Implementing Rule-based Healthcare Edits

  • Automated medical claims processing and billing is a popular application domain of information technology. Managing medical related data is a tedious job for healthcare professionals, which distracts them from their main job of healthcare. The technology used in data management has a sound impact on the quality of healthcare data. Most of Information Technology (IT) organizations use conventional software development technology for the implementation of healthcare systems. The objective of this experimental study is to devise a mechanism for use of rule-based expert systems in medical related edits and compare it with the conventional software development technology. A sample of 100 medical edits is selected as a dataset to be tested for implementation using both technologies. Besides empirical analysis, paired t-test is also used to validate the statistical significance of the difference between the two techniques. The conventional software development technology took 254.5 working hours, while rule-based technology took 81 hours to process these edits. Rule-based technology outperformed the conventional systems by increasing the confidence value to 95% and reliability measure to 0.462 (which is < 0.5) which is three times more efficient than conventional software development technology.

Direction of Healthcare Expenditure on Research and Development (보건의료 연구개발비 현황과 발전방향)

  • The quality of healthcare in Korea is very good, especially in life threatening disease. However, the level of healthcare research in Korea is not good relative to that of engineering. International university rankings also were showed that engineering was generally higher ranking than medicine. The reason of this phenomenon was deeply related to expenditure on research and development (R&D). Although Korea had a lot of gross domestic expenditure on R&D (GERD), 75% of GERD was from business enterprise that was related to engineering. Healthcare expenditure of R&D from business enterprise is small. Healthcare expenditure from government budget allocation on R&D (GBARD) was smaller than engineering. Higher education expenditure on R&D of GERD was also small and the quantity and quality of researcher in higher education was not enough. For Korea's healthcare to become the growth engine for future, GBARD should be invested heavily in the healthcare, a large part of the increased GBARD must be invested in higher education, and the higher education should invest to secure the higher quality researcher stably.

Education Topics for the Development of Doctors' Public Healthcare Competencies (의사의 공중보건 역량 개발을 위한 교육주제)

  • Needs for public healthcare have recently increased. This paper proposes education topics for competency development in public healthcare in line with the needs of the times. In Korea, various lifelong education providers have already provided public health-related education. For example, the Research Institute for Health Policy (RIHP) under the Korean Medical Association provided an "executive course for physicians' public health care competencies" in 2019 and 2020. At the end of the course, the RIHP published a comprehensive report, entitled "Curricular development and evaluation for doctors' public healthcare competencies." This article is based on a summary of that report. To develop a curriculum for public healthcare, the RIHP adopted the following methodologies for a needs analysis; reviewing already-existing education subjects, evaluating end-of-course reports, and conducting in-depth focused group interviews and questionnaire surveys with doctors at public healthcare-related institutions. The results from the needs analysis can be categorized into two domains of education topics for public healthcare. The first domain includes education subjects related to the theory and practice of public healthcare, as follows: a general overview, community or population health, organizational administration, planning and evaluation, budget and finance, responses to disasters such as infectious diseases, health policy, and the legal system. The second domain contained education topics related to general professional competencies: leadership, communication, cooperation, teamwork, and professionalism. In conclusion, the curricular content for public healthcare will be an appropriate combination of competencies specific to public healthcare and core competencies for health professionals.

  • u-Healthcare 활성화의 필요성이 꾸준히 제시되어 왔으나, 아직까지 이해자들의 공감대를 이끌어 낼 수 있는 개념정립 및 분류체계가 미비한 실정이다. 새로운 서비스 영역인 u-Healthcare 분야 산업 발전을 위해서는 정책적 전략과 제도적 정비 등의 기반조성이 필요하며, 이에 앞서 명확한 개념 정의와 영역 범주화의 필요성이 제기되고 있다. 본 연구에서는 ubiquitous computing 속성에 대한 이해를 통해 u-Health 환경과 u-Healthcare를 정의하며, 보건의료적 접근에 의한 분류를 통해 보건의료서비스에의 적용가능 분야를 제시하고자 한다.

