중년여성 당뇨병 환자의 성기능, 성 스트레스, 삶의 질에 대한 연구 (A Study on Sexual Function, Sexual Stress, and Quality of Life in Middle Aged Women Patients with Diabetes Mellitus)

  • Purpose: The purpose of this study was to examine the relationships among sexual function, sexual stress, and quality of life in middle aged women patients with diabetes mellitus. Methods: Data were collected through questionnaires distributed to 154 middle aged women patient with diabetes mellitus in one hospital. Results: The mean sexual function score was $12.99{\pm}9.11$. There were statistically significant differences in sexual function according to age, level of education, employment status, time of being diagnosed with diabetes mellitus, number of complications, self-monitoring of blood glucose, menopausal status, and level of glycosylated hemoglobin. The mean sexual stress score was $26.99{\pm}16.88$. The score of quality of life was $79.12{\pm}14.30$. There were statistically significant differences in quality of life according to level of education. Sexual function was negatively correlated with sexual stress (r=-.46 p<.001) and positively correlated with quality of life (r=.32, p<.001), while sexual stress was negatively correlated with quality of life (r=-.36 p<.001). Conclusion: Higher sexual dysfunction in middle aged women patients with diabetes mellitus was correlated with lower sexual stress and improved quality of life, while lower sexual stress was correlated with improved quality of life.

  • As popularity of dental implantation is increasing, the number of cases associated with complications also increase. Evaluation on diabetes mellitus is often neglected due to the disease's irrelevance to implantability. However, patients with diabetes mellitus are susceptible to infection due to impaired bactericidal ability of neutrophils, cellular immunity and activity of complements. Due to this established connection between diabetes mellitus and infection, a couple of cases were selected to present patients with diabetes mellitus with glycemic incontrollability, suffering from post-implantation dentigerous inter-fascial space abscess.

  • Purpose: This research seeks to identify differences between general characteristics, disease-related characteristics, glycated hemoglobin (HbAlc) levels, and aspects of chronic complications of diabetes mellitus in type 2 diabetes. Methods: This research was conducted from the 1st to the 15th of February in 2016, on 263 in patients. Patients' electronic medical records were used to identify their general characteristics, disease-related characteristics, HbAlc, and chronic diabetic complications. Chi-square test, ANOVA, ANCOVA, and the Cochran-Mantel-Haenszel test were used for data analysis. Results: Statistical significance was observed for general characteristics, based on smoking status, such as age, and education level; disease-related characteristics differed according to the duration of diabetes. Smoking status did not differ according to HbAlc level. In term of chronic diabetic complications, statistically significance was observed for diabetic nephropathy, based on smoking status. Conclusion: Patients who had a history of smoking, but were not currently smoking, were likely to display higher HbAlc levels and diabetic nephropathy. Therefore, there is need for regular checkups for diabetic complications among patients with a history of smoking and it is important to emphasize smoking cessation.

  • One of the serious long-tenn complications of diabetes mellitus is the diabetic foot lesion. Diabetic foot lesions are a major cause of morbidity, mortality, disability in patients with diabetes and frequent cause of lower-extremity amputations. The treatment of diabetic foot lesions is based on the strict control of diabetes mellitus. A patient with diabetic foot lesions was treated with Cheunhwasangamibang and improvement was seen. Results in this case support a role for Cheunhwasangamibang in controlling diabetes mellitus, and are here reported to encourage further study.

노인 당뇨병 환자를 위한 동기강화상담 당뇨병 자기관리 프로그램의 개발 및 효과 (Development and Effects of a Motivational Interviewing Self-management Program for Elderly Patients with Diabetes Mellitus)

  • Purpose: This study was conducted to develop and test the effects of a motivational interviewing self-management program for use with elderly patients with diabetes mellitus. Methods: A non-equivalent control group pretest-posttest design was used. The participants were 42 elderly diabetic patients (experimental group: 21, control group: 21). The motivational interviewing self-management program for elders with diabetes mellitus developed in this study consisted of a 12-week program in total (8 weeks for group motivational interviewing and education and 4 weeks for individual motivational interviewing on the phone). Data were collected between February 13 and May 3, 2013 and were analyzed using t-test, paired t-test, and repeated measure ANOVA with SPSS/WIN 18.0. Results: For the experimental group, significant improvement was found for self-efficacy, self-care behavior, glycemic control and quality of life (daily life satisfaction, influence of disease) as compared to the control group. Conclusion: The study findings indicate that the motivational interviewing self-management program is effective and can be recommended as a nursing intervention for elderly patients with diabetes mellitus.

The Effect of a Balance Training Program to Improve the Balance of Aged Patients with Diabetes Mellitus

  • Purpose: The purpose of this study was to determine whether a balance training program could improve the balancing ability of elderly patients with type II diabetes mellitus. If yes, the results could lead to development of a method to prevent falls in elderly patients with type II diabetes mellitus (DM). Methods: Subjects were 28 elderly patients with type II DM. Subjects were divided into a control and an experimental group with 14 patients in each group. The experimental group had a balance training program consisting of balance training and lower extremity muscle strengthening exercises. The exercise group met twice a week for 8 weeks. All subjects were evaluated using a force plate for the amount of postural sway. To determine the degree of diabetes, we measured Hemoglobin A1c. Results: Balance in the experimental group (p<0.05) increased significantly compared to the control group. Conclusion: The results suggest that a balance training program should be recommended for improving balance and preventing falls in elderly type II DM patients.

