Diagnostic Accuracy of Percutaneous Transthoracic Needle Lung Biopsies: A Multicenter Study

  • Objective: To measure the diagnostic accuracy of percutaneous transthoracic needle lung biopsies (PTNBs) on the basis of the intention-to-diagnose principle and identify risk factors for diagnostic failure of PTNBs in a multi-institutional setting. Materials and Methods: A total of 9384 initial PTNBs performed in 9239 patients (mean patient age, 65 years [range, 20-99 years]) from January 2010 to December 2014 were included. The accuracy, sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of PTNBs for diagnosis of malignancy were measured. The proportion of diagnostic failures was measured, and their risk factors were identified. Results: The overall accuracy, sensitivity, specificity, PPV, and NPV were 91.1% (95% confidence interval [CI], 90.6-91.7%), 92.5% (95% CI, 91.9-93.1%), 86.5% (95% CI, 85.0-87.9%), 99.2% (95% CI, 99.0-99.4%), and 84.3% (95% CI, 82.7-85.8%), respectively. The proportion of diagnostic failures was 8.9% (831 of 9384; 95% CI, 8.3-9.4%). The independent risk factors for diagnostic failures were lesions ≤ 1 cm in size (adjusted odds ratio [AOR], 1.86; 95% CI, 1.23-2.81), lesion size 1.1-2 cm (1.75; 1.45-2.11), subsolid lesions (1.81; 1.32-2.49), use of fine needle aspiration only (2.43; 1.80-3.28), final diagnosis of benign lesions (2.18; 1.84-2.58), and final diagnosis of lymphomas (10.66; 6.21-18.30). Use of cone-beam CT (AOR, 0.31; 95% CI, 0.13-0.75) and conventional CT-guidance (0.55; 0.32-0.94) reduced diagnostic failures. Conclusion: The accuracy of PTNB for diagnosis of malignancy was fairly high in our large-scale multi-institutional cohort. The identified risk factors for diagnostic failure may help reduce diagnostic failure and interpret the biopsy results.

Optimum Image Compression Rate Maintaining Diagnostic Image Quality of Digital Intraoral Radiographs

  • Purpose: The aims of the present study are to determine the optimum compression rate in terms of file size reduction and diagnostic quality of the images after compression and evaluate the transmission speed of original or each compressed image. Materials and Methods: The material consisted of 24 extracted human premolars and molars. The occlusal surfaces and proximal surfaces of the teeth had a clinical disease spectrum that ranged from sound to varying degrees of fissure discoloration and cavitation. The images from Digora system were exported in TIFF and the images from conventional intraoral film were scanned and digitalized in TIFF by Nikon SF-200 scanner (Nikon, Japan). And six compression factors were chosen and applied on the basis of the results from a pilot study. The total number of images to be assessed were 336. Three radiologists assessed the occlusal and proximal surfaces of the teeth with 5-rank scale. Finally diagnosed as either sound or carious lesion by one expert oral pathologist. And sensitivity, specificity and k value for diagnostic agreement was calculated. Also the area (Az) values under the ROC curve were calculated and paired t-test and oneway ANOVA test was performed. Thereafter, transmission time of the image files of the each compression level was compared with that of the original image files. Results: No significant difference was found between original and the corresponding images up to 7% (1 : 14) compression ratio for both the occlusal and proximal caries (p<0.05). JPEG3 (1 : 14) image files are transmitted fast more than 10 times, maintained diagnostic information in image, compared with original image files. Conclusion: 1 : 14 compressed image file may be used instead of the original image and reduce storage needs and transmission time.

