Susceptibility Vessel Sign for the Detection of Hyperacute MCA Occlusion: Evaluation with Susceptibility-weighted MR Imaging

  • Purpose: Susceptibility vessel sign (SVS) on gradient echo image, which is caused by MR signal loss due to arterial thrombosis, has been reported in acute middle cerebral artery (MCA) infarction. However, the reported sensitivity and diagnostic accuracy of SVS have been variable. Susceptibility-weighted imaging (SWI) is a newly developed MR sequence. Recent studies have found that SWI may be useful in the field of cerebrovascular diseases, especially for detecting the presence of prominent veins, microbleeds and the SVS. The purpose of this study was to evaluate the diagnostic values of SWI for the detection of hyperacute MCA occlusion. Materials and Methods: Sixty-nine patients (37 males, 32 females; 46-89 years old [mean, 69.1]) with acute stroke involving the MCA territory underwent MR imaging within 6 hours after the symptom onset. MR examination included T2, FLAIR (fluid-attenuated inversion recovery), DWI, SWI, PWI (perfusion-weighted imaging), contrast-enhanced MR angiography (MRA) and contrast-enhanced T1. Of these patients, 28 patients also underwent digital subtraction angiography (DSA) within 2 hours after MR examination. Presence or absence of SVS on SWI was assessed without knowledge of clinical, DSA and other MR imaging findings. Results: On MRA or DSA, 34 patients (49.3%) showed MCA occlusion. Of these patients, SVS was detected in 30 (88.2%) on SWI. The sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy of SWI were 88.2%, 97.1%, 96.8%, 89.5% and 92.8%, respectively. Conclusion: SWI was sensitive, specific and accurate for the detection of hyperacute MCA occlusion.

Edge Enhancement for Vessel Bottom Image Considering the Color Characteristics of Underwater Images (수중영상의 색상특성을 고려한 선박하부 영상의 윤곽선 강조 기법)

  • Image distortion can occur when photographing deep sea targets with an optical camera. This problem arises because sunlight is not sufficiently transmitted due to seawater and various floating particles of dust. Particularly, color distortion takes place, causing green and blue color channels to be over emphasized due to water depth, while distortion of boundaries also occurs due to light refraction by seawater and floating particles of dust. These distortions degrade the overall quality of underwater images. In this paper, we analyze underwater images of the bottom of vessels. Based on the results, we propose a technique for color correction and edge enhancement. Experimental results show that the proposed method increases edge clarity by 3.39 % compared to the effective edges of the original underwater image. In addition, a quantitative evaluation and subjective image quality evaluation were concurrently performed. As a result, it was confirmed that object boundaries became clear with color correction. The color correction and contour enhancement method proposed in this paper can be applied in various fields requiring underwater imaging in the future.

  • With the development of digital technology, the marine environment is expected to change rapidly. In the case of autonomous vessels, technology is being developed in many countries, and the international community has begun to discuss ways to operate it. Changes in ships cause changes in the marine traffic environment and urge changes to aids to navigation. This study aims to analyze the cyber security management awareness of VTS personnel to improve the cyber security system for aids to navigation. To this end, the current status of cyber security management was reviewed with a focus on VTS, and a survey was conducted on VTS personnel. The survey analysis used the IPA methodology, and as a result of the analysis, a clear difference was observed in the perception of cybersecurity between those with experience in security and those without experience. In addition, technical measures related to cyber-attack detection and blocking should be implemented with the highest priority. The results of this study can be used as basic data for improving the cyber security management system for aids to navigation.

The First Web Space Free Flap of the Foot to Reconstruct the Pulp of Fingers (수지 수질부 재건을 위한 족부의 제 1물갈퀴부 유리 피판술)

