일측성 구순열 환자에서 3차원 수치사진측량 스캔과 직접계측 방법의 비교 (Comparison of the 3D Digital Photogrammetry and Direct Anthropometry in Unilateral Cleft Lip Patients)

  • Background: In cleft lip patients, the necessity of a thorough preoperative analysis of facial deformities before reconstruction is unquestioned. The surgical plan of cleft lip patient is based on the information gained from our preoperative anthropometric evaluation. A variety of commercially available three-dimensional (3D) surface imaging systems are currently introduced to us in plastic surgery for these use. However, few studies have been published on the soft tissue morphology of unrepaired cleft infants described by these 3D surface imaging systems. Methods: The purpose of this study is to determine the accuracy of facial anthropometric measurements obtained through digital 3D photogrammetry and to compare with direct anthropometry for measurement in unilateral cleft lip patients. We compared our patients with three measurements of dimension made on both sides: heminasal width, labial height, and transverse lip length. Results: The preoperative measurements were not significantly different in both side of labial height and left side of heminasal width. Statistically significant differences were found on both side of transverse lip length and right side of heminasal width. Although the half of preoperative measurements were significantly different, trends of results showed average results were comparable. Conclusion: This is the first study in Korea to simultaneously compare digital 3D photogrammetry with traditional direct anthropometry in unilateral cleft lip patients. We desire this study could contribute the methodological choice of the many researchers for proper surgical planning in cleft lip reconstruction field.

수지 첨부 재접합술 101예의 후향적 분석 (A Retrospective Analysis of 101 Cases of Distal Digital Replantation)

  • We retrospectively evaluated our results of replantations of distal digital amputations and analyzed the factors deterrent to the survival of replanted digits. From January 2004 to 2005 June, we performed 101 cases of replantations following complete amputations at or distal to interphalangeal joint level. The study included 98 patients with a mean age of 35.6 years (range 1 to 63 years). Amputation level correlated to zone I (distal to the lunula)in 47 cases and zone II (lunula to distal interphalangeal joint) in 54 cases according to Yamano's classification. According to the mechanism of amputation, 24 cases (22.9%) suffered from guillotine type injury, 27 cases (27.1 %) from avulsion type injury and 50 cases (50%) from crush type injury. In all cases, a single arterial anastomosis was performed. Venous anastomosis on either volar or dorsal side was performed in 12 cases of amputation in zone II. Salvage procedure for venous drainage was performed in 98 cases. The mean duration of salvage procedures was 5.9 days (ranging from 4 to 14 days). Successful replantation was achieved in 96 cases (95.1%), which included 93.7% cases in zone I amputations and 96.3% cases in zone II amputations. A single venous anastomosis was performed in 12 cases of amputation in zone II. All of them survived completely. Among the 5 cases that failed to survive, 3 cases were related with avulsion injury in zone I. Initial mechanism of injury determines the survival rate of amputated parts as it is directly related with the status of vessels and soft tissues. Meticulous precaution during the salvage procedure may affect the overall survival rate of distal digital replantations.

원위 수지 재접합술 뒤 간헐적 실혈 요법 (Intermittent Bleeding Method after Replantation o the Distal Phalanx)

  • Purpose: To evaluate the effect of intermittent bleeding method in the distal phalanx replantation. Materials and Methods: From January 2007 through June 2009, authors have replanted 117 cases of distal phalangeal amputation in adults at Soo Hospital and Chonbuk National University Hospital. Cases of zone II were 60 cases and zone III 57 according to Allen classification. Male to female ratio was 8.7:1.3. The most common cause was machinery injury in the factory, 98 cases(83.8%), next one was belt injury of the machine, 11 cases(9.4%) and others, 8(6.8%). At least one digital artery and digital nerve were anastomosed under the operating microscope, but vein was impossible to anastomosis as unable to find out in the zone II and III. After anastomosis of one or more digital arteries and nerves, heparine(6,000-10,000 units) was kept to intravenous injection for 24 hours and at the same time fish mouth incision in 2-3 millimeter diameter was made in the distal radial and ulnar margin of the replanted distal phanlanx. From the first 30 minutes to an hour after replantation, incision site was swabbed with heparinized cotton ball for 5 minutes in every 30 to 40 minutes to make sure perfusion for 24 hours, every an hour at the second day, every two hours at the postoperative third to fifth day. Results: 92 cases(78.6%) was completely survived at average postoperative third week follow-up and satisfied with preservation of the finger nail, digit length, good range of motion of the distal interphalangeal joint and acceptable sensibility at average 1.2 years follow-up. Conclusions: Intermittant bleeding method in replantation of crushed distal phalanx impossible to anastomosis of vein at zone II and III of Allen classification was regarded as one of the notable salvage procedure.