Clinicopathological Significance and Diagnostic Accuracy of c-MET Expression by Immunohistochemistry in Gastric Cancer: A Meta-Analysis

  • Purpose: The aim of the present study was to elucidate the clinicopathological significance and diagnostic accuracy of immunohistochemistry (IHC) for determining the mesenchymal epidermal transition (c-MET) expression in patients with gastric cancer (GC). Materials and Methods: The present meta-analysis investigated the correlation between c-MET expression as determined by IHC and the clinicopathological parameters in 8,395 GC patients from 37 studies that satisfied the eligibility criteria. In addition, a concordance analysis was performed between c-MET expression as determined by IHC and c-MET amplification, and the diagnostic test accuracy was reviewed. Results: The estimated rate of c-MET overexpression was 0.403 (95% confidence interval [CI], 0.327~0.484) and it was significantly correlated with male patients, poor differentiation, lymph node metastasis, higher TNM stage, and human epidermal growth factor receptor 2 (HER2) positivity in IHC analysis. There was a significant correlation between c-MET expression and worse overall survival rate (hazard ratio, 1.588; 95% CI, 1.266~1.992). The concordance rates between c-MET expression and c-MET amplification were 0.967 (95% CI, 0.916~0.987) and 0.270 (95% CI, 0.173~0.395) for cases with non-overexpressed and overexpressed c-MET, respectively. In the diagnostic test accuracy review, the pooled sensitivity and specificity were 0.56 (95% CI, 0.50~0.63) and 0.79 (95% CI, 0.77~0.81), respectively. Conclusions: The c-MET overexpression as determined by IHC was significantly correlated with aggressive tumor behavior and positive IHC status for HER2 in patients with GC. In addition, the c-MET expression status could be useful in the screening of c-MET amplification in patients with GC.

Computed tomographic features of gastric and esophageal content in dogs undergoing CT myelography and factors influencing the presence of esophageal fluid

  • Background: Gastroesophageal reflux (GER) has been reported to be a common finding in dogs under general anesthesia. Objectives: The aim of this retrospective study was to assess the esophageal and gastric contents in a population of dogs undergoing computed tomographic myelography (myeloCT) examination and to evaluate the factors influencing the presence of esophageal fluid (gastric content, duration of anesthesia, body position, and intrinsic factors). Methods: Esophageal and gastric contents of 83 non-brachycephalic dogs were retrospectively assessed based on plain and myelo-CT scans. Age, weight, breed, sex, and the time between the 2 computed tomography [CT] scans were included. Results: Esophageal fluid was present in 19% (16/83) of the animals, and 14% (12/83) and 46% (37/83), respectively, had fluid or food material in their stomachs. The frequency of observing esophageal fluid on myelo-CT scans was significantly increased compared with plain CT scans (p = 0.006). The presence of gastric fluid was significantly associated with an increased frequency of observing esophageal fluid compared to other gastric contents (p = 0.049; odds ratio, 3.1). The presence of esophageal fluid was not correlated with alimentary gastric contents (p = 0.17). Increased body weight and duration of anesthesia were significantly associated with an increased frequency of observing esophageal fluid (p = 0.022, p = 0.021). Conclusions: Unlike alimentary gastric contents, fluid gastric contents were correlated with the presence of esophageal fluid upon myelo-CT. The observation of fluid in the esophagus may be consistent with GER. This study provides data additional to pH monitoring studies of GER and may support previous studies recommending shorter pre-anesthetic fasting periods in dogs.

Risk factors for canine magnesium ammonium phosphate urolithiasis associated with bacterial infection

