  • The performances of the building service equipment relay on the individual superintendent's share for the assessment of performance, fault detection, deterioration diagnosis of the building service equipment. A major use of building energy management system(BEMS) is for monitoring plant and building's energy consumption. National building management system (N-BMS) presented in this study links buildings into a network group in order to monitor and control the buildings. How to construct the N-BMS was considered to save energy resource and to conserve performance of building service equipment. The FEMIS, facility, energy/environmental management & information system, for building service offer management process integrated with BAS, FMS and EMS and so on.

  • The service quality with the various building equipments tends to depend on the individual superintendent's capability and efforts generally and this often lead to poor maintenances and managements of them. N-BMS(Networked Building Management System), proposed in this study, is a new type of building management method, which links principal equipments within many buildings into a communication network and carries out several significant missions such as fault detection, deterioration diagnosis and control the equipments and so on, as well as monitoring which is the main and unique purpose of the conventional BMS. How to construct the N-BMS was considered to keep performance of equipments high and thus to save energy and resource. LCC(Life Cycle Cost) based analysis was also performed in order to verify the effects of some maintenance and management works for the building HVAC systems.

As the result, very different quality of the performance or the durability of equipment is being obtained with his skill and effort and it is also not easy to assess that quality. This finally lead to the waste of labor force and the operating cost due to the high-cost, low-efficiency system. How to construct the N-BMS was considered to save energy, resource and to conserve performance of building service equipment. The FEMIS, facility, energy/environmental management & information system, for building service offer management process integrated with BAS, FMS and EMS and so on.

  • Lithium is mainly prescribed for manic and depressive disorder, also frequently prescribed for the other diseases such as migraine, cluster headache, alcoholism, and obsessive-compulsive disorder. An acute lithium intoxication occurs in cases of patients ingesting large amount lithium at a time, a chronic lithium intoxication occurs in patients on chronic lithium therapy. Acute or chronic lithium poisoning occurs frequently in case of patients on chronic lithium therapy ingesting larger dose than prescribed. Manifestations of lithium poisoning are various. It is possible nervous, cardiovascular, renal, gastrointestinal and endocrine systems to be involved. Due to intracellular high concentration, mortality rate is high in acute lithium intoxication patients on chronic lithium therapy. We report a case of acutely intoxicated 40-year-old male on chronic lithium therapy. His chief complaints were deterioration and high fever. On his arrival to an emergency department, he was in cardiac arrest. He restored return of spontaneous circultion (ROSC) 5 minutes later after cardiopulmonary cerebral resuscitation (CPCR) and referred to department of internal medicine for hemodialysis. Vigorous treatment was given to the patient, but he was expired at 4th hospital day.

  • The monitoring and assessment on the insulation condition of generator stator windings have been an important task of utility companies. The interest for the assessment of insulation condition has been increasing due to the need to keep old generating equipment reliable in order to extend the equipment life and to increase the generating capacity. Even though many developments and research activities for the condition assessment of generator insulation have been performed for decades, the assessment criterion in order to consistently predict the actual insulation condition is still in question. In this paper, the correlation between the parameters and the insulation condition is analyzed through the various non-destructive diagnostic tests in order to establish the assessment criterion on insulation deterioration of generator stator windings. By analyzing the correlation, PDI(Partial Discharge Index) as a novel parameter for the assessment criterion on insulation diagnosis of stator winding is proposed and verified.

  • Partial-thickness rotator cuff tear (PTRCT) is not single disease entity but one phase of disease spectrum. Symptoms of PTRCT vary from being asymptomatic to severe pain leading to deterioration in quality of life. Pathogenesis of degenerative PTRCT is multifactorial. Whereas articular sided PTRCT is usually caused by internal causes, both internal and external causes have important role in bursal sided PTRCT. A detailed history, clinical examination and magnetic resonance angiography are used in the diagnosis of PTRCT. Treatment of PTRCT is chosen based on age, demands of patients, causes and depth of tear. In most patients, non-operative treatment should be initiated. Whereas debridement can be done for less than 6 mm of articular sided PTRCT and in less than 3 mm of bursal sided PTRCT, repair techniques should be considered for higher grade PTRCT than that. Although the effect of acromioplasty is not clear, acromioplasty may be performed when the extrinsic causes appear to be the cause of tear. Either transtendon repair technique or repair after tear completion provided satisfactory clinical outcomes in treatment of articular sided PTRCT.

