패브리병(Fabry) 진단을 위한 혈장 중 Globotriaosylceramide (Gb3)의 탠덤매스 분석법 개발과 임상 응용 (Determination of plasma C16-C24 globotriaosylceramide (Gb3) isoforms by tandem mass spectrometry for diagnosis of Fabry disease)

  • 목 적 : 패브리병은 X-linked 지질 축적 질환으로 ${\alpha}$-galactosidase A (${\alpha}$-Gal A)의 결손으로 인해 스핑고당지질인 Gb3의 세포내 축적을 일으키는 병이다. 혈장 중 Gb3 측정은 패브리병 환자의 효소대체요법 후의 모니터링이나 진단에 임상적 의의가 크므로 ESI-MS/MS를 이용한 시료 전처리를 위한 노동력이 덜 들면서 간단, 신속, 고감도로 정량할 수 있는 혈장 중 Gb3분석법을 개발하고자 하였다. 방 법 : 혈장을 디옥산으로 50배 희석하여 vortex-mix 및 원심분리를 거쳐 Gb3의 추출 및 분리를 수행한다. 이 때 내부표준액인 C17:0 Gb3를 혈장에 처음부터 첨가한다. 희석과 원심분리된 혈장은 가드컬럼을 통하여 ESI-MS/MS의 다중성분 모니터링 모드에서 분석하여 내부표준액에 대한 8종 Gb3 isoform의 피크면적비를 이용하여 정량한다. 결 과 : 혈장의 바탕성분 하에서 8종의 Gb3 isoform이 완전히 잘 분리됨을 확인할 수 있었다. 혈장 중의 8종의 Gb3 isoform 중 50% 이상 차지하는 종류는 C16:0 Gb3 임이 확인되었다. Gb3 isoform이 직선성을 이루는 농도 범위는 0.001-1.0 ${\mu}g$/mL이었다. 검출한계(S/N=3)는 C16:0 Gb3의 경우 0.001 ${\mu}g$/mL 이었고 정량한계는 0.01 ${\mu}g$/mL 이었으며 회수율의 일내재현성(정확도 87-108%와 정밀도 7% 이하)과 일간재현성(정확도 87-110%와 정밀도 13% 이하)은 매우 양호 하였다. 결 론 : 본 연구에서 개발된 Gb3 분석법은 신속, 정확, 간편하게 패브리병의 1차 스크리닝이나 효소대체요법 전후의 모니터링 및 진단에 유용하게 적용될 수 있을 것이다.

6세 이후 열경련 환자의 비열성발작으로 진행되는 위험 인자 (The Prognostic Factors Affecting the Occurrence of Subsequent Unprovoked Seizure in Patients Who Present with Febrile Seizure after 6 Years of Age)

  • 목적: 열경련 환자의 경우 대부분 좋은 예후를 보이며 비열성 발작으로 가는 비율은 2-7%로 알려져 있다. 열경련이 6세 이후에도 지속되거나 새로 발생하는 경우 비열경련으로 가는 비율이 일반 열경련 환자에 비해 높은 것으로 알려져 있지만, 위험인자에 대해서는 알려진 바가 없다. 6세 이후 열경련에 대한 연구는 소수에서만 진행이 되었으며, 6세 이후 열경련 환자에 대한 비열성 발작의 위험인자를 알아보고자 하였다. 방법: 2011년 1월부터 2017년 6월까지 열경련으로 소아청소년과 신경분과 외래를 방문한 환자를 대상으로 하였으며, 그중 6세 이후에도 열경련이 지속되거나 새로이 발생한 환자들을 연구 대상으로 하였다. 정보 수집은 차트 리뷰 및 전화 조사를 통하여 시행하였다. 76명의 환자가 최종 연구 대상으로 선정되었으며, 비열성 발작으로 가는 위험 인자를 확인하기 위하여 로지스틱 회귀분석을 시행하였다. 결과: 열경련으로 방문한 896명의 환자 중, 83명에서 6세 이후에 열경련이 발생하였다. 이 중 3명은 뇌파 이상 소견 및 지속되는 열경련으로 항경련제를 복용하였고, 4명은 6세 이전에 비열성 발작이 최소 1회 이상 발생하여 연구 대상에서 제외하였다. 최종 76명이 연구 대상으로 선정되었다. 열경련의 시작 나이를 6세 기준으로 나누었을 때, 6세 전에 열경련이 시작된 군에서 열경련 가족력 및 복합열경련의 빈도가 높게 보고 되었으나, 이외 뇌파 검사 및 다른 주요 임상 변수들은 통계적인 차이를 보이지 않았다. 21%(16/76)에서 추후 비열성 발작이 발생하였으며, 비열성 발작으로의 진행에 영향을 주는 독립 인자로 뇌파 이상 소견이 확인되었다. 결론: 6세 이후 열경련 군에서 열경련의 시작 나이는 특별한 임상적인 의미를 갖지 않았다. 뇌파 이상 소견이 있는 경우 비열성 발작으로 가는 비율이 통계적으로 의미 있게 높게 나타났다. 6세 이후 열경련 환자에서 예후를 예측하는데, 뇌파 시행은 의미 있는 검사 방법이 될 수 있겠다.

