Biological Activity Review of Mume Fructus and Processed Mume Fructus (오매(烏梅)와 증숙 오매(蒸熟 烏梅)의 생리활성 연구)

  • Objectives: Presently Mume Fructus (MF) undergoes fumigation, which produces benzo[a]pyrene. As a primary analysis with the aims to minimize the production of benzo[a]pyrene and to suggest standards for processing the MF, the steaming method was chosen among the various processing methods, and reviewed through a series of experiments.Methods: Methods:Pitted and un-pitted MF were steamed and processed into samples. After testing level of benzo[a]pyrene, the samples were analyzed for amount of polyphenol and flavonoids. Scavenging activities of the samples for the DPPH and ABTS radicals were tested. In order to measure anti-inflammatory effects of the samples, cell survival rate was investigated using CCK-8 Assay. Also, water extracts of dried and steamed MF were administered to the RAW 264.7 cells to compare expressions of NO, PGE2, IL-1β, and TNF-α. In addition, anti-diarrhea effects of the herbal medicine were tested on animal models with diarrhea induced by MgSO4 and Castor oil.Results: Regardless of pitting, processed MF contained no benzo[a]pyrene. Anti-oxidation effect increased in relation to the frequency of steaming process. However, extracts of dried and steamed MF suppressed different kinds of inflammation factors, and extract of dried MF showed superior anti-diarrhea effect than extract of steamed MF.Conclusions: It is suggested that steaming method of MF is recommended for processing the herbal medicine without the production of benzo[a]pyrene. But regarding that dried and steamed MF showed differences in their anti-oxidative, anti-inflammatory, and anti-diarrhea effects, it is recommended to perform further researches on different efficacies of MF according to their processing methods.

Efficacy of Lactobacillus Acidophilus in Treatment of Acute Diarrhea in Children (소아의 급성 설사에서 Lactobacillus Acidophilus의 치료 효과)

  • Purpose: Acute diarrhea in young children is a major problem in pediatric hospitals worldwide. We evaluated the clinical efficacy of orally administered Lactobacillus acidophilus in the treatment of acute diarrhea in children. Methods: From September 2002 to July 2003 at National Police Hospital 41 children aged 3 months to 5 years with acute diarrhea were enrolled in this study. The patients were randomized to one of two groups to receive either $0.5{\times}10^8$ colony forming unit (CFU) of L. acidophilus or matching placebo on admission and every 8 hours during hospitalization. Results: The mean duration of diarrhea in all 41 children was decreased (p=0.001) in the L. acidophilus (40.5 hours) group compared to the placebo (56.6 hours) group. Stool frequency was also reduced (p=0.01) on the 3rd day in the L. acidophilus group. Rotavirus was identified in 58% of the patients. The decrease of duration of diarrhea was more significant in rotavirus-negative patients (p=0.002) compared to the rotavirus-positive patients (p=0.027). Conclusion: L. acidophilus shows to be an effective therapeutic agent in acute diarrhea in children. Further studies are needed to confirm the present findings.

Effects of Eco-friendly Multi-enzyme on Diarrhea and Immune Response of weaned Pigs (친환경 복합효소제 첨가가 이유자돈의 설사 및 면역반응에 미치는 영향)

  • This study was designed to investigate the effects of multi-enzyme on diarrhea and immune responses of weaned pigs. A total 36 weaned pigs ($5.92{\pm}0.48kg\;BW$; 28 d old) were randomly allotted to 2 dietary treatments (3 pigs/pen, 6 replicates/treatment) in a randomized complete block design. The dietary treatments were a typical diet based on corn and soybean meal (CON) and CON with 0.1% multienzyme (Multi; mixture of ${\beta}-mannanase$, xylanase, ${\alpha}-amylase$, protease, ${\beta}-glucanase$, and pectinase). Pigs were fed their respective diets for 6 wk. Frequency of diarrhea, levels of packed cell volume (PCV), white blood cells (WBC), immunoglobulins, cortisol, tumor necrosis $factor-{\alpha}$ ($TNF-{\alpha}$), transforming growth $factor-{\beta}$ ($TGF-{\beta}$), and C-reactive protein (CRP) were measured. Multi group tended to decrease (p<0.1) diarrhea frequency than CON group during 2 wk after weaning. Lower values of PCV on d 3 (p<0.05) and d 7 (p<0.1) were found in Multi group compared with CON group. There were no significant differences on WBC number and immunoglobulin (Ig) M and A between Multi and CON groups. However, Multi group tended to increase (p<0.1) Ig G on d 7 than CON group. Moreover, Multi group showed modulated immune responses, indicated by decreased levels of cortisol (p<0.05) on d 7 and 14, $TNF-{\alpha}$ on d 3 (p<0.05) and d 7 (p<0.10), $TGF-{\beta}$ on d 2 (p<0.05) and d 7 (p<0.10), and CRP (p<0.10) on d 3 and 7 after weaning compared with CON group. Consequently, inclusion of multi-enzyme in diets for weaned pigs improved gut health and modulated immune responses of weaned pigs.