고정관념의 정체와 창조적 사고를 위한 두뇌활용법 연구 (A study on Nature of the Fixed Idea and the Activation of the Brain for Creative Thinking)

  • 인지과학이 새로운 학문으로 등장하기 시작하면서 인간의 두뇌를 연구하는 움직임 또한 매우 활발히 일어나고 있다. 이와 함께 디자인영역에서도 감성적 요소가 연구의 중심을 차지하기 시작했고 이를 활용한 디자인 프로세스 개발이 다양화되어 가고 있다. 본 연구에서는 변화하고 있는 이런 현상들을 적극적으로 수용하고 이를 디자인기법으로 전환시켜 보고자 노력하였다. 그를 위해 현재 유행처럼 확산되고 있는 마인드 맵을 기법의 대상으로 삼았고 고정관념에서 벗어 날 수 있는 방법에 대한 이론적 설명을 시도하였다. 문제에 접근하기 위해 학습되는 정보가 중요하다고 판단하고 좌, 우측 뇌의 역할을 아날로그이미지와 디지틀 이미지로 규정, 언어(디지틀 이미지)와 시각적 사고(아날로그 이미지)를 자유롭게 넘나드는 방식으로 맵핑기법을 활용하여 해석하였다. 기법의 연구목표는 멥핑방식을 이용한 발상법을 초점으로 추진하였고 그 결과 디자인프로세스 상에서 문제제기로 시작된 명제를 문제 해결까지 브레인스토밍기법으로 진행하며, 문제 해결은 컨셉제시로 가시화되어진다는 논리체계[그림8]를 구축하였다. 또 제시된 컨셉에 의해 디자인적 요구에 의한 아이디어 확산도 맵핑기법을 적용 해결할 수 있다고 보고 [그림12]와 같은 사례를 결과물로 제시하였다.

Analysis of Bone Fixation Methods in Digital Replantation

  • Background Adequate fixation of replanted digits is essential not only for short-term healing but for long-term function. Various bony fixation methods using Kirschner (K-) and intraosseous wire are available in replantation. We examined clinical and radiographic outcomes of fixation methods on bone union after digital replantation. Methods A single institutional retrospective review identified 992 patients who had undergone 1,247 successful replantations between July 2009 and September 2015. Exclusion criteria included amputations of the distal phalanx, comminuted fractures, and intra-articular fractures. Patients were classified according to 5 categories of fixation methods: single K-wire, double longitudinal K-wires, cross K-wires, wire with, and wire without K-wire support. Bone union was evaluated by 5-month postoperative X-ray and fixation outcomes were compared across the 5 groups. Results The exclusion criteria were applied, and 88 patients with 103 replanted digits remained for analysis. Single K-wire fixation was used in 40 digits, double longitudinal K-wires in 30, and cross fixation in 14. Wire with and without K-wire support was required in 15 and 4 digits. Nonunion was observed in 32 digits (31.1%), of which 13 required additional operations such as bone graft or corrective osteotomy. The highest percent of nonunion was observed after cross fixation (35.7%) and the lowest after wire alone (25.0%). Conclusions In this study, contrary to general knowledge, we found that single K-wire fixation was not associated with poorer outcomes. Successful bone union outcomes may be achieved by careful selection of bone fixation methods. This study provides useful information for planning bone fixation in digital replantation.

