종합병원 소아청소년과 내원 초등학생의 비만도와 관련 요인 분석 (Obesity Index and Related Factors among Elementary School Students Visiting Pediatric Department of General Hospital)

  • The present study was conducted to investigate dietary habits and biochemical indices according to the obesity index of elementary school students visiting a hospital for diagnosis. The subjects of this study were 150 elementary school students and their mothers. The overweight rates of child subjects were 37.5% for males and 40.9% for females, whereas the rates of underweight children were 35.0% for males and 14.5% for females. Children's obesity index significantly decreased with an increase in the average monthly income of children's families. Children's obesity index, mothers' weights, BMI, and percent ideal body weight were all positively correlated. Blood biochemical values in children (hemoglobin, hematocrit, albumin, globulin, glucose, total cholesterol, and triglyceride levels) and their obesity index also showed positive correlation. For the underweight group, the frequency of iron-deficiency anemia was significantly high, whereas for the overweight group, the frequency of dyslipidemia was high. There was a significant increase in children's obesity index and a decrease in frequency of regular exercise with time spent on the TV/computer. The normal weight group recorded the highest total scores for mothers' nutritional knowledge, whereas the overweight group recorded the lowest total scores. Therefore, to maintain healthy weight in children, nutritional education programs for mothers and children should be developed.

사례연구 : 보건소 당뇨병교육 참가자의 식사교육 사례연구 (A Study on Diet Education of Participants in Diabetic Education at public Health Center)

  • This study was carried out to see the effect of individualized diet education on the improvement of blood glucose level according to the changes of nutrient intake with the subjects of 25 people. Among 25 people who participated in diabetic education program held in Chilgok-gun public health center, Gyeongbuk, there were 15 diabetic patients(3 males and 12 females) and 10 healthy people(2 males and 8 females) and after the diet education, the results were as follows. As for clinical characteristics, the diabetic group tended to have slightly higher blood pressure than the control group, especially diastolic blood pressure of diabetic group was higher than the control group with a significant level(p<0.05). Blood glucose level measured 2 hours after the meal was higher in the diabetic group than the control group with significant level(p<0.0001). For the clinical characteristic changes before and after the diet education, the blood glucose level in the diabetic group reduced significantly(p<0.01). For the daily diet intake, intake of fat in the diabetic group increased significantly compared to the one before the education(p<0.05). When comparing the ratio of carbohydrate: protein: fat, it was changed from 72.9 : 16.2 : 12.9 before the education to 63.8 : 15.1 : 21.1 after the education. From the above results, it is necessary to perform the diet education which carries out in the health center for diabetic patients with individualized goal by stepwise way rather than as one time try, and by developing the diabetic education media which is appropriate for the patients.

소도시지역 중학생의 식습관 , 식생활 행동 및 영양지식 (The Study on Dietary Habits , Dietary Behaviors and Nutrition Knowledge of Middle School Students in the Small City)

  • This study was to obtain information about the dietary habits, the dietary attitudes, nutrition knowledge, and the food consumption of middle school students and to assess relationships between the dietary habits, the dietary attitudes, and nutrition knowledge of middle school students. Data were collected from 390 middle school students in Kimcheon. In the dietary habits, the regularity of breakfast showed the highest point and the snack consumption showed the lowest point. Female students were lower in the regularity of breakfast, and male students were higher in overeating of dinner and night. In the dietary attitudes and the perceived importance of nutrition, male students showed more favorable attitudes and perceived more importance of nutrition. In the nutrition knowledge, the responses to items 'Cola or cider doesn't have calories' and 'An egg contains lots of cholesterol' were the lowest point. Female students had higher knowledge than male students. In the food intake, grains, milk.yogurt.cheese, and fruits were always eaten or frequently eaten. While fast food, spicy and salty food, seaweeds, carbonated drinks, and yellow or green vegetables were eaten in lower frequency than other foods. Male students consumed more meat.ham.egg.fish, fried.stir-fried food, fast food, carbonated drinks, and spicy and salty food than female students. The correlation between dietary attitudes and dietary habits was statistically significant. The correlation between dietary attitudes and the perceived importance of nutrition was statistically significant. But the correlation between dietary attitudes and nutrition knowledge was statistically significant for female students, not significant for male students. Nutrition knowledge was related to the perceived importance of nutrition. It might be suggested that effective nutrition education program might be developed and implemented to ensure the good eating behaviors of middle school students.