  • Purpose: The first web space of the foot has a similar thickness and skin texture of the pulp of the fingers. Moreover, it has a reliable blood vessel and sensory nerve. The purpose of this study was to evaluate the clinical results of the first web space free flap to reconstruct the pulp of fingers. Materials and Methods: Authors have performed 23 cases of first web space free flap to reconstruct the pulp defect of the fingers between June 2004 and May 2009. The age of the patients ranged from 20 years old to 55 years old. The size of the flap ranged from $1{\times}1.5cm$ to $8.5{\times}2.5cm$. The mean flap area was 5.4 cm2. In 4 cases, we elevated the flap including lateral aspect of the big toe and medial aspect of the second toe. And then we made an artificial syndactyly to reconstruct the pulps on two fingers at the same time. In all cases, we performed 1 digital artery and 1 dorsal vein anastomosis. Every donor site that had a small defect healed spontaneously without any additional operations to cover it. Results: Of this type of surgery 21 flaps (91.3%) survived, 2 flaps (8.7%) failed. There was no severe complication in the donor sites. There was no walking disturbance due to the skin defect of the donor site. The static 2 point discrimination in 11 cases that we could check ranged from 3 mm to 15 mm. Conclusion: The authors believe that the first web space free flap of the foot is a good option for the reconstruction of the pulp of the fingers and it has a minimal donor site morbidity.

  • Destruction phenomena of structure by gas explosion is due to the explosion pressure and heat. Explosion pressure is a kind of energy converted from the gas mixture explosion. In this paper, we tried to find the relationship between explosion characteristics and combustion heat of the hydrocarbon-oxygen mixtures. Experiment were carried out with the volume of $5916cm^3$ cylindrical explosion vessel. Hydrocarbon gases which used in this study were methane, ethylene, propane, and buthane Experimental parameter was the concentration of the gas mixtures. Explosion characteristics were measured with strain type pressure transducer through the digital storage oscilloscope. From the experimental result, it was found that explosion pressure depend upon the combustion heat.

  • Recently, many research works on the icebreaking vessels have been published as the possibility of passing Arctic routes has been increasing. The model ship test on the pack ice model in the ice basin is actively carried out as a way to investigate the performance of icebreaking vessels. In this test, the concentration of pack ice is important since it directly affects the performance. However, it is difficult to measure the concentration because not only the pack ice has uneven shape but also it keeps floating around in the basin. In this paper, an algorithm to identify the concentration of pack ice is introduced. From a digital image of pack ice obtained in the ice basin, the goal is to measure the area of pack ice using an image processing technique. Instead of the general global binarization that yields numerical errors in this problem, a local binarization technique, coupled with image subdivision based on the quadtree structure, is developed. The concentration results obtained by the developed algorithm are compared with the manually measured data to prove its accuracy.

The Underwater Noise of Fishing Gears in Operation (망어구의 수중소음에 관한 연구)

  • An underwater recording system was designed to measure the sound spectra of the underwater noises produced by fishing gears in operation. Recorded were noi~es from three types of fishing gears: an anchovy set net, three anchovy boat seine net and a stern trawlnet. Acoustic analysis were made using a heterodyne analyzer, a digital frequency analyzer and a level recorder. The no;'e produced by the anchovy set net was found in the high frequency region of the onset of ambient noise spectrum with a slope of - 6 dB/octave. Here the ambient noise spectrum is higher, though similar in shape, than Knudsen spectrum, and is attributed to the breaking action of the coastal wave. Measured noise spectra during the fishing operations of the anchovy boat seine nets are attributed to the background noise of the sea in the presence of the fishing vessels. The frequency distribution of the noise was 20~5, 000 Hz in the case of two steel anchovy boat seiners, and 20-3,000 Hz in the case of the wooden anchovy boat seiner. The predominant frequency range was 250~350 Hz and maximum sound pressure level was 122 dB (re $1\muPa$) in the case of the steel boat and ] 17 dB in the case of the wooden boat. The noises produced by the trawl fishing gears are remarkably higher than the background noi~e in the presence of the fishing vessel. The frequency distribution of the noi~e was 20-6,300 Hz. The predominant frequency range was 100~200 Hz and maximum sound pressure level was 137 dB ( re $1\muPa$) . The noise spectra were not so much different from that caused by vibrations of the towing cable and the structure of the ground rope of the trawl net towed in an experimental tank.

