Persistent Hymen and Pyocolpos in a Female Shih-Tzu Dog (시츄 개에서 발생한 처녀막 잔존증과 질축농증)

  • A 9-year-old intact female Shih-Tzu was presented with abdominal pain and abdominal distension. Since the animal's birth, the owner had never observed proestrous bleeding in the patient. Ultrasonography revealed segmental enlargement of the genital system. An ovariohysterovaginectomy was performed to remove the genital mass. The vagina was grossly dilated as a result of being filled with a solid black-green substance. A vaginogram was performed. Based on the surgical findings and vaginogram results, an imperforate hymen was diagnosed. One month from the first surgery, a stump pyocolpos developed between the sutured site and the obstructed portion. The persistent hymen was removed by endoscopy. However, peritonitis and sepsis developed after the procedure, and the dog died. Therefore, During operation of persistent hymen, a cruciate incision over the hymen strongly recommended to avoid additional post operative complications.

  • In the course of studying porcine enterovirus infection in piglets, the vascular endothelial cells in the CNS of colostrum-deprived piglets with polioence-phalomyelitis were investigated by electron microscope. The experimental piglets were orally infected with the porcine enterovirus serogroup 3 isolated in Korea and necropsied at 7 days postinoculation. Crystalline arrays of viral particles were found in the vascular endothelial cells of the spinal cord and cerebellum. Aggregates of immature viral particles were occasionally observed in the vascular endothelial cells in the meninges. The rough ER was deprived of ribosomes, irregularly dilated and associated with viral crystals, There were abundant cytoplasmic filaments, dilatation of perivascular space, perivascular cuffing, and the partial distruptions of endothelial cell membrane and basal lamina.

Electron Microscopic Observations of Fat-storing Cells on the Liver Of Squirrels (다람쥐 간지방섭취세포(肝脂肪攝取細胞)의 전자현미경적(電子顯微鏡的) 관찰(觀察))

  • In order to know the morphology and the function of the fat-storing cells (FSC) of the squirrels which were captured at the Pal-Gong mountain near Taegu in December, 1980 (Group A) and May, 1981 (Group B), respectively, the light and electron microscopic observations were conducted on the liver of the squirrels. The results obtained are as follows: 1. Light microscopically, the size of the lipid droplets in the FSC of group A was uniformly larger than those in the cells of group B, and number of the droplets in the FSC of group A was less than those in the cells of group B, The distribution of the FSC of group A was mainly perilobular area while those of group B was centrolobular and midzonal areas. In this point of view, the FSC of the squirrels was similar to those cells of the hamsters. 2. Electron microscopically, general morphology of the sqirrel's FSC was accorded with those of the other mammals. However, the rouph surfaced endoplasmic reticulum in the FSC of group B was more dilated than those in the cells of group A, and more lipid droplets and pinocytopic vesicles were observed in the FSC of group B than those in the cells of group A. 3. From the above evidences, it could be suggested that the metabolic rates in the FSC of the squirrels collected in the spring were higher than those in the cells of the animals collected in the winter.

  • A 70-year old male with dilated cardiomyopathy was admitted to our hospital because of a renal stone, and a supra-renal abdominal aortic aneurysm was detected during work-up. We performed a hybrid surgery using an endovascular stent because of his co-morbidities. The operation consisted of removal of the renal stone, de-branching of the visceral artery and both renal arteries from the abdominal aorta, reperfusion of the de-branched arteries with retrograde bypass surgery using two Y-graft from the left external iliac artery, and deployment of stent graft in the abdominal aorta. Therefore, we report a case of hybrid surgery for supra-renal abdominal aortic aneurysm.

