동과 분획물의 투여가 Streptozotocin 유발 당뇨 흰쥐의 혈당 및 혈장 지질수준에 미치는 영향 (Effects of Fractions of Benincasa hispida on Plasma Levels of Glucose and Lipid in Streptozotocin Induced Diabetic Rats)

  • 본 연구는 streptozotocin으로 유발된 당뇨 흰쥐에게 동과에서 추출한 각 용매분획물을 14일간 경구투여한 후 혈당과 지질함량을 분석하였으며 다음과 같은 결론을 얻었다. 당뇨대조군과 비교시 실험 14일 후 당뇨실험군에서 체중감소가 낮아졌으며, 실험 10일째부터는 당뇨실험군에서 혈당 수준이 당뇨대조군보다 감소하는 경향을 보이었다. 특히 $CHCl_3$분획물 투여군과 $H_2O$분획물 투여군은 14일째에 유의적으로 낮은 수준을 나타내었다. 당뇨실험군은 혈장 유리지방산 수준에서도 당뇨대조군에 비해 낮은 수준이었으며 $CHCl_{3}$분획물 투여군은 혈장 중성지방 수준에서도 낮게 나타났으나 유의적인 차이는 보이지 않았다. ALT와 AST 활성도는 당뇨대조군에 비해 $H_2O$분획물 투여군에서 유의적으로 낮게 나타났다. 결론적으로 당뇨 유발 흰쥐에게 $CHCl_3$분획물의 투여가 실험적 당뇨에 대하여 체중의 감소를 둔화시키며 특히 혈장 중 포도당의 함량을 유의성있게 감소시키는 것으로 보아 당뇨병에 대한 개선효과가 있다고 사료된다.

The Incidence and Characteristics of Patients with Small Ruptured Aneurysms (<5 mm) in Subarachnoid Hemorrhage

  • Objective : Small unruptured aneurysms (<5 mm) are known for their very low risk of rupture, and are recommended to be treated conservatively. However, we encounter many patients with small ruptured aneurysms in the clinical practice. We aimed to investigate the incidence and characteristics of patients with small ruptured aneurysms. Methods : We reviewed all patients admitted to our hospital with subarachnoid hemorrhage from January 2005 to December 2015. The patients were divided into two groups : those with aneurysms <5 mm (group S) and those with aneurysms ${\geq}5mm$ (group L). The patient's age and sex, size and location of aneurysms, and risk factors such as hypertension, diabetes, alcohol use, and smoking were compared between the two groups. Results : Eight-hundred eleven patients were diagnosed with ruptured aneurysms, and 337 (41.6%) were included in group S. The mean size of all aneurysms was $6.10{\pm}2.99mm$ (range, 0.7-37.7); aneurysms with a diameter of 4-5 mm accounted for the largest subgroup of all aneurysms. Female sex was significantly associated with the incidence of small ruptured aneurysms (odds ratio [OR] 1.50, 95% confidence intervals [CI] 1.02-2.19, p=0.037). Despite female predominance in the incidence of small ruptured aneurysms, the proportion of small ruptured aneurysms in young (<50 years) men was high. In men, there were no significant differences regarding the location of the aneurysms between group S and group L (p=0.267), with the most frequent location being the anterior communicating artery (ACoA) in both group S (50.9%) and group L (51.4%). However, in women, there were significant differences regarding the location of the aneurysms between group S and group L (p=0.023), with the most frequent locations being the ACoA (33.0%) in group S, and the posterior communicating artery (30.6%) in group L. In women, two locations were significantly associated with small (<5 mm) ruptured aneurysms: the ACoA (OR 2.14, 95% CI 1.01-4.54, p=0.047) and anterior cerebral artery (OR 3.54, 95% CI 1.19-10.54, p=0.023). Multiplicity and smoking were significantly associated with large (${\geq}5mm$) ruptured aneurysms in women. The use of alcohol was related to small ruptured aneurysms in men over 50 years of age (OR 2.23, 95% CI 1.03-4.84, p=0.042). Conclusion : In this study, small (<5 mm) ruptured aneurysms exhibited different incidences by age, sex, location, and risk factors such as multiplicity, smoking, and alcohol use.