한글판 뇌진탕후증후군 척도의 개발 (Development of the Korean version of Postconcussional Syndrome Questionnaire)

  • 연구목적 본 연구는 Lees-Haley가 1992년 개발한 Postconcussional Syndrome Questionnaire(PCSQ)의 한글판(K-PCSQ)을 개발하고 신뢰도와 타당도를 평가하여 임상적 유용성을 밝히고자 시행되었다. 방 법 2009년 4월 1일부터 2011년 12월 31일까지 고려대학교 안산병원 외래에 내원한 외상성 뇌 손상 환자들 중 International Classification of Disease-10(ICD-10)의 뇌진탕후증후군, 기질성 정신질환(기질성 기분장애, 기질성 불안장애, 기질성 인격장애, 기질성 감정이변성장애)의 진단기준을 만족하는 환자들을 대상으로 포함하였다. 환자들을 대상으로 자가 보고형 평가 척도인 K-PCSQ, State and Trait Anxiety inventory(STAI-I. II), Center for Epidemiologic Studies Depression Scale(CESD)를 시행하였다. 수집된 자료로 PCSQ의 신뢰도와 타당도를 평가하였으며 요인분석을 시행하였다. 결 과 K-PCSQ의 Cronbach's alpha 값은 0.956, 검사-재검사 신뢰도는 0.836이었으며 STAI-I. II, CESD와 유의한 상관관계를 보였다. 요인 분석 결과 K-PCSQ는 4개의 요인구조를 보였으며, 제 1 요인은 '기분 및 인지 증상' 요인, 제 2 요인은 '신체 증상' 요인, 제 3 요인은 '드물게 나타나는 증상' 요인, 제 4 요인은 '과장 또는 부주의한 응답' 요인으로 나타났다. 뇌진탕후 증후군 환자와 기질성 정신질환 환자 두 군에서 K-PCSQ의 총점과 4가지 하위 요인의 점수를 비교하였을 때 두 군 간에 통계적으로 유의한 차이를 보이지 않았다. '과장 또는 부주의한 응답' 항목에 답변한 대상자들은 그렇지 않은 대상자에 비해 K-PCSQ의 총점과 4가지 하위 요인의 점수가 유의하게 높았다. 결 론 본 연구는 K-PCSQ가 두부외상으로 인한 정신의학적 증상을 평가하는데 있어서 유용한 도구임을 확인하였으며, 추후 보다 많은 환자를 대상으로 하는 연구를 통해 임상적인 유용성의 평가 이루어져야 할 것으로 생각된다.

