social, industrial field and individual daily life. But there are no study about incidence and prevention program of low back pain in rural peoples. This study was conducted to find out current status and characteristics related to low back pain in rural peoples(especially engaged in agriculture), so that could provide basic data to establish prevention program for them. The data were gathered from 301 rural adults(female 62.1%, male 37.9%) with low back pain by the structured interview in the 4 communities(south and north Gyeong-sang Do and Chung-cheong Do), and sampled at local hospital, local clinic, and health clinic center in May, 2000. The results were as followed. About one third of cases(35.5%) were aged from 60 to 69, 34.6% were smoker, and 91.4% not used to regular exercise. Low back pain frequently due to intervertebral articulopathy(42.5%), follewed by herniated intervertebral disk(19.9%), sprain(17.3%), vertebral dislocation(4.0%), and etc(16.2%). Above half of cases(56.5%) were a eight-hour workday, and 61.8% complained of low back pain during work. Mostly, period of agricultural experience was more than 20 years, and cultivated more than 2 crops. Low back pain is common to rural adults, therefore adequate resting time from work and lumbar strengthening exercise program needed to prevent low back pain in rural peoples.

The Effect of Coordinative Locomotor Training on Walking in a Chronic Stroke Patient -A Single Subject Design- (협응이동훈련이 만성 뇌졸중 환자의 걷기에 미치는 효과 -단일사례설계-)

  • Purpose: The aim of this study was to investigate the effects of coordinative locomotor training in a chronic stroke patient. Methods: A left hemiplegic patient diagnosed with a right middle cerebral artery stroke participated in this research. The patient's functional conditions were assessed, and a coordinative locomotor training program was initiated to resolve the problems identified. A set of movements deemed difficult based on the brief International Classification of Functioning, Disability and Health core set for stroke and d4501 (long-distance walking) were agreed as improvement targets. The program comprised warm up, main, cool-down, and home exercises. Repeated measurements were obtained, as follows: five times at baseline (A), 10 times during the intervention (B), and five times after the intervention (A). The study period was 7 weeks, and the intervention period was 1 h per day, twice a week for 5 weeks. Various tools, including the community walking test (CWT), 10-m walking test (10 MWT), 6-min walking test (6 MWT), and timed up and go (TUG) test, were conducted to assess the patient's walking ability. Changes in functional domains before and after the ICF Qualifier were compared. The mean values of the descriptive statistics were calculated, and a visual analysis using graphs was used to compare the rates of change. Results: The results showed that the CWT, 10 MWT, 6 MWT, and TUG test scores during the intervention period improved and that this improvement remained, even during the baseline period. In addition, the ICF Qualifier before and after the comparison decreased from moderate to mild. Conclusion: Based on the results, we propose that coordinative locomotor training can have positive effects on community ambulation of chronic stroke patients.

Effects of combining both mobilization and hold-relax technique on the function of post-surgical patients with shoulder adhesive capsulitis

  • Objective: This study investigated the effects of combining both mobilization and hold-relax (HR) technique on the function of post-surgical patients with shoulder adhesive capsulitis. Design: Randomized controlled trial. Methods: Forty-five surgical patients with shoulder adhesive capsulitis participated in this study and were randomly divided into three groups; both mobilization with movement (MWM) and HR technique (HR-MWM) group (n=15), the MWM group (n=15), and control group (n=15). All participants received three different interventions; 1) MWM combined with HR technique in PNF stretching on the shoulder, 2) MWM on the shoulder, 3) general physical therapy and intervention with neither MWM or HR stretching. Pre- and post-intervention, each subject was randomly evaluated for shoulder flexion range of motion (ROM), shoulder flexor muscle strength, Visual Analogue Scale (VAS), and the Korean version of the Shoulder Pain And Disability Index (SPADI). Results: The MWM combined with HR technique group had significant effects on shoulder flexion ROM, shoulder flexor muscle strength, VAS and SPADI compared to the MWM and control group (p<0.05). The MWM group showed a significantly greater increase in shoulder flexion ROM compared to the control group (p<0.05). Conclusions: These findings suggest that combining both the MWM and HR technique on the shoulder may more effectively improve shoulder function than MWM alone or without MWM&HR technique. Therefore, combining both the MWM and HR technique is a suggested intervention for increasing function due to shoulder adhesive capsulitis after surgery.

