A Study on the Design of DICOM Integration Engine in the Ubiquitous Computing Environments (유비쿼터스 컴퓨팅 환경에서의 DICOM 설계에 대한 연구)

  • In the ubiquitous computing environments, ICT industries of current society are developed in enormous growth. Medicine or patients with mobile devices can access at any time, any place. The medical procedures at the patient bedside are out of the scope of current systems, which means that patient record and image data access during the medical visit or the execution, recording and confirmation of the medicine prescriptions, still do not enjoy computerized support. Today, the exchange of medical images and clinical information is well defined by DICOM and HL7 standards. The DICOM independent terminal equipment image access system was developed in which a DICOM Engine acts as the gateway between a PACS DB and user's terminal. Implementation system is compatible with most currently available Integration system models. This paper presents a software technology where the medical and nursing staff will be equipped with any device connected by wire and wireless to a central server that provides access to the electronic patient records and that will actively inform about tasks pending distribution. The prototype described in this article implements a medical images and structured reports server that makes the search and recovery of data stored in the DICOM standard possible.

  • The general medical information service in hospital recently has been turned into ubiquitous environment. A key requirement in ubiquitous environment is the ability for communication between image viewer realized by DICOM standard and a server supporting medical information to make it possible. In order to apply the proposed scheme, we first realized the server/client networking in mobile terminal environment. Second, we realized DICOM viewer using a PDA to be capable of wireless communication in ubiquitous environment. Third, MIPS(Medical Information Processing System) utilized RFID system to apply the patient's information efficiently. In particular, it showed the capability to communicate image based system unlike previous text based communication.

PACS in Nuclear Medicine (핵의학 PACS)

  • PACS (Picture Archiving and Communication System) is being rapidly spread and installed in many hospitals, but most of the system do not include nuclear medicine field. Although additional costs of hardware for nuclear medicine PACS is low, the complexity in developing viewing software and little market have made the nuclear medicine PACS not popular. Most PACS utilize DICOM 3.0 as standard format, but standard format in nuclear medicine has been Interfile. Interfile should be converted into DICOM format if nuclear images are to be stored and visualized in most PACS. Nowadays, many vendors supply the DICOM option in gamma camera and PET. Several hospitals in Korea have already installed nuclear PACS with DICOM, but only the screen captured images are supplied. Software for visualizing pseudo-color with color lookup tables and expressing with volume view should be developed to fulfill the demand of referring physicians and nuclear medicine physicians. PACS is going to integrate not only radiologic images but also endoscopic and pathologic images. Web and PC based PACS is now a trend and is much compatible with nuclear medicine PACS. Most important barrier for nuclear medicine PACS that we encounter is not a technical problem, but indifference of investor such as administrator of hospital or PACS. Now it is time to support and invest for the development of nuclear medicine PACS.

  • 본 연구에서는 환자와 한의사, 방사선 전문의 간에 초고속 통신망을 이용하여 원격으로 환자의 질환 상태를 파악하고 이에 따른 처방전을 내릴 수 있도록 하는 한방 원격 검진 시스템에서 환자의 X-ray 사진, CT 사진 등의 방사선 이미지들을 DICOM 프로토콜에 의거하여 저장하고 검색할 수 있도록 해주는 시스템을 구현하였다. 본 연구에서는 이미지 처리 및 관리 모듈을 Java 언어를 이용하여 구현함으로써 방사선 전문의와 한의사들이 컴퓨터의 기종에 관계없이 웹브러우저를 사용하여 환자의 X-ray나 CT 등의 방사선 사진을 살펴볼 수 있도록 하였다.

  • In this paper, we designed a emergency medical image communication system EMICS added concept of emergency medical image to the existing emergency medical information system based on DICOM. Also we suggested a emergency medical image object EMISPS of EMICS. Using EMICS, the emergency medical technician can work together with emergency doctor. Therefore the patient can take more stable care than existing emergency medical information system. Using EMISPS, the emergency medical technician can get exact situation information of the patient.