급성 복증 환자에서 현장검사 크레아티닌을 이용한 의사결정 시간의 단축 (The influence of the decision making time by using point-of-care creatinine in patients with acute abdomen)

  • Objective: Radio-contrast abdomino-pelvic computed tomography (APCT) is considered the gold standard diagnostic tool for an acute abdomen in the emergency department. On the other hand, APCT has a risk of contrast-induced nephropathy. Emergency physicians evaluate the creatinine (Cr) level prior to taking a APCT for the above reason but it takes time to evaluation the serum Cr level. This study hypothesized that Cr measured by a point-of-care test (POCT) can shorten the time to making clinically important decisions for patients with an acute abdomen. Methods: This prospective randomized study was conducted between March 2017 and October 2017. The subjects were divided into two groups (Cr measured by laboratory vs. Cr measured by POCT). To analyze the clinical acceptability for creatinine, agreement was demonstrated graphically by Bland-Altman plots. This study compared the time to make a clinically important decision by physicians and the length of stay at the emergency department in both groups. Results: A total of 76 patients were eligible for the study, 38 patients were assigned to each group. There was no statistically significant difference in the time to the first medical examination (P=0.222) and emergency department stay time (P=0.802). On the other hand, the time to recognition of the Cr level (P<0.001), time to performing APCT (P<0.001), time to decision making (P<0.001), and time to initiation of treatment (P<0.001) were shortened significantly in the point-of-care creatinine group. Conclusion: In this study, the POCT for creatinine can allow rapid decision making by shortening the time to performing the radio-contrast APCT than the laboratory for patients with an acute abdomen.

Clinical Significance of the Bacille Calmette-Guérin Site Reaction in Kawasaki Disease Patients Aged Less than 18 Months

  • 목적: 가와사키병이 있는 소아에서 진단 및 결과 예측에 있어서 Bacille Calmette-$Gu{\acute{e}}rin$ (BCG) 접종 부위 반응의 임상적 중요성을 알아보고자 하였다. 방법: 2006년 1월부터 2017년 2월까지 가와사키병으로 서울아산병원에 입원한 1,058명의 환자를 대상으로 각 연령대에서 BCG 부위 반응의 발생 빈도를 조사하였다. 이 중 생후 18개월 미만인 416명의 환자를 BCG 부위 반응과 실험실 및 임상소견들과의 연관성을 분석하기 위한 대상자들로 선정하였다. 분석은 완전형과 불완전형 발현 두 그룹으로 구분하여 수행되었다. 결과: 1,058명의 환자들을 관찰한 결과, BCG 부위 반응의 발생률은 생후 6-12개월(83%) 연령의 환자에게 가장 많았고, 12개월 후 연령이 증가함에 따라 감소했다(P<0.001). BCG 부위 반응은 생후 18개월 미만 환자의 70%이상에서 발생하였고, 경부 림프절 종대보다 빈발했다. 로지스틱 회귀분석결과, 결막염(P=0.781), 입술/구강의 변화 (P=0.963), 발진 (P=0.510), 경부 림프절 종대 (P=0.363), 사지 말단의 변화 (P=0.283) 및 관상 동맥류 (P=0.776)는 BCG 부위 반응과 관련이 없었다. 결론: BCG 부위 반응은 BCG 예방 접종을 받은 18개월 미만 소아에서 가와사키병의 주요 임상 소견과는 독립적으로 유용한 진단 도구가 될 수 있다는 것을 확인하였다. 관상동맥 합병증 결과는 완전 가와사키병 및 불완전 가와사키병 모두에서 BCG 부위 반응이 있거나 없는 그룹간에 차이가 없었다.

초음파 유도하 침 생검을 이용한 연부조직 종양의 진단에 있어 양전자방출 컴퓨터 단층촬영술의 유용성 (Availability of Positron Emission Tomography-Computed Tomography for the Diagnosis of the Soft Tissue Tumor through Ultrasound-Guided Biopsy)