Botulinum Toxin Type A Therapy in Chronic Headache Patients (만성 두통 환자에서 Botulinum Toxin Type A 치료)

  • Background: Chronic headache (CH) constitutes a significant public health problem, impacting on both the individual sufferer and society. Patients with CH, unresponsive to drug therapy or nerve block, suffer considerable disability due to the frequency and severity of attacks; therefore, they should be considered for novel therapy. Botulinum toxin type A (BoNT-A) has shown significant promise in the management of CH. In this paper, we review recent evidence on the efficacy of BoNT-A, and also report our experience with this treatment in CH patients. Methods: BoNT-A was used to treat 69 CH patients, including 47 in a chronic migraine group and 22 in a non-migraine CH group, who showed therapy-resistance to palliative drug or nerve block. We investigated the demography, dosage and site of BoNT-A injection, and used a visual analogue scale (VAS) for pain and the degree of satisfaction. The data were analyzed using t-tests and a Friedman repeated measures analysis of variance on ranks. Results: Significant decreases in the VAS for pain were found in both the chronic migraine and non-migraine CH groups, from 2, 4 and 12 weeks and from 4 and 12 weeks, respectively, after BoNT-A administration (P < 0.05). The chronic migraine group showed significantly lower VAS scores for pain than the non-migraine CH group from 2, 4 and 12 weeks after the BoNT-A administration (P < 0.05). Twenty eight patients (59.2%) in the chronic migraine group and eight (36.4%) in the non-migraine CH were satisfied with the BoNT-A treatment. Conclusions: This clinical study revealed that the use of BoNT-A demonstrated efficacy for CH patients resistant to drug therapy or nerve block. Moreover, BoNT-A proved itself more effective in the chronic migraine than non-migraine CH group.

  • The disabled and elderly people have a wide variety of functional impairments. By disability and elderly user group definition, identification of users needs and specification of content requirements were studied. Existing technologies including location, navigation and information exchange devise and communication systems were analyzed to design proper integrated system for indoor and outdoor uses. There are two types of services considered in the project: assisted living services(ALS) including health and emergency needs and assisted mobility services(AMS) with transportation needs. To develop each content, content identification and requirements was studied through interviews and expert consultations. System requirements and specification using usecase technique for disabled and elderly people are discussed.

A Biographical Study of Social Exclusion Experiences - Focused on the Physical and Cerebral Palsy Disabled - (장애인의 사회적 배제 경험에 대한 생애사 연구 -지체와 뇌병변 장애인 중심으로-)

  • The purpose of this study is to understand social exclusion experiences with the disabled through a biographical study. It is analysed the interview date conducting an interview about 10 focused on the physical and cerebral palsy disabled with social exclusion experiences. The results show that it is multi-social exclusion including the inequality of approach opportunity of education, labor, education, medical, social participation, information, social service, space, health etc. It shows their life neglected and marginalized resulted from not providing social right as citizenship. And their social relationship is isolated by social exclusion and Other-directed identity is to be shaped in accordance to the method to define as regulated social ideals. Also, there is their painful and heavy life bound with double risky factors of disability and poverty. They share common historical experiences and social events impacted on their recognition and value among participants generation in biographical narrative. On the other hand, their life is reconstructed according to each their resource and standard to interpreter and evaluate on it. It suggests political and clinical implications in order to overcome the disabled's social exclusion, which is based on practical and vivid experiential dates from the participants.

