자동차 페인트 도장공에 있어서 컴퓨터를 이용한 신경행동검사 수행기능의 평가 (Effect on Computerized Neurobehavioral Test Performance of the Car Painters Exposed to Organic Solvents)

  • A cross-sectional study was performed to evaluate the effects of chronic low-dose solvent on neurobehavioral performance of 118 male car painters. A control group of 113 workers matched for age was selected from different sections of the factory. The mean age and the mean duration of employment were 33 years and 6.7 years in both groups. Mean years of education were 11.4 years in car painters and 11.8 years in controls. Each worker completed a medical and occupational questionnaire and four tests of Swedish performance evaluation system. These included simple reaction time, symbol digit, digit span and finger tapping speed. Althougth the mean duration of employment was 6.7 years, comparison of mean performance showed a significantly poorer performance on simple reaction time (p<0.05), symbol digit(p<0.01) and digit span(p<0.05) in car painters. In univariate analysis, age and educational level contributed to poorer performance on symbol digit and digit span. Smoking appeared to slow finger tapping speed in car painters. Performance of four tests of car painters exposed to high level of solvent was poorer than that of car painters exposed to low level. In multiple regression models, controlling for age, alcohol, smoking and shift work, solvent exposure was found to be associated with performance of simple reaction time, symbol digit and digit span and exposure to high level of solvent was related to poorer performance of symbol digit and digit span.

만성적 유기용제 폭로로 인한 조선업 도장공들의 신경행동학적 영향에 관한 연구 (Neurobehavioral Effects of Chronic Exposure to Organic Solvents among Dock Yard Painters)

  • Across sectional study was performed to evaluate the chronic effects on central nerve system(CNS) of cumulative exposure of complex organic solvents, using neurobehavioral test. Subjects were 66 (male) dock yard painters of some large ship industry which is located in Ulsan. The neurobehavioral test battery used in this study was NCTB (Neurabehavioral Core Test Battery recommended by WHO(World Health Organization), which consisted of 7 items-Profile of mood states (POMS), Simple reaction time, Santa Ana dexterity test. Digit span, Digit symbol, Benton visual retention test and Pursuit aiming. The subjects were classified by 3 groups according to duration of employment(group 1 ; less than 9 years, group 2 ; 10-14 years, group 3 ; more than 15 years). The results of performance were analyzed considering of work duration, age, educational level, alcohol drinking, smoking and testing time as confounding factors. Benton visual retention test, pursuit aiming correct dot and sum of dot showed significant differences among the groups, and decreased with increasing work duration. It indicated that the mean scores of performance ability were lowering according to work duration. Besides, the tests that didn't show statistical significances but showed linear trends were depression-dejection, vigor, fatigue of POMS, slowest time of simple reaction time and digit span forward. Most of the neurobehavioral test items were correlated with age and educational level. After controlling of confounding factors-age and educational level, the results followed ; digit span backward was different significantly. Tension-anxiety, depression-dejection, anger-hostility of POMS, SD and slowest time of simple reaction time, Santa Ana dexterity test non-preferred hand, digit span forward, Benton visual retention test and pursuit aiming correct dot decreased with increasing of work duration. The correlation analysis was done in order to find out the relationship between subjective symptom and the scores of neurobehavioral core test battery. According to the results of analysis there were no items that had statistical significant relationship(p<0.05).

만성 정신분열병 환자들의 인지 기능과 정서 인식 능력의 관련성 (The Relationship between Neurocognitive Functioning and Emotional Recognition in Chronic Schizophrenic Patients)

  • Objective:The present study examined the association between basic neurocognitive functions and emotional recognition in chronic schizophrenia. Furthermore, to Investigate cognitive variable related to emotion recognition in Schizophrenia. Methods:Forty eight patients from the Yongin Psychiatric Rehabilitation Center were evaluated for neurocognitive function, and Emotional Recognition Test which has four subscales finding emotional clue, discriminating emotions, understanding emotional context and emotional capacity. Measures of neurocognitive functioning were selected based on hypothesized relationships to perception of emotion. These measures included:1) Letter Number Sequencing Test, a measure of working memory;2) Word Fluency and Block Design, a measure of executive function;3) Hopkins Verbal Learning Test-Korean version, a measure of verbal memory;4) Digit Span, a measure of immediate memory;5) Span of Apprehension Task, a measure of early visual processing, visual scanning;6) Continuous Performance Test, a measure of sustained attention functioning. Correlation analyses between specific neurocognitive measures and emotional recognition test were made. To examine the degree to which neurocognitive performance predicting emotional recognition, hierarchical regression analyses were also made. Results:Working memory, and verbal memory were closely related with emotional discrimination. Working memory, Span of Apprehension and Digit Span were closely related with contextual recognition. Among cognitive measures, Span of Apprehension, Working memory, Digit Span were most important variables in predicting emotional capacity. Conclusion:These results are relevant considering that emotional information processing depends, in part, on the abilities to scan the context and to use immediate working memory. These results indicated that mul- tifaceted cognitive training program added with Emotional Recognition Task(Cognitive Behavioral Rehabilitation Therapy added with Emotional Management Program) are promising.