  • 목적: 연부조직 종양의 진단을 위해서는 생검이 필요하지만, 육종 등 이질성을 가진 종양들의 특징상 정확한 부위의 채취가 어려워 수술적 개방 생검으로도 진단이 어려운 경우가 많다. 이 경우 초음파 유도하 침생검이 진단율을 올릴 수 있는 방법으로 사용되고 있다. 본 연구는 자기공명 관류영상(magnetic resonance perfusion)에서 발견된 연부조직 종양을 진단하는 데 있어 초음파 유도하 침 생검의 정확도를 평가하고, 양전자방출 컴퓨터 단층촬영술(positron emission tomography-computed tomography, PET-CT)의 유용성에 대해 알고자 하였다. 대상 및 방법: 2014년 1월부터 2018년 12월까지 자기공명 관류영상에서 발견된 연부조직의 종양에 대해 초음파 유도하 침 생검을 시행한 총 152예의 환자들 중 최종 진단을 얻어 초음파 유도하 침 생검의 결과와 비교할 수 있었던 86예에 대해 그 정확도를 후향적으로 평가하였다. 총 86예의 환자를 자기공명 관류영상만을 시행한 50예와 자기공명 관류영상과 PET-CT를 함께 시행한 36예로 나누어 그 정확도를 비교, 분석하여 PET-CT가 초음파 유도하 침 생검의 정확도를 향상시킬 수 있는지 알고자 하였다. 결과: 총 86예의 환자들 중 자기공명 관류영상만을 시행한 50예 중에서는 34예에서 최종 진단과 초음파 유도하 침 생검의 결과가 일치하였으며, 자기공명 관류영상과 PET-CT를 함께 시행한 36예 중에서는 32예에서 최종 진단과 초음파 유도하 침 생검의 결과가 일치하여 초음파 유도하 침 생검의 정확도에 있어 유의한 차이를 보였다. 이 중 연부 조직 육종만 따로 분류한다면 자기공명 관류영상만을 시행한 12예 중에서 6예에서 최종 진단과 초음파 유도하 침 생검의 결과가 일치하였고 자기공명 관류영상과 PET-CT를 함께 시행한 18예 중에서는 17예에서 최종 진단과 초음파 유도하 침 생검의 결과가 일치하여 마찬가지로 초음파 유도하 침 생검의 정확도에 있어 유의한 차이를 보였다. 결론: 연부조직 종양을 진단함에 있어 초음파 유도하 침 생검은 정확도가 높은 유용한 검사로 알려져 있지만 육종의 경우 이질성이 있어 자기공명 관류영상만으로는 정확한 생검 부위를 설정하는 데 어려움이 있다. PET-CT 영상은 이러한 연부조직 종양에서 침 생검 부위를 특정하고 정확도를 유의하게 향상시킬 수 있을 것으로 생각된다.

학령기 아동의 CBCL 조절곤란프로파일(Child Behavior Checklist Dysregulation Profile)의 요인구조와 특성 (Characteristics of the Factor Structure of the Child Behavior Checklist Dysregulation Profile for School-aged Children)

  • 본 연구에서는 국내 학령기 아동을 대상으로 아동·청소년행동평가척도 조절곤란프로파일(Child Behavior Checklist Dysregulation Profile; CBCL-DP)의 요인구조를 조사하고, CBCL-DP특성을 보이는 집단(임상군)과 그렇지 않은 집단(정상군)간 부적응 및 문제행동 수준에 차이가 있는지 확인하고자 하였다. CBCL-DP의 가장 적합한 요인구조를 확인하기 위해 선행연구에서 제시한 3가지 경쟁모형에서 확인적 요인분석을 실시하였으며, 그 결과 단일요인모형(One-factor)과 2차요인모형(Second order)보다 Bifactor모형이 가장 좋은 적합도를 보였다. 가장 적합한 요인구조의 타당성을 확인하기 위해 관련 변인들과의 회귀경로를 계산한 결과 Bifactor모형에서 CBCL-DP는 부모가 보고한 집행기능곤란과 담임교사가 보고한 학교적응 및 강점난점의 모든 하위요인들과 관련되어 있었으며, 불안/우울, 공격행동, 주의집중문제의 3가지 요인들은 차별화된 연관성을 보였다. CBCL-DP의 특성을 나타내는 집단의 부적응 및 문제행동 수준을 CBCL-DP 특성을 보이지 않는 정상군과 비교한 결과, CBCL-DP 특성을 보이는 아동은 정상군 아동보다 집행기능 곤란 정도가 더 높았으며, 학교생활 적응을 잘 하지 못하고 바람직한 행동을 할 가능성이 낮은 것으로 나타났다. 결과적으로 CBCL-DP는 특정 증후군(AAA)보다 여러 가지 다양한 측면에서 부정적 결과와 관련이 있었으며, 선행연구들의 요인구조와 본 연구 표본이 부합하고 있다는 점은 광범위한 조절곤란 증후군으로 CBCL-DP를 개념화하는데 일관되고 통합적인 연구결과를 제시했음을 시사한다. 또한 CBCL-DP는 정서·행동문제가 결합된 고위험군 아동을 조기발견하고 중재함에 있어 학교 및 상담 장면에서 심층적이고 전문적인 진단도구로서 유용하게 사용될 수 있을 것으로 기대된다. 이러한 연구결과를 바탕으로 본 연구의 의의, 한계점 및 향후 연구 방향에 대해 논의하였다.

