Development of Diagnostic Indicator for the Sasang Constitution Exterior-Interior Disease Based on Original Symptom (사상의학의 표리변증에 대한 소증 진단지표 개발연구: 소음인, 소양인, 태음인을 중심으로)

  • Objectives The aim of study was to suggest diagnostic indicator according to Exterior-Interior disease for the Sasang Constitution based on original symptom. Methods We investigated the literature(『Dongeuisusebowon sinchukbon』) and another study(Clinical Practice Guideline for Disease of Sasang Constitutional Medicine: Diagnosis and Algorithm). As a result, we developed diagnostic indicator of original symptom for Exterior-Interior disease in Sasang Constitutional Medicine. Results and Conclusions Diagnosis of Exterior-Interior disease in Sasang Constitution was decided by heat and cold of original symptom. Detailed indicators of diagnosis in Exterior-Interior disease were heat/cold sensitivity, the degree of sweating, the amount of drinking water, thirst, face color and somatalgia.

Percutaneous Fine Needle Aspiration Cytology of Thymoma (흉선종의 경피 세침흡인 세포학적 검색)

  • The fine needle aspiration (FNA) cytologic findings in 16 cases of histologically confirmed thymoma are reported. The aspirates were obtained under fluoroscopic guidance. The cytologic diagnoses were inadequate sample in one case, thymoma in 12(75%), small cell carcinoma or thymoma in 1, benign mesenchymal tumor in 1, and germ cell tumor in one. The cytologic features were detailed according to the constituent epithelial cell type, and into 4 small of epithelial cells and lymphocytes. Fifteen cases were classified into 4 small epithelial cell type, 6 intermediate epithelial cell type, 1 large epithelial cell type, 1 large pleomorphic epithelial cell type, and 3 spindle-shaped epithelial ceil type. Cytologic differential diagnosis was discussed, and the important criteria for the cytologic diagnosis of thymoma were reviewed. This review leads us to think that nonoperative cytologic approaches in the diagnosis of the thymoma are possible, and that correct cytologic diagnosis of thymoma with FNAs can easily be made, if adequate samples are obtained However the invasiveness and histologic type could not be predicted by cytological features only. Knowing various cytologic and histologic features of thymoma will be helpful for the diagnosis of thymoma and the differential diagnosis of modiastinal tumors.

Need for an Update for the Guideline for the Management of Mild Cognitive Impairment

  • Attention is being paid to diagnosis and treatment of mild cognitive impairment (MCI) because early diagnosis and preventive management can slow down the progression of Alzheimer's disease. In particular, in the present era, the use of biomarkers for predicting conversion into dementia is permitted in medical practice. Therefore, authors aimed to propose additional considerations when updating guidelines for the management of MCI, including predictable biomarkers, revising treatment option after additional clinical trials for cholinesterase inhibitors, and detailed regimes for lifestyle interventions. After reviewing 3 patients with MCI by detailed evaluation, we realized that cholinesterase inhibitors were not recommended. In addition, regular exercise and cognitive training were only possible recommendations for patients according to current guidelines, although all 3 patients had evidence of β-amyloid accumulation and related neurodegeneration. Furthermore, caregivers for all 3 patients were worried whether patients could keep doing regular exercise and cognitive training by themselves and asked about the economic training system which monitors patients so that they can keep training. Therefore, we propose that guidelines for managing MCI need to be updated in the present era when the use of biomarkers for predicting conversion into dementia is permitted in medical practice.

  • Integrated pest management is essential for controlling plant diseases that reduce crop yields. Rapid diagnosis is crucial for effective management in the event of an outbreak to identify the cause and minimize damage. Diagnosis methods range from indirect visual observation, which can be subjective and inaccurate, to machine learning and deep learning predictions that may suffer from biased data. Direct molecular-based methods, while accurate, are complex and time-consuming. However, the development of large multimodal models, like GPT-4, combines image recognition with natural language processing for more accurate diagnostic information. This study introduces GPT-4-based system for diagnosing plant diseases utilizing a detailed knowledge base with 1,420 host plants, 2,462 pathogens, and 37,467 pesticide instances from the official plant disease and pesticide registries of Korea. The AI plant doctor offers interactive advice on diagnosis, control methods, and pesticide use for diseases in Korea and is accessible at https://pdoc.scnu.ac.kr/.

  • In this paper, we implement embedded Linux intelligent controller and remote monitoring system for Bridge Diagnosis. Embedded controller as the hard core is consisted of 32 bit CPU and is designed to have processing of real time monitoring and FFT for Bridge Diagnosis. The prototype monitoring system can operate with world wide web in GUI environment by Java. Detailed design and functional analysis for monitoring system are performed by systems approach.