Deep vein thrombosis caused by malignant afferent loop obstruction

  • Afferent loop obstruction following gastrectomy is a rare but fatal complication. Clinical features of afferent loop obstruction are mainly gastrointestinal symptoms. A 56-year-old female underwent radical total gastrectomy with Roux-en-Y esophagojejunostomy for treatment of advanced gastric cancer. After fourteen months postoperatively, she showed gradual development of edema of both legs. Computed tomography (CT) scan showed disease progression at the jejunojejunostomy site and consequent dilated afferent loop, which resulted in inferior vena cava (IVC) compression. A drainage catheter was placed percutaneously into the afferent loop through the intrahepatic duct and an IVC filter was placed at the suprarenal IVC, and self-expanding metal stents were inserted into bilateral common iliac veins. With these procedures, sympotms related with afferent loop obstruction and deep vein thrombosis were improved dramatically. The follow-up abdominal CT scan was taken 3 weeks later and revealed the completely decompressed afferent loop and improved IVC patency. Surgical treatment should be considered as the first choice for afferent loop obstruction; however, because it is more immediate and less invasive, non-surgical modalities, such as percutaneous catheter drainage or stent placement, can be effective alternatives for inoperable cases or risky patients who have severe medical comorbidities.

A Case of Congenital Hepatic Fibrosis Accompanied by Renal Tubular Ectasia, Caroli Syndrome and Choledochal Cyst (신세뇨관 확장증, Caroli 증후군 및 총 담관낭을 동반한 선천성 간 섬유증 1례)

  • Congenital hepatic fibrosis is a relatively rare disease, characterized by bile ductular proliferation and prominent fibrosis in the portal area of liver resulting in portal hypertension. It is frequently associated with other abnormalities such as polycystic kidney, Caroli syndrome, cystic dysplasia of pancreas, intestinal lymphangiectasia, pulmonary emphysema, hemangioma, and cleft palate. We report here a case of congenital hepatic fibrosis associated with renal tubular ectasia in a 3-year-old girl, whose chief complaint was abdominal distension. Her liver function test did not reveal any abnormal findings. Hepatosplenomegaly and multiple dilated bile ducts were seen in the abdominal CT scaning. Esophageal varix was not detected by an endoscopic examination. Microscopically, diffuse portal fibrosis and widening with proliferation of blie ductules in the liver specimen and tubular ectasia in renal cortex were seen.

  • The bronchial varices in mitral stenosis are uncommon and incidentally discovered during bronchoscopy. Although bronchial varices are primarily associated with bronchial or pulmonary disease, the bronchial vein can be dilated with increased pulmonary venous pressure secondary to mitral stenosis. The bronchial varices may present massive hemoptysis. The hemoptysis can be controlled by mitral commissurotomy or mitral valve replacement in case of mitral stenosis. We report a case of bronchial varies in a patient with severe mitral stenosis. The bronchial varices were found incidentally during bronchoscopy and they were nearly disappeared by mitral valve replacement.

Effect of irradiation on the periodontal tissues in streptozotocin-induced diabetic rats (방사선조사가 당뇨 백서의 치주조직에 미치는 영향)

  • Purpose To observe the histopathological changes in the periodontal tissues of mandibular molars in streptozotocin-induced diabetic rats after irradiation. Materials and Methods : The male Sprague-Dawley rats weighing approximately 250 gm were divided into four groups: control, diabetes, irradiation, and diabetes- irradiation groups. Diabetes mellitus was induced in the rats by injecting streptozotocin. Rats in the control and irradiation groups were injected with citrate buffer only After 5days, the head and neck region of the rats in irradiation and diabetes-irradiation groups were irradiated with a single absorbed dose of 10Gy. All the rats were sacrificed at 3, 7, 14, 21, and 28 days after irradiation. The specimen including the mandibular molars were sectioned and observed using a histopathological method. Results In the diabetes group, osteoclastic activity was observed in the alveolar bone and the root throughout the period of experiment. Also, osteoblastic and fibroblastic activities were markedly decreased. In the irradiation group, the osteoclasts were observed in the alveolar bone and the dilated capillaries were increased in the early experimental phases. However, vigorous osteoblastic activity was noted in the late experimental phases. In the diabetes-irradiation group, osteoblastic activity in the alveolar bone and the root was observed in the early experimental phases. However, there were no resorption and osteoblastic activity in the alveolar bone and the root in the late experimental phases, and obvious atrophic change of fibrous tissues was noted. Conclusion : This experiment suggests that osteoblastic activity was caused by irradiation in the late experimental phases, but atrophic change of the periodontal ligament tissues was induced after irradiation in diabetic state.

