The purpose of this study was to evaluate the incidence and several radiographic features of dilacerated teeth in 2132 full month radiograms in Korean. The results were as follows: 1. The occurrence was revealed to 2.7% in total examined teeth, and these anomalies were occurred in maxilla (52.4%) more than in mandible (47.6%). 2. There was a predilection for occurrence of dilaceration in female, which included 12.8% of the female compared to 11.3% in male. 3. The frequency of dilaceration in male, which included 40.5% in maxilla and 59.5% in mandible. The frequency of dilaceration in female, which included 52.4% in maxilla and 47.6% in mandible. 4. The order of frequency of dilaceration was second premolar, first premolar, lateral incisor, canine in maxilla, and first premolar, second premolar, canine, lateral incisor, central incisor in mandible. 5. In classifying of dilacerated teeth into 3 types by following appearances such as root curvature. Distal dilaceration was by far most common containing 65.8% of the cases. The least frequent was mesial dilaceration, which included 6.5% of the cases.

Dilaceration is a tooth abnormaly occuring in the histo-morphodifferention stage resulting from disturbance between the uncalcified and already calcified portion that affects both the crown and root. The involved tooth is usually the maxillary central incisor and it also shows high prevalence of impaction. The cause of dilaceration can be either from the trauma of the primary tooth, ectopic development of the tooth germ or from cysts. And it is also found in some cases of Otodental syndrome, Hurler syndrome, Cleidocranial dysostosis. The purpose of this study is to show that periapical lesions caused by dental caries can be another factor in causing dilaceration.

Dilacerations generally involve central incisors; most often maxillary incisors rather than their mandibular counterparts. The clinical features of dilaceration include non-eruption of the responsible tooth or prolonged retention of the deciduous predecessor tooth. In Case 1, the tooth showed a dilaceration at the boundary between the crown and the root, more laterally rather than labiolingually. In Case 2, the dilacerated tooth showed a crown dilaceration with a relatively normal orientation of the dental root. In both cases, no significant space losses for eruption were observed. Moreover, it seems that unlike the maxilla with the palate, the mandibular anterior teeth are limited to show severe displacement. From these cases, it is suggested that if a mandibular permanent incisor shows a crown dilaceration or lateral dilaceration at the boundary between the crown and the root, there is a relatively high probability of spontaneous eruption of the dilacerated tooth.

Purpose : We want to identify the appearance of the buccolingual root dilaceration teeth in the panoramic views and specify the characteristics of these teeth. Materials and Methods : One thousand-six patients were examined on the basis of both panoramic and CT image criteria. We diagnosed and excluded certain teeth from the samples; both prosthodontic or pathologic lesion appearing teeth and mesiodistally dilacerated ones. We meticulously discerned buccolingually dilacerated teeth in the CT images and total 48 samples were selected. The degree of severity in dilaceration was standardized by 2 types of criteria. The samples were differentiated into 3 groups and again categorized into six types showing from the panoramic views: irregular view on the root apex area, clear blunt on the root tip, stepping on root tip, double lamina dura or double tip, arrow-target shaped root, bull's eye, normal view. Results : The types of teeth selected from total 48 buccolingual root dilaceration samples were mandibular first and second molar, premolars, canines, and lateral incisors. The direction of dilaceration was an even percentage to each buccal and lingual side for most selected teeth, however, that of both canines and lateral incisors were directed in almost a buccal side. In the panoramic views, the root types of the buccolingually dilacerated teeth were irregular view on the root apex area, clear blunt on the root tip, stepping on root tip and normal types were almost always normal view. The more severity in dilareated degree, the more chances of observation in the panoramic views were clear blunt on the root tip and stepping on root tip. Conclusion : As observed in the shape of stepping on root tip or double lamina dura in the panoramic views, there can be much more probability to diagnose as a buccolingually dilacerated root.

