Students' dietary habits, food service satisfaction, and attitude toward school meals enhance meal consumption in school food service

  • BACKGROUND/OBJECTIVES: This study aimed to compare student consumption of school meals by school level, to identify the influencing factors of school meal consumption, and to assess improvement needs of school food service among students. SUBJECTS/METHODS: A total of 1,441 elementary, middle, and high school students attending 58 schools in Gyeonggi-do, South Korea participated in the survey in 2015. A questionnaire and informed consent forms for students and legal guardians were sent home and completed responses were returned to the researcher. RESULTS: Approximately 58% of the students perceived the portion sizes of school meals as appropriate and 76.1% consumed almost all or all of the meals served. More elementary and middle school students than high school students consumed almost all or all of the meals (P < 0.001). A regression analysis revealed that the students with a higher dietary behavior score (P < 0.001), higher satisfaction with food service (P < 0.001), a higher environmental protection practice score (P < 0.05), and more positive attitudes toward school meals (P < 0.01) consumed significantly more meals. The provision of foods that taste good and reflecting student opinions on menus were the most important factors for increasing school meal consumption. CONCLUSIONS: To increase consumption of school meals, food service staff should provide students with quality meals and engage students in school food service. Nutrition education that emphasizes healthy eating behaviors and cafeteria environment modification that applies strategies based on behavioral economics can encourage students to consume more school meals.

저열량균형식, 운동 및 행동수정에 의한 비만여성의 치료에 관한 연구 (Treatment of Obese Women with Low Calorie Diet, Aerobic Exercise and Behavior Modificaiton)

  • 체지방이 30% 이상으로 비만이라 판정되는 20~62세의 지원자들에게 저열량 균형식, 무리하지 않은 유산소 운동 및 개인별 비만의 원인을 교정하고 평생체중을 조절하기 위한 영양상담과 교육을 포함한 행동수정을 통해 5주 동안 비만을 치료하고 성인병과 관련된 위험 요인들의 변화를 살펴보았다. 활동량, 식습관, 식이섭취조사를 실시하여 개인별 비만의 원인을 진단한 후 잘못된 식생활을 교정할 수 있도록 주 3회의 영양상담과 매주 1회의 영양교육을 실시하였다. 식이요법은 1일 1끼의식사와 3회의 조제식이를 통해 열량이 1200kcal 정도가 되고, 양질의 단백질, 비타민, 무기질은 성인 권장량의 100~150% 이상이 되도록 하였다. 식전에 식이섬유와 물을 복용하고 식사 기간은 20분 이상으로 천천히 섭취하도록 하였으며, 투긴음식, 가공식품, 패스트 푸두, 술, 청량음료, 달고 기름진 음식, 짜고 자극적인 음식 등은 제한하였다. 주당 3~5일간 1시간 정도의 가벼운 유산소 체조를 실험대상자 저원이 모여서 실시하였다. 먹은 식품, 활동, 운동, 감정상태, 피로여부 등을 일지로서 기록하게 하여 삼담을 통해 교정하고 관찰하여 긍정적인 내적동기를 갖도록 교육하였다. 비만치료 기간 중 탈락한 수는 총 16명 중 5명 (31.3%)이었으며, 비만도는 대상자의 대부분이 경증이고, 불규칙적인 식사, 빨리 먹는 습관, 과식, 짜게 먹는 습관, 야식, 음주 등의 비만의 원인이 되는 습관들을 가지고 있어 식습관 점수는 개선이 요구된다고 진단되었다. 5주동안의 비만치료로 체중은 평균 4.8kg(주당 0.96kg)이 감량 되었으며, 체지방, body mass index, 허리, 팔, 둔부의 둘레가 유의적으로 감소되었다. 혈압, 공복시의 혈당, 혈액증의 중성지방, 총콜레스테롤, HDL-콜레스테롤은 변화가 없었으나, LDL-콜레스테롤은 유의적으로 감소하였다.

전문가 시스템을 이용한 인터넷 체중 감량 상담 프로그램 개발 (Development of Nutritional Counseling Internet Program for Weight Reduction Using Expert System)