A Case Report of a Patient with Active Ulcerative Colitis Treated with Do-Che Decoction-based Korean Medicine (도체탕(導滯湯) 위주의 한방치료로 호전된 활동성 궤양성 대장염 환자 치험 1례)

  • Background : Sulfasalazine for treating ulcerative colitis has a clinical limitation due to its adverse effects. This case is about a patient with ulcerative colitis who could not tolerate sulfasalazine and was improved by Do-che decoction-based Korean medicine. Method and Results : Purulent and bloody diarrhea more than 20 times a day, mild fever, chilling and tenesmus were main symptoms of this patient who was diagnosed as ulcerative colitis 10 years ago. Do-che decoction-based Korean medicine which was recommended for purulent and bloody diarrhea in Dong-Ui-Bo-Gam, the classic literature of Traditional Korean Medicine was given to the patient for more than 1 month. After treatment, the frequency of purulent and bloody diarrhea, tenesmus and fever was decreased remarkably, and C-reactive protein, erythrocyte sedimentation rate, white blood cell count and maximum temperature also dropped to the normal range. Conclusion : Do-che decoction-based Korean medicine has a potential effect and may be a alternative therapeutic option in patients with ulcerative colitis who cannot tolerate sulfasalazine due to its adverse effects.

Virulence and antimicrobial resistance genes of pathogenic Escherichia coli from piglets showing diarrhea before and after ban on antibiotic growth promoters in feed (사료 첨가 항생제 금지 전후 돼지 설사증 유래 대장균의 병원성 인자 및 항생제 내성 유전자)

  • This study examined the prevalence of adherence factors, toxin genes, antimicrobial resistance phenotypes, and resistance genes in Escherichia coli (E. coli) isolated from piglets with diarrhea before and after the ban on antibiotic growth promoters (AGPs) in Korea from 2007 to 2018. In this period, pathogenic 474 E. coli isolates were obtained from diarrheic piglets. The virulence factors and antimicrobial resistance genes were assayed using a polymerase chain reaction, and the susceptibility to antibiotics was tested according to the Clinical and Laboratory Standards Institute guidelines. After the ban on AGPs, the frequency of F4 (12.5% to 32.7%) increased significantly, and LT (31.9% to 20.3%) and EAST-I (46.5% to 35.2%) decreased significantly. In addition, the resistance to streptomycin (45.8% to 67.9%), cephalothin (34.0% to 59.4%), and cefazlin (10.4% to 28.8%) increased significantly. Colistin resistance plasmid-mediated genes, mcr-1 and mcr-3, were detected after the ban on AGPs. The results of this study can provide useful data for analyzing the impact of the ban on AGPs on the virulence profiles and antimicrobial resistance of E. coli isolated from piglets with diarrhea in Korea.