The association of PBX1 polymorphisms with overweight/obesity and metabolic alterations in the Korean population

  • Pre-B-cell leukemia transcription factor 1 (PBX1), which is located on chromosome 1q23, was recently reported to be associated with type 2 diabetes mellitus. We examined whether single nucleotide polymorphisms (SNPs) of the PBX1 gene are associated with overweight/obesity in a Korean population. We genotyped 66 SNPs in the PBX1 gene and investigated their association with clinical phenotypes found in 214 overweight/obese subjects and 160 control subjects using the Affymetrix Targeted Genotyping chip array. Seven SNPs (g.+75l86C>T, g.+78350C>A, g.+80646C>T, g.+138004C>T, g.+185219G>A, g.+191272A>C, and g.+265317T>A) were associated with the risk of obesity in three models (codominant, dominant, and recessive) (P=0.007-0.05). Haplotype 1 (CAC) and 3 (TAC) of block 3 and haplotype 2 (GGAAT) of block 10 were also strongly associated with the risk of obesity. In the control group, subjects that had homozygote for the major allele for both g.+185219G>A and g.+191272A>C showed lower high density lipoprotein-cholesterol (HDL-C) level compared to those possessing the minor allele, suggesting that the association between the homozygote for the major allele for both g.+185219G>A and g.+191272A>C and HDL-C is attributable to the increased risk of obesity. This study suggests that the PBX1 gene is a possible risk factor in overweight/obese patients.

남성 건강검진 수진자들에서 알코올성 및 비알코올성 지방간과 대사증후군과의 관련성 (Association of Alcoholic & Nonalcoholic Fatty Liver Disease with Metabolic Syndrome in Health Screen Examinees of Men)

  • 최근 성인 남성에서 비만 인구가 늘어나며, 심혈관질환을 야기하는 대사증후군 유병률이 증가하고 있고, 증상이 없지만 지방간으로 진단되는 환자가 늘고 있다. 본 연구는 알코올성 또는 비알코올성 지방간 질환과 대사증후군과의 관련성이 있는지 알아보고자 하였다. 799명의 남성을 대상으로 평소 음주량을 조사하였고, 초음파 검사를 통해 지방간 유무를 확인 한 결과 297명은 정상군이었고, 206명은 알코올성 지방간, 296명은 비알코올성 지방간이었다. IDF(international diabetes federation) 기준을 적용한 대사증후군과 구성요소들을 독립변수로, AFLD(alcoholic fatty liver disease) 및 NAFLD(nonalcoholic fatty liver disease)을 종속변수로 하여 로지스틱 회귀분석을 실시하였다. 정상군과 AFLD 또는 NAFLD에서 대사증후군의 분포는 각각 7.4%, 48.8%, 34.9%이었고, 대사증후군 구성요소 중 고혈압, 고혈당, 고중성지방, 비만은 AFLD 또는 NAFLD의 위험인자였고, AFLD에선 고중성지방과 복부비만이, NAFLD에서는 복부비만이 영향력이 가장 큰 것으로 나타났다. 또한 AFLD군의 경우 대사증후군과 비만, 저HDL-콜레스테롤의 이상범위에서 지방간 중증의 교차비가 높았고, NAFLD군에서는 대사증후군과 고중성지방군의 이상범위에서 지방간 중증의 교차비가 높았다. 결론적으로 AFLD 및 NAFLD는 대사증후군 구성요소 각각의 기준과 밀접한 관련성이 있었고, 지방간 관리를 위해 꾸준하고 포괄적인 접근이 필요하다.

사구수조지방종에 의한 도르래신경마비 1예 (Trochlear Nerve Palsy Caused by Quadrigeminal Cistern Lipoma)

  • 목적: 중뇌(midbrain) 등쪽(dorsal)의 사구수조지방종(quadrigeminal cistern lipoma)에 의한 도르래신경마비를 경험하였기에 이를 보고하고자 한다. 증례요약: 65세 남자가 2년 전부터 간헐 양안 수직 복시가 있었으나 별다른 치료 없이 지내다가 2주 전부터 항상 증상이 생겨 내원하였다. 1달 전 진단받은 당뇨 이외에 다른 전신질환의 과거력은 없었다. 시력은 우안 20/25, 좌안 20/20이었고, 상대구심동공운동장애는 없었다. 안구운동검사에서 원거리 주시에서 4프리즘디옵터(prism diopters, PD)의 좌안 상사시와 근거리에서 4PD의 외사시를 보였다. 좌안은 내전시상전(elevation in adduction)이 있었다. 머리기울임검사에서 좌측 기울임에서 6PD의 좌안 상사시가 보였고, 우측 기울임에서는 정위를 보였다. 안저검사에서 우안 외회선과 좌안 내회선이 보였다. 뇌자기공명영상(brain magnetic resonance imaging)에서 중뇌 등쪽에서 나오는 도르래신경에 인접한 사구수조지방종을 확인하였다. 복시 증상의 완화를 위해 프리즘 안경을 처방하였고, 다른 신경 증상을 동반하지 않아 뇌 병변에 대한 경과관찰을 시행하기로 하였다. 결론: 중뇌 등쪽의 사구수조지방종으로 인해 도르래신경마비를 확인하였다. 종양의 압박에 의한 도르래신경마비는 드물지만, 본 증례와 같이 비특이적인 증상을 보인 도르래신경마비 환자에서는 빠른 뇌영상검사가 원인 감별에 도움을 줄 수 있다.

