The effects of diagnostic hysteroscopy on the reproductive outcomes of infertile women without intrauterine pathologies: a systematic review and meta-analysis

  • Purpose: Hysteroscopy can be used both to diagnose and to treat intrauterine pathologies. It is well known that hysteroscopy helps to improve reproductive outcomes by treating intrauterine pathologies. However, it is uncertain whether hysteroscopy is helpful in the absence of intrauterine pathologies. This study aimed to confirm whether hysteroscopy improves the reproductive outcomes of infertile women without intrauterine pathologies. Methods: We conducted a systematic review of 11 studies retrieved from Ovid-MEDLINE, Ovid-Embase, and the Cochrane Library. Two independent investigators extracted the data and used risk-of-bias tools (RoB 2.0 and ROBINS-I) to assess their quality. Results: Diagnostic hysteroscopy prior to in vitro fertilization (IVF)/intracytoplasmic sperm injection (ICSI) was associated with a higher clinical pregnancy rate (CPR) and live birth rate (LBR) than non-hysteroscopy in patients with recurrent implantation failure (RIF) (odds ratio, 1.79 and 1.46; 95% confidence interval, 1.40-2.30 and 1.08-1.97 for CPR and LBR, respectively) while hysteroscopy prior to first IVF was ineffective. The overall meta-analysis of LBR showed statistically significant findings for RIF, but a subgroup analysis showed effects only in prospective cohorts (odds ratio, 1.40 and 1.47; 95% confidence interval, 0.62-3.16 and 1.04-2.07 for randomized controlled trials and prospective cohorts, respectively). Therefore, the LBR should be interpreted carefully and further research is needed. Conclusion: Although further research is warranted, hysteroscopy may be considered as a diagnostic and treatment option for infertile women who have experienced RIF regardless of intrauterine pathologies. This finding enables nurses to educate and support infertile women with RIF prior to IVF/ICSI.

  • Korea Astronomy and Space Science Institute (KASI) has been developing a next-generation coronagraph (NGC) in cooperation with NASA to measure the coronal electron density, temperature, and speed using four different filters around 400 nm. To demonstrate technology for the measurement through the 2017 total solar eclipse across the USA, KASI organized an expedition team to demonstrate the coronagraph measurement scheme and the instrumental technology. The observation site was in Jackson Hole, Wyoming, USA. We built an eclipse observation system, so-called Diagnostic Coronal Experiment (DICE), which is composed of two identical telescopes to improve a signal to noise ratio. The observation was conducted with 4 wavelengths and 3 linear polarization directions according to the planned schedule in a limited total eclipse time of about 140 seconds.Polarization information of corona from the data was successfully obtained but we failed to get the coronal electron temperature and speed information due to a low signal-to-noise ratio of the optical system. In this study, we report the development of DICE and observation results. TSE observation and analysis by using our own developed instrument gave an important lesson that a coronagraph should be carefully designed to archive the scientific purpose. This experience through TSE observation will be very useful for a success of NASA-KASI joint missions called the Balloon-borne Investigation of the Temperature and Speed of Electrons in the Corona (BITSE) and COronal Diagnostic EXperiment (CODEX).

Diagnostic Accuracy of 18F-FDG-PET in Patients with Testicular Cancer: a Meta-analysis

  • Objective: Fluorine-18-fluorodeoxyglucose positron emission tomography (18F-FDG-PET) is a new technique for identifying different malignant tumors using different uptake values between tumor cells and normal tissues. Here we assessed the diagnostic accuracy of 18F-FDG-PET in patients with testicular cancer by pooling data of existing trials in a meta-analysis. Methods: PubMed/MEDLINE, Embase and Cochrane Central Trials databases were searched and studies published in English relating to the diagnostic value of FDG-PET for testicular cancer were collected. The summary receiver operating characteristic (SROC) curve was used to examine the FDG-PET accuracy. Results: A total of 16 studies which included 957 examinations in 807 patients (median age, 31.1 years) were analyzed. A meta-analysis was performed to combine the sensitivity and specificity and their 95% confidence intervals (CIs), from diagnostic odds ratio (DOR), positive likelihood ratios (PLR), negative likelihood ratio (NLR). SROC were derived to demonstrate the diagnostic accuracy of FDG-PET for testicular cancer. The pooled sensitivity and specificity were 0.75 (95% confidence interval (CI), 0.70-0.80) and 0.87 (95% CI, 0.84-0.89), respectively. The pooled DOR was 35.6 (95% CI, 12.9-98.3). The area under the curve (AUC) was 0.88. The pooled PLR and pooled NLR were 7.80 (95% CI, 3.73-16.3) and 0.31 (95% CI, 0.23-0.43), respectively. Conclusion: In patients with testicular cancer, 18F-FDG-PET demonstrated a high SROC area, and could be a potentially useful tool if combined with other imaging methods such as MRI and CT. Nevertheless, the literature focusing on the use of 18F-FDG-PET in this setting still remains limited.