가공식품의 영양강화 현황과 영양표시 (Current Status of Nutrient Fortification in Processed Foods and Nutrition Labeling)

  • Current status of nutrient fortification in processed food in Korea were presented by analyzing the information shown on food labels. The obtained information was assessed by the regulations on food fortification in both Korea and other countries including Codex. The most current regulations were gathered from internet. The results are summarized as follows. 1. Major nutrients fortified were calcium, Vit C, Vit B complex, iron and fiber. The forfified foods were not limitted to certain food group with more frequent fortification in snackfoods, cereal, ramyun, retort pouch foods, milk, and youguart. The descriptive terms of nutrition label for the fortification were various including high, supplemented, added, source, fortified, and abundance though the difference among these terms were not distinct. 2. Current regulation on nutrition label requires to give the content of the fortified nutrient and % RDA. However not all of food items carry above information. Also some ingredients such as chitosan, DHA, taurine, $\omega$-3 fatty acid, chondrichin, bifidus were supplemented mainly to the snack foods which FDA(USA) does not allow to be fortified. 3. The nutrient most frequently fortified was calcium and general practice of fortification appears to follow the regulation in Korea. Presently the regulation itself is not well described, this nutient fortification can cause toxic effect. Since calcium was supplemented to wide range of food group consumers who are not conscious of the safe upper limit may intake the fortified food up to the level of 2g/day. 4. For the effective fortification in Korea, the regulation on fortification should be reformed in accordance with the international guideline Codex and the regulations in other countries especially in America and Japan.

기본생계비를 위한 식품비 산정에 관한 연구 (Estimated Food Cost to Maintain Basic Living Expenditure)

  • In this study, the food cost for the urban worker's living expenses was estimated for the Korea Labor Union. The urban workers living expenses are minimum of healthy and decency level expenses with allowances for minimum quality of life. Thus, the food cost should be enough for purchasing proper kinds and amount of foods which can supply sufficient nutrient to maintain health and which should reflect current food consumption patterns. To estimate the food cost, the Korean Recommended Dietary Allowances was used to calculate the amount of nutrients which should be supplied. The National nutrition survey and the Food balance sheet, were used to estimate the current consumption patterns for the kind and amount of food. To estimate price for each food item, the market survey was executed in six large cities. Also, to verify the estimated food cost, actual food costs were surveyed. For 5 kinds of model household, dietary allowances were calculated for the each nutrient. Using the Korean food guides, the number of serving for each food group were decided for the model households. In each food group, the amount and kind of foods were decided by the current food consumption pattern. The kind and amount of food were adjusted by the amount of calculated nutrient. When the amount of nutrient was between 90% and 110% of the recommended dietary intake, it was accepted. With these amount and kinds of foods, the food cost were calculated using the market survey. Considering extra expenses for the eating-out and processed foods, extra expenses are added. As a results, for single person family, the estimated food cost was 149,210won per month. For two, three, four and five person family, the estimated food costs were 245,179won, 381,182won, 501,669won and 687,980won per month, respectively. The estimated food cost were lower in the single and two person family than the actual food cost by the survey. The cost for eating-out gave major differences. In the future, to estimate food cost, the food consumption patterns for the different kind of household, sex and age should be studied carefully. Also, the pattern of eating-out should by analyzed.