Fatty liver associated with metabolic derangement in patients with chronic kidney disease: A controlled attenuation parameter study

  • Background: Hepatic steatosis measured with controlled attenuation parameter (CAP) using transient elastography predicts metabolic syndrome in the general population. We investigated whether CAP predicted metabolic syndrome in chronic kidney disease patients. Methods: CAP was measured with transient elastography in 465 predialysis chronic kidney disease patients (mean age, 57.5 years). Results: The median CAP value was 239 (202-274) dB/m. In 195 (41.9%) patients with metabolic syndrome, diabetes mellitus was more prevalent (105 [53.8%] vs. 71 [26.3%], P < 0.001), with significantly increased urine albumin-to-creatinine ratio (184 [38-706] vs. 56 [16-408] mg/g Cr, P = 0.003), high sensitivity C-reactive protein levels (5.4 [1.4-28.2] vs. 1.7 [0.6-9.9] mg/L, P < 0.001), and CAP (248 [210-302] vs. 226 [196-259] dB/m, P < 0.001). In multiple linear regression analysis, CAP was independently related to body mass index (${\beta}=0.742$, P < 0.001), triglyceride levels (${\beta}=2.034$, P < 0.001), estimated glomerular filtration rate (${\beta}=0.316$, P = 0.001), serum albumin (${\beta}=1.386$, P < 0.001), alanine aminotransferase (${\beta}=0.064$, P = 0.029), and total bilirubin (${\beta}=-0.881$, P = 0.009). In multiple logistic regression analysis, increased CAP was independently associated with increased metabolic syndrome risk (per 10 dB/m increase; odds ratio, 1.093; 95% confidence interval, 1.009-1.183; P = 0.029) even after adjusting for multiple confounding factors. Conclusion: Increased CAP measured with transient elastography significantly correlated with and could predict increased metabolic syndrome risk in chronic kidney disease patients.

Dietary Exposure to Transgenic Rice Expressing the Spider Silk Protein Fibroin Reduces Blood Glucose Levels in Diabetic Mice: The Potential Role of Insulin Receptor Substrate-1 Phosphorylation in Adipocytes

  • Type 2 diabetes mellitus (T2DM) is characterized by insulin resistance (IR). T2DM is correlated with obesity and most T2DM medications have been developed for enhancing insulin sensitivity. Silk protein fibroin (SPF) from spiders has been suggested as an attractive biomaterial for medical purposes. We generated transgenic rice (TR) expressing SPF and fed it to diabetic $BKS.Cg-m+/+Lepr^{db}$ mice to monitor the changes in blood glucose levels and adipose tissue proteins associated with energy metabolism and insulin signaling. In the present study, the adipocyte size in abdominal fat in TR-SPF-fed mice was remarkably smaller than that of the control. Whereas the adenosine monophosphate-activated protein kinase (AMPK)-activated protein kinase and insulin receptor substrate 1 (IRS1) protein levels were increased in abdominal adipose tissues after TR-SPF feeding, levels of six-transmembrane protein of prostate 2 (STAMP2) proteins decreased. Phosphorylation of AMPK at threonine 172 and IRS1 at serine 307 and tyrosine 632 were both increased in adipose tissues from TR-SPF-fed mice. Increased expression and phosphorylation of IRS1 at both serine 307 and tyrosine 632 in adipose tissues indicated that adipocytes obtained from abdominal fat in TR-SPF-fed mice were more susceptible to insulin signaling than that of the control. STAMP2 protein levels decreased in adipose tissues from TR-SPF-fed mice, indicating that STAMP2 proteins were reducing adipocytes that were undergoing lipolysis. Taken together, this study showed that TR-SPF was effective in reducing blood glucose levels in diabetic mice and that concurrent lipolysis in abdominal adipocytes was associated with alterations of AMPK, IRS1, and STAMP2. Increased IRS1 expression and its phosphorylation by TR-SFP were considered to be particularly important in the induction of lipolysis in adipocytes, as well as in reducing blood glucose levels in this animal model.