Subsyndromal Depression (아증후군적 우울증)

  • Subsyndromal depression (SSD) is found to be more prevalent than major depressive disorder (MDD) and minor depressive disorder (MnDD). SSD is also associated with adverse clinical outcomes, increased risk of suicide, increased social dysfunction and disability, increased risk for future mood disorders, and increased uses of medical and mental health services. DSM-IV diagnostic criteria are not suitable for capturing SSD. Although there is no agreement on gold standard to define SSD so far, three definitions of SSD are available. First, SSD is defined as having two or more current depressive symptoms without core depressive symptoms (depressive mood or loss of interest) and with time threshold (most of the day and nearly every day over at least two weeks). Second, SSD is defined as having two or more current depressive symptoms with core depressive symptoms and without time threshold. Third, SSD is defined by using cutoff points of depression rating scales. SSD may represent a prodromal, residual, or interepisode symptomatic state in the course of MDD and MnDD. More than a half of SSD patients became any type of depressive disorders (SSD, MnDD and MDD) at 1 year. SSD may represent a discrete category of its own, without prior or consequent episodes of MDD. Considering clinical significance of SSD such as its high prevalence, significant psychosocial impairment and chronicity and serious outcomes, researchers and clinicians should be more vigilant in capturing and caring for patients with SSD.

Effects of The Oldwomen's Muscular Strength Flexibility and IADL by Exercise Therapy (운동요법이 노인의 근력, 유연성 및 IADL에 미치는 영향)

  • The purpose of this study was to investigate the effects of exercise therapy to oldwomen's muscle strength, flexibility and IADL. To identify the effect of exercise therapy on oldwomen's muscle strength, flexibility and IADL, this study attempted to determine grip strength, back strength, leg strength, flexibility of upper and low(flexion, extension, abduction), flexibility of trunk(flexion, extension) and IADL, using dynamometer, goniometer and Geriatric center IADL, for 34 female. Thirty-four subjects participating in this study consisted of A group(from sixty-five to sixty-nine, eleven person), B group(from seventy to seventy-four, twelve person), C group(over seventy-five years old, eleven person). Data from the findings of this experiment were computed for and standard deviation by using repeated measurement of MINI TAB. As a result, the following conclusion was drawn : 1. Subject group showed significantly higher scores on effect of exercise between three age group after regular exercise. 2. Subject group showed significantly higher scores on muscle strength(grip strength, back strength, leg strength) and there is a difference between three age group after regular exercise of muscle strength. 3. Subject group showed significantly higher scores on flexibility (upper, lower, trunk) and there is a difference between three age group after regular exercise of flexibility. 4. Subject group showed significantly higher scores on IADL(Instrumental activities of daily living) and there is a difference between three age group after regular exercise. As a result of this study, the effect of exercise training program had improved muscle strength, ROM (range of motion) of joint, IADL. Thus exercise training program could be beneficially applied for the prevention of disability and promotion of health and wellbeing in the aged easily and safely.

Development of a Predictive Model and Risk Assessment for the Growth of Staphylococcus aureus in Ham Rice Balls Mixed with Different Sauces (소스 종류를 달리한 햄 주먹밥에서의 Staphylococcus aureus 성장예측모델 개발 및 위해평가)

  • This study compared the predictive models for the growth kinetics of Staphylococcus aureus in ham rice balls. In addition, a semi-quantitative risk assessment of S. aureus on ham rice balls was conducted using FDA-iRISK 4.0. The rice was rounded with chopped ham, which was mixed with mayonnaise (SHM), soy sauce (SHS), or gochujang (SHG), and was contaminated artificially with approximately $2.5{\log}\;CFU{\cdot}g^{-1}$ of S. aureus. The inoculated rice balls were then stored at $7^{\circ}C$, $15^{\circ}C$, and $25^{\circ}C$, and the number of viable S. aureus was counted. The lag phases duration (LPD) and maximum specific growth rate (SGR) were calculated using a Baranyi model as a primary model. The growth parameters were analyzed using the polynomial equation as a function of temperature. The LPD values of S. aureus decreased with increasing temperature in SHS and SHG. On the other hand, those in SHM did not show any trend with increasing temperature. The SGR positively correlated with temperature. Equations for LPD and SGR were developed and validated using $R^2$ values, which ranged from 0.9929 to 0.9999. In addition, the total DALYs (disability adjusted life years) per year in the ham rice balls with soy sauce and gochujang was greater than mayonnaise. These results could be used to calculate the expected number of illnesses, and set the hazard management method taking the DALY value for public health into account.