  • 본 연구에서는 인터넷상에서 일반 대중들이 행동 수정 방법을 통해 올바른 식습관을 비롯한 생활 습관을 가짐으로 체중 감량을 통해 정상적인 체중을 장기간 유지할 수 있도록 하기 위하여 전문가 시스템 툴인 KEA를 사용하여 실시간 상담을 해줄 수 있는 프로그램을 개발하였다. 전문가 시스템인 KEA는 MCRDR 이론을 사용하여 특정 분야의 전문가가 전문가 시스템에 관한 전문지식 없이도 시스템을 직접 구축 운용할 수 있는 환경을 구축하여 지식의 전문가가 직접 지식데이터 베이스를 구축하고 유지.보수하여 저렴한 비용으로 장기간 운영이 가능하다는 장점이 있다. 처음에 KEA로 상담할 수 있도록 지식을 학습시키기 위하여 체중 감량에 관련된 설문지를 작성하여 과체중 또는 비만한 사람 150명을 대상으로 설문을 받아 전문 영양사가 이에 대한 상담을 하고 이것을 KEA에 학습시켰다. 설문지는 인터넷 상담 프로그램과 같은 설문지를 사용하여 각 대상으 비만의 원인, 식습관, 생활습관, 운동 습관을 조사하였다. 체중 감량 상담은 행동 수정 이론을 적용하여 내담자가 스스로 잘못된 식습관 및 생활습관을 수정함으로써 점차 체중을 감량하여 평생 적절한 체중을 유지할 수 있는 방법을 터득할 수 있도록 상담하는 것을 원칙으로 하였다. 지식을 KEA에 학습시키는 방법은 조사한 설문을 바탕으로 case에서 적절한 항목값으로 추출하고 KEA에 지식을 만들기 위한 조건으로 추가한 후 이러한 조건에 맞는 상담 결론을 선택하고 추가하는 방식으로 KEA에 학습시켰다. 이 프로그램은 인터넷을 통해 일반인이 사용할 수 있도록 인터페이스를 만들었고 사용자가 프로그램에 들어가서 실시간으로 전문가의 상담을 받을 수 있다. 사용자가 인터넷 체중감량 상담 프로그램을 사용하기 위하여 화면에 있는 설문에 답을 기입하면 이미 축적된 KEA의 지식으로 실시간으로 상담의 결론을 내려주었다. 만약 사용자의 상황이 KEA에 축적되어 있는 지식으로 결론을 내릴 수 없으면 '결론없음'이라는 말이 뜨게 되고 이때는 전문가각 직접 이 상담 case를 분석하여 KEA에 지식을 축적시켜 점차 KEA의 지식을 확충시켰다. 결론적으로 전문가 시스템인 KEA를 이용하여 체중 감량의 원칙을 행동 수정 방법에 따라서 사용자의 개인에 맞는 방법을 체중 감량을 할 수 있도록 전문가가 일대일로 실시간으로 상담할 수 있도록 하여 건강하게 체중 감량을 할 수 있도록 하였다.

  • 비만의 치료와 관리에 있어서 식이요법과 운동요법은 비교적 단기간의 효과는 입증 되었지만, 장기간 체중유지가 되지 못한다는 문제가 있다. 그래서 비만연구가들은 치료의 현장에서 행동수정요법을 활용한다. 그러나 행동수정요법의 식습관 및 생활습관 개선만으론 체중관리에 있어서 지속적인 결과유지는 여전히 큰 문제로 남아 있다. 그래서 본 연구에서는 기존 비만치료의 한계를 조금이나마 해결하고자, 행동수정요법의 장점에 인공지능을 활용한 비만관리를 제안한다. 인공지능 비만관리는 대상자들에게 인공지능으로 테이터화 된 문자메시지를 각자의 상황에 맞게 보내는 프로그램이다. 무엇보다도 인공지능 일대일 맞춤형 비만관리 메시지는 유연한 설득, 격려, 동기부여를 통한 운동지속의 효과를 강화시키는 연구 제안이다.

문제 행동을 가진 초등학생의 영양 상태와 모발 미네랄 함유량 (Nutritional Status and Hair Mineral Content of Elementary School Children with Behavioral Problems)

  • This study was performed to assess the nutritional status and hair mineral content of children with behavioral problems and compare the values with a gender, age-matched control group. The subjects were recruited from S elementary school children in Seoul, Korea. Students scored higher than 60 points were diagnosed with behavioral problems according to the Korean-Child Behavior Checklist. Nutritional assessment of the two groups (behavior problem group: male n=15, female n=24; $9.6{\pm}1.6$ years, control group: male n=16, female n=18; $9.5{\pm}1.9$ years) was performed using a nutritional survey and by measuring hair mineral contents. In the results, food frequency questionnaire analysis showed that the intakes of anchovy (P<0.05), soybean curb (P<0.01), radish (P<0.05), bean sprouts (P<0.05), spinach (P<0.05), carrot (P<0.05), pumpkin (P<0.05), lettuce (P<0.05), cabbage (P<0.01), apple (P<0.05), and milk (P<0.01) were higher in the control group than the behavior problem group, whereas intakes of ramyeon (P<0.05), cookies (P<0.05), and coke (P<0.01) were higher in the behavior problem group than the control group. Intakes of most nutrients such as plant-derived protein (P<0.05), fiber (P<0.05), plant-derived calcium (P<0.05), phosphorus (P<0.05), plant-derived iron (P<0.05), vitamin B2 (P<0.05), vitamin B6 (P<0.05), vitamin C (P<0.01), vitamin E (P<0.05), and folate (P<0.05) were significantly higher in the control group than the behavior problem group. Hair analysis showed that the levels of arsenic (P<0.05), mercury (P<0.001), uranium (P<0.05), iron (P<0.001), boron (P<0.01), and germanium (P<0.001) were lower, but the levels of phosphate (P<0.05), chromium (P<0.001), sodium (P<0.05), and sulfur (P<0.001) were higher in the behavior problem group than the control group. Conclusively, behavioral problems constitute a complicated condition in which nutritional factors may play major roles. However, it is still under investigation as to whether or not modification of dietary habits or nutritional supplementation can improve children's behavior, since symptoms require a broad understanding of the environmental and genetic interactions.