The Study on Concentration of Serum Copper and Zinc in Stomach Cancer Patients (위암 환자의 혈청내 구리 및 아연 농도에 관한 연구)

  • In order to investigate whether serum Cu, Zn and Cu/Zn ratio are the useful indicater for the diagnosis of cancer. serum Cu and Zn levels were determined from 35 normal subjects and 33 stomach cancer patients by Atomic absorption spectrophotometer. The following results are obtained : Serum copper levels of stomach cancer patients were significantly increased than the those of the normal controls. whereas serum Zn levels significantly decreased than those of the normal contros(P<0.01) Therefore the copper to zinc ratio revealed a significant in the serum Cu and Zn levels. The serum protein and albumin levels were significantly lower in cancer patients(P<0.01) And the correlation analysis indicated that there was no significant correlation between serum Cu/Zn ratio and protein or albumin. These data suggest that the serum Cu. Zn and Cu/Zn ratio may be used as a diagnostic test in stomach cancer patients.

  • This report was conducted to quantify the pulse/respiration ratio and set up the normal range of wan-maeck(緩脈). In order to objectify the pulse diagnosis and use as basic clinical index of Cold-Hot diagnosis, we developed the hardware and software for detection and interpretation of pulse/respiration ratio, pulse/expiration ratio, pulse/respiration ratio, inspiration time, expiration time, respiration frequency, respiration time, duration of one pulse and pulse and pulse rate per minute, The results were as follows; pulse/respiration ratio is $4.30{\pm}1.03$ times, pulse/respiration ratio is $1.60{\pm}0.32$ times, pulse/respiration ratio is $2.37{\pm}0.75$ times, inspiration time is $1.35{\pm}0.20$ sec, expiration time is $1.89{\pm}0.39$ sec, respiration frequency is $17.16{\pm}3.49$ times/min, total respiration time is $3.63{\pm}0.71$ sec, duration of a pulse is $0.86{\pm}0.15$ sec, pulse rate is $71.51{\pm}12.30$ times/min.

Serum IL-33 as a Diagnostic and Prognostic Marker in Non-small Cell Lung Cancer

  • Background: Interleukin-33 (IL-33) has recently been implicated in tumor immunity. The aim of this study was to explore the clinical role of serum IL-33 in patients with non-small-cell lung cancer (NSCLC). Methods: Sera collected from 250 healthy volunteers (HV), 256 patients with benign lung diseases (BLD) and 262 NSCLC cases were subjected to IL-33 ELISA and relationships between serum IL-33 and clinical characteristics were evaluated. Results: Circulating IL-33 levels were higher in the NSCLC group in comparison with the HV and BLD groups (p<0.001). Using a cut-off level 68 pg/ml (95% specificity in the HV group), IL-33 showed a good diagnostic performance for NSCLC. Multivariate survival analysis indicated that serum IL-33 was an independent prognostic factor in the entire NSCLC group [hazards ratio (HR) = 0.64 for low versus high IL-33 levels, 95% confidence interval (CI) 0.50-0.82; p<0.001] and in 165 selected patients with locally advanced or metastatic disease receiving chemoradiotherapy or chemotherapy (HR 0.70, 95% CI 0.52-0.94; p=0.013). Conclusions: IL-33 is a promising potential diagnostic and prognostic marker in NSCLC, independent of the therapeutic intervention.

