Effects of Caloric Restriction on Blood Glucose and Serum Lipid Levels of OLETF Rats (칼로리 급여량 제한이 OLETF 쥐의 혈당 및 혈청 지질량에 미치는 영향)

  • To determine the effects of caloric restriction on obese type 2 diabetes we measured body blood glucose and serum lipid level in dietary restricted Otsuka Long Evans Tokushima Fatty (OLETF) rats. OLETF rats (obese diabetic rats) and Long Evans Tokushima Otsuka (LETO) rats (control rats) were grouped into 2 groups; control (free feed) group and 30% caloric restricted (30% CR) group. At 24 weeks of age the 30% CR animals were provided a diet at a level of 30% less food (by weight) than control rats consumed during the previous week. Blood glucose levels and serum triglyceride, total cholesterol, and high density lipoprotein (HDL)-cholesterol levels of CR rats were determined every 2 weeks for 8 weeks total. Blood glucose, triglyceride and total cholesterol levels of OLETF rats were significantly higher compared to LETO rats. In OLETF rats, the blood glucose levels were decreased to 61% by 8 weeks in the 30% CR compared to the non-CR control group, but changes of blood glucose levels were not observed in LETO rats during the 8weeks in the 30% CR. The serum triglyceride levels of OLETF rats were decreased significantly in the 30% CR but no change in the serum of LETO rats was found. The total cholesterol level was not changed by dietary restriction in LETO rats, but significant changes were observed in OLETF rats by 30% dietary restriction. HDL-cholesterol levels were also increased by dietary restriction in both LETO and OLETF rats. These results suggested that elevated blood glucose, triglyceride and total cholesterol levels in diabetes II patients may be reduced by caloric restriction.

Alteration of Hemorrhagic Aldosterone Response During Sodium Restriction, Potassium Supplement and Diuresis (Na 섭취제한, K 투여 및 이뇨 기간중 실혈성 Aldosterone 반응의 변동)

  • Effect of sodium restriction with or without potassium supplement and furosemide diuresis on plasma aldosterone response to mild hemorrhage were studied in normotensive young volunteers. After an overnight fast, blood were drawn just before and 10, 20, 30, 50, 70, 90, and 120 minutes after the $^3H$-aldosterone injection. The sum of blood delivered reached over 100ml(during two hours). Plasma aldosterone and renin were measured by means of radioimmunoassay. The results were as followed; 1. Hemorrhage resulted in a moderate increase in plasma aldosterone level of volunteers with normal diet. 2. The mean figures of plasma aldosterone in subjects with sodium restriction and diuresis were likewise significantly increased by hemorrhage, however, the figure of the subjects with potassium supplement who already shown higher plasma level was without effect on hemorrhage. 3. Hemorrhage produced slight decrease in serum sodium concentration in every experimental conditions, although the changes were not significant. 4. Plasma renin activities after the hemorrhage followed a similar pattern with that of aldosterone, increased during sodium restriction or diuresis and unaffected during potassium supplement.

  • Inherited metabolic disease is rare disorders that show symptoms mainly in pediatric age and early treatment is important for preventing complications of the disease. Recent development in molecular and biochemical techniques help clinicians with proper diagnosis of patients, however, many of the disease still remain lack of effective therapeutic strategies. Better understanding on biochemical and molecular basis of pathogenesis of the disease combined with advanced medical care would provide new sight on the disease that can also improve the quality of life and long-term prognosis of patients. Traditionally, there are several modalities in the treatment of metabolic diseases depend on the biochemical basis of the disease such as diet restriction, removing or blocking the production of toxic metabolites, and stimulating residual enzyme activity. The inherited metabolic disease is not familiar for many clinicians because the diagnosis is troublesome, treatment is complicated and prognosis may not as good as expected in other diseases. Recently, new therapeutic regimens have been introduced that can significantly improve the medical care of patients with metabolic disease. Enzyme replacement therapy has showed promising efficacy for lysosomal storage disease, bone marrow transplantation is effective in some disease and gene therapy has been trying for different diseases. The new trials for treatment of the disease will give us promising insight on the disease and most clinicians should have more interest in medical progress of the metabolic disease.

