High Level Physical Activity and Prevalence of Cardiovascular Disease Using the Korea National Health and Nutrition Examination Survey Data, 2007-2013

  • Objectives: The purpose of our study was to evaluate the association between the intensity of physical activity (PA) and prevalence of cardiovascular disease (CVD) using Korean representative data. Methods: We analyzed 39 804 participant data from the Korea National Health and Nutrition Examination Survey, 2007-2013. Exposure variable was three levels of PA (low, medium, and high) in a week, and outcome variable was prevalence of CVD based on patient self-recognition and doctor's diagnosis. Complex logistic regression analysis was performed to evaluate the relationship between level of PA and CVD adjusted by body mass index, hypertension, hypercholesterolemia, diabetes mellitus, stress recognition, household income, smoking, and current drinking. The indices of association w ere estimated as crude prevalence odds ratio (POR), adjusted POR, and their 95% confidence interval (CI). All statistical analyzes were performed using complex sample analysis procedure of the SPSS version 23.0. Results: When all variables were adjusted, only high level PA in women showed a significant association with stroke (adjusted POR by patient's self-recognition, 0.57; 95% CI, 0.32 to 0.99, adjusted POR by doctor's diagnosis, 0.55; 95% CI, 0.34 to 0.87) and CVD (adjusted POR by doctor's diagnosis, 0.68; 95% CI, 0.48 to 0.96). Conclusions: High level PA in women has a significant reverse association with prevalence of stroke and CVD in Korea. Further study for elucidating the mechanism will be needed.

Fourth molar: A retrospective study and literature review of a rare clinical entity

  • Purpose: The prevalence of supernumerary teeth has been reported to be between 0.1% and 3.8%. The aim of this study was to determine the prevalence, clinical significance, and associated pathologies of fourth molars based on a retrospective study and a literature review. Materials and Methods: A 5-year retrospective prevalence study was conducted at the Department of Oral Diagnosis and Dentomaxillofacial Radiology of Okayama University Hospital, Okayama, Japan. The study involved extracting data from the digital records of patients from January 1, 2013 through December 31, 2017. The sampling frame included all patients who had panoramic radiographs, cone-beam computed tomography (CT), and multislice CT images during the period under review. Results: A total of 26,721 cases were reviewed and 87 fourth molars were identified. The prevalence of fourth molars in the 5-year study at Okayama was calculated as 0.32%. The mean age of patients with a fourth molar was 30.43 years, and the male-to-female ratio was 1:0.98. The vast majority of cases were in the maxilla (92%) and had normal shapes(89.7%); furthermore, 82.8% of cases were unerupted. Conclusion: The prevalence of fourth molars in the study population was found to be 0.32%, and fourth molars occurred with approximately equal frequency in males and females. Fourth molars were more common in the maxilla and were predominantly unerupted and small.

Alternative accuracy for multiple ROC analysis

  • The ROC analysis is considered for multiple class diagnosis. There exist many criteria to find optimal thresholds and measure the accuracy of diagnostic tests for k dimensional ROC analysis. In this paper, we proposed a diagnostic accuracy measure called the correct classification simple rate, which is defined as the summation of true rates for each classification distribution and expressed as a function of summation of sequential true rates for two consecutive distributions. This measure does not weight accuracy across categories by the category prevalence and is comparable across populations for multiple class diagnosis. It is found that this accuracy measure does not only have a relationship with Kolmogorov - Smirnov statistics, but also can be represented as a linear function of some optimal threshold criteria. With these facts, the suggested measure could be applied to test for comparing multiple distributions.

Analysis of Temporomandibular Joint Disorders in Children and Adolescents: Diagnosis and Treatment Pattern by Age

  • Temporomandibular joint disorders (TMDs) can occur at any age, including childhood and adolescence, and pain-related TMDs can affect growth and quality of life. In the present study, recent trends in the diagnosis and treatment of TMDs in children and adolescents were analyzed over a 10-year period. Using 10 years of data from the Health Insurance Review and Assessment Service (HIRA) and Jeonbuk National University (JBNU) Dental Hospital, patients between 0 and 19 years of age diagnosed with K07.6 (temporomandibular joint disorders) were analyzed by 5-year bins. Both datasets indicated a higher prevalence in females (1.2-fold in HIRA, 1.5-fold in JBNU) and in ages 15 to 19 years (72.1% in HIRA, 74.7% in JBNU). HIRA reported a 42.3% increase in prevalence per 100,000 people, from 651.4 in 2011 to 927.0 in 2020. JBNU reported K07.66 (masticatory muscle disorders) as the most common diagnosis in subjects under 10 years of age and K07.60 (internal derangement of temporomandibular joint) in those over 10 years of age. In addition, both were treated mainly by a combination of physical therapy and medication, and the treatment rate increased in accordance with age. Because TMDs can affect various structures in the orofacial region and cause pain that tends to differ with age, an early and specific diagnosis appropriate for age is important for treatment. Therefore, pediatric dentists need to promptly recognize TMDs in children and adolescents and consult with specialists as the prevalence increases.