  • Objectives : Atopic dermatitis(AD) assume an remarkable clinical aspect and it s diagnosis almost depends on clinical symptoms. Therefore, we aimed to study the clinical diagnostic standard of AD for more accurate treatment. We repert as follows; Methods : For 6 months from March to August in 2000 we selected fifty outpatients who were prognosis of AD in the department of dermatology, Oriental medical hospitol, Dong-eui University. Results and Conclusions : 1. We classified of the grade, the condition of AD patient was slight and severe, by the sum of total by the clinical index of AD (diagnostic features). 2. By consulting previous oriental medical theories, we divided symptom-complex of AD into two type ; one was damp-heat type and the other was deficiency of blood- wind-dryness type. 3. Male to female ratio was 17 : 33 and the third stage, more than half of the patients were adolescents. 4. According to the results of symptom-complex of AD patients, on the first examination damp-heat type was more than deficiency of blood-wind-dryness type and in progressing treatment, the condition has been change to deficiency of blood-wind-dryness type. 5. In the lesions of AD, arm and knee were most serious skin lesions and in symptoms of AD, pruritus was most complained, and in progressing treatment, erosion and erythema were greatly improved. 6. When we measured the levels of serum Total IgE, that of thirty eight patientswere higher than that of normal, but elevation of serum IgE levels was not correlated with the severity of AD.

  • The author investigated the effect of some variables such as age, sex and the experience of past vaccination on the validity of PHA. The changing pattern of the validity with the change of PHA diagnostic criteria, and the relationship between PHA test result and RIA Ratio Unit were also studied. The results obtained were as follow; 1) No statistically significant difference was found in sensitivity, specificity and negative predictability by sex, but positive predictability was significantly higher in male than that in female. 2) Positive predictability was shown to become higher with the increase of age and nagative predictability was found to be significally different among age groups, but no statistically significant difference was found in sensitivity and specificity by age group. 3) Significantly low specificity and high positive predictability were found in past vaccined group, but no statistically significant difference was found in sensitivity and negative predictability between past vaccined group and non-vaccined group. 4) False negative cases by PHA were found to be the weak positive reactors by RIA and false positive rate of PHA was as high as 46.3 per cent. 5) Sensitivity and specificity of PHA at the diagnostic criteria of HBsAb titer 1:2 were 98.4% and 53.8% respectively, but after increasing the HBsAb titer to 1:64 as the diagnostic criteria the results were 60.0% and 95.6% respectively.

A Comparative Study on Output of Four Type Diagnostic X-ray Equipments (정류방식에 따른 진단용 X-선 장치의 출력비교)

  • There are 4 types of equipment in diagnostic radiography. These are single phase, three phase, inverter type and condenser type X-ray generators. It is very confusing to make an adequate exposure factor and to know the usage of different type of X-ray generators. In this experiment, I explored a comparative study of outputs in 4 different type of X-ray units. I expect that this experiment could be helpful for manufacturer to make both the X-ray equipment better, In terms of Ideal exposure factors, thereby reducing the patient dose. Experimental results are as follow : 1) X-ray output The ratio of X-ray output of single, three phase and inverter type of X-ray generator was 1 : 1.6 : 2 without absorber and 1 : 2 : 2.6 with 20 mm aluminium absorber. 2) Beam quality The X-ray beam quality of single phase generator was proved to be softer than three phase and inverter type of generators by 0.4 mmAL and 0.55 mmAl HVL respectively. 3) Reproducibility Linearity of X-ray output Retroducibility of X-ray output met the regulation below CV 0.05 and linearity also met the regulation below 0.1 in 4 types of diagnostic X-ray generators. 4) The comparison of incident dose Three phase X-ray generator was 20% higher than two other X-ray generators in radiation dose to make same film density.

Contrast-Enhanced Magnetic Resonance Angiography: Dose the Test Dose Bolus Represent the Main Dose Bolus Accurately?

  • Objective: To determine whether the time-intensity curves acquired by test and main dose contrast injections for MR angiography are similar. Materials and Methods: In 11 patients, repeated contrast-enhanced 2D-turbo-FLASH scans with 1-sec interval were obtained. Both test and main dose timeintensity curves were acquired from the abdominal aorta, and the parameters of time-intensity curves for the test and main boluses were compared. The parameters used were arterial and venous enhancement times, arterial peak enhancement time, arteriovenous circulation time, enhancement duration and enhancement expansion ratio. Results: Between the main and test boluses, arterial and venous enhancement times and arteriovenous circulation time showed statistically significant correlation (p < 0.01), with correlation coefficients of 0.95, 0.92 and 0.98 respectively. Although the enhancement duration was definitely greater than infusion time, reasonable measurement of the end enhancement point in the main bolus was impossible. Conclusion: Only arterial and venous enhancement times and arteriovenous circulation time of the main bolus could be predicted from the test-bolus results. The use of these reliable parameters would lead to improvements in the scan timing method for MR angiography.