Evidence-based customized nutritional intervention improves body composition and nutritional factors for highly-adherent children and adolescents with moderate to severe obesity

  • BACKGROUND/OBJECTIVES: Evidence-based customized nutritional interventions are required for effective treatment of moderate to severe obese children and adolescents. SUBJECTS/METHODS: Sixty six (64.1% of 103) of the eligible participants who joined the usual care or physical activity group in the clinic were involved in 16-week intervention. Customized nutritional intervention was implemented for each participant based on a nutrition care process (NCP) model. Sociodemographic assessment, anthropometrics data, health- and dietary-related behaviors, and dietary intake of the study subjects were assessed at baseline and follow-up. All participants engaged in 30-minute nutritional sessions on a monthly basis. RESULTS: After 16 weeks, there were significant improvements in body composition [BMI (-0.8 ± 0.9, P < 0.05), BMI z-score (-0.3 ± 0.2, P < 0.001), body fat (kg) (-1.3 ± 2.1, P < 0.05), and body fat (%)(-1.5 ± 1.9, P < 0.05)] as well as macronutrient intake [total energy intake (kcal) (-563.7 ± 656.8, P < 0.05), energy (%) (-26.5 ± 30.0, P < 0.05) and fat (g) (-28.3 ± 40.6, P < 0.05)] in the adherent group than the non-adherent group. The SOC was higher in both groups after the intervention (P < 0.001). CONCLUSIONS: Our results highlight the positive effects of an evidence-based approach as a multidisciplinary intervention for people-centered nutritional care and weight management.

과체중 대학생에서 저열량식이 또는 저열량식이와 운동병합에 의한 체중 감량 효과 (The effects of weight loss by a low-calorie diet and a low-calorie plus exercise in overweight undergraduate students)

  • Korean undergraduate students attempt to lose weight but often fail due to utilizing ineffective weight loss strategies. Some diet programs have succeeded, yet, they have not provided adequate skills for long-term weight maintenance. The aim of the study was to determine the effect of a low calorie diet and exercise with nutritional education on weight loss, serum lipid profiles, nutrient intakes, and dietary behavior modification in overweight and obese undergraduate students. The subjects in the low calorie diet group (LCD; n = 12) and the low calorie diet plus exercise group (LCDE; n = 13) had the same goal of losing 4 kg during a 12 week program. Nutrient intakes were assessed by the 24 hour recall method. Also, food habits and dietary behaviors were investigated by self-administered questionnaires before and after the weight control program and one month after completing the program. LCD and LCDE groups lowered body weight by 2 kg and 1 kg, respectively, although they decreased calorie intake by 355 and 287 kcal per day compared to intakes prior to the study. Body fat mass decreased in both the LCD and LCDE groups; however, the decrease was greater in the LCDE group. In addition, only the LCDE group increased muscle mass. The LCD group had a slightly better effect in reducing body weight, body fat, and waist circumference than the LCDE group. However, their decrease was reversed after the mid-study check in the LCD group; the reduction was better maintained and decreased more in the LCDE group. However, serum lipid profiles were already in borderline prior to the study; moreover, they were not modified after losing weight. The dietary behavior program helped students to develop better dietary habits. In conclusion, the combination of a low calorie diet and exercise is necessary in order to maintain longer weight loss by increasing muscle mass and decreasing body fat.