Intraarterial Nimodipine Infusion to Treat Symptomatic Cerebral Vasospasm after Aneurysmal Subarachnoid Hemorrhage

  • Objective : Cerebral vasospasm leading to cerebral ischemic infarction is a major cause of morbidity and mortality in the patients who suffer with aneurysmal subarachnoid hemorrhage. Despite adequate treatment, some patients deteriorate and they develop symptomatic vasospasm. The objective of the present study was to investigate the efficacy and clinical outcome of intraarterial nimodipine infusion on symptomatic vasospasm that is refractory to hemodynamic therapy. Methods : We retrospectively reviewed the procedure reports, the clinical charts and the transcranial doppler, computed tomography and digital subtraction angiography results for the patients who underwent endovascular treatment for symptomatic cerebral vasospasm due to aneurysmal SAH. During the 36 months between Jan. 2005 and Dec. 2007, 19 patients were identified who had undergone a total of 53 procedures. We assessed the difference in the arterial vessel diameter, the blood flow velocity and the clinical outcome before and after these procedures. Results : Vascular dilatation was observed in 42 of 53 procedures. The velocities of the affected vessels before and after procedures were available in 33 of 53 procedures. Twenty-nine procedures exhibited a mean decrease of 84.1 cm/s. We observed clinical improvement and an improved level of consciousness with an improved GCS score after 23 procedures. Conclusion : Based on our results, the use of intraarterial nimodipine is effective and safe in selected cases of vasospasm following aneurysmal SAH. Prospective, randomized studies are needed to confirm these results.

  • The context of innovation in which we evolve today, subtracts us in a spacial reality and virtuality (digital) that aimed less and less to interact with natural processes which could converge to new possible relationships in the world. We constantly live in presence of fluctuations and imperceptible natural energies (wind, solar radiation, etc.) defined by flows, their own physicality, which remains without being virtual, elusive. This study first outlines how these energies already exploited within the framework of production, could be thought as interactive of our habitat's space dimension, as a prolongation of a physical and material environment built by men and for men, giving rise to new social, cultural dynamics, and making natural complexity of our space vivid, comprehensible with new visual and physical clues. In recent days, where lifestyles are changing, architecture no longer needs to limit its scope of creation to only built structures. Based on a deeper understanding of human and through new potential advanced technologies (kinetic system, etc), it is time to fundamentally diagnose what environments or devices contribute to our lives. Architecture becomes ${\ll}interface{\gg}$, step up its fundamental role, and newly defines the sturdy image and tectonics of existing environment, establishing a stance to search for a new typology. In the end, building will show two simultaneous and distinctive connections related to its physical existence: reality in its function and irreductibility, in its ability to forge new dynamic connections with its environment, hybridizing the spatial dimension to a new form of physicality, adaptive and incessantly flexible in the dimension time, becoming a vessel for ever changing contemporary lifestyles.

Should Cerebral Angiography Be Avoided within Three Hours after Subarachnoid Hemorrhage?

  • Objective : While the risk of aneurysmal rebleeding induced by catheter cerebral angiography is a serious concern and can delay angiography for a few hours after a subarachnoid hemorrhage (SAH), current angiographic technology and techniques have been much improved. Therefore, this study investigated the risk of aneurysmal rebleeding when using a recent angiographic technique immediately after SAH. Methods : Patients with acute SAH underwent immediate catheter angiography on admission. A four-vessel examination was conducted using a biplane digital subtraction angiography (DSA) system that applied a low injection rate and small volume of a diluted contrast, along with appropriate control of hypertension. Intra-angiographic aneurysmal rebleeding was diagnosed in cases of extravasation of the contrast medium during angiography or increased intracranial bleeding evident in flat-panel detector computed tomography scans. Results : In-hospital recurrent hemorrhages before definitive treatment to obliterate the ruptured aneurysm occurred in 11 of 266 patients (4.1%). Following a univariate analysis, a multivariate analysis using a logistic regression analysis revealed that modified Fisher grade 4 was a statistically significant risk factor for an in-hospital recurrent hemorrhage (p=0.032). Cerebral angiography after SAH was performed on 88 patients ${\leq}3$ hours, 74 patients between 3-6 hours, and 104 patients >6 hours. None of the time intervals showed any cases of intra-angiographic rebleeding. Moreover, even though the DSA ${\leq}3$ hours group included more patients with a poor clinical grade and modified Fisher grade 4, no case of aneurysmal rebleeding occurred during erebral angiography. Conclusion : Despite the high risk of aneurysmal rebleeding within a few hours after SAH, emergency cerebral angiography after SAH can be acceptable without increasing the risk of intra-angiographic rebleeding when using current angiographic techniques and equipment.
