개에서 자궁축농증과 병발한 척추원반염 1례 (Discospondylitis with pyometra in a dog)

  • Discospondylitis is an infection of intervertebral discs, associated end plates, and adjacent vertebral bodies causing destruction and proliferation. A 10-year-old intact female Rhodesian Ridgeback dog was referred to the Veterinary Medical Teaching Hospital, Seoul National University with paraparesis and severe pain at T13-L1 region for one week duration. The treatment with PDS for one week by referring veterinarian showed no clinical improvement. At time of presentation, clinical signs of the patient included depression, vaginal discharge, and fever. In T-L spine radiography, osteolytic change was found between caudal endplate of T13 and cranial endplate of L1. The affected vertebral bodies were shortened and intervertebral disc space was widened. In abdominal radiography, a dilated soft tissue opacity tubular structure was found in mid-caudal abdomen. The abdominal ultrasonography revealed the uterus dilated with echogenic fluid. The CT findings showed concentric lysis of T13-L1 endplates. The dog was diagnosed as diskospondylitis at T13-L1 and pyometra. Urine and blood were cultured, and Staphylococcus (S.) intermedius was identified. The patient had surgical treatment for pyometra but died for septicemia.

Investigation of entanglement dynamics of flexible polymer chains via comparison of dielectric and viscoelastic properties: a review of recent findings

  • This review article summarizes results of recent viscoelastic and dielectric studies for entangled cis-polyisoprene (PI) chains. The PI chains have the so-called type-A dipoles parallel along the chain backbone, and their slow viscoelastic and dielectric relaxation processes reflect the same global chain motion. However, this motion is differently averaged in the viscoelastic and dielectric properties, the former representing the isochronal orientational anisotropy of individual entanglement segments while the latter detecting the orientational correlation of the segments at two separate times (0 and t). On the basis of this difference, the viscoelastic and dielectric data of the entangled PI chains were compared to elucidate detailed features of the chain dynamics. Specifically, the molecular picture of dynamic tube dilation (DTD) incorporated in recent models was tested for linear and star PI chain. The comparison revealed that the DTD picture was valid for linear PI chains but failed for the star PI chains in the dominant part of the terminal relaxation. The failure for the star chains was related to the pre-requisite for the DTD process, rapid equilibration of successive entanglement segments through their constraint release (CR) motion: For the star chains, the dilated tube diameter expected in the terminal regime was considerably large because of a broad distribution of motional modes of the chains, so that the CR-equilibration required for DTD could not occur in time. The terminal relaxation of the star chain appeared to occur through the CR process before the expected DTD process was completed. The situation was different for the linear chain exhibiting narrowly distributed motional modes. The dilated tube expected for the linear chain was rather thin and the required CR-equilibration occurred in time, resulting in the success of the DTD picture. These detailed features of the chain dynamics was revealed only when the viscoelastic and dielectric properties were compared, demonstrating the importance of this comparison.

동물원 사자의 Clostridium perfringens에 의한 장독혈증 감염증례 (Clostridium perfringens enterotoxicosis in a lion of zoo)

  • A 3-year-old male lion at Gwangju Uchi Zoo presented for acute onset of haemorrhagic diarrhea and died. The lion showed reddening of the anus as the cause of haemorrhagic enteritis. Necropsy revealed a severe haemorrhagic colitis. Grossly, lesions included icterus, excess pericardial fluid. dark kidneys, and an enlarged, friable liver. The intestines were flaccid, thin-walled, dilated, and 9as-filled. The spleen was enlarged and pulpy because of congestion. Most of organs were rapidly postmortem autolysis. Histopathologically, the intestines were edema and transient leukocyte infiltration of the lamina propria, followed by necrosis. Especially of the intestinal submucosa was edematous, haemorrhagic, or filled with leukocytes. The crypts remained intact or dilated. C perfringens was isolated from a lion at bloody feces, and identified C perfringens type A, confirming the presence of C perfringens $\alpha-toxin$ by PCR. These results were suggested that the case were diagnosed as enterotoxicosis in the lion. More studies are needed on lion enterotoxemia. especially of its etiopathogenesis, in order to develop more efficient prevention for this disease.