A Study on Weight Control Program for 4th and 5th Grade Obese Children in Elementary School

  • This study examined the proper roles of dietitian and nurse-teacher in the weight control program (WCP) in schools and the effect of the WCP on subjects with respect to anthropometric measurements, nutrition knowledge, dietary attiude, and behavior changes. The program consisted of six sessions of nutrition education and frequent face-to-face nutrition counseling. Subjects were 22 obese children in the 4th and 5th grade who underwent counseling and 18 obese children in another school who served as a control group. After two months of WCP, obesity index such as .elative body weight (RBW, from 135.7 to 132.5), tricep skinfold thickness (TSFT, from 34.9 to 32.8 mm), and body fat content (from 32.0 to 29.8%) had decreased significantly in the experimental group, while the control group showed no significant differences in these indices. The reductions in obesity indices were maintained in the experimental group except for fat content (32.6%), which returned to its original value within six months. The control group significantly increased fat content in the same period (from 31.2% to 36.2%). Both groups decreased RBW, TSFT, and fat content while midarm circumference and waist/hip ratio remained the same after one year. Subjects' nutrition knowledge was improved with average test scores from 15.1 to 16.7 while nutrition attitude and behavior test scores remained unchanged suggesting that behavior modification may require more time than knowledge acquirement. These results suggest that proper nutrition counseling can initiate weight reduction. However, the maintenance of controlled weight requires changes in attitude and behavior which have not been achieved by the present WCP. The role of school dietitian for WCP in this study was limited to assisting the nurse-teacher in nutrition education. Expansion of dietitian's role in nutrition education and counseling is needed.

대구시 일부지역 중학생들의 건강관련 위험행동과 관련요인조사 (Youth Risk Behavior Survey of Middle School Students in Daegu Metropolitan City)

  • This research was conducted to understand and analyze the health-related behaviors of middle school students and get fundamental research data essential to provide efficient student guidance and public health service at school. The interview using Youth Risk Behavior Surveillance System(YRBSS). Translation and modification for Korean students of the YRBS. The Korean version of YRBS(Youth Risk Behavior Surveillance System) that translation and modification for Korean students of the YRBS developed by the Centers for Disease Control and Prevention(CDC)was used to assessment to health-related behaviors of youth. The interviewees were 1040 enrolled students at middle school in Daegu metropolitan city. YRBSS monitors six categories of priority health behaviors among youth and young adults behaviors that contribute to unintentional and intentional injuries; tobacco use; alcohol and other drug use, sexual behaviors that contribute to unintended pregnancy and sexually transmitted diseases(STDs)(including human immunodeficiency virus infection); unhealthy dietary behaviors; and physical activity. The result shows that over 30% of students rarely or never used safety belt and almost students were rarely or never wore a bicycle helmet. During the 12 months preceding the survey, 21.9% female students had felt so sad or hopeless almost every day for $\geq$ 2weeks in a row that they stopped doing some usually activities 20.5% of male middle school students have ever tried cigarette smoking. 26.2% of male students and 27.2% do female students had had over one drinks of alcohol during their lifetime(lifetime alcohol use). 47% of male students had had over one drinks of alcohol on $\geq$ 1 of the 30 days preceding the survey(current alcohol use). Over one half of female student were thought they were overweight. These results suggest that some risk behaviors be very prevalent in a korean middle school students and priority health-risk behaviors, which contribute to the leading cause of mortality and morbidity among youth and adult, often are establish during middle school age, extend into adulthood, are interrelated. Among both children and adults, the leading causes of death are closely linked to these behaviors. Among adults, chronic diseases such as cardiovascular disease, cancer, and diabetes are the national leading killers. Practicing healthy behaviors, such as eating low-fat, high-fruit-and-vegetable diets, getting regular physical activity, and refraining from tobacco use, would prevent many premature deaths. Because health-related behaviors are usually established in childhood, positive choices need to be promoted before damaging behaviors are initiated or become ingrained.

소아 지방간 환자에서 체중조절에 의한 간기능 개선 효과 (Improvement of Liver Function by Weight Control in Children with Fatty Liver)

  • Obese children have increaed in Korean in recent years as socieconomic status have got more higher than past years. Obese children have many health problems and some kinds of complications. Obese patients who have fatty liver with abnormal liver function should be managed with weight control. This study was intended to investigate the effect of weight control on hepatic abnormalities of obese children with fatty liver. We managed 6 obese children who had fatty liver with abnormal liver finction for weight control containing behavior modification, diet and exercise during 3 months. After 3 months, we compared body weight, liver function test, dietary intake and exercise. Body mass index(BMI) showed significant decrease from 28.1$\pm$2.3kg/$m^2$ to 26.2$\pm$1.7kg/$m^2$ (p<0.01). Aspartate aminotransferase(AST) showed significant improvement from 56$\pm$15IU/l(p<0.01). Alanine aminotransferase(ALT) also showed significant improvement from 108$\pm$38IU/l to 33$\pm$9IU/l(p<0.01). Therefore if we can rule out the other causes of hepatic abnormalities in obese children with fatty liver, these patients showed benefit by weight control as it can improve both obesity and liver functions.
