Sarcoidosis is a multisystem disease characterized by noncaseating granulomas. Cardiac involvement is known to have poor prognosis because it can manifest as a serious condition such as the conduction abnormality, heart failure, ventricular arrhythmia, or sudden cardiac death. Although early diagnosis and early treatment is critical to improve patient prognosis, the diagnosis of CS is challenging in most cases. Diagnosis usually relies on endomyocardial biopsy (EMB), but its diagnostic yield is low due to the incidence of patchy myocardial involvement. Guidelines for the diagnosis of CS recommend a combination of clinical, electrocardiographic, and imaging findings from various modalities, if EMB cannot confirm the diagnosis. Especially, the role of advanced imaging such as cardiac magnetic resonance (CMR) imaging and positron emission tomography (PET), has shown to be important not only for the diagnosis, but also for monitoring treatment response and prognostication. CMR can evaluate cardiac function and fibrotic scar with good specificity. Late gadolinium enhancement (LGE) in CMR shows a distinctive enhancement pattern for each disease, which may be useful for differential diagnosis of CS from other similar diseases. Effectively, T1 or T2 mapping techniques can be also used for early recognition of CS. In the meantime, PET can detect and quantify metabolic activity and can be used to monitor treatment response. Recently, the use of a hybrid CMR-PET has introduced to allow identify patients with active CS with excellent co-localization and better diagnostic accuracy than CMR or PET alone. However, CS may show various findings with a wide spectrum, therefore, radiologists should consider the possible differential diagnosis of CS including myocarditis, dilated cardiomyopathy (DCM), hypertrophic cardiomyopathy, amyloidosis, and arrhythmogenic right ventricular cardiomyopathy. Radiologists should recognize the differences in various diseases that show the characteristics of mimicking CS, and try to get an accurate diagnosis of CS.
대학 교육에서 외국어 교육 중요성의 증대와 아울러 대단위 강좌에 대한 효율적인 온라인 외국어 평가 및 학습시스템의 도입이 절실히 요구된다. 본 논문에서는 대단위 외국어 강좌에 대하여 온라인 평가가 가능할 뿐만 아니라 개인별 능력에 맞춘 학습기능과 진단기능을 갖춘 평가 및 학습 시스템을 제안한다. 제안한 시스템의 사용은 대단위 강좌에서 시험 평가에 따른 시공간의 효율적 활용을 가능하게 하고 강사와 분반에 관계없이 모든 학생들에게 동일한 수준의 교과 진행을 가능하게 하여 강의의 질 향상에도 기여한다.
Diabetic polyneuropathy (DPN) is the most common form of diabetic neuropathy, and causes a significant morbidity with an impact on the quality of life in the patients with diabetes. Since DPN frequently induces foot deformity and ulceration, which finally leads to foot amputation, the early detection and treatment is very important for the prevention of a permanent structural change. In the early stage of DPN, the diagnostic methods which can evaluate the function or structure of small nerve fibers should be employed because small nerve fibers are first involved in the course of DPN. However, the nerve conduction study cannot reflect the function of the small nerve fibers, and thus, has a definite limitation in the early diagnosis of DPN. For the early detection of DPN, electrodiagnostic data should be interpreted on a clinical context, along with the careful evaluation of the small nerve fiber functions using the tests such as the analysis of intraepidermal nerve fiber density.
Acute kidney injury (AKI), formerly referred to as acute renal failure (ARF) is defined as the sudden impairment of kidney function (estimated from the glomerular filtration rate [GFR]) that results in the lack of excretion of waste products. More than 30 definitions of AKI exist in the literature, most of which are based on serum creatinine. Lack of a uniform and multidimensional AKI definition has led to failure to recognize significant renal injury, delays in treatment, and inability to generalize single-study results. The RIFLE criteria were developed to standardize the diagnosis of ARF and in the process the term AKI has been proposed to encompass the entire spectrum of the syndrome from minor changes in renal function to requirement for renal replacement therapy. Large prospective studies are needed to test definitions and to better understand risk factors, incidence, independent outcomes, and mechanisms that lead to poor short- and long-term outcomes. Early biomarkers of AKI need to be explored in critically ill neonates.
In this paper, a new expert system scheme, called Logic Based Expert System (LBES), is proposed for real time fault diagnosis of power system. In LBES, Expertise is represented by logical connectives and converted into a Boolean function. The set of Prime Implicants (PIs) of the Boolean function contains all the sound inference results which can be obtained from the expertise. Therefore, off-line inference is possible by off-line PI identification, which reduces the on-line inference time considerably and makes it possible to utilize-the LBES in real-time environment.
본 논문은 현재 사용되고 있는 케이블의 진단을 위해서 열화가중치라는 새로운 접근법을 사용하였다. 케이블은 상시에 높은 온도로 운전된다는 점, 수분이 접촉할 확률이 크다는 점에서 신뢰성 있고 효율적인 열화진단은 필수적이라고 할 수 있다. 종래의 연구는 수트리가 발생되는 절연층을 주 대상으로 삼았으나, 현대에 와서는 절연층뿐만 아니라, 내·외부 반도전층 또한 활발한 연구가 이루어지고 있다. 본 연구에서는 최근에 국내에서 제안된 열화가중치법을 이용하여 케이블의 열화인자가 될 수 있는 부분에 대하여 특성분석을 수행한 다음, 열화가중치를 부여하여 케이블의 최종적인 상태를 파악해 보았다.
Temporomandibular disorder(TMD) is described as a cluster of disorders characterized by pain in the preauricular area and/or the muscles of mastication; limitations or deviations in mandibular range of motion; and noises in the TMJ during mandibular function. The most common symptom in TMD patients is pain that is aggravated by chewing or other jaw function. These symptoms are appeared when the stimuli loaded in TMJ are over the physiologic tolerance. The primary goal in treatment of TMD is to alleviate pain and lor mandibular dysfunction. TMD treatment can be divided into 2 categories: reversible and irreversible methods. Reversible methods include medication, thermal therapy, habit modification, physical therapy, appliance therpy and arthrocentesis and lavage and irreversible methods include arthroscopic lysis, surgery, occlusal adjustment et al. It is widely accepted that reversible methods are ther first choice of treatments. However if reversible ones are not effective, irreversible methods are considered.
In this research, a novel battery charge system with embedded diagnosis function is proposed by using online impedance spectroscopy. The impedance spectroscopy technique is employed to investigate the impedance variation of the battery thereby estimating the state of health of the battery. A small voltage perturbation is applied to the battery by the voltage controller of the bidirectional converter with no additional hardware and the impedance of the battery is then calculated by the digital lock-in amplifier embedded in the DSP of the charger. The design procedure of the proposed charger is detailed and the feasibility of the system is verified by the experiments.
Among many dimensional and dimensionless amplitude parameters, kurtosis (4-th normalized moment of probability density function) is generally regarded as a sensitive good parameter for machine diagnosis. However, higher order moment may be supposed to be much more sensitive. Bicoherence is an absolute deterioration factor whose range is 1 to 0. The theoretical value of n-th moment divided by n-th moment calculated by measured data would behave in the same way. We propose a simplified calculation method for an absolute index of n-th moment and name this as simplified absolute index of n-th moment. Some favorable results are obtained.
Depression is a frequently developed psychiatric symptom after stroke. Poststroke depression(PSD) causes not only psychological distress related to depression but also other disturbances in many areas such as cognitive function, activities of daily living and quality of life. Therefore, a comprehensive understanding of accurate detection and appropriate treatment of depression is mandatory in patients with stroke. This review focused on the current knowledge of PSD.