  • Medical imaging device에서 이미지 정보의 교환은 PACS의 성공을 결정짓는 중요한 요소이다. 즉 이미지 정보를 획득한다는 것은 네트워크상에서 정보를 신속 정확히 얻을 수 있다는 장점을 말한다. 더욱 중요한 것은 이미지파일의 호환성이다. 의료영상저장전달시스템 즉 PACS는 병원의 모든 정보와 서로 연동될 수 있어야 한다. 데이타 통합과 시스템의 안정성은 PACS에 있어서 가장 중요한 필수요건이다. 위에 언급한 요구조건들을 모두 만족시키기 위해서는 시스템 상호간을 네트워크할 수 있는 공통의 통합 프로토콜이 필요하다. 이와같은 문제를 해결하기 위해서 The American College of Radiology(ACR)과 National Electrical Manufacturers Association(NEMA)는 1983년초에 업체간의 장비공유와 방사선 실무자들의 통합되는 프로토클을 제정하기 위해 위원회를 설립하였다. 여기서 제정된 프로토콜을 DICOM이라 정하였다. DICOM의 역사적 배경, 의미, 필요성과 시스템상에서 어떻게 역활해왔는가와 DICOM을 통한 PACS시스템의 실제 활용예에 대하여 알아보겠다.

  • 본 논문은 의료영상진단시스템에서의 의료정보보호를 위한TLS 프로세서의 구현에 관하여 기술하였다. DICOM 3.0 standard의 ‘Part 15. DICOM Security Profile’에서는 TLS와 ISCL 두 가지의 Secure Transport Connection Profile을 정의하고 있고, 인증, 데이터의 무결성 보장, 프리아버시 보호의 기능을 할 수 있도록 몇가지의 알고리즘을 사용할 것을 정의하고 있다. 그 중 TLS Security Profile에는 Triple DES CBC모드와, RSA and SHA를 정의하고 있다. 그리하여 본 논문에서는 세 알고리즘의 개별적인 동작 검증을 마친 후 통합된 TLS Processor를 설계하고 검증하였다. 일반적인 Mux만을 이용한 설계는 임베디드 시스템 적용에 있어서 면적을 많이 차지하는 단점이 나타났다. 따라서 면적을 많이 차지하는 레지스터를 줄이기 위해서 세 알고리즘 블록이 같은 레지스터를 공유하도록 설계하였다. 그리고 임베디드시스템 개발 키트인 IFC-ETK100장비의 FPGA에 회로를 올리고 검증하였다.

Design and Implementation of Web-Based MINI-PACS using the DICOM (DICOM을 이용한 웹 기반 MINI-PACS 설계 및 구현)

  • In recent years, medical procedures have become more complex, while financial pressures for shortened hospital stays and increased efficiency in patient care have increased. As a result, several shortcomings of present film-based systems for managing medical images have become apparent. Maintaining film space is labor intensive and consumes valuable space. Because only single copies of radiological examinations exist, they are prone to being lost or misplaced, thereby consuming additional valuable time and expense. In this paper, mini-PACS for image archiving, transmission, and viewing offers a solution to these problems. Proposed mini-PACS consists of mainly four parts such as web module, client-server module, internal module, acquisition module. In addition, mini-PACS system includes DICOM converter that non-DICOM file format converts standard file format. In client-server module case, proposed system is combined both SCU (service class user: client) part and SCP(service class provider: sewer)part therefore this system provides the high resolution image processing techniques based on windows platform. Because general PACS system is too expensive for medium and small hospitals to install and operate the full-PACS. Also, we constructed web module for database connection through the WWW.

  • Application of information system to hospital would bring innovative improvement on efficiency of business management and provide high quality services toward patients as well as the retrenchment of operating funds. PACS(Picture Archiving and Communication System) including X-ray film that manages the medical image information effectively, has drawn considerable attention to essential structural elements to the sophisticated information system for hospital. PACS system should be connected to the network after making a form of standard medical image file from different style of image information obtained from various medical instruments. In this paper, to solve this problem, we construct Mini-PACS that converts the form of Non-DICOM file to the form of standard file by designing the DICOM converter. This system is designed to be managed under Web environment. Comparing with the existed Mini-PACSs, consisting of the client and server module, our system is designed and implemented with integration of these functions in order to be strongly combine strongly between system.