국소진행성 유방암에서 F-18 FLT-PET 적용 가능성에 대한 예비 연구: F-18 FDG-PET와 비교 (A Pilot Study for the Feasibility of F-18 FLT-PET in Locally Advanced Breast Cancer: Comparison with F-18 FDG-PET)

  • 목적: 최근에 세포증식성 PET 방사성의약품으로 소개되고 있는 FLT를 이용하여 국소진행성 유방암 환자에서 FLT-PET 영상을 시행하여 종양 영상으로서의 향후 적용 가능성을 살펴보고, FDG와 FLT의 섭취양상에 대한 비교 및 정상 장기에 대한 섭취분포 양상을 알아보고자 하였다. 대상 및 방법: 본 연구는 2005년 8월부터 2006년 11월까지 총 22명(모두 여성, 평균나이; 42세)의 국소진행성 유방암 진단 받은 환자들을 대상으로 하였으며, 모든 종양의 조직형은 침윤성관상피암이었다. 모든 환자를 대상으로 하여 conventional imaging workup, FLT PET/CT, FDG-PET를 시행하였다. MRI로 측정 한 종양 장경의 평균은 $7.2{\pm}3.4$ cm 이었다. 원발 병소와 림프절 섭취에 대한 육안적인 분석과 반정량적인 분석(SUVmax, $SUV_{75}$, TB ratio)을 하였고, 림프절 섭취에 대한 FDG-PET와 FLT-PET의 일치도를 살펴보았다. 정상장기에서의 FLT와 FDG 분포양상을 비교하기 위해 간과 골수의 SUVmax와 $SUV_{75}$을 측정하였다. 결과: 모든 원발 종양근 FLT-PET와 FDG-PET에서 섭취 증가를 나타냈다. MR로 측정한 종양 장경과 FLT-PET 또는 FDG-PET의 SUVmax간에 유의한 상관관계는 없었다(p>0.05). 종양의 SUVmax (FLT $6.5{\pm}5.2$ vs FDG $8.3{\pm}4.9$, p=0.02)와 $SUV_{75}$ (FLT $5.3{\pm}4.3$ vs FDG $6.9{\pm}4.2$, p=0.02)에서는 FDG가 FLT보다 유의하게 높았으며, 중등도의 유의한 상관관계를 보였다(SUVmax; rho=0.450, p=0.04 / $SUV_{75}$; rho=0.472, p=0.03). 반면, TB 섭취비는 FLT가 FDG보다 현저히 높았다(FLT $11.7{\pm}7.7$ vs FDG $6.3{\pm}3.8$, p=0.001). 림프절 섭취 분포에 대한 비교에서는 FDG와 FLT-PET는 34개 구역에서 33개가 일치하였다. FLT와 FDG의 정상 장기의 섭취분포를 보았을 때 FLT는 간(FLT $4.2{\pm}1.2$ vs FDG $1.8{\pm}0.4$)과 골수(FLT $7.4{\pm}1.2$ vs FDG $1.6{\pm}0.4$)의 섭취가 FDG보다 높고 환자마다 다양하였다. 결론: 국소진행성 유방암에서 FTLT는 FDG보다 종양 섭취도는 낮지만 종양과 전이 림프절 섭취가 PET에서 잘 관찰되었다. FLT-PET는 종양 대 주변섭취의 비가 FDG-PET보다 높았으며, 전이 림프절의 진단에서도 FDG-PET와 높은 일치율을 보여 종양 진단 영상으로 유용할 것으로 생각된다. FLT는 정상 간과 골수 조직의 섭취가 높고 개인차가 커서 이들 장기의 전이를 진단하는 데는 제한적일 것으로 생각된다.