Subgingival pathogens in chronic periodontitis patients affected by type 2 diabetes mellitus: a retrospective case-control study

  • Purpose: The aim of this study was to compare the prevalence and bacterial load of 6 main periodontal pathogens between pairs of periodontal patients with and without type 2 diabetes mellitus. Porphyromonas gingivalis and Aggregatibacter actinomycetemcomitans genotypes were also investigated. Methods: Twenty patients affected by chronic periodontitis and type 2 diabetes were retrospectively selected and matched to 20 patients without diabetes on the basis of the degree and severity of periodontal disease. Microbiological data of subgingival biofilms were analysed and compared for the examined pathogens: A. actinomycetemcomitans, P. gingivalis, Prevotella intermedia, Treponema denticola, Fusobacterium nucleatum, and Tannerella forsythia. Results: The pairs were balanced in terms of demographic and clinical parameters, except for bleeding on probing and suppuration. In the microbiological test sites (4 for each patient), the mean probing pocket depth was 6.34±1.63 mm in patients with diabetes and 6.41±1.78 mm in patients without diabetes. No significant difference between pairs in the prevalence of P. gingivalis or the distribution of its genotypes was recorded. Patients with diabetes had a significantly greater amount of total bacterial load, P. gingivalis, T. denticola, T. forsythia, and F. nucleatum (P<0.05). Moreover, patients with diabetes had a higher number of sites with a greater cell count than patients without diabetes. When compared to the total bacterial load, only T. forsythia maintained its relative load in patients with diabetes (P=0.001). Conclusions: This retrospective matched study supports the hypothesis that microbiological differences exist among periodontal patients with and without diabetes mellitus.

한국인 당뇨병에 관한 조사연구 (Studies on diet theraphy of diabetes mellitus among Koreans)

  • The incidence of diabetes mellitus among Koreans as shown an increasing tendency recently, probably due to the various factors such as the improving living conditions. The majority of people are ignorant or indifferent to the nature, progress and prognosis of diabetes mellitus in Korea. 107 cases of diabetes mellitus which had been admitted to Severance Hospital between January and August, 1971 were Studied. Of these 107 cases, 22 cases were interviewed thoroughly during their hospital stay and the response to their diet therapy was carefully checked. 1) Of the 107 cases 69 cases were male and 38 cases were female; the sex ratio was 18:1. The age of the onset of the disease was as follows: 2 cases were under 20 years of age; 20 cases (18.7%) were under 40 years of age and 85 cases (79.5%) were over 40 years of age. Juvenile diabetes was less frequent when compared with developed countries. 2) Patients complaints and symptoms on admission, complications of sickness, and duration of sickness until the female discharge were also studied. We found that the incidence of tuberculosis complication in diabetes melltius was alarming (13. 8%). 3) In most cases, the control of diabetes was inadequate and diet practisis by the patients was also very poor even when they had known of the diabetes mellitus for a considerable period of time. During hospitalization 75 cases (70.0%) were controlled by diet and oral medications alone. Only 16 cases (15.0%) needed insulin injection, the remaining 16 cases required both diet control and insulin injection. 4) In general, patients received hospital diet satisfactorily. Only a few cases complained of difficulties with milk intake because of no previous dietary experience or of excessive meat orfish because they preferred vegetables and fruit. 5) Patients responded well to the dietitians interviews in the hospital but follow up study and care were poorly organized after discharge from the hospital. 6) The diet exchange 1ist published by the Korean Diabetic Association was not well received by the patients or the general puplic because it is not inexpensive and detailed instructions were not given at the time of discharge from the hospital.

당뇨병 환자의 혈당 조절 정도에 대한 인식과 HbA1C 값 비교 (The discrepancy between the estimation for diabetic control and the HbA1C value in diabetic patients)

  • Background : Diabetes mellitus is chronic condition which needs patients' understanding of disease status and effort for disease control. We investigated whether the patients' estimation for blood glucose control was reliable or not comparing with their HbA1C value, Method: Among people who visited one health promotion center in Seoul from 1995 to 2003,those who answered as having diabetes mellitus on self reported questionnaire were identified. Subsequent question was whether his or her diabetes was well controlled or not. We compared these replies with their HbA1C values. When their HbA1C values were below7% with answering not well controlled, we analyzed if there was any variable which can predict the discrepancy. And the same analyze we did for the opposite situation. Result: A total of 1193 patients were eligible. Those who answered as well controlled formed32.9% of the group with HbA1C values above 7%. In the cases with well controlled though significantly associated with this discrepancy, Conclusion: Considerable proportion of diabetic patients' estimation for diabetic control status was not matched with glycemic control guideline. For successful management of diabetes mellitus, more efforts for patient education and communication skills are essential.

The Effects of Elderly Diabetes Mellitus Patients' Self-care Behavior and Health Conservation on Cardiovascular Risk Factors

  • Purpose: This study was performed in order to identify the relationships among self-care behavior, health conservation, and cardiovascular risk factors and to examine the influence of self-care behavior and health conservation on cardiovascular risk factors among Korean elders with diabetes mellitus. Methods: The participants were 105 elders with diabetes mellitus using senior welfare centers and elderly leisure houses in Daegu. Data were collected through interviews during the period from April to May in 2014. Self-care behavior was measured with Kim's (1997) Self-care Behavior Scale, health conservation with Sung's (2005) Health Conservation Scale, and cardiovascular risk factors with the Arizona Heart Institute Cardiovascular Risk Factor Questionnaire. Collected data were analyzed through one-way ANOVA, independent t-test, Pearson's correlation, and stepwise multiple regression using the SPSS/WIN 19.0 program. Results: A negative correlation was found between self-care behavior and cardiovascular risk factors, and between health conservation and cardiovascular risk factors. Self-care behavior explained 6% and health conservation did 49% of variance in elderly diabetes mellitus patients' cardiovascular risk. Conclusion: The results indicate that, in order to reduce cardiovascular risk factors among Korean elders with diabetes mellitus, we need nursing interventions for increasing health conservation and self-care behavior.
