손톱위 피부피판을 이용한 손톱형태 복원의 치험례 (Restoration of the Injured Fingertip with Eponychial Cutaneous Flap)

  • The restoration of the fingernail is not simple in case of amputated distal fingertip which involved the pulp and nail. The nail should maintain a length of at least 2 mm from the eponychium for an adequate grip and decent appearance. Various methods to reconstruct the fingernail bed are available. The nail bed graft from amputated finger or great toe, and free onychocutaneous flap are commonly used. The nail bed of the injured tip tends to be atrophied, deformed and failed as a graft. And the great toe is often turned down as a donor. We have restored satisfactorily the nail beds of three injured finger tips with eponychial cutaneous flaps. The pulps were reconstructed with either a reverse dorsal digital island flap or free pulp graft. Repeated again. A mean follow- up was six months. The nail grew up to the average of 3.7 mm. All patients were satisfied with the length of the nail and met with good cosmetic results. An eponychial cutaneous flap is useful to restore the nail of the distal fingertip amputation. The procedure is relatively simple and morbidity is minimum.

  • Background First suggested by Brent in 1979, the pocket principle is an alternative method for patients for whom a microsurgical replantation is not feasible. We report the successful results of a modified palmar pocket method in adults. Methods Between 2004 and 2008, we treated 10 patients by nonmicrosurgical replantation using palmar pocketing. All patients were adults who sustained a complete fingertip amputation from the tip to lunula in a digits. In all of these patients, the amputation occurred due to a crush or avulsion-type injury, and a microsurgical replantation was not feasible. We used the palmar pocketing method following a composite graft in these patients and prepared the pocket in the subcutaneous layer of the ipsilateral palm. Results Of a total of 10 cases, nine had complete survival of the replantation and one had 20% partial necrosis. All of the cases were managed to conserve the fingernails, which led to acceptable cosmetic results. Conclusions A composite graft and palmar pocketing in adult cases of fingertip injury constitute a simple, reliable operation for digital amputation extending from the tip to the lunula. These methods had satisfactory results.

  • 최근 3D 애니메이션은 급속한 하드웨어와 소프트웨어의 기술적 발전에 힘입어 실사에 가까운 애니메이션 연출이 가능하게 되어 영화, 광고, 게임, 디지털 영상물 등의 엔터테인먼트적인 요소로 많이 사용되고 있다. 3D 애니메이션 제작과정은 전통적인 애니메이션과는 달리 다양한 제작기술과 표현요소를 요하게 된다. 3D 애니메이션 제작에서 제작기술과 표현요소는 각각의 요소로 작용하는 것이 아니라, 서로 결합하여 제작하여야만 완성도가 높은 애니메이션을 얻을 수가 있다. 이에 본 고는 3D 애니메이션 Computer Process에 의한 작업 과정을 하드웨어와 소프트웨어를 바탕으로 한 기술적인 표현방식과 미학적, 조형적, 디자인적, 회화적 요소를 바탕으로 한 예술적 표현요소를 분석하고 기술적인 완성도와 표현력이 있는 애니메이션을 제작하는 방법을 모색해 본다.

수지 재접합 실패시 허혈 상태의 수지골과 피판술을 이용한 구제술 (Salvage of Failed Digital Replantation Using Necrotizing Phalangeal Bone and Flap Coverage)

  • Failure of reattachment of finger is inevitable in replantation surgery and that failure rate is about 10 % are reported in many authors. Management of the failed finger replantation is challenge to microsurgeons. We report 7 cases of thumb reconstruction after failure of replantation. The reconstructive surgery composed with early debridement of soft tissue that are under gangrenous processing, extract the phalangeal bone without any soft tissues. Osteosynthesis of the extracted phalangeal bone with host phalangeal bone. The exposed bony portion covered with vascularized flaps such as reverse radial forearm pedicled flap, free radial forearm flap and neurovascular island finger flap. This procedure underwent within a week after vascular insufficiency developed. All of the flaps are survived, bone union achieved within 3 months. The function and external appearance of the reconstructed thumb were encouraging; pinch power was average 1.2 pounds. Early removal of necrotizing soft tissue followed by covering none vascular phalangeal bone which extracted from the dead phalanx with vascularized flap is one of the useful alterative solutions in failed replantation surgery in hand.

  • Since G.N. Hounsfield's clinical use of computed tomography in 1971, digital imaging technique using computers has shown an eye opening progress. Progress has made 3-dimensional understanding of not only facial bones but muscles and other connective tissues possible through 3-dimensional reconstruction of preexisting tomographical images. Also, quantitative analysis of density, distance, volume has become possible, allowing objective analysis of preoperative and postoperative states through imaging. The authors measured the masseter muscle volume of 20 normal individuals and 8 female patients through 3-D reconstructive CT imaging and made a statistical analysis of the measurements. The method used in our study may be applied to the diagnosis of disease causing the change of the facial volume and presurgical design as a useful tool to provide objective information on the evaluation of surgery outcome.