  • Pulmonary lymphangioleiomyomatosis is a rare disease occurring almost exclusively in woman of reproductive age. It is characterized by rapid deterioration of respiratory functions and results in death within an average of 10 years. This disease is caused by a progressive proliferation of atypical smooth muscle in the pulmonary lymphatic vessels, blood vessels, and airways leading to the development of distal cystic changes which causes frequent recurrent pneumothoraces. We experienced a case of pulmonary lymphangioleiomyomatosis in a 30-year old woman who had a history of bilateral, recurrent pneumothoraces. The patient underwent lung biopsy through right thoracotomy which revealed the diagnosis of pulmonary lymphangioleiomyomatosis. We report a case with a review of the literature.

Sellar-Suprasellar Extraventricular Choroid Plexus Papilloma : A Case Report and Review of the Literature

  • Choroid plexus papillomas (CPPs) are relatively rare neuroectodermal tumors that develop from choroid plexus epithelial cells and are usually restricted to the ventricles. Extraventricular CPPs are very unusual and can be difficult to diagnose and treat. A 50-year-old male patient was admitted to our clinic complaining of headache and visual deterioration. Neurological examination found no abnormalities except decreased light perception and secondary optic atrophy in the left eye. Endocrine testing revealed normal levels of hormones produced by the pituitary and target glands. Magnetic resonance imaging of the brain revealed a huge regular-shaped lesion in the sellar-suprasellar region occupying the sella turcica and extending into the suprasellar cistern and planum sphenoidale. The lesion was completely excised by microsurgery via an ordinary left-sided pterional approach. Histopathology identified the lesion as a choroid plexus papilloma. Following the case report, literature on the origin, differential diagnosis, and treatment of this rare tumor is reviewed.

Disseminated Postnatal Cytomegalovirus Infection in a Preterm Neonate: Autopsy Case Report

  • Treatment guidelines for postnatal cytomegalovirus (pCMV) infection in preterm have not been established yet. Neutropenia, thrombocytopenia, hepatitis, colitis, and sepsis-like disease are among the clinical manifestations, which range from moderate to serious. We present a case of autopsy diagnosed as pCMV infection in a premature infant delivered at gestational age of 24 weeks and 5 days. On the 7th and 14th days of birth, urinary CMV polymerase chain reaction samples were negative, ruling out congenital CMV infection. However, autopsy examination revealed that the patient had disseminated pCMV infection. CMV inclusion bodies were found in the majority of tissues, including the lung, liver, pancreas, breast, kidney, and adrenal gland, but not the placenta. The thymus exhibited significant cortical atrophy and T-cell immunodeficiency, possibly induced by dexamethasone treatment for bronchopulmonary dysplasia or by pCMV infection itself. If dexamethasone treatment is extended or high doses are considered, it may be beneficial to test the CMV infection status to prevent aggravation of infection. This case demonstrates that, despite the low prevalence, pCMV infection should be considered a differential diagnosis in preterm if other conditions or etiology cannot justify clinical deterioration.

Insulation Diagnosis of Gas-Insulated Switchgears by TEV Detection (TEV 검출에 의한 가스 절연 개폐기의 절연 진단)

  • Transient earth voltage (TEV) signals propagate on metal surfaces when partial discharge (PD) occurs due to the deterioration of insulation performance in the operation of gas-insulated switchgears (GIS). A TEV sensor has advantages of high sensitivity and convenient installation for detecting PD defects. However, the TEV sensor depends on imports in domestic and detailed studies have not been conducted. In this study, a sensor was designed and fabricated by the TEV principle and its response characteristics were evaluated for detecting PD defects, which were simulated as protrusion on conductor (POC), protrusion on enclosure (POE), and free moving particle (FMP) defects. Finally, the PD-induced TEV signals were measured and phase-resolved partial discharge (PRPD) patterns were analyzed to identify the type of defect.