  • Objectives : Though a proportion of Alzheimer's disease(AD) patients treated with donepezil have shown positive response on cognition, but the responders' characteristics are still uncertain. This study attempts to identify whether a single oral dose of donepezil(5mg) can change cognition and the relationship between single dose responder items and long-term responder are examined. Methods : Twenty-three AD patients for single donepezil challenge study group and eleven AD patients for controls were participated in the study. Seven days after baseline study for neuropsychological test and EEG, same studies were rechecked after donepezil medication in study group. In donepezil study groups, 12 weeks after donepezil medication, neuropsychological test and EEG were rechecked. Results : After single donepezil challenge, forward digit span, Rey-Osterrieth Complex Figure Test copy, SVLT delayed recall were significantly improved, and beta spectra power in anterior, theta spectra power in posterior field were significantly decreased. According to linear regression analysis, forward digit span after single donepezil challenge was significantly positive correlated with long-term responders. Conclusions : This study suggests that single donepezil medication can significantly change cognitive functions and EEG in AD patients. Among these responsive items, forward digit span was significantly correlated with long-term responder.

Effect of Bi-/Unilateral Masticatory Training on Memory and Concentration - Assessor-blind, Cross-over, Randomized Controlled Clinical Trial

  • Objectives: This study aimed to explore the short-term effects of bilateral masticatory training using an intraoral device on memory and concentration, which is an advanced form of Gochi, compared to the unilateral form with gum. Methods: Thirty young healthy participants (age, 16-30 years) were screened and randomly assigned to one of two sequences in a crossover design. The participants assigned to sequence A (n=15) performed bilateral mastication using an intraoral device with a total of 300 taps, followed by unilateral mastication using gum with the same number of repetitions and frequency, separated by a 7-day washout period. A reverse order was used for sequence B. The primary and secondary outcomes were the digit span test result and the symbol digit modality test and the word list recall results, respectively, which were conducted before and after each intervention. Results: Symbol digit modality test scores increased by 12.03±8.33 with bilateral mastication, which was significantly higher than that obtained with chewing gum (5.17 points;95% confidence interval: 0.99, 9.34; p<0.05). Changes in the digit span test and word list recall scores were not significantly different between the two groups. In the digit span test forward, symbol digit modality test, and word list recall test, bilateral mastication was not inferior to unilateral mastication in improving memory and concentration. Conclusions: Bilateral masticatory exercises using an intraoral device are not inferior to unilateral mastication with gum for improving memory in healthy young individuals. Further research is needed to determine the efficacy of bilateral masticatory training on cognitive function.

Relationships Between Cognitive Function and Gait-Related Dual-Task Interference After Stroke

  • Previous studies have reported that decreased cognitive ability has been consistently associated with significant declines in performance of one or both tasks under a dual-task walking condition. This study examined the relationship between specific cognitive abilities and the dual-task costs (DTCs) of spatio-temporal gait parameters in stroke patients. The spatio-temporal gait parameters were measured among 30 stroke patients while walking with and without a cognitive task (Stroop Word-Color Task) at the study participant's preferred walking speed. Cognitive abilities were measured using Computerized Neuropsychological Testing. Pearson's correlation coefficients (r) were calculated to quantify the associations between the neuropsychological measures and the DTCs in the spatio-temporal gait parameters. Moderate to strong correlations were found between the Auditory Continuous Performance test (ACPT) and the DTCs of the Single Support Time of Non-paretic (r=.37), the Trail Making A (TMA) test and the DTCs of Velocity (r=.71), TMA test and the DTCs of the Step Length of Paretic (r=.37), TMA test and the DTCs of the Step Length Non-paretic (r=.36), the Trail Making B (TMB) test and the DTCs of Velocity (r=.70), the Stroop Word-Color test and the DTCs of Velocity (r=-.40), Visual-span Backward (V-span B) test and the DTCs of Velocity (r=-.41), V-span B test and the DTCs of the Double Support Time of Non-paretic (r=.38), Digit-span Forward test and the DTCs of the Step Time of Non-paretic (r=-.39), and Digit-span Backward test and the DTCs of the Single Support Time of Paretic (r=.36). Especially TMA test and TMB test were found to be more strongly correlated to the DTCs of gait velocity than the other correlations. Understanding these cognitive features will provide guidance for identifying dual- task walking ability.