  • Background: With limited information available, the association among urinary tract infections, urease-producing bacteria and the presence of magnesium ammonium phosphate (MAP) urolithiasis in canines in Thailand requires more study. Objectives: This study aimed to investigate the association between demographic characteristics of canines and the presence of MAP urolithiasis in canines, and to evaluate antimicrobial susceptibility patterns of bacteria isolated from canine uroliths. Methods: A total of 56 canines admitted for treatment with surgical removal of uroliths were recruited. Demographic characteristics and clinical chemistry data were recorded. Bacteria isolated from the removed uroliths were identified. Chemical compositions of the uroliths were analyzed by Fourier transform infrared spectrometer. Potential risk factors were determined with univariable and multivariable logistic regression analyses. Results: Of 56 canine urolithiasis, bacteria were isolated from uroliths of 38 canines (27 MAP and 11 non-MAP) but not from uroliths of 18 canines (5 MAP and 13 non-MAP). The most common bacteria found in nidus of MAP uroliths was Staphylococcus pseudintermedius (approximately 51%). An antimicrobial resistance was frequently found in Staphylococci isolates (42.86%). Multivariate logistic regression analysis showed that the predictors of MAP urolith in canine urolithiasis were being female (p = 0.044; adjusted odds ratio [OR], 10.22; 95% confidence interval [CI], 1.06-98.24) and the positive urolith culture (p = 0.012; adjusted OR, 8.60; 95% CI, 1.60-46.30). Conclusions: Our results indicate that S. pseudintermedius (a urease-producing bacterium) is the major causative bacteria of MAP uroliths. A positive urolith culture and being female are risk factors of MAP urolithiasis in canines.

  • 모음낱자들을 한 쌍씩 제시하되,그 크기를 변화시켜 크기 비의 차이가 동시적 동일상이대응과제(simultaneous same-different matching task)에서 의 판단반응시간에 어떻게 영향을 미치고 있는지를 검토하였다.자극 낱자들의 크기,자획의 연속성,제시 위치등을 조작하영 크기 비의 효과 배후의 표상과 알고리즘에 대한 유비 부호변형설,위치설,진단적 정보활용성,판단 기준설의 예언 등을 검토하였다.판단해야할 모음낱자들 간의 크기의 차이는 동일(same)반응에만 체계적으로 영향을 미쳤으며,낱자를 구성하는 수 직선과 수평선 중 수평선의 길이의 변화가 또한 동일반응 시간에 큰영향을 미치고 있었다.본 연구의 결과들을 수렴해서 진단적 정보 활용설을 지지하는 반면,다른 가설들이 문제있음을 보여 주었다.

정상인 및 각종 갑상선 질환 환자의 혈청 Thyroxine (Determination of Serum Thyroxine Levels in Normal Korean Subjects and Various Thyroid Diseases)

  • The serum thyroxine levels were measured by method of Tetrasorb Kit in 69 subjects including 13 subjects in euthyroid state, 31 with hyperthyroidism, 5 with hypothyroidism, 13 with nontoxic diffuse goiter, and 7 with nontoxic nodular goiter. Three parameters of the thyroid function test including thyroxine ($T_4$) levels, $^{131}I$ uptake (24 hrs) values and $PB^{131}I$ conversion ratio were correlated with clinical manifestations and courses of the disease. 1. The serum $T_4$ levels in the normal subjects were in range of $6.0{\mu}g/dl\;to\;14.4{\mu}g/dl$. (The mean $9.4{\mu}g/dl$). 2. The diagnostic compatibility of the serum $T_4$ was 93.5% in hyperthyroidism, 100% both in hypothyroidism and in nontoxic diffuse goiter, 86% in nontoxic nodular goiter, or 95.8% in the entire series. (Table 1). 3. The diagnostic compatibility of $^{131}I$ uptake (24 hrs) values and $PB^{131}I$ conversion ratio were less than the serum $T_4$ levels as summarized in Tables 2 & 3. The serum $T_4$ determination by Tetrasorb Kit is a simple, accurate and very useful test of the thyroid function.