가공식품의 영양표시에 대한 중학생과 학부모의 이용 및 인지도 실태 (Use and recognition of nutrition labelings in processed foods among middle school students and their parents)

  • In order to investigate the use and recognition of nutrition labelings of processed foods which adolescents frequently purchase, 277(boys 125, girls 152) middle school students and their parents were conveniently selected from the west Chonan City, and were surveyed by self-recording questionnaire on June, 2000. Nutrition labels were indicated only in 14.9% of 222 processed snack foods. At purchasing foods the students considered taste most importantly and in turn price, quantity, and nutrition. Of the subjects 79.4% of students and 89.2% of parents had experiences of reading nutrition labels, and the reasons were mainly to just know contained nutrients(76.8%, 72.1%, respectively) and for their health care(35.9%, 47.8%, respectively). The main reason not to confirm nutrition labels was that they were not interested in nutrition labels. But too small size and rudeness or hardness of understanding was also pointed out as a reason of not reading nutrition labels. The students ranked mineral, protein, and carbohydrate as the most important three nutrients at labelings, while the parents ranked protein, mineral, and calorie. Cholesterol, dietary fibers, and sodium were shown the lowest rank in both students and parents. Significant nutrients for specific processed foods did not well recognised in the students. Conclusively, nutrition labelings should be applied to all processed foods which students frequently buy, particularly in clear and large size. Also it is necessary to prepare education programs for both students and parents about accurate recognition of nutrition knowledge about processed foods and how to use nutrition labels.

거창지역 중.고등학생의 우유 섭취실태, 우유에 대한 인식 및 지식조사 (The Consumption Pattern, Perception and Knowledge of Middle and High School Students on Milk in Geochang Area)

  • This study was performed to investigate the consumption pattern, perception and knowledge on milk of middle and high school students. The subjects were 1,195 students(590 middle school and 605 high school students) living in Geochang area. The survey was conducted by using a self-administered questionnaire in November, 2004. The results were as follows. Fifty-one point nine percent of the subjects drank milk 1-3 cups per day, and 51.7% of the subjects drank milk more than 5-6 times in a week. Amount and frequency of milk intake were higher in middle school students and male students than those in high school students and female students(p<0.001), and the consumption of milk was significantly higher in students with highly educated mother and nuclear family(p<0.01). Eighty-two point one percent of subjects perceived that they should drink milk or as much as possible, and 89.5% of them milk intake required per day is 1-3 cups, and male students perceived more than female students that milk should be taken a lot(p<0.001). In addition, half of the subjects slightly knew that milk contains various kinds of nutrients and a third knew it well, and the perception rate was higher in male students than those in female students(p<0.001). The average score of knowledge about milk was 5.71 out of 10 points and 5.94 points in female students, which was significantly higher than 5.48 points in male ones (p<0.001).

교정시설 급식에서의 위생관리 실태조사 (Food Sanitation Management at Foodservice of Correctional Institutions in Korea)

  • The purpose of this study was to investigate the equipment ratio of sanitary facility/equipment as well as the sanitary management performance level at foodservice of correctional institutions in Korea. For this purpose, a total of 47 questionnaires were distributed to dietitians working at correctional institutions during the period from March 20th to May 18th of 2008. A total of 38 questionnaires (response rate 81%) was analyzed using SPSS (windows ver. 14.0). The majority of the respondents were females (65.8%), 35 years or older (55.3%), with 7 years or longer experiences (65.8%), and with education level of university or higher (60.5%). Among the institutions, 39.5% had less than 500, 28.9% had 501 or more but less than 1,200, and 31.6% had 1,201 or more inmates. The equipment ratio of the sanitary facilities/equipment was 49.7%, which was relatively low. Most dietitians perceived 'limited availability of facilities and equipment' and 'the lack of support from financing department' as the major barriers in implementing a desirable sanitary system. On the other hand, perceived sanitary management performance was rated by the respondents as being between 2.55 to 4.50 (5-point Likert scale)-'Cleaning hands properly as specified' showed the lowest performance, whereas 'Sampling preserved meals by standard methodology' showed the highest. The results of this study suggest that a sanitary education program designed for inmate food handlers is needed for successful sanitary management.

