A Feasibility Study of Constitution Discrimination Using a Measurement Device for Dynamic Friction Coefficients of the Back of a Hand (손등피부 운동 마찰계수 측정기를 이용한 체질 판별 가능성 연구)

  • 1. Objectives Our goal is to observe the feasibility of constitution discrimination from computing quantitative roughness index from dynamic friction coefficients and their gradients with the measurement device of skin friction with 3-Axis load cell sensor. 2. Methods In the traditional Korean medicine, skin diagnosis is one of the examination methods to discriminate Sasang constitution since it was known that Tae-eumin has rough skin, and Soyangin has smooth one. It is based on the skin roughness on the back of one's hand for the discrimination. The measurement device of skin friction with 3-axis load cell sensor has been developed in order to provide quantitative skin roughness through dynamic friction coefficients. The effective interval of the coefficients is obtained from the automatic sampling algorithm to use their curvature and slope. Then, Fisher's discriminant function of them makes the discrimination. 3. Results The success rate of extracting the effective interval was about 90% and the discriminant accuracy between Tae-eumin and Soyangin was 70% and 68% for men and women, respectively. The entire methods showed the possibility to distinguish between Tae-eumin and Soyangin by using stochastic properties of roughness index, which can make the entire system to include the measurement, the computation of the roughness index and the discrimination of constitution automatical. 4. Conclusions The measurement device, the automatic sampling algorithm of dynamic friction coefficients and the constitution discrimination algorithm were developed, respectively, and their combination can become the serial and automatic procedure for quantitative and objective skin diagnosis, which mimics the movement of the Oriental medical doctors' skin diagnosis. It can be applied to healthcare as well as the diagnosis of constitution in a u-Health system soon.

The Study for Correlation Characteristics on Radial Artery and Floating/sinking Pulse with BMI (BMI에 따른 요골동맥의 혈관특성과 부/침맥과의 상관관계 연구)

  • Pulse diagnosis refers to the process of diagnosing a patient by feeling an artery on the wrist based on the shape that the pulse take s while the hold-down pressure increase. The styloid process artery on the wrist is usually felt, and the pulse is taken on Chon, Gwan and Cheok using three fingers. This study is to examine the structural difference in the location of pulse diagnosis by measuring and analyzing blood diameter, blood depth, and blood flow velocity of the location of pulse diagnosis by using ultrasonic wave (VOLUSION730 PRO, GE Medical, U.S.A). This study also attempted to grasp whether the characteristics of blood vessels differ depending on Body Mass Index (BMI) and analyzed their correlation with Oriental medical pulse diagnosis. The male subjects without cardiovascular diseases were divided into the normal BMI group, the underweight group and the overweight group and 10 people of each group were measured, Blood depth, blood diameter and blood flow velocity at the location of pulse diagnosis (Chon, Gwan, Cheok) of the wrists of left and right hands were measured and the pulse wave was measured by using pulse diagnosis instrument (3-D Mac, DaeyoMedi, Korea).The results of this study showed that the characteristics of blood vessels differ depending on the degrees of obesity, and the characteristics of floating pulse and sinking pulse of Oriental medical pulses were related to the degrees of obesity. This shows that the characteristics of the blood vessels of subjects and BMI information are the major indicators for diagnosis and are the matters that must always be considered when developing the algorithm of pulse diagnosis.

