Geometry and Property Database for Korean Spine Research (한국인 척추 연구를 위한 형상 / 물성 정보 구축)

  • The Korean spine geometry and property data for researchers were made by KISTI and Catholic Institute for Applied Anatomy. We took whole spine CT, X-Ray, BMD scan for making high resolution cross-sectional spine images using more 20 donated cadavers(60 - 80 years). Then we constructed 3-dimensional volume model using serial CT images by Mimics software. The major morphometric parameters of vertebrae were measured. Mechanical motion and property data were obtained by the same cadavers using the DEXA for BMD and the spine simulator. The Korean spine geometry and property data could be used for research and development of medical device.

Statistical Observation for Measurement of Bone Mineral Density of the Spine in Korean Women Screening in a Health Clinic (건강진단상 여성골밀도 측정의 통계학적 관찰)

  • During the period from January to September 1995, 1.010 Women(premenopausal 466 cases and postmenopausal 544 cases) of employees and family member were observed for bone mineral density of lumbar spine using Dual Energy X-ray Absorptiometry(DEXA) at Medical Department of Korea Life Insurance Co. Ltd. Results obtained were as follows: 1) In premenopausal women 466 cases, the ratio of decreased bone mineral density of lumbar spine according to age groups shows 3rd decade were 5 cases(9.80%) among 51 cases, 4th decade were 31 cases(8.68%) among 357 cases, and 5th decade were 10 cases(17.24%) among 58 cases. 2) In postmenopausal women 544 cases, the ratio of decreased bone mineral density of lumbar spine according to age groups shows 3rd decade were 2 cases(66.67%) among 3 cases, 4th decade were 28 cases(31.11%) among 90 cases, 5th decade were 168 cases(48.70%) among 345 cases, 6th decade were 73 cases(73.0%) among 100 cases, and 7th decade were 4 cases(66.67%) among 6 cases. 3) Bone mineral density begins to decrease with age as early as the 4th decade, even in the premenopausal state. In the 4th decade, premenopausal women exhibit an average 8.68% decrease in bone density. Postmenopausal women in this same age group, however, demonstrate an average bone density decrease of 31.11%.

Biomechanical analysis of pullout strength of the pedicle screws in relation to change bone mineral density (반복 하중 후 골밀도 감소에 따른 척추경 나사못의 고정력(Pullout Strength)감소 형태 분석)

  • Screw loosening and subsequent pullout can be attributed to the reduction in bone mineral density in the vertebrae manifested by osteoporosis in which the decrease in fixation strength between the cancellous bone and screw threads are accelerated by repeated loads exerted by patients own weight and activities following the surgery. In this study, the change in pullout strength of the pedicle screws was investigated before and after repeated loads were imparted. For this purpose. Diapason pedicle screws $(6.7\times40mm)$ were inserted onto fresh porcine spine specimens (T1-L5) after bone mineral density was measured using a DEXA. With an MTS, an axial load was applied at a loading rate of 0.33mm/sec until failure to measure the maximum pullout strength. Flexion moment of 7.5N-m was then imparted at 0.5Hz for 2000 cycles. It was found that the maximum pullout strength was exponentially related to BMD regardless of load types ($107.71\;\times\;\exp^{(1.43{\times}BMD)}r^2=0.93$, P<0.0001 without repeated load; ($107.71\;\times\;\exp^{(2.19{\times}BMD)}r^2=0.78$, P<0.0001 with repeated load). The results suggest that the reduction in pullout strength for pedicle screws is far more prominent in osteoporotic spine than in normal spine especially as number of repeated load was increased. More importantly, it was demonstrated that the level of bone mineral density and the activity level of the patient should be evaluated in more detail for successful implementation of pedicle screw systems in spinal surgery.

The Examination of Pre-Menopause Women's Bone Mineral Density and Its Related Factors by Using the Dual-Energy X-Ray Absorptionmetry (이중에너지 X선 흡수계측법을 이용한 폐경 전 여성의 골감소증 관련요인)

  • The purpose of this study is to examine factors that affect the bone mineral density of pre-menopause women by using the dual energy x-ray absorptionmetry, ultimately contributing to preventing women's osteoporosis that tends to be aggravated since menopause. Out of the subjects, 20.2% were suffering osteopenia. Age was found most important in estimating the level of bone mineral density. Meanwhile, the older women were, the significantly lower their bone mineral density was. It was found that taking exercise has a more positive effect on boss mineral density than not taking. Exercising in a suitable amount was helping women keep their bone mineral density better. Preferring meat to vegetarian diets were significantly affecting women's bone mineral density. Meanwhile, it was found that the shorter menstrual cycle is, the significantly lower bone mineral density is. A multi=regression analysis of bone mineral density and its related factors showed that the older women were, the significantly lower their bone mineral density was. In other words, age was found as the most risk factor of osteoporosis.

