Classification of Nursing Activities and Workload Analysis in a New Open Hospital (환자중심 간호업무 향상을 위한 간호업무 측정에 관한 연구)

  • The purpose of this study was to confirm the classification of nursing activity and to analyze the time of nursing workload in a new open hospital. The data were collected from 20 nurses working in 6 general nursing units by 4 trained observers. The tools used for this study were an observation recording sheet and a classification sheet of nursing activity. The classification sheet was constructed to be adaptable to each hospital system based on the instrument described in the literature. The results of the study are as follows : The direct nursing activities consisted of 6 sections, 33 subsections and the indirect nursing activities consisted of 14 sections, 53 subsections. The direct nursing activities included medication, measuring and observation, care of therapies, care of physical comfort, laboratory and treatment. The indirect nursing activities included preparation of medical utensils, collection of information and assessment, recording, phone communication, professional interaction related to patients, personal time, assigning work to staff, patient eaucation and training, interaction with lab, transfer of administration of utensils, checking physician's order, dietary service, management of pollution and contagion, guide direction. Nurses spent 127.6min for direct nursing activity during day duty. It was 24.5% of total nursing activity. Within that activity medication had the highest percentage of time(40.09%), followed by communication and education with patient(24.76%), measuring and observation (16.93%), laboratory and treatment (12.85%), care of therapies(3.21%) and care of physical comfort (2.16%). The time breakdown for indirect nursing activities is as follows ; the preparation of medical utensils 22.3%, collection of information and assessment 20.29%, recording 20.27%, phone communication 8.14%, professional interaction related to patients 7.33%, personal time 7.24%, with the remaining timeshared by staffing, patient education and training, interaction with lab, transfer of administration of utensils, checking physician's order, dietary service, management of pollution and contagion, guide direction. In the analysis of the relationships between the working time and the work allocation characters of the nurses(including nurse's experiences. nurse-patients ratio, nurse-rooms ratio, and character of nursing unit) ; There were no significant differences in direct-indirect nursing times between nurse's career years. There was significant difference in direct nursing time between assigned patient numbers. The nurses assigned larger number of patients spent significantly more time in direct nursing care than that of the smaller. On the other hand, there was no significant difference in indirect nursing workload between the assigned patient numbers. There were no significant differences in direct-indirect nursing time between an allocated patient's room numbers. There was significant difference in working time between working places. The nurse in the medical unit spent more time in direct nursing care than her counterpart in the surgical unit. However there was no difference in direct nursing time between two groups. The study results indicate that nurses spent less time in the direct nursing care than in the previous studies even though the hospital system has been modernized. On the other hand they spent much more time for the coordinating role within the interdisciplinary team and for the overlapping paperwork. Therefore it is recommended that patient oriented job description and more efficient usage of modernized utilities be made.

Evaluations and Comparisons of Body Surface Doses during Breast Cancer Treatment by Tomotherapy and LINAC Radiotherapy Devices

  • Effects on skin caused by the dose from linear accelerator (LINAC) opposing portal irradiation and TomoDirect 3-D modeling treatment according to the radiation devices and treatment methods were measured, and a comparative analysis was performed. Two groups of 10 patients each were created and measurements were carried out using an optically stimulated luminescence dosimeter. These patients were already receiving radiation treatment in the hospital. Using the SPSS statistical program, the minimum and maximum average standard deviations of the measured skin dose data were obtained. Two types of treatment method were selected as independent variables; the measured points and total average were the dependent variables. An independent sample T-test was used, and it was checked whether there was a significance probability between the two groups. The average of the measured results for the LINAC opposing portal radiation was 117.7 cGy and PDD 65.39% for the inner breast, 144.7 cGy and PDD 80.39% for the outer breast, 143.2 cGy and PDD 79.56% for the upper breast, 151.4 cGy and PDD 84.11% for the lower breast, 149.6 cGy and PDD 83.11% for the axilla, and 141.32 cGy and PDD 78.51% for the total average. In contrast, for TomoDirect 3-D conformal radiotherapy, the corresponding measurement values were 137.6 cGy and PDD 76.44%, 152.3 cGy and PDD 84.61%, 148.6 cGy and PDD 82.56%, 159.7 cGy and PDD 88.72%, and 148.6 cGy PDD 82.56%, respectively, and the total average was 149.36 cGy and PDD 82.98%. To determine if the difference between the total averages was statistically significant, the independent sample T-test of the SPSS statistical program was used, which indicated that the P-value was P=0.024, which was 0.05 lower than the significance level. Thus, it can be understood that the null hypothesis can be dismissed, and that there was a difference in the averages. In conclusion, even though the treatment dose was similar, there could be a difference in the dose entering the body surface from the radiation treatment plan; however, depending on the properties of the treatment devices, there is a difference in the dose affecting the body surface. Thus, the absorbed dose entering the body surface can be high. During breast cancer radiotherapy, radiation dermatitis occurs in almost all patients. Most patients have a difficult time while undergoing treatment, and therefore, when choosing a radiotherapy treatment method, minimizing radiation dermatitis is an important consideration.

