Giant paraesophageal hiatus hernia associated with severe anemia [Belsy mark V procedure] (심한 빈혈증상을 동반한 거대한 식도주위허니아 치험 1예 (Belsey Mark V 수술치험))

  • Paraesophageal hiatus hernia represents a rare hiatal hernia that are treated surgically. The completely asymptomatic paraesophageal hernia often does not reach the clinician or surgeon. But the presence of a symptomatic paraesophageal hernia is sufficient indication for surgery. The paraesophageal hernia may be approached either transthoracically or transabdominally. The general technique is essentially the same, whichever route is used. From either transthoracic or transabdominal approach, following reduction of the viscus and elimination of the sac, the diaphragmatic opening is then closed with interrupted heavy dacron or silk sutures in paraesophageal hiatus hernia. But if the phrenoesophageal membrane often is destroyed when the esophagogastric junction and the distal esophagus have been mobilized, it becomes important to fix the esophagogastric junction below the diaphragm, so that it does not slide through the hiatus and produce a sliding-type hiatus hernia in future. We have experienced one case of paraesophageal hiatus hernia which was accompanied with severe anemia in child. We preferred to approach through left thoracotomy incision and then pushed down the stomach into the abdominal cavity with complete excision of the hernial sac. We employed Belsey Mark V procedure using of Teflon felt pledgets with the mattress sutures against development of sliding-type hiatus hernia in postoperative period. postoperative course has been uneventful and good for about 3 months to this time.

  • This is a report of a case of congenital esophageal hiatus hernia, occurring in a younger child at the age of one year and two months. The child was suffered from vomiting (intermittently coffee-ground vomitus or blood-flecked vomitus), coughing, and high fever for about 3 months.Diagnosis was confirmed by the radiologic barium study of the gastrointestinal tracts. The herniated organs were entire stomach and a part of the transverse colon. They were located in the right chest. The etiologic pathology was mainly congenital developmental defect of the musculature of the diaphragm, forming the posterior margin of the esophgeal hiatus, and additionally slight shorted esophagus. After reduction of herniated organs back to the abdominal cavity through abdominal approach, herniorrhaphy was performed through transthoracic approach with modified Allison's procedure. At the same time, any associated anomalies, such as hypertrophic pyloric stenosis, were not seen. Postoperative course was uneventful.

  • Endometriosis involving sites outside the pelvic cavity is a relatively uncommon occurrence and its frequency is cited as being less than one percent of all occurences of endometriosis. Sites previously reported in literature have described involvement of the extraperitoneal portion of the round ligament, the rectosigmoid, the appendix, and the ileum. Other more distal sites reported to have been found to have endometriosis lesions include the inguinal ligaments, the umbilicus, abdominal incisional wounds, thoracic lesions involving the pleura or diaphragm which may result in catamenial pneumothorax, the kidneys, and vulvar episiotomy wounds. Endometriosis have even been reported to occur in males. In this presentation, three cases of endometriosis, two involving the vulvar area and one involving a Pfannenstiel incision wound, is presented with a review of the pertinent literature in order to discuss its etiology, pathology, diagnosis and treatment.

NUMERICAL STUDY ON SYNTHETIC-JET-BASED FLOW SUPPLYING DEVICE (합성제트 기반의 유량 공급 장치에 대한 수치적 연구)

  • Flow characteristics of synthetic jet based flow supplying devices have been computationally investigated for different device shapes. Jet momentum was produced by the volume change of a cavity by two piezoelectric-driven diaphragms. The devices have additional flow path compared with the original synthetic jet actuator, and these flow path changes the flow characteristics of synthetic jet actuator. Four non-dimensional parameters, which were functions of the shapes of the additional flow path, were considered as the most critical parameters in jet performance. Comparative studies were conducted to compare volume flow rate and jet velocity. Computed results were solved by 2-D incompressible Navier-Stokes solver with k-w SST turbulence model. Detailed computations revealed that the additional flow path diminishes suction strength of the synthetic jet actuator. In addition, the cross section area of the flow path has more influence over the jet performances than the length of the flow path. Based on the computational results, the synthetic jet based flow supplying devices could be improved by applying suitable shape of the flow path.

Right Bochdalek Hernia - The Third Case Report in Korea - (우측 Bochdalek Hernia - 1례 보고 -)

  • Bochdalek hernia is the most common congenital diaphragmatic hernia and that of the foramen of Bochdalek is rare in right side of the diaphragm. Two cases of right Bochdalek hernia were reported in literatures published in Korea. The first case was associated with hypoplasia of the lung and could not be survived. The second case [four months old male infant] was operated on August 28, 1971. at Ewha University Hospital arid survived. The third case was a three months old male infant who had been born at 8th lunar month in a private clinic. During the period of three months prior to admission this premature infant was suf-fered from mild respiratory distress measured as pneumonia. On December 20. 1972, this infant was admitted at Kyung Hee University Hospital with severe respiratory distress developed abruptly. Chest X-ray examination [barium study] demonstrated right diaphragmatic hernia. Right hemithorax was filled with intestines and right lobe of the liver which were repositioned back into the peritoneal cavity through the foramen of Bochdalek. 3. 0 cm x3.5 cm in size, and the defect was closed with interrupt mattress sutures. There was hypospadia with chordee but no other associated anomalies such as hypoplasia of the lung, malrotation or malfixation of intestines, and cardiovascular anomalies. The Postoperative hospital course was not eventful and the baby was discharged on the 14th postoperative day.

