Peripheral Arterial Bypass using Interpositional Vein Graft in the Hypothenar Hammer Syndrome: A Case Report (소지구 망치증후군에서 정맥이식을 이용한 동맥우회술: 증례 보고)

  • Purpose: The clinical syndrome of unilateral finger ischemia, caused by digital artery occlusions from embolization from the palmar ulnar artery associated with repetitive striking of the palm, has been called the hypothenar hammer syndrome (HHS). We report the case of a man with this unique disease probably caused by manual work. Methods: A 52 - year - old male left - hand dominant manual worker complained of pain and coldness in the left 4,5th finger. On physical examination, there was a tip necrosis and the result of Allen's test was mildly positive(sluggish filling of hand from the ulnar artery). Arteriograms confirmed occlusion of the distal ulnar artery without direct perfusion of the superficial palmar arch and distal digital artery. Surgical bypass with reverse autologous vein grafting was performed between ulnar artery and superficial palmar arch, common digital artery. Results: He had an uneventful postoperative course and has remained asymptomatic for 18 months since discharge. Patency has been confirmed by color doppler with resolution of cold intolerance and successful digital preservation. Conclusion: We introduce very unique pattern of vascular ischemic disease and recommend the arterial bypass with vein interpositional grafting.

Effects of Using Convergence Digital Contents for High-Intensity Interval Exercise on Growth Hormone and Fatigue Elements in Middle Aged Women (융복합 디지털 콘텐츠를 활용한 고강도 인터벌 운동이 중년여성의 성장호르몬과 피로물질에 미치는 영향)

  • This study was tested growth hormone and fatigue materials after eight-week high-intensity interval exercise to middle aged women (40's~50's women) who have been obesity without other disease. Two group of 20 candidates were randomly divided 10 persons as exercise group and control group. Exercise was conducted 8 weeks 4 days 35 minutes, before the study each group was checked vital sign for fatigue materials. It showed the following results. First, it has been increased growth hormone level after exercise program compared each group, significantly. Secondly, it has been decreased fatigue materials due to the high-strength interval workout for 8 weeks compared control as significantly. As a result, eight-week high-intensity interval exercise could be increased growth hormone levels by reducing fatigue and it might be preventing fatigue materials levels. We would suggest that high-strength interval workout for 8 weeks could have a positive effect for preventing and reducing fatigue and related disease, obesity.

  • As life expectancy has increased, health status becomes an important factor on life. The Fourth Industrial Revolution has led to the development of a variety of healthcare devices and applications that make it easier and more convenient to manage health. In particular, the area of individual genetic analysis with Single Nucleotide Polymorphism (SNP) has expanded, and genetic information is used to treat and prevent diseases according to individual differences. This study analyzes the genetic information of individuals, that is, SNP variant, and identifies specific chronic disease risk groups such as obesity, diabetes and hypertension. After then we will propose a customized nutrition information service. In addition the service of regional exercise management will be provided, to encourage exercise based on walking courses and sports center information in residential areas. Based on the GPS, it will design the service of exercise management that can encourage exercise by providing walking course and sports center information in the residential area.

CAD for Detection of Brain Tumor Using the Symmetry Contribution From MR Image Applying Unsharp Mask Filter

  • Automatic detection of disease helps medical institutions that are introducing digital images to read images rapidly and accurately, and is thus applicable to lesion diagnosis and treatment. The aim of this study was to apply a symmetry contribution algorithm to unsharp mask filter-applied MR images and propose an analysis technique to automatically recognize brain tumor and edema. We extracted the skull region and drawed outline of the skull in database of images obtained at P University Hospital and detected an axis of symmetry with cerebral characteristics. A symmetry contribution algorithm was then applied to the images around the axis of symmetry to observe intensity changes in pixels and detect disease areas. When we did not use the unsharp mask filter, a brain tumor was detected in 60 of a total of 95 MR images. The disease detection rate for the brain was 63.16%. However, when we used the unsharp mask filter, the tumor was detected in 87 of a total of 95 MR images, with a disease detection rate of 91.58%. When the unsharp mask filter was used in the pre-process stage, the disease detection rate for the brain was higher than when it was not used. We confirmed that unsharp mask filter can be used to rapidly and accurately to read many MR images stored in a database.