The purpose of this study was to evaluate the prevalence and distribution according to the types of tooth shape anomalies in permanent teeth of 6,082 persons by means of analysis of the full mouth periapical radiographs. And the following results were obtained ; 1. Among tooth shape anomalies, the highest incidence was observed on dilaceration (20.14%), foll owed by dens invaginatus(3.02%), peg lateralis(1.48%), taurodontism (0.34%), dens evaginatus (0.33%), talon cusp(0.20%), fusion(0.07%) and gemination(0%) in descending order of frequency. 2. Peg lateralis, dens invaginatus and dilaceration occurred more prevalent in females, and other types of tooth shape anomalies occurred more prevalent in males. 3. Dens evaginatus and taurodontism occurred with bilateral tendency, but other anomalies occur- ed with unilateral tendency. 4. As to the involved teeth, maxillary lateral incisors were the most frequently involved teeth on peg lateralis, dens invaginatus and talon cusp. And the mandibular premolars were the most frequently involved teeth on dens evaginatus, the maxillary first molars were on taurodontism, the maxillary second premolars were on dilaceration, and the mandibular incisors and canines were on fusion.

Purpose: To describe the features of impacted upper canines and their relationship with adjacent structures through three-dimensional cone-beam computed tomography (CBCT) images. Materials and Methods: Using the CBCT scans of 79 upper impacted canines, we evaluated the following parameters: gender, unilateral/bilateral occurrence, location, presence and degree of root resorption of adjacent teeth (mild, moderate, or severe), root dilaceration, dental follicle width, and presence of other associated local conditions. Results: Most of the impacted canines were observed in females (56 cases), unilaterally (51 cases), and at a palatine location (53 cases). Root resorption in adjacent teeth and root dilaceration were observed in 55 and 47 impacted canines, respectively. In most of the cases, the width of the dental follicle of the canine was normal; it was abnormally wide in 20 cases. A statistically significant association was observed for all variables, except for root dilaceration (p=0.115) and the side of impaction (p=0.260). Conclusion: Root resorption of adjacent teeth was present in most cases of canine impaction, mostly affecting adjacent lateral incisors to a mild degree. A wide dental follicle of impacted canines was not associated with a higher incidence of external root resorption of adjacent teeth.

Dilaceration is a developmental disturbance in shape of teeth. It refers to an angulation, or a sharp bend or curve, in the root or crown of a formed tooth. Dilacerations are estimated to occur in 3% of all permanent dentitions. However, the etiology of dilaceration is not yet fully understood. Traumatic injury to the deciduous predecessors and ectopic development of the tooth germ are the two commonly cited causes of this anomaly. The involved tooth is usually the maxillary central incisor and it also shows high prevalence of impaction. At first, We tried to do surgical exposure with orthodontic traction but to no avail. After then surgical repositioning of the dilacerated tooth was performed. Finally, Satisfactory results were obtained at the end of treatment, with improvement in dental esthetics and functional aspects.

Biomechanical preparation of root canals with accentuated curvature is challenging. New rotatory systems, such as Reciproc, require a shorter period of time to prepare curved canals, and became a viable alternative for endodontic treatment of teeth with root dilaceration. Thus, this study aimed to report a clinical case of endodontic therapy of root with accentuated dilaceration using Reciproc single-file system. Mandibular right second molar was diagnosed as asymptomatic irreversible pulpitis. Pulp chamber access was performed, and glide path was created with #10 K-file (Dentsply Maillefer) and PathFile #13, #16 and #19 (Dentsply Maillefer) up to the temporary working length. The working length measured corresponded to 20 mm in the mesio-buccal and mesio-lingual canals, and 22 mm in the distal canal. The R25 file (VDW GmbH) was used in all the canals for instrumentation and final preparation, followed by filling with Reciproc gutta-percha cones (VDW GmbH) and AH Plus sealer (Dentsply Maillefer), using thermal compaction technique. The case has been receiving follow-up for 6 mon and no painful symptomatology or periapical lesions have been found. Despite the difficulties, the treatment could be performed in a shorter period of time than the conventional methods.

Impaction of maxillary incisor is rare than the third molar and canine, but its rate is higher than the other anterior teeth due to frequent mesiodens, trauma and variation of root formation (root dilaceration etc.). It is often observed in the dental age of about eight years and over. It will be occurred that the space loss, midline deviation and cyst formation due to the impaction of maxillary incisor. So it is important to evaluate the precise location of impacted tooth and to make appropriate treatment plan. Treatment would be surgical extraction or expectation for spontaneous eruption. If the impacted tooth has no pathologic change and development of the root is favorable, orthodontic traction is recommended for recovery of function and esthetics. In these cases, we performed orthodontic traction for the eruption of impacted maxillary incisors, and obtained satisfactory results.