가상현실 기술을 이용한 문화재의 디지털 복원 (Digital Restoration of Cultural Properties Based on Virtual Reality)

  • 오늘날 현대 사회에서 첨단 과학의 눈부신 발전은 전 세계 인류에게 다양한 정보를 손쉽고 빠르게 획득할 수 있는 기회를 제공하고 있다. 이러한 첨단 과학의 비약적인 발전 중에 디지털 영상매체 분야는 여러 산업 분야와 일상생활에 적용되어 멀티미디어 정보사회를 대체 할 수 있는 수단으로 등장하게 되었다. 특히 최근 문화재의 복원에 대한 관심과 열의가 높아지고 있다. 따라서 문화재의 보존이나 복원 및 이들의 학문적 가치를 판정하고 기록에 남길만한 정보의 수집 등을 위한 효율적인 방법이 필요해졌으며, 이의 한 방안으로써 컴퓨터를 이용한 영상처리 기술이 문화재 복원에 도입되었다. 이러한 기법의 문화재 복원은 원본을 손상시키지 않고 문화재의 파괴된 부분을 복원시킬 수 있다는 장점으로 인해 선진 각 국에서 활발한 연구가 진행중이다 그러나, 유구한 역사 속에 무수한 문화유산을 갖고 있는 우리나라로서는, 이와 관련된 연구가 부족한 상황이다 따라서 본 연구에서는 먼저 가상현실에 대하여 고찰하고 이 기술을 문화재 복원에 적용하는 방법에 대하여 살펴 본 후, 실제 디지털 기술을 적용한 사례를 국외 국내로 나눠 소개하고 결론적으로 가상현실 기술을 통한 디지털 문화재 복원에 대한 의의를 제안해 보고자 한다.

디지털시대 대학도서관의 경영기법에 관한 연구 (Management Skills for Academic Libraries in the Digital Age)

  • 이 연구는 디지털시대에서 대학도서관을 보다 원활히 운영하고 사서들이 서비스를 향상하기 위해 어떠한 경영기법이 필요한가를 모색하였다. 이 연구를 위해 관련 자료를 수집, 분석하였으며 미국 대학도서관의 여러 사서들과 의견을 교환하는 과정을 거쳤다. 이 연구에서 제시된 경영기법들은 학습조직, 지식경영, 팀 관리, 고객지향적 서비스, 변화관리 둥과 같이 주로 영리를 추구하는 기업에서 활용되는 개념들로부터 나온 것이다. 이 연구의 결과 대학도서관이 서비스를 향상하고 대학사회에서 위상을 강화하기 위해 이러한 경영기법을 활용하는 것은 별다른 무리가 없음을 확인할 수 있었다. 다만 대학도서관 사서진은 디지털시대에서도 학술광장이자 비영리조직인 대학도서관의 핵심적 가치를 염두에 두고 도서관을 경영하며 서비스의 지평을 넓혀갈 필요가 있다.

  • A computer program which can differentiate compounds whose absorbance spectra are very similar was developed. The program. [SPECMAN PLUS], written in Pascal provides automated spectral comparison techniques, utilizing the values of Root Mean of Squares (RMS) of differences. This comparison routine of the program can deal with spectra of compounds different concentrations and different spectral recording resolutions. In addition, the program was designed applicable to any spectral data of digital form. The program was applied to the UV spectra of 13 pencillins and 5 cephalosporins, whose absorbance spectra are so similar. As a result, all compounds examined could be differentiated from each other.

The Meaning, Method and Tool to Build the Ewha Music Database (EMDB)

  • The Ewha Music Database (EMDB) is an online database comprised of primary source materials related to music education from East Asia during the modern era (1880 to 1945) when Korea, Japan, and China were geopolitically and culturally intertwined. We developed the incipit search in EMDB as an embedded tool. This is the first attempt in Korea to implement a unique search function of musical data using alphabets of musical notes. Unlike in traditional search system that uses general literature information search conditions, such as author, title, publisher, year, number of pages, etc., it offers a new way of searching a musical piece/work and sheet music. This study confirms that digital information technology is an important methodology for research of music culture as a field of humanities.

소아에서의 원위 수지 절단의 재접합술에 대한 임상적 고찰 (Fingertip replantations in children)

  • From Jan. 1998, 28 young children patients with complete amputations at zone I or II were replanted using a microscopic technique. Children's average age was 4.5 years and the amputation levels were zone I in 12 cases and zone II in 16 cases. Overall survival rate was 57%(16 cases). In children, the advantages of the replantation for distal digital amputations are nearly normal length, good soft tissue coverage with nail and cosmetically high patient satisfaction but surgical technical difficulties are much greater than in adults.