Effects of Dietary Herbaceous Peat and Vitamin C on Immunity and Growth Performance in Holstein Calves (허브부식토 및 비타민 C 급여가 홀스타인 송아지의 면역력 및 증체에 미치는 영향)

  • This study was conducted to determine the effects of supplementing herbaceous peat preparation and vitamin C in order to find out its effects on immunity and growth performance in Holstein calves. Twenty Holstein calves ($39.3{\pm}4.21kg$ average initial body weight) were allocated into four treatments by completely randomized design. Dietary treatments included: 1) control (Basal diet), 2) treatment 1 (control +5% herbaceous peat), 3) treatment 2 (control+10g vitamin C) and 4) treatment 3 (control+10g herbaceous peat+10g vitamin C). The duration of the experiment was 38 days in this experiment, and blood metabolism, dry matter intake, body weight and diarrhea frequency were investigated. For blood metabolism, white blood cells (WBC), neutrophils (NE), lymphocytes (LY), and red blood cells (RBC) did not show any significant differences among treatments. Average daily gain and feed conversion were highest in T1 group feeding with the herbaceous peat preparation compared to the control groups and diarrhea frequency showed lowest in T3 group. Overall results of the present experiment indicated that dietary supplementation with herbaceous peat influenced positively on growth performances in Holstein calves.

Digestive Tolerance and Safety of an Anti-Regurgitation Formula Containing Locust Bean Gum, Prebiotics and Postbiotics: A Real-World Study

  • Purpose: Infant regurgitation is associated with other functional gastrointestinal disorders and signs and symptoms that have a major impact on the quality of life of infants and their families. This study evaluated the safety, tolerance, and real-world effectiveness of an anti-regurgitation formula containing locust bean gum (LBG), prebiotics, and postbiotics to alleviate digestive symptoms beyond regurgitation. Methods: This 3-month study involved infants with regurgitation requiring the prescription of an anti-regurgitation formula according to usual clinical practice. Outcomes included evaluation of the evolution of stool consistency and frequency; occurrence of colic, constipation, and diarrhea; and assessment of regurgitation severity. Infant crying, parental assessment of infant well-being, and parental satisfaction with the stool consistency were also evaluated. Results: In total, 190 infants (average age: 1.9±1.1 months) were included. After three months, stool frequency and consistency remained within the normal physiological range, with 82.7% of infants passing one or two stools per day and 90.4% passing loose or formed stools. There was no significant increase in the number of infants with diarrhea, whereas a decrease was observed in the number of infants with constipation after 1 month (p=0.001) and with colic after both 1 and 3 months (p<0.001). Regurgitation severity and crying decreased and parental satisfaction with stool consistency, formula acceptability, infant well-being, and sleep quality increased. Monitoring of adverse events did not reveal any safety concerns. Conclusion: Formulas containing LBG, prebiotics, and postbiotics were well tolerated and provided an effective strategy for managing infant regurgitation and gastrointestinal discomfort.

Antibiotic-Associated Diarrhea in 3 to 6 Month Old Infants with Febrile Urinary Tract Infections (영아 발열성 요로감염 환자의 항생제 연관 설사)

  • Purpose : This study attempted to investigate the frequency, duration, and risk factors of antibiotic-associated diarrhea (AAD) in infants hospitalized due to febrile urinary tract infection (UTI). This is a basic research on the probiotics used in the prevention and treatment of AAD in infants. Methods : Medical records of the infants aged 3-6 months hospitalized in Gachon University Gil Hospital from January 2008 to September 2010 due to the febrile UTI were retrospectively reviewed. The episodes of loose or watery stool were investigated for frequency, onset, and duration. Those who had AAD and those who did not (non-AAD) were compared. The antibiotic regimens and the episodes of diarrhea were investigated in AAD group. Result : Total 147 infants were included. Fifty-four (36.7%) showed AAD. Intravenous third-generation cephalosporin ($3^{rd}$ CS) single therapy was used for 102 patients (69.4%), the $3^{rd}$ CS and non-$3^{rd}$ CS combination therapy for 24 (16.3%), and non-$3^{rd}$ CS combination therapy for 21 (14.3%). There was no significant difference in the dose of cefotaxime between AAD and non-AAD group (P=0.601). According to the antibiotic therapies above, in AAD group, there was no significant difference in the onset and duration of diarrhea respectively (P=0.717, P=0.830). Although the frequency of diarrhea was higher for the $3^{rd}$ CS and non-$3^{rd}$ CS combination therapy subgroup with $9.25{\pm}5.30$ times/day than the other two subgroups ($7.58{\pm}2.97$ times/day in $3^{rd}$ CS single therapy subgroup, $6.75{\pm}4.40$ times/day in non-$3^{rd}$ CS combination therapy subgroup), there was no statistical significance (P=0.078). Conclusion : AAD seems common to the infants aged 3-6 months with febrile UTI, regardless of regimen and amount of antibiotics in usual dosage. Further research on the effects of probiotics used in the prevention and treatment of AAD in infants is warranted.