지역사회 노인에서의 수면의 질과 인지기능의 관련성에 대한 연구 (A Study on the Relationship between Sleep Quality and Cognitive Function in Community Elderly)

  • 목 적 : 나이가 들수록 수면의 질은 나빠진다고 알려져 있으며 수면의 질과 인지기능 사이의 관계를 조사한 전향적 연구는 현재까지 거의 없었다. 본 연구에서는 60세 이상의 지역사회 노인을 대상으로 6년 동안 추적 조사한 자료를 바탕으로 초기 수면의 질과 인지기능 사이의 관련성을 알아보고자 하였다. 방 법 : 본 연구에서는 60세 이상의 지역사회 노인 622명을 대상으로 하였고 최종 분석에는 322명의 노인이 포함되었다. 다음과 같은 임상척도가 사용되었다. 피츠버그 수면질 척도(Pittsburgh sleep quality index, PSQI)와 한국판 신경심리 평가집(Korean version of Consortium to Establisha Registry for Alzheimer's Disease, CERAD-K). 초기 수면의 질과 6년 이후의 인지기능의 관련성을 평가하기 위해 다중 선형 회귀분석을 시행하였다. 결 과 : 초기 수면의 질(PSQI)은 6년 이후의 숫자 외우기 검사(digit span test), 시계그리기 검사(CLOX 1), 단어 목록 회상 검사(word recall test)의 결과와 유의한 관련성이 있었다. 수면의 질이 좋을수록(β = -0.167, p = 0.026) 숫자외우기 검사 점수가 증가했으며 수면의 질이 좋을수록(β = -0.157, p = 0.031) 시계 그리기 점수(CLOX 1)가 증가했다. 또한 수면의 질이 좋을수록(β = -0.140, p = 0.039) 단어 목록 회상 검사 점수(word recall test)가 증가했다. 결 론 : 본 연구에서 수면의 질과 유의한 관련성이 있는 나온 숫자외우기 검사(digit span test), 단어목록 회상 검사(word list recall), 시계 그리기 검사 (CLOX 1)는 임상에서 비교적 단시간 내에 간편하게 수행 할 수 있는 검사이다. 수면의 질이 낮은 노인을 대상으로 이른 시기에 인지기능을 평가하는 것은 중요하며 추후 위의 3가지 검사가 수면의 질이 낮은 노인에게 조기 치매 진단의 유용한 선별검사가 될 수 있음을 밝히는 연구가 더 진행되어야 할 것이다.

Musical Aptitude as a Variable in the Assessment of Working Memory and Selective Attention Tasks

Evidence points to the low performance of nonmusicians in visual and auditory cognitive tasks (working memory and attention) compared with musicians. This cannot be generalized to all nonmusicians, as a sub-group in this population can have innate musical abilities even without any formal musical training. The present study aimed to study the effect of musical aptitude on the working memory and selective attention. Subjects and Methods: Three groups of 20 individuals each (a total of 60 participants), including trained-musicians, nonmusicians with good musical aptitude, and nonmusicians with low musical aptitude, participated in the present study. Cognitive-based visual (Flanker's selective attention test) and auditory (working memory tests: backward digit span and operation span) tests were administered. Results: MANOVA (followed by ANOVA) revealed a benefit of musicianship and musical aptitude on backward digit span and Flanker's reaction time (p<0.05). Discriminant function analyses showed that the groups could be effectively (accuracy, 80%) segregated based on the backward digit span and Flanker's selective attention test. Trained musicians and nonmusicians with good musical aptitude were distinguished as one cluster and nonmusicians with low musical aptitude formed another cluster, hinting the role of musical aptitude in working memory and selective attention. Conclusions: Nonmusicians with good musical aptitude can have enhanced working memory and selective attention skills like musicians. Hence, caution is required when these individuals are included as controls in cognitive-based visual and auditory experiments.

폐경기 여성에서 에스트로젠 치료가 우울증상과 주의집중력에 미치는 영향 (The Effects of Estrogen Replacement Therapy on Depressive Symptoms and Attention in Postmenopaual Women)

  • 본 연구는 폐경기 여성에서의 에스트로젠 치료에 따른 우울정도와 주의집중력의 차이를 평가하기 위해 폐경이 된 지 1년이 지난 폐경기 여성을 대상으로 사용군과 비 사용군으로 나누어 각각 30명을 대상으로 했다. 검사 도구는 한흥무 등(1986)이 표준화 한 Beck의 Beck depression inventory(BDI)를 사용하여 사용군과 비사용군으로 나누어 평가하였다. 그리고 주의 집중력의 평가는 K-WAIS(염태호 등 1992)의 소검사 항목인 숫자외우기와 바꿔쓰기을 사용하였다. 에스트로젠 사용군과 비사용군에서 각각의 검사 항목에 대해 t-test로서 유의한 차이가 있는지 비교하였다. 더불어 사용군과 비사용군에서 혈중 에스트라디올 농도를 측정하여 각 검사항목과 상관관계가 있는지 Pearson's correlation(r)를 통해 확인하였다. 결과는 BDI와 숫자외우기 중 바로 따라 외우기에서만 유의한 차이가 있었으며 다른 항목에서는 차이가 없었다. 그리고 혈중 에스트라디올 농도와 검사 항목간의 유의한 상관관계는 없었다. 본 실험을 통해 에스트로젠이 폐경기 여성에서 우울증상의 경감에 효과적이나 기억력 및 주의집중력에는 효과가 없음을 확인할 수 있었다.