개에서 단독으로 발생한 급성 비장 염전과 만성 비장 염전의 비교 (Acute and Chronic Isolated Splenic Torsion in Two Dogs)

  • An intact male, 3-year old, Sapsari, weighing 29 kg with a history of respiratory distress, anorexia, abdominal distension, and depression for 1 day was referred. Radiographic findings included a C-shaped soft tissue mass in the mid-abdominal cavity and loss of serosal detail. Ultrasonographs showed splenomegaly with hypoechoic coarse parenchyma, loss of venous flow in the dilated splenic veins, and thrombi. Based on image findings, acute isolated splenic torsion was diagnosed. Splenectomy and prophylactic gastropexy was performed. Another case was a spayed female, 8-year old, Shih-tzu, weighing 3.7 kg with a history of intermittent abdominal pain and a palpable abdominal mass for 1 year. Radiographic findings showed splenomegaly characterized by a soft tissue round mass. Ultrasonographs revealed displaced splenic hilum and absence of venous flow in the dilated splenic vein. Chronic isolated splenic torsion was diagnosed and also splenectomy and gastropexy was performed. Two dogs were recovered normally after surgery without any complications. Acute form of isolated splenic torsion showed acute systemic clinical signs, severe C-shaped splenomegaly on radiographs, and loss of all of splenic venous flow, perivenous hyperechoic triangle sign, and thrombi on ultrasonographs. Chronic form of isolated splenic torsion showed chronic obscure clinical signs, moderate splenomegaly on radiographs, and partial loss of splenic venous flow on ultrasonographs.

실험적 설암에서 방사선 조사전후의 혈관분포에 관한 연구 (A STUDY ON THE PRE-AND POST-IRRADIATION EFFECT OF BLOOD VESSELS IN THE EXPERIMENTALLY INDUCED TONGUE CANCER)

  • The author observed the changes of vasculature of pre-and post-irradiation on DMBA induced rat tongue cancer. The study was performed by using vascular corrosion resin casting, and scanning electron microscopy. The results were as follows. 1. The capillaries runned parallely and formed bundles and, sometimes, plexus. The endothelial cells were arranged regularly and small pores were observed. 2. In irradiated normal tongue the capillaries were curved slightly and formed plexus on initial day of post-irradiation. On third day the capillaries and capillary pores were dilated and the endothelial cell arrangement was irregular. The effects of irradiation were gradually increased from initial to the 3rd day, though it was decreased after 7th day. 3. The vasculature of DMBA induced tongue cancer group were very irregular, and large avascular lesions were formed according to the cancer necrosis or tumor cell nest and the vasculature was narrowed and paralleled around the avascular lesion by compression of cancer cell nest. The vascular wall was roughened and dilated, forming club shaped or varix. 4. The vessels were curved and formed reticular network in irradiated DMBA induced tongue carinoma group. The free end of newly formed capillaries had regular width, and also irregular club shaped or aneurysmal dilatation were observed. The vascular structures were destroyed and vessels were fused in tumor necrosis lesion. The radiation effects were marked on the first and third day of irradiation and the effects were decreased after seventh day and showed capillary regeneration.

특징 융합과 공간 강조를 적용한 딥러닝 기반의 개선된 YOLOv4S (Modified YOLOv4S based on Deep learning with Feature Fusion and Spatial Attention)

  • 본 논문은 특징 융합과 공간 강조를 적용하여 작고 페색된 객체 검출을 위한 개선된 YOLOv4S를 제안하였다. 기존 YOLOv4S은 경량 네트워크로 깊은 네트워크 대비 특징 추출 능력 부족하다. 제안하는 방법은 먼저 feature fusion으로 서로 다른 크기의 특징맵을 결합하여 의미론적 정보 및 저수준 정보를 개선하였다. 또한, dilated convolution으로 수용 영역을 확장하여 작고 폐색된 객체에 대한 검출 정확도를 향상시켰다. 두 번째로 spatial attention으로 기존 공간 정보 개선하여 객체간 구분되어 폐색된 객체의 검출 정확도를 향상시켰다. 제안하는 방법의 정량적 평가를 위해 PASCAL VOC 및 COCO 데이터세트를 사용하였다. 실험을 통해 제안하는 방법은 기존 YOLOv4S 대비 PASCAL VOC 데이터세트에서 mAP 2.7% 및 COCO 데이터세트에서 mAP 1.8% 향상되었다.

WaveNet과 Work Forward Validation을 활용한 시계열 데이터 분석 (Time Series Data Analysis using WaveNet and Walk Forward Validation)

  • 복잡하고 비선형적인 특징을 갖는 시계열 데이터를 예측하기 위해 딥러닝 기법이 널리 사용되고 있다. 본 연구에서는 최근에 개발된 WaveNet을 개선하고 워크포워드 검증 기법을 적용하여 전력 소비량 데이터를 24시간 이전에 예측하고자 한다. 원래 WaveNet은 오디오 데이터 예측에 사용하고자 고안되었으며, 장기간의 데이터를 효과적으로 예측하기 위해 1차원 팽창인과 합성곱(1D dilated causal convolution)을 사용한다. 먼저, WaveNet이 부호화된 정수 값이 아니라 실수 값을 출력하여 전력 데이터를 예측하기 적합하도록 개선하였다. 다음으로 학습 과정에 적용된 하이퍼파라미터(입력 기간, 배치 크기, WaveNet 블록 개수, 팽창 비율, 학습률 변경)를 조정하여 적절한 성능을 나타내도록 하였다. 마지막으로 성능 평가를 통해 전통적인 홀드아웃 검증 기법보다 본 연구에서 사용한 워크포워드 검증 기법이 전력 소비량 데이터 예측에 우수함 성능을 나타냄을 확인하였다.