Systematic Review of the Diagnosis of Pelvic Deviation for Chuna Manual Therapy (추나치료에 적용된 골반변위 진단법에 대한 체계적 문헌고찰)

  • Objectives This systematic review aimed to analyze research about pelvic deviation diagnosis for Chuna manual therapy (CMT) and to review the diagnosis methods, indices, and results of diagnosis. Methods Ten electronic databases were systematically searched up to January 4th 2022. Clinical studies and reviews containing pelvic deviation diagnosis for CMT or using CMT as a treatment of pelvic deviation were selected and evaluated. CMT diagnosis in clinical studies and reviews were isolated and analyzed by 2 independent reviewers. Results Thirteen clinical studies and three reviews were included in the evaluation. X-ray analysis and manual testing were the two main methods used in CMT diagnosis of pelvic deviation. For manual testing in clinical studies, leg length insufficiency testing was the most frequently used measurement index and the most common diagnostic results were anterior and posterior rotation. In the X-ray analysis, Obturator foramen and femur head line were the most frequently used measurement index and the most common diagnostic results were anterior rotation and posterior rotation. Conclusions The systematic review found that manual testing and X-ray analysis were mainly used for the diagnosis of pelvic deviation in CMT among clincial and review articles. As there was little research about diagnosing pelvic deviation in CMT and any existing research presented only low standards of evidence, further research should be updated with using a more standardized approach.

Analysis of The Behavior of Kurtosis By Simplified Model of One Sided Affiliated Impact Vibration

  • Among many amplitude parameters, Kurtosis (4-th normalized moment of probability density function) is recognized to be the sensitive good parameter for machine diagnosis. Kurtosis has a value of 3.0 under normal condition and the value generally goes up as the deterioration proceeds. In this paper, simplified calculation method of kurtosis is introduced for the analysis of impact vibration with one sided affiliated impact vibration which occurs towards the progress of time. That phenomenon is often watched in the failure of such as bearings’ outer race. One sided affiliated impact vibration is approximated by one sided triangle towards the progress of time and simplified calculation method is introduced. Varying the shape of one sided triangle, various models are examined and it is proved that new index is a sensitive good index for machine failure diagnosis. Utilizing this method, the behavior of kurtosis is forecasted and analyzed while watching machine condition and correct diagnosis is executed.

Is the Hospital Caseload of Diagnosis Related Groups Related to Medical Charges and Length of Stay? (DRGs(Diagnosis Related Groups)별 환자집중도 수준에 따른 입원진료비와 재원일수의 차이 분석)

  • This study analyzes the effects of hospital caseload on medical charges and length of stay for inpatients. Hospital caseload, representing the level of concentration of patients, was measured with the Internal Herfindal Index for three diagnosis related group (DRG) codes (appendectomy, operations on anus, and operations on uterus and adnexa). Ordinary least squares regression was used for analysis. Results showed that medical charges per inpatient and average length of stay significantly differed with respect to hospital concentration indices, and that hospital caseload was inversely related to operational performance for appendectomy and operations on uterus and adnexa. The significant negative relationship between concentration index and length of stay may decrease the total medical charges. The results imply that the expansion of the DRG payment system to hospitals will have a negative influence on their gross sales.

  • This paper presents a failure diagnosis and accommodation strategy which is capable of tolerating faulty actuators of a submersible vehicle. The proposed method is mainly based on a sliding mode control technique. The primary ideas include a performance index to describe the effectiveness of actuators, and a controller reconfiguration strategy using the actuator effectiveness index. The actuator effectiveness proposed in this work is defined as the relationship between the sliding surface and the controlled system behavior. The resulting actuator effectiveness is then used in reconfiguring the controller in order to counteract for the deteriorated control performance in the presence of a faulty actuator. The effectiveness of the proposed method is demonstrated by means of numerical simulations with a submersible vehicle.

  • To evaluate the contribution of head/neck posture to signs/symptoms of craniomandibular disorder(CMD), the author clinically, cephalo-metrically, analyzed craniomandibular index (CMI), clinical dysfunction index(CDI), occlusal index(OI) and head posture, cervical spine, cervicovertebral anatomy, craniofacial variables from 30 CMD patients and control. And the author analyzed difference between patients and control, and the correlation between CMI, CDI and head posture, cervical spine, cervicovertebral anatomy, craniofacial variables in CMD patients and control. The obtained results were as follows : 1. There was very high significant correlation between CMI and CDI(r=0.8969, p<0.01). 2. There was significant difference between patients and control in head/neck posture, head/neck anatomy(p<0.05). 3. There was significant correlation between CMI, CDI and head posture, cervical spine, cervicovertebral anatomy, craniofacial variables(p<0.05). 4. There was more significant correlation between DI and head/neck posture, head/neck anatomy than between PI and head/neck posture, head/neck anatomy in CMD patients(p<0.05)