Effects of Sodium Restriction and Potassium Supplement on Aldosterone Secretion Rate In the Normal Korean (한국인의 Aldosterone 분비율에 미치는 Na 섭취제한 및 K 투여의 영향)

  • Author have already reported that urinary aldosterone excretion of the Korean who usually eat high sodium diet is significantly lower comparing with the American, although the plasma aldosterone concentration is identical in the former with that of the latter. Measurement of urinary aldosterone excretion and Plasma concentration only is insufficient to establish the pressence and/or mode of evolution of the Korean. In this experiments, aldosterone secretion rate(ASR) was measured in normotensive Korean during high and low dietary sodium intake with or without additional potassium supply. Results were as follows; 1) In normal Korean, dietary sodium restriction resulted in appreciable increase in ASR, and a sustained increase in urinary aldosterone excretion with an increase in plasma level. 2) Oral potassium loading easily stimulated the adrenal cortex of the Korean who already adapted to a high sodium diet when dietary sodium is still identical with not·mal American. 3) Quantitative relationships between aldosterone secretion rate, plasma concentration and urinary excretion of aldosterone were altered by potassium loading. 4) Urinary aldosterone excretion didn't reflect concurrent increase aldosterone secretion in subjects with Potassium intake. It was discussed that the changes of tile relationships and of adrenal hyper response on Potassium Beading in the Korean will be elucidated by measuring the metabolic clearance rate.

Clinical Implications of Intermittent Fasting (간헐적 단식의 임상적 적용)

  • Dietary therapy is one of the most important treatments for obesity. In general, it is difficult to maintain the recommended diet for weight control for a long time; therefore, it is difficult to achieve weight loss or maintain weight. Intermittent fasting has recently become one of the most popular diets for weight loss. Intermittent fasting is a strategy of repeating intermittent energy restriction and eating, unlike conventional diets of continuous calorie restriction. Studies on intermittent fasting have shown positive results not only in terms of weight loss but also improvement in metabolic indicators, such as glucose control and reduction of blood pressure. Therefore, it is important to maintain a long-term dietary strategy to prevent weight loss in obese individuals. The effect on weight loss was similar to that of an existing continuous energy-restricted diet. However, long term studies and safety data are still lacking, and large-scale studies with various populations are needed. If more evidence is secured for various individuals, it can be expected that intermittent fasting, including time-restricted eating, will be applied clinically in the future.

Compensatory nutrition-Mediated Lactation Potential and Milk Protein Gene Expression in Rats (보상성장에 의한 에너지 섭취량 조절이 흰쥐의 유생산능력과 유단백질 유전자 발현에 미치는 영향)

  • The objective of this study was to determine the extent to which the compensatory nutrition regimen modulates lactation performance and milk protein gene expression in the first and second lactation cycles. Female rats(28 days of age) were assigned to 1)control ad libitum ; 2) stari-step compensatory nutrition(SSCN) regimen an alternating 3-2-3-4-week schedule beginning with an energy restriction diet(40% restriction) for 3 weeks followed by the control diet(ad libitum) for 2 weeks and then alternating another 3-4 week feeding regimen. The SSCN rats were received an overall 20% energy restriction(average from all stair-step periods) compared with the conventionally fed control group. Rats were bred during the first week of the second realimentation. All pups were weaned on day 21 of lactation. About 1 week after weaning all dams were mated for the second pregnancy. Mammary tissues were obtained from pregnant and lactating rats during the first and second lactation cycles. During these lactation cycles the SSCN group had a 11% increase in average lactation performance over that of control. The SSCN group had significantly increased levels of milk protein gene($\alpha$- and $\beta$-casein) expression in mammary tissues during the first lactation cycle compared with those of the control group. During the second lactation period the levels of milk protein gene expression in lactating mammary tissues of the SSCN group were also higher than those of the control group. These results suggest that the effects of compensatory growth imposed at an early age extend to the second lactation cycle with regard to increased lactation performance and milk protein gene expression.