Loss of Primary Cilia Results in the Development of Cancer in the Murine Thyroid Gland

  • Communications at the interface between the apical membrane of follicular cells and the follicular lumen are critical for the homeostasis of thyroid gland. Primary cilia at the apical membrane of thyroid follicular cells may sense follicular luminal environment and regulate follicular homeostasis, although their role in vivo remains to be determined. Here, mice devoid of primary cilia were generated by thyroid follicular epithelial cell-specific deletion of the gene encoding intraflagellar transport protein 88 (Ift88). Thyroid follicular cellspecific Ift88-deficient mice showed normal folliculogenesis and hormonogenesis; however, those older than 7 weeks showed irregularly dilated and destroyed follicles in the thyroid gland. With increasing age, follicular cells with malignant properties showing the characteristic nuclear features of human thyroid carcinomas formed papillary and solid proliferative nodules from degenerated thyroid follicles. Furthermore, malignant tumor cells manifested as tumor emboli in thyroid vessels. These findings suggest that loss-of-function of Ift88/primary cilia results in malignant transformation from degenerated thyroid follicles.

백악-골화섬유종에서 보이는 동맥류성 낭종변화의 면역조직화학염색 배열분석 (Immunohistochemical Array Analysis of Cemento-Ossifying Fibroma Exhibiting aneurysmal Cystic Changes)

  • A 31 years old female had been suffered from a bony swelling in right premolar region of the mandible for 12 years, recently grown rapidly. A fistula tract developed on the right anterior mandibular border, but the lesion was relatively asymptomatic. In the radiological examination, the tumor mass was irregularly mixed with radiolucent and radiopaque areas, forming multiple cystic spaces. Under the diagnosis of calcifying odontogenic cyst, the mandibular mass was resected and examined pathologically. After decalcification, the dissected tumor mass showed multiple small cystic spaces and calcifying fibrous tissue, mimicking calcifying odontogenic cyst or ameloblastoma. Histological observation showed many calcifying cementoid materials and ossifying trabeculae. The cystic spaces were turned out to be dilated vascular channels lined by endothelial cells, containing plasma fluid. However, the main lesion was diagnosed as cemento-ossifying fibroma (COF), and the atypical vascular channels were greatly dilated and gradually expanded the whole tumor mass. The present COF was examined through immunohistochemical (IHC) array, and investigated for tumor cell characteristics, exhibiting abnormal ossification and aneurysmal cystic changes. IHC array disclosed that the tumor cells grew progressively in the lack of apoptosis, and that they showed lower expression of RUNX2 than BMP-2, RANKL, and OPG, and increases of protein expression in $HIF-1{\alpha}$, VEGF-A, and CMG2. These data suggested that the reduced expression of RUNX2, osteoblast differentiation factor, be relevant to abnormal ossification of COF, and that the consistent expressions of angiogenesis factors be relevant to de novo angiogenesis in COF, subsequently resulted in aneurysmal cystic changes.

Expandability of Cephalic Veins after Brachial Plexus Block in Arteriovenous Fistula Formation for Hemodialysis

  • Background: Arteriovenous fistula (AVF) for hemodialysis is essential for patients with end-stage renal disease. However, it is difficult to maintain AVF reliably. It is vitally important to select proper blood vessels for AVF formation. In a previous study, a minimum diameter of 3 mm for the autologous vein was proposed. However, patients who did not meet the minimum vascular diameter before anesthesia, but fulfilled other criteria, showed satisfactory venous dilatation after brachial plexus block (BPB). This study investigated the extent of vein expansion by BPB and the surgical outcomes of dilated veins after BPB. Methods: Sixty-one patients who underwent AVF formation using an autologous vein between August 2018 and December 2019 were included in the study. The clinical characteristics of the patient groups, hemodynamic parameters including the diameter of blood vessels before and after BPB, and complications were investigated. Based on the venous diameter measured by sonography before anesthesia, patients were divided into group A (26 patients) and group B (35 patients), with venous diameters <3 mm and ≥3 mm, respectively. Results: The venous diameter expanded after anesthesia by 41% overall, by 62% in group A, and by 25% in group B. This difference between groups A and B was statistically significant (p=0.001). No other variables showed statistically significant differences. Conclusion: Sufficient venous dilatation was observed after BPB. Therefore, if the vein is sufficiently dilated after BPB, even in patients with a pre-anesthesia venous diameter <3 mm, surgery may still be performed with an expected desirable outcome.