한국 성인의 비만이 인슐린, 공복혈당 및 당화혈색소에 미치는 영향 (Obesity as associated factors for Insulin, Fasting blood sugar, and Hemoglobin A1c in Korean adults)

  • 본 연구의 목적은 체질량지수가 인슐린, 혈당 및 당화혈색소에 미치는 영향을 확인하는 것이다. 연구방법은 국민건강영양조사 2015년 자료를 분석하여, 서술적 통계, t-test, ANOVA, Pearson correlation coefficient과 단계적 다중회귀분석을 적용하였다(SPSS WIN 25.0). 주요결과는, 체질량지수는 인슐린(r=0.397, p<0.001), 공복혈당(r=0.241, p<0.001) 및 당화혈색소(r=0.237, p<0.001)와 양의 상관관계를 나타내었다. 인슐린에 영향요인은 과체중(${\beta}=.130$, t=8.193, p<0.001),비만(${\beta}=.390$, t=24.390, p<0.001), 연령이며 설명력은13.1% (F=53.584, p<0.001)였다. 공복혈당에 영향요인은 과체중(${\beta}=.072$, t=4.478, p<0.001), 비만(${\beta}=.186$, t=11.485, p<0.001), 연령이며 설명력은 10.6%(F=42.351, p<0.001)였다. 당화혈색소에 영향요인은 과체중(${\beta}=.059$, t=3.712, p<0.001), 비만(${\beta}=.190$, t=11.895, p<0.001), 연령, 성별이며 설명력은 14.1% (F=58.134, p<0.001)였다. 본 연구결과를 바탕으로 당뇨병 예방 및 관리프로그램에 비만관련 중재가 포함되어야 한다.

Molecular mechanism of protopanaxadiol saponin fraction-mediated anti-inflammatory actions

  • Background: Korean Red Ginseng (KRG) is a representative traditional herbal medicine with many different pharmacological properties including anticancer, anti-atherosclerosis, anti-diabetes, and anti-inflammatory activities. Only a few studies have explored the molecular mechanism of KRG-mediated anti-inflammatory activity. Methods: We investigated the anti-inflammatory mechanisms of the protopanaxadiol saponin fraction (PPD-SF) of KRG using in vitro and in vivo inflammatory models. Results: PPD-SF dose-dependently diminished the release of inflammatory mediators [nitric oxide (NO), tumor necrosis factor-${\alpha}$, and prostaglandin $E_2$], and downregulated the mRNA expression of their corresponding genes (inducible NO synthase, tumor necrosis factor-${\alpha}$, and cyclooxygenase-2), without altering cell viability. The PPD-SF-mediated suppression of these events appeared to be regulated by a blockade of p38, c-Jun N-terminal kinase (JNK), and TANK (TRAF family member-associated NF-kappa-B activator)-binding kinase 1 (TBK1), which are linked to the activation of activating transcription factor 2 (ATF2) and interferon regulatory transcription factor 3 (IRF3). Moreover, this fraction also ameliorated HCl/ethanol/-induced gastritis via suppression of phospho-JNK2 levels. Conclusion: These results strongly suggest that the anti-inflammatory action of PPD-SF could be mediated by a reduction in the activation of p38-, JNK2-, and TANK-binding-kinase-1-linked pathways and their corresponding transcription factors (ATF2 and IRF3).

의사결정나무분석을 이용한 심혈관질환자의 재입원 위험 요인에 대한 융합적 분석 (Convergence Analysis of Risk factors for Readmission in Cardiovascular Disease: A Machine Learning Approach)

  • 본 연구는 의사결정나무 통계분석법을 톨해 국민건강영양조사 자료를 2차 분석하여 심혈관질환자의 재입원 위험 요인을 확인하는 기초자료를 마련하고자 하였다. 연구대상자는 국민건강영양조사 4-6기 자료대상자 총 65,973명 중 협심증이나 심근경색 진단 병력이 있는 총 1,037명의 성인이며, SPSS window 21 Program을 이용하여 분류 분석 중 CHAID 의사결정나무 방법으로 분석하였다. 뿌리 마디(Root node)는 경제활동상태((χ2=12.063, p=.001), 자식 마디(Child node)는 개인 소득수준(χ2=6.575, p=.031), 최근 1년간 체중 변화(χ2=12.758, p=.001), 거주지역(χ2=4.025, p=.045), 직접흡연(χ2=3.884, p=.049), 교육수준(χ2=9.630, p=.024)으로 확인되었다. 끝마디(Terminal node)는 고혈압(χ2=3.854, p=.050), 당뇨(χ2=6.056, p=.014), 직업형태(χ2=7.799, p=.037)로 분석되었다. 이를 통해 심혈관질환자의 재입원 관리를 위해 다양한 요인의 통합적 접근을 고려한 프로그램의 개발 및 운영이 필요함을 제언한다.