A Comparison of Bone Mineral Density in Osteoporotic Facture of the Proximal Femur Using Dual Energy X-ray Absorptiometry (대퇴 근위부 골절환자에서 이중에너지 방사선흡수계측법을 이용한 부위별 골밀도 비교)

  • There were some controversies about direct cause of hip fracture. We attempted to look at 40 osteoporotic proximal femur fractures in women over 50 years between March in 1999 and Febuary in 2000. The bone density of the fracture group and the healthy 85 control group was measured by Dual Energy X-ray absorptiometry(DEXA). The result was compared using age matched paired T test. The results were as follows ; 1. The femoral neck fractures were 14 cases and the trochanteric fractures were 26 cases. Mean age at a fracture was 67.1 years in neck fracture group and 76.5 years in trochanteric fracture. 2. In the control group, the bone density of both side of the proximal femur was measured and it showed statistically no difference between both sides in same person. 3. The bone density of neck, Ward's triangle, trochanter(P<0.05) and lumbar spine(P<0.001) was significantly reduced in the proximal femoral fracture group comparing with the control group. 4. The bone density of neck, Ward's triangle, trochanter(P<0.05) was significantly reduced in the proximal femoral neck fracture group comparing with the control group, but there was no statistical difference in lumbar spine comparing with the control group. 5. The bone density of neck, Ward's triangle, trochanter and lumbar spine(P<0.001) was significantly reduced in the proximal femoral neck fracture group comparing with the control group. We concluded that the bone mineral densities(BMD) of proximal femur and lumbar spine had decreased in hip fractures but that the bone mineral density and T-score % of the proximal femur were statistically lower than that of the lumbar spine. We suggest that measuring the bone mineral density of the proximal femur may reflect the weakness of the proximal femur more precisely than measuring the bone mineral density of the lumbar spine.

A Study of Nutrient Intake and Serum Levels of Osteocalcin, Ca, P, and Mg and Their Correlation to Bone Mineral Density in Korean Postmenopausal Women Residing in Rural Areas (농촌 지역 폐경 후 여성의 골밀도에 따른 영양소 섭취 상태 및 혈청 오스테오칼신, 칼슘, 인, 마그네슘 함량과 이들간의 상관성 연구)

  • Postmenopausal women lose more bone mass than men as a result of estrogen deprivation. The resultant low bone mineral density (BMD) is a major risk factor in the development of osteoporosis. Calcium, phosphorus and magnesium are main components of bone. The purpose of this study is to investigate nutrient intake and serum osteocalcin, Ca, P and Mg and their correlation to bone mineral density in Korean postmenopausal women residing in rural areas. We conducted 24 hour dietary recalls, anthropometric measurements and blood analysis on 60 postmenopausal women. The BMD of the lumbar spine (L2$\rightarrow$L4) and the femoral neck were measured by dual energy X-ray absorptiometry (DEXA). Subjects were assigned to one of three groups:normal (T-score> -1, n=20), osteopenia (-2.5> T-score $\leq$ -1, n=23), and osteoporosis ( T-score $\leq$ -2.5, n=17). The mean age, height, weight and BMI were 62.37 yr, 154.36 cm, 55.28 kg and 23.18 $kg/m^2$ respectively. The mean daily energy and protein intakes were 76.35% and 87.41% of RDA for Koreans. The mean intakes of calcium, phosphorus, and magnesium were 463.62 mg (66.23% of RDA), 955.32 mg (136.47% of RDA), 345.87 mg respectively. The mean serum levels of calcium, phosphorus and magnesium were 8.76 mg/dl, 3.80 mg/dl, and 2.10 mg/dl, respectively, and there were no significant differences among the three groups. However, the BMD of the femoral neck showed a significantly negative correlation with serum magnesium (p<0.05). To summarize the results, most nutrient intakes (especially calcium) in postmenopausal women did not reach the RDA values for Koreans. Also, increase of serum magnesium levels may be related to bone loss.

Relationship of bony trabecular characteristics and age to bone mass (연령과 골소주 특성의 골량에 대한 연관관계)

  • Purpose : Bony strength is dependent on bone mass and bony structure. So this study was designed to investigate the relationship between the bone mass and bony trabecular characteristics. Subjects and Methods : Study subjects were 51 females (average age 68.6 years) and 20 males (average age 66.4 years). Bone mineral density (BMD, $grams/cm^2$) of proximal femur was measured by a dual energy X-ray absorptiometry (DEXA). Regions of interest (ROIs) were selected from the digitized radiographs of proximal femur. A customized computer program processed morphologic operations (MO) of ROIs. 44 skeletal variables of MO were calculated from ROIs on the Ward's triangle and greater trochanter of femur. WHO BMD classes were predicted by MO variables of the same ROI. Classification and Regression Tree analysis was used for calculating weighted kappa values, sensitivity and specificity of MO. Results : The discriminating factors of morphologic operation were branch point, branch point [per cm sq]. Age also played important role in distinguishing osteoporotic classes. The sensitivity of MO at Ward's triangle and Greater Trochanter was 91.8%, 65.6%, respectively. The specificity of MO was 100% at Ward's triangle and Greater Trochanter. Conclusion : Bony trabecular characteristics obtained using radiological bone morphometric analysis seem to be related to bone mass.