Three cases of rare SRY-negative 46,XX testicular disorder of sexual development with complete masculinization and a review of the literature

  • Purpose: To identify the clinical characteristics of SRY-negative male patients and genes related to male sex reversal, we performed a retrospective study using cases of 46,XX testicular disorders of sex development with a review of the literature. Materials and Methods:SRY-negative cases of 46,XX testicular disorders of sex development referred for cytogenetic analysis from 1983 to 2013 were examined using clinical findings, seminal analyses, basal hormone profiles, conventional cytogenetic analysis and polymerase chain reaction. Results: Chromosome analysis of cultured peripheral blood cells of 8,386 individuals found 19 cases (0.23%) with 46,XX testicular disorders of sex development. The SRY gene was confirmed to be absent in three of these 19 cases (15.8%). Conclusion: We report three rare cases of SRY-negative 46,XX testicular disorders of sex development. Genes on autosomes and the X chromosome that may have a role in sex determination were deduced through a literature review. These genes, through differences in gene dosage variation, may have a role in sex reversal in the absence of SRY.

Development and Reconsideration of Korea Healthcare System (보건의료체계의 발전과 성찰)

  • During last 65 years, Korea has achieved very rapid economic growth and social reformation including healthcare system. Many foreigners have praised that Korea healthcare system is very good in the respect of ease accessibility to healthcare under the lowest cost among the industrialized countries. Whole population are covered by the National Health Insurance. Also utilizations of healthcare among different income classes are even. However Korea healthcare system faced with several challenges, in terms of the an aging population and a rise in chronic disease problem, new threats of communicable disease due to globalization, the rapid increase of healthcare expenditure and high financial burden of patients even though they are insured. To cope with these challenges, we need reconsider the healthcare system as followings; to set up ideology of healthcare as normative public goods, to rebuild paradigm of healthcare for 21 century, to reform public health for strengthening health promotion, to develop new method for healthcare management including quality improvement and consumer responsiveness, to build new governance for health and to view new perspective on healthcare as a kind of industry.

  • The fast-paced development in the field of U-Healthcare, which is available anytime and anywhere, is being underway in accordance with the development of IT technology. U-Healthcare technology has various security threats because it is based on network. The purpose of this paper is to examine the threats of DOS / DDOS attacks based on network attacks, and to propose the response technique that fit the situation of the U-Healthcare service by modifying the existing Detecting Early DOS / DDOS attacks through Packet Counting.

  • This paper presents development of a management system for the health and diseases of the elderly. This study aimed to the promotion of the u-healthcare industry and to the increase in its competitive power by developing and expanding a system for managing the health and diseases of the elderly, in cooperation with u-healthcare companies. The study is underway through the following four substudies. In the first substudy titled development of the bio-signal collection and analysis technology using smart media, a technology that supports the collection of bio-signals in the elderly using portable terminals. In the second substudy titled development of the patient-specific healthcare platform expansion and enhancement technology, a technology is being developed for making medical decisions and taking measures based on the results of the processing of the collected bio-signals. In the third substudy titled development of the N-screen based healthcare contents open service technology, a technology is being developed to provide information on health, diseases, and medicine to platforms. In the fourth substudy titled development of the oriental medicine diagnosis and analysis technology for senile diseases. This study is expected to help ensure an excellent workforce and new technologies in the healthcare sector using smart phones, and to help reduce medical expenses by improving the health of citizens.

Current Issues of Healthcare Research and Development in Korea (한국 보건의료 연구개발의 현황과 과제)

  • There has been much efforts to facilitate healthcare innovation and many desirable outcomes were produced in Korea. However, some structural deficiencies were found. They are misalignment of research and development (R&D) subjects with healthcare system, lack of flexibility of current healthcare system to accommodate the radically innovative products, and lack of cooperation among innovation agent. Some suggestions to correct these deficiencies are discussed. The suggestions are as follows: relating Korean healthcare R&D to healthcare system, enhancement of institutional flexibility to allow innovative application of new technology, improvement of the R&D process, and reexamination of the role of academic medical centers.

Status Quo of Evidence-Based Design Research and Its Research Trends in Korean Healthcare Architecture (한국 의료건축연구의 근거기반설계에 대한 최근 연구동향)

  • Purpose: The purpose of this study is to provide understandings about EBD research trends in Korean healthcare architecture research. Method: The systematic literature review was conducted, while comparing the characteristics of research papers in Journal of Korea Institute of Healthcare Architecture with EBD related research papers among them. Results: It is found that the difference of research method utilized between healthcare research in general and EBD research. Specifically, a research method most utilized in general is a field study. In EBD research, however, literature review is most frequently used. Implications: This study can be used as a reference point for further development of EBD research in Korean healthcare architecture. In addition, research results can be used as a communication tool with healthcare specialists such as doctors and consultants in a healthcare project team. Thus, this study is anticipated to play a preliminary role for the development of healthcare architecture design in Korea.