  • Si wafer direct bonding(SDB) technology is very attractive for both Si-on-insulator(SOI) electronic devices and MEMS applications. This paper presents on pre-bonding according to HF pre-treatment conditions in Si wafer direct bonding. The characteristics of bonded sample were measured under different bonding conditions of HF concentration and applied pressure. The bonding strength was evaluated by tensile strength method. A bond characteristic on the interface was analyzed by using FT-IR, and surface roughness according to HF concentration was analyzed by AFM. Si-F bonds on Si surface after HF pre-treatment are replaced by Si-OH during a DI water rinse. Consequently, hydrophobic wafer was bonded by hydrogen bonding of Si-OH$\cdots$(HOH$\cdots$HOH$\cdots$HOH)$\cdots$OH-Si. The pre-bonding strength depends on the HF pre-treatment condition before pre-bonding. (Min : $2.4kgf/cm^2{\sim}$Max : $14.9kgf/cm^2$)

  • In order to develop the ion exchange membranes which would be used in direct methanol fuel cell (DMFC), the polysulfone polymer was sulfonated using chlorosulfonic acid (CSA) and trimethylchlorosilane(TMCS). It has been characterized in terms of ion conductivities, methanol crossover, swelling degree and ion exchange capacities for the heat untreated and treated membranes at $150^{\circ}C.$ Typically, the methanol permeability and ion conductivity at the mole ratio of 1.4 between polysulfone repeating unit and sulfonating agents showed $2.87{\times}10^{-7}\; cm^2/s$(without heat treatment), $1.52{\times}10^{-7}\; cm^2/s$(with heat treatment) and $1.10{\times}10^{-2}\; S/cm$(without heat treatment), $0.87{\times}10^{-2}\;$ S/cm(with heat treatment), respectively. After the mole ration of 1.4 both values indicated mild increase.

Seawater-driven forward osmosis for direct treatment of municipal wastewater

  • Direct treatment of municipal wastewater by forward osmosis (FO) process was evaluated in terms of water flux decline, reverse salt diffusion, pollutants rejection and concentration efficiency by using synthetic seawater as the draw solution. It was found that when operating in PRO mode (active layer facing the draw solution), although the FO membrane exhibited higher osmotic water flux, more severe flux decline and reverse salt diffusion was also observed due to the more severe fouling of pollutants in the membrane support layer and accompanied fouling enhanced concentration polarization. In addition, although the water flux decline was shown to be lower for the FO mode (active layer facing the feed solution), irreversible membrane fouling was identified in both PRO and FO modes as the water flux cannot be restored to the initial value by physical flushing, highlighting the necessity of chemical cleaning in long-term operation. During the 7 cycles of filtration conducted in the experiments, the FO membrane exhibited considerably high rejection for TOC, COD, TP and $NH_4{^+}-N$ present in the wastewater. By optimizing the volume ratio of seawater draw solution/wastewater feed solution, a concentration factor of 3.1 and 3.7 was obtained for the FO and PRO modes, respectively. The results demonstrated the validity of the FO process for direct treatment of municipal wastewater by using seawater as the draw solution, while facilitating the subsequent utilization of concentrated wastewater for bioenergy production, which may have special implications for the coastline areas.

Treatment of Heavy Metals and Phenol in Contaminated Soil Using Direct Current and Pulse Voltage (직류 전원과 펄스 전원을 이용하여 오염된 토양에서의 중금속과 페놀 처리)

  • In this work, the treatment of heavy metals and phenol in the contaminated soil was investigated by applying direct current (DC) and pulse voltage. When the DC was used, the removal efficiencies for Cu, Zn, As, and Pb were 73, 88, 10, and 10%, respectively, and more than 95% for phenol was removed. Furthermore, when a pulse voltage was employed the removal efficiencies for Cu, Zn, As, and Pb were 88, 92, 40, and 40%, respectively, and 87% of phenol was removed. The results indicate that the application of a pulse voltage for the treatment of contaminated soil reduced electro-osmosis, but increased the rate of electric current movement of heavy metals. In addition, the removal efficiencies for As and Pb have been improved due to the enhanced adsorption capacity of clay components in the soil. Therefore, these experimental results could be effectively applied in remediation technology for the treatment of various heavy metals and phenol.