토끼 뇌종양 모델에서의 관류 CT 영상에 관한 연구 (Research on Perfusion CT in Rabbit Brain Tumor Model)

  • VX2 암종을 이식한 토끼 뇌종양 모델에서, perfusion CT(computed tomography, CT)를 이용하여 종양과 정상 뇌조직의 혈류 특성을 알아보고자 하였다. 체중 2.4~3.0kg(평균 2.6kg)의 토끼(New Zealand white rabbit) 수놈 9마리를 대상으로, 토끼 뇌에 VX2 세포 현탁액 $1{\times}10^7$ cells/ml, 0.1 ml을 이식하고 종양이 5mm 정도 크기로 자라면 perfusion CT를 시행 하였다. GE사의 AW(advantage windows workstation, version 4.2)로 종양의 용적과 perfusion 값을 산출 하였다. 뇌종양의 평균 용적은 $316{\pm}181mm^3$ 이었고, 가장 큰 종양은 497 $mm^3$, 가장 작은 종양은 195 $mm^3$ 이었 이식된 종양 모두 단일 결절형으로 만들어졌고, 두개강 내로 전이는 발견되지 않았다. perfusion CT에서 종양 중심부의 혈류량(cerebral blood volume, CBV)은 $74.40{\pm}9.63$ 이었고, 종양쪽 정상 뇌조직에서는 $16.08{\pm}0.64$ 이었으며, 종양 반대쪽 정상 뇌조직에서는 $15.24{\pm}3.23$이었다. 혈류 속도(cerebral blood flow, CBF)는 종양 중심부에서 $962.91{\pm}75.96$ 이였고, 종양쪽 정상 뇌조직에서는 $357.82{\pm}12.82$ 이었으며, 종양 반대쪽 정상 뇌조직에서는 $323.19{\pm}83.24$ 이었다. 평균 통과시간(mean transit time, MTT)은 종양 중심부에서 $4.37{\pm}0.19$ 이었고, 종양쪽 정상 뇌조직에서는 $3.02{\pm}0.41$ 이었으며, 종양 반대쪽 정상 뇌조직에서는 $2.86{\pm}0.22$ 이었다. 투과성 표면적(permeability surface, PS)은 종양 중심부에서 $47.23{\pm}25.45$ 이었고, 종양쪽 정상 뇌조직에서는 $14.54{\pm}1.60$ 이었으며, 종양 반대쪽 정상 뇌조직에서는 $6.81{\pm}4.20$이었다. 또한, 종양 중심부에서 최고치 도달 시간(time to peak, TTP)은 $19.33{\pm}0.42$ 이었고, 종양쪽 정상 뇌조직에서는 $16.43{\pm}1.72$ 이었으며, 종양 반대쪽 정상 뇌조직에서는 $15.14{\pm}0.88$이었지만 통계적으로 유의하지 않았다.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The important parameters of this process are the conduction loss to laser power absorption (defined as the conduction-to-laser power parameter, Nk), the convection heat transfer at the surface to conduction loss (defined as Bi), dimensionless extinction coefficient (defined as Br.), and dimensionless irradiation time (defined as Fo). Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Fo and explosion depth (from the surface to the point of maximun temperature) are increased when Nk and Bi are increased.To find out the minimum laser power for explosive evaporative removal process, steady state analysis has been also made. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury.
