  • Three neonates with congenital diaphragmatic eventration underwent intrathoracic operation had marked improvements in symptoms postoperatively. Two were one day of ages, one was 1 month of age, and they were all female and had other congenital abnormalities of lung hypoplasia, cleft palate, nasal polyps and neonatal hepatitis. The right diaphragm was more affected than left as 2:1. The repair for diaphragmatic eventration was performed successfully by plication of remnant diaphragm, and there were no complications postoperatively.

  • Anomalies of the diaphragm, particularly eventration, are not encountered frequently in clinical practices. Diaphragmatic eventration is generally accepted as an anomaly high position of part or all of the diaphragm, usually associated with a marked decrease in muscle fibers and a membranous appearance of the abnormal area. We experienced 3 cases of the congenital diaphragmatic eventration at the department of the thoracic and cardiovascular surgery, Maryknoll Hospital, which were treated successfully.

  • Since diaphragmatic injuries are difficult to diagnose, those that missed may present with latent symptoms.Delayed diaphragmatic hernia is very rare and occurs commonly after penetrating or blunt trauma. Recently we had experience with two cases of delayed diaphragmatic hernia after left nephrectomy. They were operated by herniolysis and closure of diaphragm using prosthetic patch[Dura, Dacron through the left thoracotomy. Postoperative courses were uneventful.

  • The vibration isolation table system for the high precision Processing machinery has been developed. The system uses air spring as its isolation elements. An investigation of the model and the test results showed that the diaphragm has a role in the mathematical model. The vibration levels at various floors in the laboratory were investigated during operating the large shaking table for the selection of optimum installation location. The vibration test on the designed system showed good isolation performances.

  • Tubular joints of jacket structures are usually reinforced using thicker can section, internally ring stiffeners, diaphragm, or externally gusset plates to increase load carry capacity. In this paper, the effect of reinforcement type and geometric parameters of stiffener on the ultimate strength of tubular X-joints subjected to brace compression have been studied numerically Three reinforcement methods were considered; (1)can reinforcement (2)internally ring stiffener (3)internally longitudinal diaphragm. The ANSYS software was used nonlinear strength analysis. It was found that there is significant strength enhancement for reinforced joints.

  • This paper has aimed to prevent excessive heat input by controlling arc distribution and heat input capacity with pulse GTAW in order to improve weld quality in 0.08mm pressure gauge diaphragm and flange welding parts. A design of experiment was designed using Box-Behnken method to optimize a welding process. The pulse GTAW parameters such as pulse current, base current, pulse duty, frequency and welding speed were set to input variables while hydraulic pressure that represents welding characteristics in diaphragm and flange joint were set to output variables. Based on the test result, a second regression equation was obtained between input and output variables and turned out significant. Besides, an influence of parameters has been confirmed through response surface analysis using the second-order regression equation and optimum welding condition was obtained through a grid-search method. The optimum welding condition was set to pulse current 84.4(A), base current 29.6(A), pulse duty 58.8(%), frequency 10(%), and welding speed 596(mm/min). Then, decent bead shape was acquired with no excessive heat input under the $2.3kgf/cm^2$ of hydrostatic pressure.

  • In this work, a new type speaker which features various resonant frequencies is proposed utilizing a magneto-rheological (MR) fluid and its performance is evaluated in terms of the change of the field-dependent sound pressure level. In order to achieve this goal, a whole concept of the speaker system is firstly discussed and subsequently a controllable diaphragm is made using MR fluid whose rheological properties such as viscosity are controllable by the magnitude of magnetic field. Then, the proposed speaker system consisting of the inner structure and the squeeze mode type of MR diaphragm is established in an anechoic room The effectiveness of the proposed speaker system is experimentally evaluated at two different conditions; with and without the magnetic field. It is shown from experimental tests that the sound pressure level at different sound source can be controlled which is not able to achieve using one conventional speaker system.

  • Background: Surgical correction needs to be considered when diaphragm eventration leads to impaired ventilation and respiratory muscle fatigue. Plication to sufficiently tense the diaphragm by VATS is not as easy to achieve as plication by open surgery. We used pneumatic compression with carbon dioxide ($CO_2$) gas in thoracoscopic diaphragmatic plication and evaluated feasibility and efficacy. Methods: Eleven patients underwent thoracoscopic diaphragmatic plication between January 2008 and December 2013 in Pusan National University Hospital. Medical records were retrospectively reviewed, and compared between the group using $CO_2$ gas and group without using $CO_2$ gas, for operative time, plication technique, duration of hospital stay, postoperative chest tube drainage, pulmonary spirometry, dyspnea score pre- and postoperation, and postoperative recurrence. Results: The improvement of forced expiratory volume at 1 second in the group using $CO_2$ gas and the group not using $CO_2$ gas was $22.46{\pm}11.27$ and $21.08{\pm}5.39$ (p=0.84). The improvement of forced vital capacity 3 months after surgery was $16.74{\pm}10.18$ (with $CO_2$) and $15.6{\pm}0.89$ (without $CO_2$) (p=0.03). During follow-up ($17{\pm}17$ months), there was no dehiscence in plication site and relapse. No complications or hospital mortalities occurred. Conclusion: Thoracoscopic plication under single lung ventilation using $CO_2$ insufflation could be an effective, safe option to flatten the diaphragm.