의료보험 심사 업무의 작업자세(Work Postures) 특성과 누적외상성질환(CTDs) 발생에 관한 연구 (Relationships between Work Postures and Upper Extremity Cumulative Trauma Disorders in Medical Insurance Bill Reviewers)

  • The purpose of this study is to investigate the relationship between the work postures and upper extremity cumulative trauma disorders(CTDs) in female medical insurance bill reviewers(n=448). This study included diagnosis for CTDs, anthropometry and job analysis (workstations and posture). The characteristics of subject were 33.6 years of average age and 8.7 years of average work duration. The results were as fallows. Ergonomic conditions of workstation were unsuitable compared with anthropometry result. The height of work surface(79.5 cm) and chair(43.0 cm)were high. work space was small, and legs space under the table was limited. Work postures were awkward compared with recommended neutral postures. Neck flexion($21.0-36.0^{\circ}$), elbow elevation, shoulder abduction ($46.0-47.0^{\circ}$). wrist radial and ulnar deviation, forearm supination and pronation, and wrist repeated motion(12.7-21.5 freq./min) were analyzed as hazardous risk factors by job analysis. The prevalence of CTDs was 32.8 % by medical diagnosis. The prevalence rate of CTDs in hazardous work posture group was significantly greater than safe work posture group both neck-shoulder (relative prevalence = 5.2, p<0.001) and wrist-hand (relative prevalence = 2.5, p<0.05).

새로 진단된 제1형 및 제2형 당뇨병 환자에서 말초신경이상 (Peripheral Nerve Abnormalities in Patients with Newly Diagnosed Type I and II Diabetes Mellitus)

  • Background: Early detection of neuropathy may prevent further progression of this complication in the diabetic patients. The purpose of this study was to evaluate the prevalence of early neuropathic complication in patients with newly diagnosed type 1 and type 2 diabetes. Methods: Nerve conduction studies (median, ulnar, posterior tibial, peroneal, and sural nerves) were performed for 49 type 1 (27 males, mean $14.1{\pm}7.5$ years) and 40 type 2 (27 males, $42.0{\pm}14.1$ years) diabetic patients at onset of diabetes. Children with age at onset under 4 years and adults over 55 years were excluded to eliminate the aging effect and the influence of obstructive arteriosclerosis. Neuropathy was defined as abnormal nerve conduction findings in two or more nerves including the sural nerve. Results: Mean HbA1c level was $12.6{\pm}3.3%$ for type 1 and $10.5{\pm}2.9%$ for type 2 diabetes. The prevalence of neuropathy was 12.2% for type 1, and 35.0% for type 2 diabetes, respectively. There were significant trends in the prevalence of neuropathy with increasing age (p<0.05). The effect of the mean level of glycosylated hemoglobin on the prevalence of polyneuropathy at onset of diabetes was borderline (p=0.0532). Neither sex of the patients nor the type of diabetes affected the neurophysiologic abnormalities at the diagnosis. Conclusions: Even in a population with diabetes at the diagnosis, the prevalence of subclinical neuropathy was not low. Neuropathy has been significantly associated with increasing age indicating the possibility of longer duration of undetected diabetes among them, especially in type 2 diabetes.

생활습관 요인이 대사증후군 유병 위험에 미치는 영향 (Impact of Life Style Characteristics on Prevalence Risk of Metabolic Syndrome)

  • Purpose: The goal of this study was to evaluate the impact of life style characteristics on the prevalence risk of metabolic syndrome (MS). Methods: A total of 581 adults were recruited from a cardiovascular outpatient clinic. A newly developed comprehensive life style evaluation tool for MS patients was used, and patient data related to the MS diagnosis were reviewed from the hospital records. Results: The overall prevalence of MS was 53.2%, and the mean of MS score was 2.6 for patients at a cardiovascular outpatient clinic (78% of the patients had hypertension). Dietary habits among the life style characteristics had significant influence on the prevalence risk of MS and MS scores. And also interestingly, the classification and regression tree (CART) model suggested that the high prevalence risk groups for MS were older adults (61.5$\leq$age<79.4), and adults between 48.5 and 61.5 yr of age with bad dietary habits. Conclusion: This study indicates that nurses should focus on dietary habits of patients (especially patients classified as high prevalence risk for MS) for improvement and prevention of MS prevalence risk.