단체급식 안전관리에 대한 영양사 인식 조사(II) - 안전시스템 및 안전교육 현황 - (Dieticians' Perception of Safety Supervision in Institutional Foodservices (II) - Status of Safety System and Safety Education -)

  • The purpose of this study was to investigate the status of the safety system and safety education in institutional foodservices in the Changwon and Masan areas. The survey was conducted from February 1 to March 31, 2009 via questionnaires that were sent to 300 dietitians, and 142 dietitians responded. It was determined that most of the safety managers were 'dietitians (87.1%)', whereas facilities/equipment managers consisted of 'dietitians (45.7%)', 'department of facilities management in the organization (36.4%)' and 'outsourced company of facilities management (17.9%)'. Out of the 11 safety practices, seven safety practices showed less than 50% of total implementation ratio, which meant that the safety systems were not functioning properly. Except for 'non-skid shoes (85.9%)', other safety equipment was seldom used. The survey respondents recognized that safety education was very necessary (4.47 points); however, they responded so-so (3.46 points) to the question of whether or not the actual frequency and time spent on safety education were enough. The average time spent on safety education was 28 minutes 11 seconds. Regarding the difficulties in performing safety education, 'not many safety education materials and media (3.44 points)', 'not many varieties in the subjects and contents for safety education (3.40 points)', and 'not much organizational support on safety education (3.33 points)' showed higher scores than 'lack of education time due to workload (3.20 points)'. The following were cited as education materials that should be developed as it is currently difficult to obtain relevant information and data: 'root causes of musculoskeletal injury and preventive measures (15.8%)', 'healthcare and disease preventive exercises for employees who do simple and repetitive works (14.9%)', and 'instruction on safe handling of chemicals (12.7%)'

의료기관 평가제도 영양부문 기준 및 결과의 시계열 변화 분석 (The Time Series Analysis of Standards and Results of Nutritional Domain in Hospital Evaluation Program)

  • The purpose of this study was to evaluate the current state of foodservice and clinical nutrition management in a hospital-based nutrition department. Nutritional guidelines and survey reports of hospital evaluation programs from 2004 to 2009 were analyzed. In total, 275 hospitals in the first period and 288 hospitals in the second period were evaluated. The division of knife and chopping board use decreased from 97.2% in 2005 to 89.7% in 2008, the maintenance of a proper freezer temperature (below $-18^{\circ}C$) increased from 82.1% in 2004 to 97.7% in 2007 (88.9% to 97.4% from large hospitals and 69.8% to 86.5% from small/medium hospitals in 2005 and 2008, respectively). In tube-feeding management, the performance rate of material cold storage and the offer rate of tube-feeding were 65.9% and 94.2% in 2007, respectively. The cold storage of material, proper use within 24 hours after opening or production, and the use of an appropriate label were 47.3%, 71.2% and 67.2% in 2009, respectively. The rate of a management system for undernourished patients was 86.0% in 2007 (56.4% for large hospitals, 18.9% for small/medium hospitals) and 14.3% in 2009. In standards of nutrition support management, the performance rates of constructing a nutrition support team, the nutrition support team activity, and organizing multidisciplinary team were 66.7%, 43.6%, and 64.1% respectively, in 2004. For large hospitals, those rates were 61.1%, 36.1%, and 58.3%, in 2005, 93.0%, 62.8%, and 91.9% in 2007, and 69.2%, 43.6%, and 69.2% in 2008, respectively. The results of this study suggest standards on sanitary foodservice preparation, production, and tube-feeding production need to correspond with HACCP regulations for small/meidium hospitals in standards of a healthcare accreditation system. It will be necessary to understand the operating conditions of nutrition departments in convalescent hospitals, psychiatric hospitals, and geriatrics hospitals. As the application of accreditation is required from 2013, standards will need to be improved and continuously updated for healthcare accreditation.