Effects of Dietary Restriction on the Body Weight and Antioxidant Enzymes in Various Organs of Diabetic Rats (당뇨병 흰쥐에서 식이 제한 급여가 장기의 항산화효소 활성도 및 체중에 미치는 영향)

  • The effects of dietary restriction (DR) on antioxidant enzymes were studied in liver, lung and erythrocytes of diabetic rats. Experimental animals used Sprague-Dawley (SD; body weight 350$\pm$20g) male rats and Otsuka Long Evans Tokushima fatty (OLETE; body weight 5--$\pm$30g) male rats, as a model of type 2 diabetes mellitus. Type I diabetes was induced in SD rats by intramuscular injection of alloxan (80 mg/kg BW). Animals were randomly assigned either to continue the ad libitum diet or 40% DR (60% intake of ad libitum diet) groups. The body weight was measured at every 2 weeks to 4 months following DR. The activities of antioxidant enzymes (superoxide dismutase (SOD), catalase, glutathione peroxidase (GSHPx) were measured in liver, lung and erythrocytes and the concentration of TBARS as a marker of reactive oxygen species-induced tissue injry was also measured in rats after 4 months 40% DR. The body weight 4 months after 40% DR of control SD, alloxian-diabetid SD and OLETE rats were 80%, 98% and 75% of each control groups, respectively. The activities of SOD, catalase and GSHPx in lung and erythrocytes of rats were not change by 40% DR but in 4 month 40% DR rat liver, the activities of SOD and catalase were increased in control SD, alloxan-diabetic SD, and OLETF groups. The concentration of TBARS in lung and erythrocytes was also not changed by 40% DR, while liver TBARS concentration was decreased in OLETF and control SD rats compared to each non-DR control rats. These results suggested that the body weight changes in diabetic rats by DR was more prominent in type 2 diabetes and changes of antioxidant enzymes is most prominent in liver by DR either type 1 and 2 diabetic rats.

The effects of elimination diet on nutritional status in subjects with atopic dermatitis

  • A food allergy is an adverse health effect arising from a specific immune response that occurs reproducibly upon exposure to a given food. In those with food allergies that are thought to cause aggravation of eczema, food avoidance is important. The objective of this study was to research the nutritional status of patients with food allergies. A total of 225 subjects diagnosed with atopic dermatitis underwent a skin prick test as well as measurement of serum immunoglobulin E. Food challenge tests were conducted using seven food items: milk, eggs, wheat, soybeans, beef, pork, and chicken. At post-food challenge visits to the test clinic, participants completed a three-day dietary record, which included two week days and one weekend day, in order to evaluate energy intake and diet quality during the challenge. We analyzed nutrient intake based on differential food allergens. Subjects with a food allergy to milk showed lower intake of Ca, Zn, and vitamin B2, and subjects with a food allergy to egg showed lower intake of vitamin A, B1, B2, niacin, and cholesterol. Subjects with a food allergy to wheat and soybean showed lower intake of Ca, P, Fe, K, Zn, vitamin B2, vitamin B6, and niacin; and subjects with a food allergy to beef, pork, and chicken showed lower intake of Fe and higher intake of K, vitamin A, B2. Subjects with atopic dermatitis were lacking in several nutrients, including vitamin A and vitamin C. A greater number of food allergies showed an association with a greater number of nutrient intake deficiencies. Allergen avoidance is the basic treatment for atopic dermatitis. However, when the allergen is food, excessive restriction can lead to nutrition deficiency. Findings of this study suggest the necessity for enhanced nutritional education in order to provide substitute foods for patients with food allergies who practice food restriction.

Comparison of Perception of the Neutropenic Diet between Nurses and Patients (호중구감소증 환자의 식품별 섭취허용에 대한 간호사와 환자의 인식 비교)

  • Purpose: This descriptive study aimed to compare the perception of the acceptable foods for the neutropenic diet between nurses and patients by food type. Methods: The participants were 225 nurses working at chemotherapy wards and 71 patients in chemotherapy treatment. Data were collected with a self-administered questionnaire from January 2 to February 24, 2012, and analyzed with SPSS 12.0 program using descriptive statistics and the ${\chi}^2$-test. Results: Eighty-eight point eight percent of nurses and 76.1% of cancer patients thought the patients needed the neutropenic diets. The most important decisional criteria to determine dietary restriction was neutrophil count for nurses and food type for patients. The two groups showed significantly different allowance to raw fruits and raw vegetables, sterilized canned juice, fried vegetables, yogurt, uncooked tofu, salted fish, cooked fish, cooked shellfish, uncooked grain powder, jellified food, home-made bread/cookies, nuts including peanuts, instant coffee or tea and tea brewed. In general, patients were more permissive about the neutropenic diet than nurses. Conclusion: It is recommended to consider patients' preference as well as nurses' professional knowledge and publish standardized clinical diet guidelines for neutropenic patients with collaboration between nurses and patient representatives.