Histologic Study on the Effect of Two Types of Bovine Bone Powder in Extraction Socket of Beagle Dogs (성견 발치와에 매식한 2종의 Bovine Bone Powder가 치유에 미치는 영향에 관한 조직학적 연구)

  • Several extraction cases with advanced bone loss as a result of periodontal disease, root or labial bone fracture, extensive caries, and periapical lesions occur esthetic, functional problems and severe bone loss. Therefore, to treat these cases used several surgical methods and socket preservation among this therapies have been evaluated simple, effective and good prognosis in the implant placement. Socket preservation therapy have been used with barrier membranes or/and graft materials. Deproteinized bovine bone mineral have been evaluated ideal grafting materials. Recently, calcium-phosphate thin film coated bovine bone powders were developed in our country, but the study for these material wasn't reported. When two types of xenograft materials were implanted in extraction sockets of Beagle dogs, the effects of these were analyzed after 4 weeks and 8 weeks histological views. The results of this study were as follows. 1. In control groups, 4 weeks after implantation, the extraction sockets were filled with connective tissue which has dilated vessels and epithelial growth. And after 8 weeks, irregular connective bundles were observed. But new bone formation was not seen. 2. In Bio-Oss groups, epithelial growth was not seen and bone powder was covered with connective tissue fiber. New bone formation was found around the interproximal bone. There was no special change seen after 8 weeks, connective tissue fibers became more regular, and bone growth near bone powder was not made well. 3. In Ca-P BBP groups, epithelial cells didn't grow in the extraction sockets, there was a lot of new bone made around the bone powder after 8 weeks, new bone around bone powder was replaced with mature bone. It is thought that bone powder grafting into the extraction sockets is very useful for conservation of ridge, and Ca-P BBP is more effective in bone formation than Bio-Oss.

The Effect of the Aflatoxin $B_1$ on Liver Tissue of Chick-Embryo (Aflatoxin $B_1$이 계배 간조직의 미세구조에 미치는 영향)

  • To investigate the effect of aflatoxin $B_1$, on survival rate and ultrastructure of liver during chick embryogenesis electron microscopic methods were used. After injection of aflatoxin $B_1$ into the yolk, ultrastructural changes in the liver of chicken embryo were observed. The results were as followed. 1. 12-day old chicken embryos were treated with single injection of aflatoxin $B_1$ with the dose of $0.0005{\mu}g,\;0.005{\mu}g,\;0.05{\mu}g,\;0.5{\mu}g,\;2.5{\mu}g,\;5.0{\mu}g$ each. Chicken embryos treated with the dose of $0.5{\mu}g$ of aflatoxin $B_1$ had survival rate of 22%. The embryos treated with $2.5{\mu}g$ of aflatoxin $B_1$ hardly survived. 2. Chicken embryos treated with $0.05{\mu}g$ of afatoxin $B_1$ had hatched in 30%, but once hatched, they all survived. 3. After administration of $0.05{\mu}g$ of aflatoxin $B_1$ into the 12-day old chicken embryo, the electron microscopic studies were examined during development stages. The nuclei of hapatocytes became irregularly shaped and the structures of endoplasmic reticulum were changed to spherical types at 20-day old chicken embryo. Also, mitochondria became to be dilated and severe fibrosis was induced in the cytoplasm. However, the hepatocytes became almost normal in 30-day old young chicken.