Comparison of microwave and steam-type drying processes for dehydrated sludge in a wastewater treatment plant (탈수슬러지의 마이크로웨이브와 증기 건조공정의 성능비교)

  • There are two primary sludge drying methods such as the direct heating microwave method and the indirect heating steam one. In this study, the drying treatment facility at sewage treatment plant A applied both of these drying methods. The research aimed to investigate the optimal operation approach for the drying facility, considering the input sludge and the moisture content data after the drying process. Moisture content and removal rate data were executed at the research facility from January 2016 to December 2018. First, the microwave, a direct heating drying method, performed intensive drying only on the outer surface of the sludge by directly applying heat to the sludge using far infrared rays, so effective sludge drying was not achieved. On the other hand, the steam method of the indirect heating method used steam from a gas boiler to maximize the utilization of the heat transfer area and reduce energy of the dryer, resulting in an effective sludge drying efficiency. The sludge moisture content brought into the sludge drying facility was about 80%, but the moisture content of the sludge that went through the drying facility was less than 10% of the design standard. Therefore, the steam method of the indirect heating method is more effective than the microwave method of the previous direct heating method and is more effective for maintenance It has proven that it is an efficient method of operating construction facilities.

DIASTEMA CLOSURE TREATMENT DECISION FOR AN ADOLESCENT PATIENT WITH CEREBRAL PALSY (뇌성마비인 청소년의 치간이개 치료법 결정 : 증례보고)

  • Diastema is thought to be a problem related to aesthetics, pronunciation, or malocclusion. Due to its extent and patient conditions, orthodontic treatment, prosthodontic treatment, and conservative direct resin restoration are the treatment options for diastema closure. Additional factors need to be considered when deciding on the most appropriate treatment of diastema, particularly for patients with cerebral palsy. A 13-year-old girl visited the Department of Pediatric Dentistry at Yonsei University Dental Hospital with a chief complaint of the large gap between her upper front teeth. After clinical and radiographic examinations, midline diastema of 4.5 mm, protrusive maxilla incisors, congenital missing teeth, retained primary teeth, etc. were identified. Prosthodontic treatment with intentional root canal treatment was not appropriate because of the patient's age. Dental spaces can be closed effectively via orthodontic appliances. However, additional prosthodontic and restorative intervention is unavoidable, which incurs significant costs and requires more time. Instead of orthodontic and prosthodontic treatment, direct resin restoration can address the chief complaint; these restorations are reversible, less harmful to other oral structure and teeth, relatively easy to apply, less expensive than other treatments, and require shorter office visits. Midline diastema can be treated in several ways. For diastema closure in patients with cerebral palsy, conservative resin restorations are a short, simple, and appropriate treatment compared with orthodontic or prosthodontic treatments.

Effects of Gibberellin Application and Bagging on Ripening and Quality in 'Delaware' Grape Berries (Delaware 포도에서 Gibberellin 처리와 봉지씌우기가 과립의 성숙과 품질에 미치는 영향)

  • This experiment was carried out to clarify the effects of gibberellin(GA) aplication and bagging on repeening and quality in 'Delaware' grape berries. Treatments are 4 plots(2X2 factorial experiment); GA, GA+bagging, bagging and control. The clusters were dipped twice in 100 ppm GA with GA treatment : 10 days before and after the full bloom. The results obtained as follows: 1. GA treatment made the seedless grape berry reduced in the fresh weight but it hastened the ripening period about 2 weeks. 2. Total soluble solid(TSS), viscosity and pH value of berry juice increased with maturation. The concentration of TSS and viscosity were higher in GA treatment plot than GA non-treatment. 3. Berry-hardness, titratable acidity and alcohol inslouble solid(AIS) decreased with maturation. Expically berry-hardness and AIS decreased more greatly in GA non-treatment than GA treatment. 4. The concentration of anthocyanin increased with ripening but pectic substance didn't fluctuate nearly. These of anthocyanin and pectin were higher in GA non-treatment plot than GA treatment. 5. By analysis of factorial experiment GA treatment was highly significant with the $^{o}$Brix/Acidity ratio, juice viscosity and AIS, but high negatively, significant with berry-hardness and berry fresh weight. And it was significant with T S S and negatively, titratable acidity. Bagging was significant with $^{o}$Brix/Acidity ratio and AIS content, but negatively, titratable acidity. 6. Qualitative characters were high correlated with the $^{o}$Brix/Acidity ratio in simple correlation but direct effect by the path-coefficient analysis didn't coincide with simple correlation. The direct effect of pH was large and juice viscosity, the next. And that of berry-hardness was negligible but, AIS, small negatively.