  • A 55-year-old female presented to Pusan National University Yangsan Hospital with left neck and shoulder pain. An anterior mediastinal mass was detected on chest CT and there were no other specific lesions in the lung or pleural cavity. An infected pericardial cysts was suspected and excision was performed through a left-sided VATS approach. The patient was discharged on the second post-operative day with left diaphragm palsy and praziquantel was prescribed after paragonimaisis was confirmed on pathology. The patient has not shown any particular problems at my outpatient clinic.

Design of Blank Support Structure for Large and Curved Thick Plate Forming (대면적 후곡판 성형을 위한 블랭크 지지구조 설계)

  • As one of the functional metal parts in steam turbine diaphragm assembly, the hollow-partitioned turbine nozzle (stator) has large and thick geometries, as well as an asymmetric configuration. Therefore it is hard to support a metal blank in the die cavity. To ease this situation and control posture and position of metal blank (workpiece), a blank support structure is newly introduced. The blank support structure is basically composed of enlarged arms from the blank, guide pins and linear bearings. It can help to control the intermediate blank without a critical sliding phenomenon. The operation mechanism of this blank support structure, during thick plate forming for the hollow-partitioned turbine nozzle stator, is first evaluated. A series of FEM-based numerical simulations, with respect to the width of the guide arm as geometric design parameters, are carried out to investigate its applicable range. As the results, it is observed the blank support structure for this thick plate forming can guide the workpiece to have stable posture during the plate forming process.

Peritoneopericardial Diaphragmatic Hernias in Four Dogs (복막 심낭 횡격막 허니아의 진단 영상 4례)

  • Peritoneopericardial diaphragmatic hernia(PPDH) is uncommon and congenital disease in dogs and cats. In PPDH, the peritoneal organs such as liver, small intestine, stomach and omentum are displaced into the congenital defect between pericardial sac and diaphragm and cause the abnormal round and enlarged cardiac silhouette. Abnormal cardiac silhouette contacts with the cranial diaphragmatic border consistently and soft tissue- and/or gas- density structures are summated over the cardiac density in radiography. The contrast medium flows from peritoneal cavity into the pericardial sac and demonstrates the herniated abdominal organs and the abnormal defect in positive peritoneography. In this study, 4 dogs was diagnosed as PPDH using radiography, peritoneography and thoracic ultrasonography and showed various clinical signs according to the kind, amount and clinical state of herniated abdominal organs.

MRI Findings of a Malignant Solitary Fibrous Tumor of the Diaphragmatic Pleura: a Case Report

  • Solitary fibrous tumors (SFT) are rare mesenchymal tumors that most commonly develop in the pleura; they rarely involve the diaphragm. MRI has not been widely used to evaluate SFTs of the thoracic cavity, though it may be highly useful in assessing local invasion, predicting malignant potential, and helping in the differential diagnosis. However, MRI findings of malignant SFTs of the diaphragmatic pleura have been described in only two cases. We report a rare case of a malignant solitary fibrous tumor of the diaphragmatic pleura in an 82-year-old man. We describe the clinical and characteristic imaging features, including computed tomography, conventional MRI, and diffusion-weighted imaging. Contrast-enhanced MRI is more accurate than is CT in identifying the origin of SFTs, predicting whether they ae benign or malignant, and assessing local invasion. This imaging modality proved helpful in deciding on the treatment strategy for these rare tumors.

Clinical Application for Video-Thoracoscopy in Lung Cancer Surgery Patients (폐암 수술환자에서 흉강경수술의 적용)

  • Background: Video-thoracoscopy is known to be an useful method to provide accurate pre-resectional staging in patients with lung cancer in addition to the conventional radiologic studies and mediastinoscopy, for the pleural cavity is inspected directly and biopsy specimens call be obtained. This study is undertaken to evaluate how video-thoracoscopy can be used in deciding pre-resectional stage Material and Method: Video-thoracoscopy was performed in patients with lung cancer who were scheduled for surgical resection based on the radiologic staging and mediastinoscopic biopsy. 37 patients were included in this study. Pre-thoracoscopically 18 cases were in TNM stage 1, 7 in stage 2, and 12 in stage 3. Result: In 15 of 37 cases, video-thoracoscopy could not be performed effectively due to heavy adhesions in the pleural cavity, diaphragmatic and chest wall invasion of tumor and bulky tumor mass es. Mediastinal lymph nodes were positive postresectionally in 6 of these 15 cases. In 22 cases, video-thoracoscopy was performed as usual. Positive mediastinal lymph nodes were identified in 2 cases and exploratory thoracotomy was prevented. Surgical resection were carried out in remaining 20 cases and 5 cases among them had positive mediastinal lymph nodes. Conclusion: We believe that it is difficult to perform pre-thoracotorny video-thoracoscopy for all lung cancer patients for there were many cases that thoracoscory could not be undertaken doe to heavy adhesions in the pleural cavity, tumor involvement of the chest wall and/or diaphragm and bulky tumor mass. However we think it is helpful in preventing unnecessary exploratory thoracotomy for some patients with lung cancer whom pre-thoracotomy video-thoracoscopy was carried out.