A Case Report on Parkinson's Disease Soyangin Patient with Abdominal Pain and Constipation (만성복통 및 변비를 호소하는 소양인 파킨슨병 환자의 치험1례)

  • Objectives The purpose of this case was to report significant improvement of abdominal pain, constipation and a slight improvement of tremor after treatment by Sasang Constitutional Medicine to the patient with Parkinson's disease.Methods The patient was diagnosed as Soyangin according to the result of Sasang constitutional diagnosis, and treated by Sasang constitutional medications and acupuncture. The progress was evaluated with the Global Assessment Scale (GAS).Results The abdominal pain was reduced from Numeric Rating Scale (NRS)9 to NRS1 and headache was reduced from NRS5 to NRS3. The tremor was reduced to GAS 10. Unified Parkinson Disease Rating Scale (UPDRS) was reduced from 40 to 27. The difference of heat distribution between left and right side was reduced and the abdomen was more warmer on Digital Infrared Thermal Imaging (DITI).Conclusions Sasang constitutional medicine can be a solution for the non motor symptoms caused by Parkinson's disease.

  • This is a case report of Crouzon's disease as a kind of craniofacial dysostosis by premature closure of unilateral coronal suture, showed plagiocephalic skull. 5-year-old boy was visited for the treatment of dental caries and oral examination. Physical examination showed hypertelorism, internal strabismus, and saddle nose. Intraoral radiographs showed congenital missing of upper right and left deciduous and permanent lateral incisors. Cephalometric analysis showed shortening the posterior cranial base length, clockwise growth pattern and class III and open bite tendency. Posterior-anterior and submentovertex view showed multiple radiolucencies-digital impression on inner surface of cranial vault. Maxillo-facial and neuro-surgical treatment was required to improvement of facial esthetics and optic complications. Continuous examination was needed to the growth and development.

Supraceliac Aorta Bypass Surgery For Juxtarenal Aortic Occlussive Disease -2 Cases - (신장근접 대동맥 폐색증의 상복강대동맥 우회술)

  • Total atherosclerotic obstruction of the juxtarenal abdominal aorta is a relatively rarely encountered form of atherosclerotic vascsular disease, accounting for less than 5% of all arterial obstrutive disease. We have encountered two patients with such lesions, both of whom were admitted for intermittent claudication of the lower extremities and symptoms of vascular ischemia. Digital subtraction angiography[DSA] was performed on both patients, the results of which revealed total obstruction of the aorta just inferior to the renal arteries without involving the latter. Operative technique involved the use of the sup-raceliac aorta as the site of proximal anastomosis of aortofemoral bypass followed by a fem-orofemoral bypass graft with Smm sized Woven Dacron[Vascutek] through a subcutaneous tunnel within the retroperitoneal space. Both patients experienced restoration of blood flow distal to the obstruction postoperatively without any complications, and OPD follow-up one month postoperatively and postoperative DSA showed evidence of continued graft patency with persistent symptomatic improvement.

The threat of Monkeypox in the Philippines: another problematic preparation and management for the healthcare system?

  • The Philippines is still in a tight battle with the coronavirus disease 2019 pandemic since many cases are detected daily. With the continuous spread of another disease worldwide-monkeypox, many Filipinos are alarmed if the country's healthcare system is prepared enough, especially with the detection of its first case. Learning from the unfortunate experiences of the country during the current pandemic is essential in facing another health crisis. With this, recommendations for a robust healthcare system are proposed centered on: a massive digital information campaign about the disease; training healthcare workers to raise awareness about the virus and its transmission, management, and treatment; an intensified surveillance and detection procedure to monitor cases and execute contact tracing properly; and a persistent procurement of vaccines and drugs for treatment, with a well-designed vaccination program.

The Impact of Self-Efficacy on Nurses' Well-Being: Does Digital Competence Matter?

  • Purpose: Drawing on person-environment fit theory and social cognitive theory, this study aimed to examine how self-efficacy affects nurses' workplace well-being via person-job fit and the moderating role of digital competence. Methods: A two-wave survey was conducted to collect data. Data were collected from six hundred and ninety-five nurses at three Chinese hospitals between May 2022 and September 2022. We employed hierarchical regression analysis and bootstrapping to analyze the data. Results: Self-efficacy positively influenced person-job fit (β = .55, p < .001), which positively affected nurses' workplace well-being (β= .32, p < .001). Person-job fit mediated the effect of self-efficacy on nurses' workplace well-being. Additionally, digital competence strengthened the positive impact of self-efficacy on person-job fit (β = .12, p < .001). Conclusion: Recruiting nurses with both self-efficacy and digital competence benefits hospitals. It is critical for nurses to improve their digital competence for achieving person-job fit and attaining workplace well-being in the post-coronavirus disease 2019 (COVID-19) era.