학령전기와 학령기 아동의 천식 유병률과 위험인자 (Prevalence and Risk Factors of Asthma in Community Childhood)

  • Purpose: This study was conducted to estimate the prevalence of asthma and its risk factors in childhood asthma. Method: Random samples of 10,236 were selected from 43 kindergarten (1,418) and 57 elementary (8,718) in K city between september and November (2007). 1,079 (kindergarten children) and 7,271 (elementary children) were in the final analysis. The Korean-translated modified version of the questionnaire for the International Study of Asthma and Allergies in Childhood was used in this cross-sectional survey. Parents were surveyed to answer for the questionnaire. Result: The lifetime and 12-month prevalence of wheezing were 11.50%; 11.06% in kindergarten children and 19.24%; 4.80% in elementary children. The lifetime prevalence of asthma diagnosis and the 12-month prevalence of asthma treatment were 11.59%; 4.43% in kindergarten children and 4.43%; 10.78% in elementary children. The 12-month prevalence of night cough and exercise-induced wheezing were 12.90%; 3.33% in kindergarten children and 20.72%; 4.74% in elementary children. Risk factors analysis showed that age, paternal and maternal asthma, allergic disease, carpet use, monthly income, indoor environment were associated with a higher risk of asthma. Conclusion: The study suggests that prevalence of asthma has increased among the community children. These data have been used to manage a possible role of risk factors as predictors of childhood asthma.

한국인 구취발생 빈도에 관한 연구 (Epidemiologic Study on Oral Malodor for Korean)

  • Oral malodor is a problem that traverses history, culture, race and sex. But, up to the present the study on prevalence of oral malodor in normal popualtion is short, especially there is no study on oral malodor prevalence in Korean. In our study, we investigated self-evaluation of oral malodor, self-rating intensity of oral malodor, patients efforts for curing their oral malodor, and degree of satisfaction with their efforts. Investigation was carried out on 368 public Koreans resident in a big city(174 males, 194 females), by method of self-reporting to prepared questionnaire. There was no difference in self-reporting prevalence of oral malodor in sexes(57.5% in male, 58.8% in female), and the prevalence rate was increased with aging. Self-rating intensity of oral malodor in oral malodor patients group(by self-reporting) was ranked in order of slight, weak, middle, strong in both sexes, but the number of self-rating strong female group is significantly larger than that of self-rating strong male group(9.3% in male, 17.9% in female). In question on duration of oral malodor, most subjects answered that their oral malodor was developed in certain circumstances(61.3% in male, 76.3% in female), and oral malodor-developing circumstances was ranked in order of in the early morning, in hunger state, in fatigue, in stressful situations. Subjects effort for curing their oral malodor was ranked in order of more tooth brushing, gum chewing, use of commercial product for oral hygiene, visit dental clinic, and rates of no effort subjects were 15.3% in males and 10.7% in females. The degree of satisfaction with their efforts for curing their oral malodor was very low, only 5.6% of males and 6.6% of females were answered that there were satisfactory improvement in their oral malodor. Collectively, our result revealed that prevalence of oral malodor in public Koreans and their interest in oral malodor were much higher than our anticipation, and that patients satisfaction with improvement in oral malodor was short of their expectations. Considering the life quality of patients suffering from oral malodor, we conclude that developments of more improved diagnostic tools and treatment methods for oral malodor is indispensable in future.

학령전기와 학령기 아동에서의 아토피 피부염 유병률과 위험인자 (Prevalence and Risk Factors for Atopic Dermatitis in Pre-school and School Aged Children)

  • Purpose: This study was done to estimate the prevalence of atopic dermatitis (AD) and its risk factors for AD in children living in the community. Method: Random samples of 10,236 were selected from 43 kindergarten (1,418) and 57 elementary (8,718) students in K city. Data from 1,079 (kindergarten children) and 7,271 (elementary) students were used in the final analysis. The Korean-translated modified version of the International Study of Asthma and Allergies in Childhood (ISSAC) questionnaire was used in this cross-sectional survey. Parents answered the survey questionnaire. Results: The lifetime and last 12-month prevalence of AD were 40.15%; 30.86% in kindergarten children and 33.56%; 25.37% in elementary children. The lifetime and last 12-month prevalence of AD diagnosis were 40.8%; 18.68% in kindergarten children and 34.36%; 12.63% in elementary children. The lifetime prevalence of AD treatment was 25.93% in kindergarten children and 22.07% in elementary children. Multivariate logistic regression analysis revealed that risk factors for AD were age, allergic disease, age of house, carpets, pets. Conclusion: The study suggests that prevalence of AD has increased among children in the community. These data can be used to manage possible risk factors that are predictors of childhood AD.
