  • Objectives: The aim of this study was to identify the causal relationship between use of levocetrizine or cetrizine, and liver injury, by comparing frequency and pattern of hepatotoxicity in levocetrizine or cetrizine prescribed patients. Methods: This is a retrospective observational study, using data retrieved from electronic medical record system. Among 1164 patients prescribed levocetrizine or cetrizine during study period (Jul, 2009 - Jun, 2010) at Seoul National University Hospital, 543 patients with more than 4- time liver function test (LFT) results were included in final analysis. Liver injury was defined as greater than 3 times elevated level of alanine aminotransferase or 2 times elevated level of alkaline phosphatase or total bilirubin, compared to upper limit of normal, in patient with normal liver function at baseline. The frequency and pattern of liver injury were assessed. Results: Incidence of liver injury in patients prescribed with levotcetrizine or cetrizine were 1.48% and 2.94%, respectively. With few exceptions, most injuries were shown to be hepatocellular type. Rapid recovery was observed after drug cessation and long term use tends to be associated with incidence of liver injury. In patient with digestive system disorder, rate of liver injury was significantly higher (p=0.011). Conclusion: The result of this study implies potential need of liver toxicity monitoring, especially in patients taking long term levecetrizine or cetrizine or in patient with digestive system disorder. However, prospective large scale observational study is needed to confirm liver injury associated with the use of levocetirizine or cetirizine.

  • Diseases caused by indurate tissues of the internal organs are liver cirrhosis and abdominal sclerosis. The cause of chronic gastro-intestinal disease is a digestive system disorder and a defecation disorder. They impede peristaltic movement and digestive system with the symptom that indurate tissues. The purpose of the present study was to determine the disease grade quantitatively by measuring an indurated standard of tissues and organs. For the measurement of elasticity, we designed the system that measure the displacement of the substance and approved pressure using ultrasound transducer. For verification of developed system, we compared elasticity as results of experiment between the developed system and public elasticity measurement machine at individual plastic phantoms made by plastic hardener and softener. Elasticity of the plastic phantoms is averagely 0.007MPa lower measured by developed system than Micro-indenter, and less than 10% errors. Comparing with economical value and accuracy between developed system and Micro-indenter, the system is significant of measurement for tissue elasticity. Thus, it is possible to measure a elasticity at human tissue and organ. A chronic gastro-disease as well as grade could be decided objective validity using this system.

  • In an attempt to evaluate working conditions and hazardous environmental factors, a survey on health problems was conducted for 721 employees from 18 industrial plants in Young Dung Po and Sung Dong districts of Seoul in the period from May 1975 to Feb, 1976. The questionnaire and survey was inclusive of all illnesses known as most common to industrial workers (according to the report by Korea Industrial Health Association) of this country and envi ronmental factors pointed out by the workers. The results were as following. 1. The over whelming majority of complaints were psychiatric, ear, eye, and digestive disorders. 2. Those whose work time is the longest have a propotionately higer incidence of complaints especially in eye, ear, neuromusculoskeletal, and neurosensory systems. 3. In females whose occupational history is relatively short, a higher incidence of digestive and skin section disorders was noted and this is especially true among those working in night shift. 4. Among machinery industry workers, most complaints were of eye, ear, and neuro musculoskeletal disturbances, and this fact seems to be closely related to physical over works. 5. Dust and noise were pointed out by the largest number of workers as undisirable conditions in work environment. 6. Distribution of complaints are closely related to the type of industry, and in comparison with the result reported by Korea Industrial Health Association in 1972, the prevalence rates of occupational disorder were similar in our study,

  • Objectives Migratory pathogenic factor(六淫) occupies an important position in the etiology of Korean Medicine. This paper shows how Dampness, one of Migratory pathogenic factor(六淫), is explained in Donguibogam(東醫寶鑑). And, based on this, we will figure out how to make a judgement of Dampness through diagnosis. Method 1. Collect parts of Dampness mentioned in Donguibogam(東醫寶鑑). 2. From the collection, extract contents about mechanism and symptom of Dampness, which is considered necessary for diagnosis. 3. Put all the extraction together, suggest the diagnosis element which can be criteria of judgement of Dampness through diagnosis. Result & Conclusions The occurrence of Dampness come from a wet climate and environment externally, and overeating of greasy food and digestive disorder internally. There are many different kinds of symptoms throughout the body cause of poor circulation. Dampness is classed as Cold-dampness, Damp-heat, and Dampness-phlegm depending on characteristic symptoms, and mainly shows musculoskeletal disease and digestive troubles. Typical symptoms are pitting edema, distention and fullness, moderate and thready pulse, volume of perspiration increase, loose feces, urine volume decrease, pain of joint and muscle, restriction of movement, etc.

  • Progressive familial intrahepatic cholestasis type 3 (PFIC3) is an autosomal recessive disorder of cholestasis of hepatocellular origin, typically seen in infancy or childhood caused by a defect in the ABCB4 located on chromosome 7. Here we report on an older patient, aged 15, who presented with biochemical testing that led to an initial consideration of a diagnosis of Wilson disease (WD) resulting in a delayed diagnosis of PFIC3. Diagnosis of PFIC3 was later confirmed by molecular studies that identified novel mutations in the ABCB4 gene. Cholestasis due to PFIC3 can cause elevated hepatic copper and increased urine copper excretion that overlap with current diagnostic criteria for WD. Molecular diagnostics are very useful for establishing the diagnosis of PFIC3. Ursodeoxycholic acid ameliorates cholestasis in PFIC3, and may help mediate a reduction in hepatic copper content in response to treatment.