Correlation between Nutrient Intakes and Bone Mineral Density in Carpus of Female University Students (일부 여대생의 영양섭취와 손목 골밀도와의 상관성)

  • The purpose of this study was to examine the correlation between nutrient intakes and bone mineral density in female university students. A total 27 female university students were measured the anthropometric characteristics, dietary intake and the bone mineral density of carpus using DEXA. The average age, height, weight, % body fat, WHR, and BMI of the subjects were 22.7 years, 161.5 cm, 57.0 kg, 29.9%, 0.8, 21.8 kg/$m^2$, respectively. Bone mineral density of ultradis and distal carpus as T-value were -1.5 and -0.4, respectively. The daily energy intake of the subjects was 1589.0kcal. And the intakes of energy, calcium, iron, zinc, vitamin $B_2$, and folic acid did not meet the Korean RDAs. The daily total food intake of the subjects was 1011.0g and food intake from cereals and vegetables was high. The major food groups of mineral intake were vegetables/cereals/milks/fishes for calcium, cereals/meat/vegetables/fishes for phosphorus, cereals/vegetables/meats for iron, seasonings/vegetables/cereals for sodium, vegetables/cereals/seasonings for potassium, and cereals/meats for zinc. The body weight and body mass index were significantly positive correlated to the bone mineral density of average carpus, respectively. The intakes of animal calcium and vitamin $B_2$ were significantly negative correlated to the bone mineral density of average carpus, respectively. In conclusion, bone mineral density in carpus and nutrient intakes of some female university students were low. Therefore, health management and the study on relation between bone mineral density in various site and long-term intakes of nutrients in many subjects are required.

  • Objective : The aim of this study is to investigate the effect of Chamaecyparis Obtusa Oil to NC/Nga mice induced in Atopic dermatitis-like skin lesions by DNCB Mthods.Methods : NC/Nga mice which have been induced to Atopic dermatitis-like skin lesions by DNCB are divided into 3 groups, the one is the mice which have been spread with Chamaecyparis Obtusa Oil 2% cream, the other is which have been spread with dexamethasone (Dexa.) 0.5% on their Atopic lesion, another is the control group. then we take the blood sample of each group to measure state of the dorsal skin, the number of immunocytes. and resect the skin lesion to analyze the state of cells.Result & Conclusion : There are meaningful results of measuring the number of IgE, IL-4, IFN-γ and the state of the skin lesion and cells in the group with hamaecyparis Obtusa Oil 2% cream in comparison with the other groups.

Effect of DHEA Administration before, during and after Dexamethasone Treatment on Body Weight and Mass of TypeI,II Muscles in Rats (DHEA의 투여 시기가 Dexamethasone 치료 쥐의 체중과 TypeI,II 근육무게에 미치는 영향)

  • This study was to determine the effect of DHEA administration before, during, and after dexamethasone treatment on body weight and TypeI,II muscle weight of rat receiving dexamethasone treatment. Method: Wistar rats were divided into 6 groups: control(C), dexamethasone(D), DHEA administration for 3days after dexamethasone treatment for 7days(7D+3DH), dexamethasone treatment for 7days after DHEA administration for 3days(3DH+7D), DHEA administration during dexamethasone treatment for 4days after dexamethasone treatment for 3days(3D+4DDH), DHEA administration during dexamethasone treatment for 7days(7DDH). Dexamethasone was injected by subcutaneously daily at a dose of 5mg/kg. DHEA was orally administered daily at a dose of 5mg/kg for 7 days. Soleus(TypeI) muscle, and both plantaris and gastro- cnemius(TypeII) muscles were dissected on the 7th day of experiment. Result: Body weight of both 3DH+7D group and 3D+4DDH group increased significantly compared with that of 7D group. Body weight of 7D+3DH group decreased significantly compared with that of 7D group, 7DDH group, 3DH+7D group and 3D+4DDH group. Muscle weight of both plantaris and gastro- cnemius tended to decrease compared with that of 7D group. Muscle weight of 7DDH group, 3D+4DDH group and 3DH+7D group increased significantly compared with that of 7D+3DH group. Muscle weight of gastrocnemius of both 3DH+7D group and 3D+4DDH group increased significantly compared with that of 7D group. Conclusion: Based on these results, it can be suggested that DHEA administration before and during dexamethasone treatment can increase both body weight and mass of atrophied TypeII muscle induced by dexa- methasone treatment.