Safe clinical photography: best practice guidelines for risk management and mitigation

  • Clinical photography is an essential component of patient care in plastic surgery. The use of unsecured smartphone cameras, digital cameras, social media, instant messaging, and commercially available cloud-based storage devices threatens patients' data safety. This paper Identifies potential risks of clinical photography and heightens awareness of safe clinical photography. Specifically, we evaluated existing risk-mitigation strategies globally, comparing them to industry standards in similar settings, and formulated a framework for developing a risk-mitigation plan for avoiding data breaches by identifying the safest methods of picture taking, transfer to storage, retrieval, and use, both within and outside the organization. Since threats evolve constantly, the framework must evolve too. Based on a literature search of both PubMed and the web (via Google) with key phrases and child terms (for PubMed), the risks and consequences of data breaches in individual processes in clinical photography are identified. Current clinical-photography practices are described. Lastly, we evaluate current risk mitigation strategies for clinical photography by examining guidelines from professional organizations, governmental agencies, and non-healthcare industries. Combining lessons learned from the steps above into a comprehensive framework that could contribute to national/international guidelines on safe clinical photography, we provide recommendations for best practice guidelines. It is imperative that best practice guidelines for the simple, safe, and secure capture, transfer, storage, and retrieval of clinical photographs be co-developed through cooperative efforts between providers, hospital administrators, clinical informaticians, IT governance structures, and national professional organizations. This would significantly safeguard patient data security and provide the privacy that patients deserve and expect.

  • 아이폰을 선두로 하여 세계적으로 스마트폰이 각광을 받기 시작했다. 더불어 스마트폰의 성능이 향상됨에 따라 사용자가 점점 증가하고 있다. 이에 따라, 스마트폰을 이용한 다양한 연구가 진행되고 있고 응용 프로그램(Application)도 다양하게 개발되고 있다. 본 논문에서는 스마트폰과 디지털 아카이브즈 시스템을 접목하여 자료 수집 전문 연구원(아키비스트)에게 보다 쉬운 자료 수집 환경을 제공한다. 또한, 일반 사용자들에게는 아카이브 자료를 쉽게 이용 할 수 있도록 편이성을 제공하고자 한다. 이를 위해 개발자에게 편리한 개발환경과 부대비용이 들지 않는 Google의 안드로이드(Android) 플랫폼을 선택하여 스마트폰 상의 디지털 아카이브즈 시스템을 구현 하였다.

Reconstruction of Multiple Digital Defects by Temporary Syndactylization Using a Lateral Arm Free Flap

  • Background Soft tissue defects of the multiple finger present challenges to reconstruction surgeons. Here, we introduce the use of a lateral arm free flap and syndactylization for the coverage of multiple finger soft tissue defects. Methods This retrospective study was conducted based on reviews of the medical records of 13 patients with multiple soft tissue defects of fingers (n = 33) that underwent temporary syndactylization with a microvascular lateral arm flap for temporary syndactylization from January 2010 to December 2020. Surgical and functional outcomes, times of flap division, complications, and demographic data were analyzed. Results Middle fingers were most frequently affected, followed by ring and index fingers. Mean patient age was 43.58 years. The 13 patients had suffered 10 traumas, 2 thermal burns, and 1 scar contracture. Release of temporary syndactyly was performed 3 to 9 weeks after syndactylization. All flaps survived, but partial necrosis occurred in one patient, who required a local transposition flap after syndactylization release. The mean follow-up was 15.8 months. Conclusion Coverage of multiple finger defects by temporary syndactylization using a free lateral arm flap with subsequent division offers an alternative treatment option.

Electronic Theses and Dissertations' Development in University Libraries in Nigeria

  • The study was meant to examine electronic theses and dissertations' development in Nigerian federal university libraries. The study adopted descriptive survey research design. The study population comprised forty-eight institutional repository librarians in eighteen Nigerian federal university libraries. The sample consisted of forty three librarians. A questionnaire was used for data collection. The data collected were analyzed with mean and standard deviation. Findings reveal that the policies available for developing electronic theses and dissertations (ETDs) in the libraries require submission of electronic copies of theses and dissertations by students, participation of digital/IR librarians in ETD development and provision of free access to ETDs; the libraries comply with standards of Open Archives Initiative Protocol for Metadata Harvesting and Search/Retrieval via URL using DSpace for building ETDs and taking advantage of such available ICT facilities as computers, scanners, CD-ROM, LAN, internet connectivity and printer. The libraries achieve ETD through digitisation, mandating submission of electronic versions of TDs by students and creation of IR. However, the process is hindered by technological obsolescence, financial problems, absence of clearly-defined ETD development policy, required ICT facilities and trained staff, copyright, irregular power supply and unstable internet connectivity. This study represents a fresh empirical investigation into electronic theses and dissertations' development in federal university libraries in Nigeria.