Long-Term Evaluation of Conservative Treatment for Craniomandibular Disorders (두개하악장애환자의 보존적 치료에 관한 장기평가)

  • In order to evaluate the prognosis of conservative treatment for Craniomandibular Disorders, 127 patients were subjected at the Dept. of Oral Medicine, Pusan National University Hospital from 1983 through 1991. All the changes of patients' symptoms and related factors were analyzed before treatment, after treatment and at follow-up examination by means of subjective and objective symptom indicies (Ai, AAI, SDS, Di, CDS, MCO). 1. All the indices were reduced and MCO became increased at follow-up examination (p<0.01). 2. As the duration after treatment became longer, all the indices became reduced and MCO became increased. 3. There were no significant differences in index scores according to sex and age. 4. Long-term patients showed higher index scores and lower MCO than short-term patients(p<0.01). 5. Chronic group showed higher index scores and lower MCO than acute group (p<0.05). 6. Macrotrauma group showed higher index scores and lower MCO than microtrauma group (p<0.05) 7. While muscle group showed the lowest MCO, muscle and joint group showed the highest index scores (p<0.01). 8. The long-term success rate of conservative treatment was more than 80% (p<0.01)

Analyzing the Specialization Status of Hospital's Services in Korea (의료기관 서비스 전문화 수준의 측정 및 분석)

  • This study evaluated the specialization status of Korean hospitals by applying index measures that were developed to determine how hospitals are specialized. In addition, multivariate regression analysis was applied to assess how the measures responded to the internal and external factors of hospitals. National Health Insurance claims for 2004 were used to calculate the information theory index, internal Herfindahl index, number of distinct diagnosis-related groups (DRGs) treated, and percent of the five most common DRGs. Data from the Ministry of Health and Welfare and Korean Hospital Association were used to determine the size, ownership, teaching status, organization type, and location of the hospitals. The four indexes analyzed showed that there were significant differences in the specialization status of providers, depending on the provider size, organization type, and location. Hospitals that were smaller and located in metropolitan areas tended to provide specialized services; this is considered to constitute a competitive strategy for hospitals. It is expected that specialized hospitals will increase given the current market structure. Therefore, policy makers will need an index for measuring how hospital services are specialized. Information from such an index could provide a picture of how hospital services are mixed and change over time.

Assessment of Anthropometric Obesity Indexes by Correlation with Body Composition (비만치료에 유용한 단순비만지표의 선별)

  • Object : Waist circumference(WC), waist-hip ratio(WHR), waist-stature ratio(WSR), and body mass index(BMI) are commonly used for evaluating obesity. This Research were done to determine what is more sensitive obesity indexes(WC, WHR, WSR, BMI) Correlated with body composition such as body fat mass, body fat(%), visceral fat area, and fat free mass. And what is more sensitively correlated obesity indexes with % changes of body composition during weight reduction treatment. Methods : This clinical retrospective research were carried out 127 cases of female obese outpatients with weight reduction treatment during 1 month. Bioelectrical impedence analysis(for body composition) and body size(for anthropometric obesity indexes) were estimated in pre-treatment and post-treatment to evaluate the obesity indexes. Pearson correlation coefficients were used to select useful obesity index. Result & Conclusion : BMI is useful index for diagnosis and evaluation of obesity. WSR is sensitively correlated with visceral fat area and body fat(%). So, WSR is useful index for evaluating abdominal obesity and risk factors of metabolic syndrome. WC is correlated with both body fat mass and fat free mass. WHR is not optimal for diagnosis and evaluation of obesity.