Tongue Indices with Upper Respiratory Tract Infection for Application in Diagnostic Systems Without Face-to-face Visits

  • Although upper respiratory tract infections (URTIs) are common diseases, there have been no studies of their relationship with the indices obtained from tongue image analyses. The purpose of this study was to identify a tongue index through an analysis of tongue images (TIs) showing significant changes before and after treatment in patients with URTIs. A computerized tongue image acquisition system was developed to acquire TIs from subjects in the same environment. An image was taken from each of 39 URTI patients and 39 healthy controls. For the patients, images were acquired before and after treatment to identify changes. The tongue area was classified into a tongue body and a tongue coating, and the coating ratio between the two areas, the average value of the colour of each area, and teeth marks were calculated. No significant difference was observed in age or sex between the URTI patients and control participants. Heart rates were slightly different. The analysis of TIs showed that the luminance of the tongue coating and the coating area ratio were decreased, while the reddish value of the tongue body at the centre area increased as the treatment progressed. Tongue coating and body in URTIs had different colour and shape from those in the normal. It is expected that this result will contribute not only to the objectification of traditional Chinese medicine but also to diagnostic methods that do not involve face-to-face physician visit during the pandemic.

TOWARD A NEXT GENERATION SOLAR CORONAGRAPH: DIAGNOSTIC CORONAGRAPH EXPERIMENT

  • The Korea Astronomy and Space Science Institute (KASI) has been developing a next-generation coronagraph (NGC) in cooperation with NASA to measure the coronal electron density, temperature, and speed simultaneously, using four different optical filters around 400 nm. KASI organized an expedition to demonstrate the coronagraph measurement scheme and the instrumental technology during the 2017 total solar eclipse (TSE) across the USA. The observation site was in Jackson Hole, Wyoming, USA. We built an eclipse observation system, the Diagnostic Coronal Experiment (DICE), composed of two identical telescopes to improve the signal-to-noise ratio. The observation was conducted at four wavelengths and three linear polarization directions in the limited total eclipse time of about 140 seconds. We successfully obtained polarization data for the corona but we were not able to obtain information on the coronal electron temperature and speed due to the low signal-to-noise ratio of the optical system and strong emission from prominences located at the western limb. In this study, we report the development of DICE and the observation results from the eclipse expedition. TSE observation and analysis with our self-developed instrument showed that a coronagraph needs to be designed carefully to achieve its scientific purpose. We gained valuable experience for future follow-up NASA-KASI joint missions: the Balloon-borne Investigation of the Temperature and Speed of Electrons in the Corona (BITSE) and the COronal Diagnostic EXperiment (CODEX).

사상변증내용(四象辯證內容) 설문조사지(設問調査紙)(I)의 타당화(妥當化) 연구(硏究) (The Study on The Validity of "Questionnaire of Sasang Constitution Classification(I)")

  • This study was conducted for the purpose of finding out an objective classification method for Sasang Constitutional medicine, which divides people into 4 groups of constitution and presents comprehensively physiology, pathology, diagnosis, therapy and recuperation regarding each constitution. Questionnaire of Sasang Constitution Classification(I) was administered to 328 inpatients at Kyung Hee Oriental Medicine Hospital. Data was collected during 10 months from June 1994 to Mar. 11, 1995. For the purposes of this study, the collected data was analyzed by crosstabs, variation analysis, and discrimination analysis. The analyzing program was SPSS PC+V4.0. For the purposes of this study, the collected data was analyzed by crosstabs, cariation analysis, and discrimination analysis. The analyzing program was SPSS PC+V4.0. The results were as follows : 1. There was significant differences of each group scales through variation analysis. The questions of each group had Sasang constitutional diagnostic discrimination abilities 2. The diagnostic discrimination abilities(Hit-ratio=56.10%) of the Questionnaire for Sasang Constitution Classification(I) were found to have over 20% improvement than the propotional chance criteria(33.3%), Especially, Hit-ratio for $So{\breve{u}}m$-In(63%) was higher than that of SoYang-In(55%) and $Ta{\breve{e}}um$-In(56.3%). 3. Through discrimination analysis on good questions of each group, the diagnostic discrimination abilities of the Questionnaire for Sasang Constitution Classification(I) was 57.93%. 4. This would be on the ground that the Questionnaire for Sasang Constitution Classification(I) could be used as a tool for Sasang constitution classification.