Frequency of Clostridium difficile-associated Diarrhea and Relevant Medical Treatment in a Tertiary Care Hospital in Korea (3차 의료기관에서 Clostridium difficile-associated Diarrhea의 발생빈도 및 치료에 관한 연구)

  • 배경: 약물로 인한 Clostridium difficile-associated diarrhea (CDAD)는 널리 알려져 있으며 우리나라에서 항생제와 프로톤 펌프 억제제 소모량을 고려할 때 질환 치료과정에서의 CDAD 발생빈도 및 CDAD 유발 이전에 투여한 약물의 사용빈도와 CDAD의 치료방법을 조사할 필요성이 있다. 방법: 경상대학교 병원에서 2011년 1월부터 6월까지의 입원환자를 대상으로 대변 독소 검사에 의해 CDAD로 판명된 환자의 성별, 연령분포, 질환명, 입원병동, 재발률을 조사하였으며 CDAD 판명이전에 투여한 약제 및 CDAD 판명후 치료약제를 조사하였다. 결과: 연구기간 동안 CDAD 대변 독소 검사 의뢰된 환자수는 1,500명이었으며 CDAD 양성은 111명(9.3%)이었고, 재발은 29명(26.1%)이었다. CDAD를 주소로 입원한 환자는 17명 (15.3%)이었고, 나머지는 입원기간 중에 발생하였다. CDAD 양성인 환자의 연령대는 60대에서 32.4% (36/111명) 이었고, 내과병동에서 34.2%를 나타내었고, 재발률은 외과계 병동에서 41.4%로 가장 높게 나타났다. CDAD 환자의 17% (19/111명)은 항암제 투여 동안 발생하였으며 CDAD 발생 전 사용약물로는 세팔로스포린계 항생제가 162회로 가장 빈번하게 사용 되었으며, 히스타민2 수용체길항제 107회, 스테로이드 82회, 비 스테로이드 항염제 79회, 프로톤 펌프 억제제 77회, 하제 59회, 항암제가 33회 처방되었다. CDAD 치료약제로는 8종의 약제가 241회 처방 되었으며 metronidazole이 99회로 가장 빈번하게 사용되었고, vancomycin이 37회로 나타났다. 결론: 입원환자에 있어서 CDAD양성은 특히 고령의 암환자가 많아 항암제 투여 시에는 CDAD 발생에 주의해야 할것으로 보인다. CDAD의 치료약제로는 metronidazole이 vancomycin 보다 많이 사용되는 것으로 나타났다.

The Cross-sectional Study on Characteristics between Soyangin Gyolhyungjeung and Soyangin Heat-related Diarrhea Accompanied by Headache in Patients of Indigestion (소화불량을 주소로 내원한 소양인(少陽人) 결흉증(結胸證)과 신열두통망음증(身熱頭痛亡陰證)의 소증(素證) 특성에 관한 단면연구)

  • 1. Objectives : The aim of this study was to survey the Original symptoms of Soyangin's and to compare the tendency of their symptoms between Soyangin Gyolhyungjeung and Soyangin Heat-related diarrhea accompanied by headache in patients of indigestion. 2. Methods : We recruited 76 Soyangin subjects in the Korean Medicine clinic and researched their original symptoms. Subjects were diagnosed by the Sasang Constitutional specialists and Sasang Constitutional Medication. Subjects were two groups, confirmed to Soyangin Gyolhyungjeung and Soyangin Heat-related diarrhea accompanied by headache. All subjects answered the questionnaire about their Original symptoms. We found the differences between 2 groups using the Mann-Whitney U Test. Analysis was conducted using SPSS 18.0 for Korean. 3. Results : In Original symptoms, there was no difference between 2 groups in sleep, appetite, digestion, eating habits, water temperature, function of bowel movement, urination, sweating, sensitivity to coldness and sensitivity to warmness. But there was difference between 2 groups in frequency of defecation & hardness of feces(p=0.027). 4. Conclusions : We found the differences of heat and cold in symptoms between two Soyangin syndromes.