  • Chronic abdominal pain (CAP) is a common complaint encountered in pediatric clinics and a great concern for patients and their caretakers as well as health care professionals. A constant challenge is detecting individuals with organic diseases or psychosomatic disorders from the majority of patients who have a functional disorder including functional dyspepsia, irritable bowel syndrome, functional abdominal pain, and abdominal migraine. Beginning with a detailed history and physical examination, physicians must determine a differential diagnosis of CAP by applying the symptom-based Rome III criteria to positively identify a functional disorder. These findings should then be further analyzed based on diagnostic clues and red flags that indicate the presence of specific organic diseases and/or the need for further testing. Once a functional diagnosis has been made or an organic disease is suspected, physicians can initiate an empiric therapeutic trial. Since psychological distress accompanies both organic and non-organic abdominal pain in children, a cooperative diagnostic approach involving pediatricians and psychiatrists is recommended.

  • A new diagnosis model was constructed by combining atomic force microscopy (AFM), wavelet, and neural network. Plasma faults were characterized by filtering AFM-measured etch surface roughness with wavelet. The presented technique was evaluated with the data collected during the etching of silicon oxynitride thin film. A total of 17 etch experiments were conducted. Applying wavelet to AFM, surface roughness was detailed into vertical, horizon%at, and diagonal components. For each component, neural network recognition models were constructed and evaluated. Comparisons revealed that the vertical component-based model yielded about 30% improvement in the recognition accuracy over others. The presented technique was evaluated with the data collected during the etching of silicon oxynitride thin film. A total of 17 etch experiments were conducted

  • This paper introduces new diagnostic criteria and differential diagnosis of orthostatic dizziness to help clinicians to diagnose hemodynamic orthostatic dizziness. Clinicians need to be able to discriminate hemodynamic orthostatic dizziness from other types of dizziness that are induced or aggravated when standing or walking. Measurements of the orthostatic blood pressure and heart rate are important when screening hemodynamic orthostatic dizziness. Detailed history-taking, a physical examination, and laboratory tests are essential for finding the cause of hemodynamic orthostatic dizziness. The differential diagnosis of hemodynamic orthostatic dizziness is crucial because it can be caused by various autonomic neuropathies.

  • Autism spectrum disorder (ASD) is diagnosed by the clinical decision of a trained professional based on the Diagnostic and Statistical Manual for Mental Disorders, Fifth Edition or International Classification of Diseases 11th Revision diagnostic criteria. To obtain information for diagnostic formulation, professionals should explore detailed developmental history, and can use structured or semi-structured assessment tools to observe interaction between the child and parents or strangers. Diagnostic assessment should include a profile of the strength and weaknesses of the individual and should be conducted using an optimal approach by a multidisciplinary team with appropriate techniques and experience. Assessment of language, cognitive, neuropsychological, and adaptive functioning should be conducted in ASD individuals prior to establishing an individualized treatment plan. Genetic testing, brain magnetic resonance imaging or electroencephalogram testing can be considered for identification of underlying causes.

  • Pulse diagnosis has been highly valued in many East Asian medical books, since "Huangdaineijing(黃帝內經)" published. Gigoo-inyoung pulse method(氣口人迎脈法) is one of the four pulse diagnostic methods originally written in"Huangdaineijing(黃帝內經)". At the late 3rd century, "Maijing(脈經)" redefined the positions of Gigoo-inyoung pulse(氣口人迎脈). According to "Maijing(脈經)", both spots were 1fen(分) in front of guan(關) but Gigoo on the right hand and Inyoung on the left hand. At the middle of 13th century, "Maekgyolejijangbyungsikdoseol(脈訣指掌病式圖說)" had detailed view points about the manner of reading Gigoo-inyoung pulse. "Maekgyolejijangbyungsikdoseol(脈訣指掌病式圖說)" explained clearly the position of 1fen(分) in front of guan(關) by sketch of wrist. The location indicated the border of cun(寸) and guan(關). It also explained clearly corresponding certain cun-guan-chi(寸關尺) with Gigoo-inyoung pulse by sketch. "Euihackipmoon(醫學入門)" is a comprehensive book on medicine written by Lee-Cheon(李梴) in Ming(明) dynasty. In this book, pulse diagnosis is the most emphasized method among the four techniques of diagnosis(四診). Furthermore, it introduces detailed information about Gigoo-inyoung pulse method(氣口人迎脈法) in "Gigoo-inyoungmaekgyole(氣口人迎脈訣)". Lee-Cheon concured in the theory of "Yeongruegumbang(永類鈐方)" and "Maekgyolejijangbyungsikdoseol(脈訣指掌病式圖說)" and put a short essay about Gigoo-inyoung pulse named "Gigoo-inyoungmaekgyole(氣口人迎脈訣)" quoted from the two books. Therefore, throughout many chapters of "Euihackipmoon(醫學入門)", Gigoo-inyoung pulse is used in order to distinguish internal damage(內傷) and external contraction(外感). "Maekgyolejijangbyungsikdoseol(脈訣指掌病式圖說)" and "Euihackipmoon(醫學入門)" understood Gigoo-inyoung pulse commonly by corresponding certain cun-guan-chi(寸關尺) with Gigoo-inyoung pulse. Therefrom, Gigoo-inyoung pulse can distinguish seven emotions(七情) of internal damage(內傷) or six excesses(六淫) of external contraction(外感).
